The Development and Verification of Balance Insole for Improving the Muscle Imbalance of Left and Right Leg Using based Sound Feedback (청각 피드백이 적용된 좌우 불균형 개선을 위한 밸런스 인솔 개발 및 검증)

  • This study was to develop the balance insole system for detecting and improving the muscle imbalance of left and right side in lower limbs. We were to verify the validation of balance insole system by analyzing the strategy of muscular activities and foot pressure according to sound feedback. We developed the balance insole based FSR sensor modules for estimating the muscle imbalance using detecting foot pressure. The insole system was FPCB have 8-spot FSR sensor with sensitivity range of 64-level. The participants were twenty peoples who have muscle strength differences in left and right legs over 20%. We measured the muscular activity and foot pressure of left and right side of lower limbs in various gait environment for verifying the improvement effect of muscle imbalance according to sound feedback. They performed gait in slope at 0, 5, 10, 15% and velocity at 3, 4, 5km/h. The result showed that the level of muscle imbalance reduced within 30% for sound feedback of balance insole system contrast to high level of muscle imbalance at 169.9~246.8% during normal gait for increasing slope and velocity. This study found the validation of balance insole system with sound feedback stimulus. Also, we thought that it is necessary to research on the sensitivity of foot area, detection of muscle imbalance and processing algorithm of correction threshold spot.

Dedifferentiated Parosteal Osteosarcoma - A Case Report - (역분화성 방골성 골육종 - 증례 보고 -)

  • Dedifferentiated parosteal osteosarcoma is an uncommon variant of osteosarcoma. Dedifferentiation is known to be associated with a greater metastatic potential and a more rapid lethal clinical course. Thus recognition of dedifferentiation is important to establish the treatment strategy. But there may be few significant clinical clues to distinguish between dedifferentiated parosteal osteosarcoma and conventional one. A 29-year-old woman presented with 2-year history of discomfort and swelling in her proximal thigh. Examination showed a large, hard, non-mobile mass. Radiographs revealed a large ossified mass attached to the proximal femur. Diagnosis of parosteal osteosarcoma was established by MRI and needle biopsy. But she had a history of abrupt severe thigh pain and increased swelling before surgery. Follow up MRI showed enlargement of mass with invasion to muscle around tumor. The patient underwent an en-bloc resection of tumor and reconstruction. Histological examination showed parosteal sarcoma with dedifferentiation. The patient expired due to local recurrence of tumor and distant lung metastasis 2 months after the surgery. In case with rapid growth of a lesion or unusual severe pain, one must have a high index of suspicion with regard to dedifferentiation.

Pathological Changes in Cultured Korean Catfish (Silurus asotus) Artficially Infected with Aeromonas veronii (Aeromonas veronii의 실험적 감염에 의한 양식 메기, Silurus asotus의 병리학적 변화)

  • Recently, a new disease showing symptoms such as epidermal exfoliation and muscular necrosis occurred in cultured Korean catfish. Although the mortality of fishes was low but the economic damages owing to loss of commercial value were severe. The authors isolated the causative agent from diseased fish and observed pathological changes both in naturally and artificially infected fish. The causative bacteria was identified as Aeromonas veronii. Subsequently we observed the daily death and pathological symptoms of artificially infected fish with Aeromonas veronii. Symptoms of artificially infected fish were similar to those of naturally infected fish and all fish died within 7 days after infection. Histopathological changes on the naturally infected fish revealed severe congestion and necrotic degeneration in the liver, spleen and kidney. Some bacterial aggregates with inflammatory degeneration were observed in the heart, and congestion and fibrosis in the lamina propria of digestive tube were predominant. In artificially infected fish, skin erosion and necrotic degeneration of muscle tissue around injected region were particularly manifested. Degeneration of hepatocytes in liver and hyalic degeneration around ellipsoids in spleen were partially observed. However, there were no predominant signs in digestive tube in artificially infected fish.

In Vitro Fertilization and Embryo Culture in Immature Rats induced to Superovulate (미성숙 흰쥐에 있어서 과잉배란 난자의 체외수정 및 수정란의 배양에 관한 연구)

  • The ability of fertilization in vitro and subsequent development of superovulated oocytes was assessed in a controlled environment using an in vitro fertilization technique. The in vitro fertilization percentage of oocytes with cumulus mass declined significantly(P<0.05) with increased doses from 4~10 to 16~40IU of PMSG for superovulation. However, the porportion of polyspermic penetration varied from 2.3 to 9.7% and there was no significant difference between treatments in incidence of polyspermy. When morphologically normal ova with cumulus mass were cultured for 66~72h in a plastic mini-straw to undergo fertilization in vitro, the mean percentage of embryos developed to 2-16 and 4-16cell stage was 61.8 and 17.6%; it was slightly(P<0.05) superior to the corresponding results from petri dish. A total of 52 two-cell embryos fertilized in a mini-straw were transferred to seven pseudopregnant rat. Among these recipients, two normal young were born from one recipient which received a total of six embryos. These results suggest that superovulated oocytes are proportionately less competent than normally ovulated oocytes to undergo fertilization in a controlled environment using an in vitro fertilization technique. Also, a plastic mini-straw designed was slightly superior to petri dish as a culture vessel for fertilization and embryo development in vitro.

The purpose of this study was to examine the mechanical properties and patterns of bilateral and unilateral movement under varying velocities and movement patterns. The unilateral and bilateral isokinetic knee extension and flexion were taken for three speeds of 13 healthy male subjects. Although there was bilateral facilitation at the speed of $450^{\circ}$/s for the bilateral movement of knee extension, as a whole there was less resultant torque and power of bilateral movement than summed unilateral under knee flexion and extension of 3 velocity condition. There was significant correlations between bilateral deficits within individuals observed for an the same movement($120^{\circ}-240^{\circ}$ flexion, $120-240^{\circ}$ extension, $240^{\circ}-450^{\circ}$ extension), which means that same agonist was recruited. On the contrary, although there was a tendency of a similar pattern of the individuals bilateral deficit according to the varying velocities, there was not a significant correlations between bilateral deficits of flexion and extension within individual, which means that different agonist was recruited. With the analyses of this results the individuals neuromuscular characteristics and the effecting factors for bilateral movement can be speculated upon.

Gastric anisakiasis cases in Cheju-do, Korea (고래회충유충증 107례 보고 및 어류감염 실태 조사)

  • Human anisakiasis may occur after ingestion of raw marine flesh infected with nematode larvae of Anlsi,hidae. Anisakiasis caused by the migration of the larva into the wall of stomach, small intestine and other portion has been reported in Korea. This prospective study was made of all cases referred to parasitological laboratory in Cheju-do between. Tune 1989 and June 1992. Gastric anisakiasis was confirmed if larvae invading the gastric wall were observed by gastrofiberscopy One hundred and seven cases were diagnosed, most of which were in 30-49 years old. Most of the patients complained acute epigastric pain with history of eating raw marine fish. This symptom usually occurred about 12 hours to 1 day after ingestion of infected marine fish. Edema, erosion or ulcer of the mucosa and hemorrhage from the gastric wall were observed in the involved areas. Ninety larvae removed from the stomach were identified; the larva of Anisqkis siwlex was the most prevalent species, and the Iarva of Pseudntewanoua decipien was also detected. The important species of marine fish from which the patients became infected was demonstrated as yellow corvina, sea eel, ling, cuttle fish, yellowtail and others. Five species of marine fish as a possible source of infection were examined, and Anisakis simplex larvae and Pseunotewqnoua decipirens larvae were collected from the mackerel and rock cod. This study demonstrates that anisakiasis is recognized as a public health problem in Korea.

  • Recently, mass mortality of the young abalone Haliotis discus hannai has occurred in commercial seed production farms in Korea. The mortality rate was above 50% of the total cultured organisms in the farm, and the shell length of the moribund organisms was about 3cm. The mortal phenomenon was that the young abalones were weakly scattered on the bottom of the pond from the attachment matrix, or that they could not be moved back to their normal positions. The diseased farmed Pacific abalone had abdominal edema. From the edema in the moribund individuals, three bacterial strains were isolated and all the strains were identified as Vibrio harveyi. These strains were compared with thirty six strains isolated from the fish. The results was that the Vibrio harveyi from the fish were sorted into genogroup A or B; however, the three strains of the diseased farmed Pacific abalone were sorted into genogroup A and the new genogroup C. The identical mortality and pathological symptoms of the naturally infected organisms were reproduced by artificial infection with WA AG-1 and WA CS-5 strains. The $LD_{50}$ of WA AG-1 and WA CS-5 were each $1.0{\times}10^3cfu\;animal^{-1}$ and $1.7{\times}10^4cfu\;animal^{-1}$.

  • A new scale named LCL(Logically Classified Level) has been developed to judge the status of the spasticity quantitatively implementing a pcndulum test in this study. Total 30 parameters have been used to develop the new scale. One representative parameter that can represent the dominant characteristics of spasticity has been chosen through statistical analysis. 20 parameters among 30 parameters uscd in the statistical analysis were newly developed and 10 parameters were from previous studies. The new 20 parametcrs were developed using combinations of pcndulum test characteristics. ie anglc, angular velocity. musclo midel. and EMG. 11 parameters among 30 parameters have showed strong correlations each other, Finally. L11 that showed consistency at every case has been chosen to be a representative parameter among the 11 parameters. 28 patients data were separated into 4 groups. A regression equation to predict the trend of patients of L11 has been made. The paramcter L11 was tested to prove its usabilitics for various cases of patients. The new LCL scale is expected to be a quantitative scale, and to replace the MAS(Modified Ashworth Scale) that is not a quantitative scale. Especially it is also expected that the new scale could be used to plan a treatmcnt period. methods, and intensity. as it can evaluate the status of patient's in detail.

The First Report on Clinical Manifestation of Cryopyrin-Associated Periodic Syndrome in Korean Children

  • Purpose: The aim of this study was to evaluate the clinical characteristics of children diagnosed as cryopyrin-associated periodic syndrome (CAPS) in Korea. Methods: Diagnosis was made based on clinical features and confirmed by a mutation in the cold-induced autoinflammatory syndrome 1 (CIAS1) gene. Especially, osteocartilaginous overgrowth in the patella or distal femur was so characteristic that its presence warranted a diagnosis of chronic infantile neurologic cutaneous and articular/NOMID. Results: We observed the clinical features of 9 Korean CAPS patients. All the patients suffered from an urticarial rash with recurrent fever. Among the 9 patients, 6 presented with rash and 4 with fever on the 1st or 2nd days of birth. Eight patients showed myalgia, and 7 patients showed arthralgia in the joints, and 6 patients showed radiologic findings of arthropathy including cupping of the metaphysis, excessive growth of the epiphysis, osteopenia or overgrowth of the cartilage. Four patients showed brain atrophy, enlarged ventricles or leptomeningeal enhancement on magnetic resonance imaging. Intellectual disability was observed in 1 patient. Five patients had eye involvement as conjunctivitis, uveitis, chorioretinitis, avascular area or papillary edema, and 3 patients showed progressive hearing loss. All 9 patients showed increased C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). Conclusions: All the patients carried a mutation on exon 3 of the CIAS1 gene. After the anakinra (interleukin-1 receptor antagonist) therapy, the fever and rash immediately disappeared, and CRP and ESR were improved.

DENTAL MANAGEMENT OF A PATIENT WITH MUSCULAR DYSTROPHY UNDER GENERAL ANESTHESIA: CASE REPORT (근이영양증(muscular dystrophy) 환자의 전신마취 하 치과치료 : 증례보고)

  • Muscular dystrophy (MD) is a heterogeneous group of inherited neuromuscular disorders, characterized by progressive muscle weakness. Severity of the disease ranges from mild to severe, and the disease is mostly caused by mutations in a number of genes. These genetic mutations cause lack of proteins which are essential for muscle cell stability. Muscle fibers are gradually replaced by fat and fibrous tissue. The muscles of the head and neck are affected in several types of MD that manifest as altered craniofacial morphology and dental malocclusion. A 3-year-10-month old, 15.0 kg boy with MD presented to Seoul National University Dental Hospital, Seoul, South Korea because of extensive carious teeth. A number of dental caries in primary dentition were identified during clinical oral examination. Due to dental anxiety and underlying systemic disease, general anesthesia was considered. General anesthesia was induced and maintained with intravenous anesthetics, propofol and remifentanil. Caries treatments - resin restoration, pulpectomy, zirconia crown restoration, stainless steel crown restoration - were performed. Under general anesthesia, successful dental procedure was done. Total intravenous anesthesia (TIVA) was performed instead of inhalation anesthesia in order to avoid risk of complications such as malignant hyperthermia and life-threatening rhabdomyolysis. With decreasing muscle function, plaque control becomes more difficult and leads to gingivitis. Especially, the open-mouth posture worsens gingivitis and can leads to malocclusions and problems in swallowing. Regular and periodic dental care is essential for maintaining oral health for patients with MD.
