  • Eosinophilic fasciitis(EF) is a very rare clinical syndrome, especially during childhood. It is characterized by diffuse fasciitis and peripheral eosinophilia. Little is known about the pathogenesis of EF, and it is suggested that immunologic alteration may play a role. Epstein-Barr virus(EBV) is known to cause a variety of diseases via immune mechanism. We report a 22 month old boy with EF following EBV infection, which may be associated with pathogenesis of EF.

  • The purpose of this study was to investigate the effects of 8 weeks Self-Myofascial Release exercise on body composition, vascular and metabolic functions in obese female college students. The subjects were 19 obese female college students (n = 9) who were randomly assigned to an exercise group(n=9) and control group(n=9), with an exception of except 1 abandonment. The Self-Myofascial Release exercise group performed exercises twice a week for 8 weeks and for 40 minutes at one time using a foam roller, and the control group maintained normal life pattern for the same period. Body composition was measured by body weight, body fat percentage, vascular function was measured by pulse wave velocity(PWV), augmentation index(AIx), brachial artery vascular flow-mediated dilation(FMD), and metabolic function was measured by fasting glucose and triglyceride levels. The Self-Myofascial Release exercise with foam roller showed no significant effect on body weight and body fat percentage. The Self-Myofascial Release exercise with foam roller showed no significant effect on heart rate, FMD, PWV but, SBP, CSBP and AIx were significant decreased. The Self-Myofascial Release exercise with foam roller showed no significant effect on fasting glucose and triglyceride levels. In conclusion, 8-week Self-Myofascial Release exercise for obese female college students showed no significant effect on body composition and metabolic function, but improved SBP, CSBP and AIx levels significantly. Therefore, Self-Myofascial Release exercise will be an effective way to improve the vascular function of obese female college students.

  • Temporomandibular disorders(TMD) is a collective term which is embracing a number of clinical problems that involve the masticatory musculature, the TMJ and associated structures, or both. Myofascial pain, which is a kind of masticatory muscle disorder of TMD, is the sensory, motor, and autonomic symptoms caused by myofascial trigger points. There has been some controversies regarding etiologies of TMD and MFP. Especially the issue of occlusal conditions has been a critical issue for long time. Despite much efforts, the results of studies regarding occlusal conditions were contradictory. These controversies might be mostly due to various factors resulting from the complex nature of TMD, however, inaccurate and inappropriate study design, selection criteria, methodologies also play significant roles. Recently, a computerized occlusal analysis system, T-Scan II which made it possible to reveal quantifiable time data and relative force data for analyzing occlusion, was introduced. Some authorities suggested that the concept of disclusion time and prolonged disclusion time of posterior tooth and MFP are related using T-Scan II. But the previous studies which used T-SCAN II are not reliable for they did not provide accurate diagnostic criteria of MFP. Morever they did not compare with controls, and had many other problems. The purpose of this study was to evaluate the relationship between MFP and prolonged disclusion time of posterior tooth, which is one of the occlusal factors of TMD, by selecting 30 subjects as the study group through strict criteria and comparing them with 38 controls using T-SCAN II, computerized occlusal analysis system. The results, statistically analyzed, are summarized as follows: 1. Cronbach ${\alpha}$ coefficient of repeated measurements of disclusion time was 0.92. 2. There were no statistically significant differences at repeated measured disclusion time of both side between control and study group. 3. There was no statistically significant diffefence in the disclusion time between right and left side. From the results above, we can suggest that there was no relationship between MFP and disclusion time, so irreversible treatments leading to the reduction of disclusion time for treating MFP would not be appropriate. However more controlled, large scaled study, which consider various occlusal factors, and quantification of symptoms using Helkimo index would be necessary in the future.

  • Infections involved with the oral and maxillofacial area are associated with various anatomical structures. If the proper treatment is not done in an immediate period, the infections will be quite fatal. The causes of the infections are numerous, but the most common cause of odontogenic infections in children is a dental caries. It is known to lead to some kinds of diseases such as periapical abscess, cellulitis, osteomyelitis, Ludwig's angina, toxic shock syndrome and so on. The common pathogenic sequence of fascial abscess is a necrotic pulpal inflammation in the form of dentoalvelor abscess which spreads over and gradually penetrates into the fascial membranes through the cortical bones and finally contracts the potential fascial spaces. If the infections of oral maxillofacial area were penetrated into the surrounding soft tissues, then they would diffuse into the directions of the least tissue resistance along with the connective tissues and the fascial spaces. These infections can be properly cured by tooth extraction, endodontic therapy, surgical treatment including Incision & drainage and antibiotics. The purpose of the cases is to report the satisfactory treatment results in the patients derived from the canine fascial space abscesss or buccal fascial space ones of the odontogenic origin.

  • Myofascial pain is a condition associated with regional pain and muscle tenderness characterized by the presence of myofascial trigger points. In this case report, a subject complaining of nighttime bruxism was clinically assessed, and a latent trigger point of the anterior temporalis muscle was identified with manual palpation. A surface electromyographic (SEMG) exam of the anterior temporalis muscle harboring the latent trigger point demonstrated several SEMG features, including post-contraction irritability, delayed relaxation following contraction and accelerated muscle fatigue. It was concluded that a SEMG exam may detect abnormal masticatory muscle function and, therefore, assist in the evaluation of myogenous temporomandibular disorders.

  • Majority of toothache is caused by abnormality of pulpal or periodontal tissues. However, there are numerous nonodontogenic sources that may be responsible for pain felt in the tooth. Nonodontogenic toothache may result from muscle, maxillary sinus, neuropathic, neurovascular, even cardiac and psychogenic problems. Myofascial pain is one of the most common abnormality characterized by palpation of a hard band which acts as the trigger point that causes stiffness and fatigue of the muscle, referral pain in tooth, tension-type headache, and hyperalgesia. Masseter muscle particularly induce referral pain in maxillary and mandibular molars. This case reported the treatment of the pain in right mandibular molar originated from myofascial pain of the right masseter. The pain is improved by general and reversible treatments such as muscle exercise, physical therapy, and medication. Nonodontogenic toothache should undergo proper differential diagnosis in order to avoid unnecessary dental treatments, such as endodontic, periodontic treatment, and tooth extraction, which would fail to alleviate the symptom of the patient.

  • The definition of fascial spaces are latent spaces between fascial planes. If infections which spread from dental origin to soft tissue are mild, they are restricted by fascial planes. But, when infections are severe, fascial spaces are often used as a natural pathway which spread to the deep cervical region. If they are not treated at early stage, they may result in the fatal complications as followings; airway obstruction, septicemia, cerebral abscess, and thrombophlebitis etc. The early treatment of fascial space abscess is very important for young children. These case reports present the successful result of fascial space abscess treatment through intracanal drainage without surgical excision. It is proven that the treatment through intracanal drainage has some benefits to the surgical excision, which are as follows: 1) It is economic to the patients or their parents avoiding admission. 2) The treatment procedure is more simple. 3) Childrens can avoid the fearful environment.

  • Nodular faciitis is generally considered to be benign proliferation of fibroblasts and myoblasts, and it measures dimension up to 3cm. The characteristics such as rapid growth, abundant cellularity, and mitotic activity occasionally cause these lesions to mimic sarcoma. The authors experienced two cases of nodular fasciitis of the thigh, which were unusually large with dimension of more than 5cm. All of these two cases mimicked sarcoma and one of two cases, which initially mimicked sarcoma clinically and histologically in our hospital, was finally diagnosed as nodular fasciitis after requesting external consultation to several experienced pathologists. All of two cases had no evidence of recurrence at 2 years postoperatively. The lesions of unusually large dimension such as in our cases must be included in the differential diagnosis of soft tissue sarcoma. So we report two cases of nodular fasciitis of the thigh with a review of the current literature.

  • Nodular fasciitis is a benign fibroblastic proliferation rarely reported in the breast. We report the case of a 55-year-old woman who presented with imaging findings that resembled a malignancy. Mammography revealed an isodense nodule with partially indistinct margin in the right breast, showing the abnormality score 75% on artificial intelligence. Ultrasonography revealed an oval hypoechoic nodule with microlobulated margin, echogenic halo, increased blood flow, and soft elasticity. After core needle biopsy and excision, nodular fasciitis was diagnosed. Although nodular fasciitis of the breast is rare, it may mimic malignancy; therefore, it should be considered as a differential diagnosis to prevent unnecessary intervention.
