Objective: The purpose of this study was to investigate the effect of electroacupuncture on acute pancreatitis (AP). Method: The search engines that were used included PubMed, CNKI, OASIS, NDSL, the Korean Traditional Knowledge Portal, and J-STAGE. The search period was from the creation of the search engine to May 6, 2019; there were no language limitations. Extractions and selections from the literation were made by two authors. The study included randomized controlled trials with electroacupuncture for patients with acute pancreatitis. Cochrane's risk of bias (RoB) was used as the methodological quality assessment scale. Results: Nine studies were finally selected. It was observed that using electroacupuncture with general Western medicine treatment for AP was more effective than using only general Western medicine treatment. Although most of the studies showed a high risk of bias, there were significant differences among the indicators. Conclusion: The electroacupuncture treatment with Western medicine may be more effective in reducing symptoms of AP than Western medicine alone. However, due to the limitations of the research design, it was not enough to obtain reliable information. As a result, high quality research will be additionally required.
Octreotide는 somatostatin 동족체로서 8개의 aminoacid로 구성된 합성 polypeptide이다. 이는 외분비선과 내분비선, 특히 위의 분비기능과 췌장분비 기능을 억제하는 작용이 강하며 somatostatin에 비하여 체내 반감기가 길고 장기에 대한 선택성이 높기 때문에 소화성 궤양에 의해 유발되는 출혈, 급성췌장염, 그리고 소화기관의 분비선에 나타나는 종양의 치료에 대한 적용이 시도되고 있다. 저자등은 somatostatin이 말초 교감신경에 존재하며, 신경전달인자 또는 신경조정인자로서 작용할것이라는 보고와 임상적용시 혈압과 심박동수를 감소시킨다는 보고에 흥미를 가지고 그 유도체인 octreotide가 말초 교감신경에 미치는 작용을 관찰하기 위하여 다음과 같은 실험을 하였다. 흰쥐 (Sprague-Dawley)의 전립선 부위 정관 절편을 적출 근편 실험조에 현수하여 등척성 장력을 측정하였다. 흰쥐 적출정관은 전기장 자극(duration : 1 mSec, voltage: 50 V, frequency: 5 Hz or 30 Hz, train: 10 Sec)에 대하여 first phasic component (FPC)와 second tonic component (STC)로 구성된 수축 반응을 나타내었으며, 이러한 수축반응은 tetrodotoxin($10^{-6}M$) 전처치로 소실되었다. Octreotide는 전기장 자극에 의해 유발되는 수축 반응을 FPC, STC 모두 농도의존적으로 억제하였다. 그리고 24 시간 전에 reserpine(1 mg/kg, IP)을 전처치하여 norepinephrine을 고갈시킨 흰쥐에서 적출한 정관 절편과 beta-gamma-methylene ATP를 전처치하여 퓨린성 수용체를 탈감작시킴으로써 ATP의 작용을 배재한 정관절편의 전기장 유발 수축을 공히 억제하였다. 이러한 octreotide의 모든 수축 억제 작용들은 5 Hz의 전기장 자극을 가하였을 때, 30 Hz의 전기장 자극을 가하였을 때 보다 더 강하게 나타났다. 그러나 octreotide는 정관 절편의 기본장력에는 영향을 미치지 않았으며, 외부에서 첨가한 norepinephrine과 ATP에 의해 유발되는 수축반응에는 영향을 미치지 않았다. 이상의 결과로 미루어 보아 octreotide는 흰쥐적출 정관의 교감신경성 신경원으로부터 ATP와 norepinephrine의 유리를 억제함으로써 그 수축성을 억제하는 것으로 사료된다.
In this study, we aimed to investigate the effect of GamiChungYi-tang(GCY-t) on caerulein-induced acute pancreatitis (AP). It is performed by detecting oxidative stress markers and observing histopathological examination. Thirty adult male rats(Sprague-Dawley) were divided into six groups as follows: normal (NOR,n=5), caerulein-induced (CON,n=5), caerulein+Cefotaxime Sodium(CT,n=5), caerulein+GCY-t (130 mg/kg, CHA,n=5), caerulein+GCY-t (260 mg/kg, CHB,n=5) and caerulein+GCY-t (520 mg/kg, CHC,n=5) groups. Pancreatic tissues of rats from all groups were removed for apoptosis and light, and electron microscopic examination. Blood of rats from all groups were collected for oxidative stress markers inspection and pathological examination. Pancreatic oxidative stress markers were evaluated by the measurements of leukocyte, serum amylase and platelet activating factor (PAF), Interleukin-6 (IL-6) levels were determined spectrophotometrically. CON group has a significant increase (p<0.05) in amylase compared with NOR, but CT and CHA, CHB, CHC groups reduced the levels of these enzyme. The levels of Platelet activating factor (PAF) were increased in CON compared with NOR, but decreased in CT and CHA, CHB, CHC groups compared with CON. Interleukin-6 (IL-6) levels were increased significantly in CON compared with NOR, but reduced in CT and CHA, CHB, CHC groups. In the observations of Optical microscopy and electron microscopy, The experimental groups showed the significant decreases in pancreatic tissue inflammation, edema, vacuolization, necrosis compared to the control group. After all, GCY-t is potentially capable of limiting pancreatic damage produced during AP by restoring the fine structure of acinar cells and tissue.

Methods : We examined changes of pancreatic weight, histological, immunohistochemical and gene expression of cyclooxygenase (COX-2). Thirty-six adult male Sprague-Dawley rats were divided into six groups as follow: normal(Nor), caerulein-induced (Con), caerulein + cefotaxime sodium(CT), caerulein + SJS 3 mg/kg(SJSA), caerulein + SJS 6 mg/kg(SJSB) and caerulein + SJS 12 mg/kg(SJSC) groups. Pancreatic tissues of rats from all groups were removed for histological observation and light, and electron microscopic examination. Platelet activating factor(PAF) and Interleukin-6(IL-6) levels were determined spectrophotometrically. Results : The ratio of pancreas/body weight was significantly(p<0.05) increased in the Con compared with Nor, but significantly(p<0.05) decreased in SJSA, SJSB, SJSC and CT groups compared with Con. Caerulein administration significantly increased(p<0.05) the levels of amylase, but SJSA, SJSB, SJSC and CT significantly(p<0.05) reduced the levels of these enzymes. The levels of platelet activating factor(PAF) increased in Con compared with Nor, but decreased in SJSA, SJSB, SJSC and CT groups compared with Con. Interleukin-6(IL-6) levels increased significantly in all groups compared to Nor at 6 hrs, but significantly(p<0.05) reduced in SJSA, SJSB, SJSC and CT groups compared with Con at 24 hrs. The levels of tumor necrosis factor(TNF)-${\alpha}$ levels increased in all groups compared to Nor at 6 hrs, but significantly(p<0.05) reduced in SJSA, SJSB, SJSC and CT groups compared with Con at 24 hrs. The COX-2 positive materials were observed in the pancreas of the Con, but these positive materials were decreased in the SJS extract treatment group. Conclusion : SJS is potentially capable of limiting pancreatic damage during AP by restoring the fine structure of acinar cells and tissue; therefore, we conclude that SJS may have beneficial effects in the treatment of caerulein-induced AP.
Objectives : We examined the effects of electroacupuncture on the cholecystokinin-octapeptide-induced acute pancreatitis in rats. Methods : Rats were administered with $75{\mu}g/kg$ cholecystokinin-octapeptide subcutaneously three times (1, 3 and 5h after shaving) for 5days. Three days after finishing cholecystokinin-octapeptide administration, high frequency electroacupuncture (100Hz) and low frequency electroacupuncture (2Hz) were applied to acupoint equivalent to ST36 (Zusanli) for 7 days. The author determined the pancreatic weight/body weight ratio, the levels of pancreatic heat shock protein HSP60 and HSP72. The author also assay the secretion of ${\beta}-amylase$, lipase and pro-inflammatory cytokines in serum. Repeated cholecysokinin-octapeptide treatment resulted in the typical laboratory and morphological changes of experimentally induced pancreatitis. Results : Eelectroacupuncture significantly decreased the pancreatic weight/body weight ratio in cholecystokinin-octapeptide-induced acute pancreatitis, increased the pancreatic levels of HSP60 and HSP72, and decreased ${\beta}-amylase$ and lipase levels in cholecystokinin-octapeptide-induced acute pancreatitis. Additionally, the secretion of $Interleukin-1{\beta}$ and tumor necrosis $factor-{\alpha}$ was decreased in the animals treated with electroacupuncture. Conclusion : These results suggest that electroacupuncture may have protective effects against cholecystokinin-octapeptide-induced acute pancreatitis.
We performed this study to evaluate the potential clinical marker of urinary trypsinogen-2 together with amylase, lipase and urinary amylase creatinine clearance ratio (ACCR) for the diagnosis of acute pancreatitis in dogs. In the experiment on daily changing patterns of amylase, lipase and ACCR measurements in experimentally induced pancreatitis dogs, compared to values measured in pre-induction state, significant difference was seen in amylase until 5th day of induction, and for lipase significant difference was found during the 7th day of observation period (p < 0.05). No significant difference was found in ACCR for the study period (p > 0.05). On SDS-PAGE analysis of urine from experimentally induced pancreatitis dog, The 26kd band was markedly increased compared with that of normal state and that band was confirmed trypsinogen-2 using substrate interaction and isoelectric focusing assay after being eluted. When assessing the appearance of 26kd band on urine SDS-PAGE 87.1% (range: 50~100%) of experimentally induced pancreatitis dogs showed positive results, whereas no corresponding band was seen in dog without pancreatic disorders. With this result, determination of urinary trypsinogen-2 assay was found to have a high diagnostic value with a 70% of sensitivity and 100% of specificity as a routine test for pancreatitis, although the detection of trypsinogen-2 in urine can be varied on the progression stage of pancreatitis at the initial visit to animal clinic. We therefore suggest that the promising results in this study be used for the development of dipstick test for detecting acute pancreatitis in the future research.

Methods : Male Wistar rats weighing 200 to 250 g were divided into three groups. The first was normal untreated group, the second was in treatment with DMG group; DMG was administered orally, followed by 75 ${\mu}g/kg$ CCK subcutaneously three times, after 1, 3 and 5 h. This whole procedure was repeated for 5 days. The third was in treatment with saline group, the protocol was the same as in treatment group with DMG. We determined the pancreatic weight/body weight ratio, the levels of pancreatic HSP60, HSP72, ERK, p38 MAPK and the secretion of pro-inflammatory cytokines. Repeated CCK treatment resulted in the typical laboratory and morphological changes of experimentally induced pancreatitis. Results : DMG was significantly decreased the pancreatic weight/body weight ratio in CCKinduced AP and DMG increased HSP60 and HSP72 compared with CCK-induced AP. DMG suppressed ERK and p38 MAPK activation. Additionally, the secretion of $IL-1{\beta}$ and $TNF-{\alpha}$ and the levels of amylase and lipase were lower than that saline. Conclusions : DMG has an effect to treatment for CCK-induced AP.

Altogether, this indicates EBS is potentially capable of limiting pancreatic damage produced during AP by restoring the fine structure of acinar cells and tissue. Conclusions : We concluded that EBS may have beneficial effects in the treatment of caerulein-induced AP.

Normal untreated group, in treatment with PH group; PH (1g/kg) was administered orally, followed by $75\;{\mu}g/kg$ CCK subcutaneously three times, after 1, 3 and 5 h. This whole procedure was repeated for 5 days. In treatment with saline group, the protocol was the same as in treatment group with PH. The author determined the pancreatic weight/body weight ratio, the levels of pancreatic HSP60, HSP72 and the secretion of pro-inflammatory cytokines. Methods : Repeated CCK treatment resulted in the typical laboratory and morphological changes of experimentally induced pancreatitis. PH was significantly decreased the pancreatic weight/body weight ratio in CCK-induced AP. Futhermore, the author demonstrated that PH increased HSP60 and HSP72 compared with CCK-induced AP. Additionally, the secretion of $TNF-{\alpha},\;IL-1{\beta}$ and IL-6 the levels of amylase and lipase were lower than that saline. Conclusions : These results suggest that PH may has a inhibitory effect against CCK-induced AP.

DWT was significantly decreased the pancreatic weight/body weight ratio in CCK-induced AP. Futhermore, The author demonstrated that DWT increased HSP60 and HSP72 compared with CCK-induced AP. Additionally, the secretion of $IL-1{\beta}\;and\;TNF-{\alpha}$ and the levels of amylase and lipase were lower than that saline. Conclusions : These results suggested that DWT may has a protective effect against CCK-induced AP.
