Purpose: The aim of this study was to evaluate the clinical usefulness of ultrasound examination of children performed by a pediatrician. Methods: One thousand children who presented with symptoms of a gastrointestinal disorder and underwent abdominal ultrasound evaluation in the Department of Pediatrics, between January 2003 and June 2006, were included in this study. We analyzed the patient's medical records and ultrasound results retrospectively. Results: Among the 1,000 patients, 58.4% were male and 41.6% were female. The mean age of the patients was $4.7{\pm}4.0$ years. The main reasons for ultrasound were abdominal pain (43.9%), vomiting (17.3%), elevated liver enzymes (11.8%), and jaundice (9.8%). Abnormal ultrasound findings were present in 57.9% of cases. The major abnormal findings were mesenteric lymphadenitis (29.2%), fatty liver (12.1%), hepatitis (6.4%), hepatosplenomegaly (6.2%), and acute appendicitis (4.8%). The time interval between the initial medical evaluation and the ultrasound evaluation was within 24 hours in most cases (78.5%). The main findings in children with abdominal pain were mesenteric lymphadenitis (32.6%), fatty liver (5.9%), intussusception (2.7%), and acute appendicitis (2.7%). The main findings in children with vomiting were mesenteric lymphadenitis (12.7%), hypertrophic pyloric stenosis (10.4%), and acute appendicitis (3.5%). The major ultrasound findings in children with urinary tract diseases were hydronephrosis (45.4%), urolithiasis (21.5%) and cystic renal disease (18.1%). Conclusion: Ultrasound examination played an important role as a non-invasive and prompt screening examination for detection of abdominal diseases. Ultrasound was an important tool for pediatricians to determine timely information for patient management.
Background: We evaluated a sensitive and quantitative method utilizing fragment analysis of the fms-like tyrosine kinase 3-internal tandem duplication (FLT3-ITD), simultaneously measuring mutant allele burden and length, and verified the analytical performance. Methods: The number and allelic burden of FLT3-ITD mutations was determined by fragment analysis. Serial mixtures of mutant and wild-type plasmid DNA were used to calculate the limit of detection of fragment analysis, conventional PCR, and Sanger sequencing. Specificity was evaluated using DNA samples derived from 50 normal donors. Results of fragment analysis were compared to those of conventional PCR, using 481 AML specimens. Results: Defined mixtures were consistently and accurately identified by fragment analysis at a 5% relative concentration of mutant to wild-type, and at 10% and 20% ratios by conventional PCR and direct sequencing, respectively. No false positivity was identified. Among 481 AML specimens, 40.1% (193/481) had FLT3-ITD mutations. The mutant allele burden (1.7-94.1%; median, 28.2%) and repeated length of the mutation (14-153 bp; median, 49 bp) were variable. The concordance rate between fragment analysis and conventional PCR was 97.7% (470/481). Fragment analysis was more sensitive than conventional PCR and detected 11 additional cases: seven had mutations below 10%, three cases represented conventional PCR failure, and one case showed false negativity because of short ITD length (14 bp). Conclusions: The new fragment analysis method proved to be sensitive and reliable for the detection and monitoring of FLT3-ITD in patients with AML. This could be used to simultaneously assess ITD mutant allele burden and length.
Objective : The present study was carried out to determine if Juglandis Semen Aquacupuncture(JSA) exerts beneficial effect against the urine concentrating defect induced by glycerol injection in rabbits. Methods : In order to test the effect of JSA, rabbits were acupunctured with $0.5m{\ell}$ of 1%JSA for 7days at both sides of $Sh{\grave{e}}ns{\bar{u}}$(BL23) before the glycerol injection. The other animals were pretreated with an equal volume of saline instead of JSA. Results : The urine flow was reduced, but the urine osmolality was significantly lower than the basal period in glycerol-injected animals, indicating that glycerol causes the urine concentrating defect. The fractional excretion of Na and K was increased in glycerol-treated animals. The Free-water clearance and fractional water excretion(V/GFR) were increased in animals treated with glycerol. Conclusion : These results indicate that glycerol injection resulted an impairment in the urine concentrating ability in rabbits. Such changes were prevented by JSA. JSA may be used as a method to treat and prevent glycerol-induced acute renal failure.

Polyoma Virus Enhancer core Binding Protein (PEBP2)는 유전자의 전사를 조절하는 hetero-dimeric transcription factor로서 $\alpha$와 $\beta$ subunit으로 구성되어 있다. $\alpha$ subunit을 coding 하는 유전자중 하나인 PEBP2aB는 급성백혈병과 관련되어있는 t(8;21) 또는 t(12;21)에 의하여 변형됨으로서 백혈병 발병의 원인이 되고 있다 (Miyoshi et al., 1993; .Romana et al., 1995). $\beta$ subunit을 coding 하는 PEBP2$\beta$도 inv(16)에 의하여 변형됨으로서 백혈병을 유도하는 주요 원인이 되고 있다 (Liu et al., 1993). 이 유전자들의 생물학적 활성을 밝히기 위한 연구가 gene targeting에 의한 knockout mouse 생산 방법으로 수행되었다. 그 결과 PEBP2$\alpha$B와 PEBP2$\beta$ 유전자가 definitive hematopoiesis에 있어서 결정적으로 중요한 역할을 하고 있음이 관찰되었다 (Okuda et al., 1996, Wang et al., 1996a, 1996b), 이는 이들 유전자가 bematopoietic master switch 유전자임을 밝힌 중요한 결과로서 이로부터 혈액학 연구 분야의 새로운 장이 열리게 되었다. 또한 이러한 연구 결과들은 PEBP2 family에 속하는 다른 유전자의 생물학적 활성의 연구를 촉진하는 계기가 되었다. 최근 PEBP2$\alpha$A 유전자가 결손된 마우스가 생산되었는데 이 유전자의 경우에는 모든 종류의 뼈의 생성이 완전히 결손됨이 관찰되었다 (Komori et al., 1997). 이는 PEBP2$\alpha$A 유전자가 뼈의 생성을 지배하는 master switch 유전자임을 보여주는 중요한 관찰로서 bone biologist 들의 큰 관심을 모으고 있다. 본 연구팀은 PEBP2 family 유전자 중 유일하게 아직 생물학적 활성이 규명되지 않은 PEBP2$\alpha$C 유전자의 활성을 knockout 마우스를 생산하는 방법에 의하여 분석하였으며 소화기관의 형성에 중요한 역할을 하고 있음을 확인하였다.

The current paper reports on a case of subdural empyema secondary to frontal sinusitis in an otherwise healthy child. Sinusitis is a common and benign condition in most pediatric cases. Because of the widespread use of antibiotics, intracranial extension of pediatric sinusitis is rarely seen today; however, complications (e.g., cavernous sinus thrombosis, orbital infection, meningitis, and subdural empyema) are potentially life threatening. A 15-year-old right-handed male presented with a 3-day history of fever, headache, and left-sided palsy. Computed tomography revealed right-sided subdural empyema with right frontal sinusitis and maxillary sinusitis. A postoperative inpatient neurological consultation was requested 2 months post-surgery due to motor function deficits. The results suggested that early and accurate diagnosis of subdural empyema leads to prompt treatment and a favorable outcome for the patient.
We encountered two children with lead poisoning who were administered herb medicinal pills recommended by their clergyman. These patients presented anemia and severe coliky abdominal pain, but no neurologic symptoms. For this reason, they were initially misdiagnosed with gastrointestinal hemorrhagic disease. However, we got a clue that they took herb medicinal pills. Finally, based on the assay of blood lead level, we made a correct diagnosis of lead poisoning in these patients. These patients underwent chelating therapy. Subsequently, the concentration of blood lead was decreased. Finally, we drew a conclusion that the possibility of lead poisoning must be considered in children who complained of colicky abdominal pain accompanying anemia. Here, we report two pediatric cases of lead poisoning with a review of literature.
Overseas direct investment is constantly on the rise, while domestic investment has been slowing and has led to concerns that the expansion of overseas investment may be weakening domestic investment. Considering the change of environment as economic growth, this paper analyzes the relationship between domestic and overseas investment using a regression tree. The result shows that overseas investment substituted domestic investment in the past (before 1985); however, they compensated for each other during the rapid growth period based on exports (1986-1997). The relationship turns out to be insignificant after the Asian currency crisis(after 1998). In addition, this paper also examines the factors determining domestic facilities investment and overseas direct investment at each stage of the changes in their influence due to globalization. Taking into account the results, we need to re-evaluate the current pattern of corporate investment apart from the past restricted point of view to help domestic enterprises efficiently utilize the international production network and resources.

These volatile organic compounds include benzene, toluene, acetone, and styrene, as well as the representative formaldehyde, and these substances are slowly released over a long period of time, causing acute or chronic diseases to residents. As a method for removing organic volatile substances, physical methods using adsorption, chemical methods for converting volatile substances to other substances, or a mixture of the two are mainly used. In this paper, a sustained release chlorine dioxide gel pack obtained by a method for controlling the reaction rate of a reactant and the release of a product is mixed with a zeolite adsorbent having an optimized hole diameter to adsorb and decompose and remove formaldehyde suspended in the air. I would suggest an effective method.

Henoch-Sch$\ddot{o}$nlein purpura (HSP) is a leukocytoclastic vasculitis of small vessels with deposition of IgA, commonly resulting in skin, joint, gastrointestinal, and kidney involvement. HSP is an uncommon disorder in adults and accounts for 0.6% to 2% of adult nephropathy. We report a case of HSP with acute renal failure successfully treated with corticosteroid. In this case, the patient presented with vasculitic purpuric rash on lower extremity, arthralgia in the wrist, abdominal pain, hematochezia, oliguria and azotemia. Abdominal CT showed wall thickening of the small and large bowels. Skin biopsy revealed leukocytoclastic vasculitis. Percutaneous renal biopsy showed no crescent formation, but mesangial IgA and $C_3$ deposits were observed by immunofluorescence. The patient was treated with corticosteroid (1mg/kg per day) and hemodialysis. After treatment, renal function improved and purpuric lesion, arthralgia and abdominal pain disappeared. Thus, when adults present with purpuric rash and rapidly progressive glomerulonephritis (RPGN), HSP should be a diagnostic consideration.
A 13-year-old boy presented with anterior chest wall depression and dyspnea on exertion(NYHA II). He underwent Ravitch operation for pectus excavatum 7 years ago. A preoperative echocardiographic study revealed secundum atrial septal defect. He had no other abnormality of laboratory test, except FVC and FEVI were decreased into 2.03 L(7 %) and 1.82 L(71 %). He underwent repair of secondary anterior chest wall deformity and secundum atrial septal defect. We used unique method, raising sternum at right angle to secure good operative field for open heart surgery. Acute respiratory insufficiency was developed on postoperative day 1. Mechanical ventilation was applied which could be weaned on postoperative day 6 and thereafter hospital course was uneventful without any other sequale. He was discharged on postoperative day 19.
