The Relationship Factors Geriatric Syndrome of the Elderly Women in Poverty (빈곤층 노인여성의 노인증후군 관련요인 분석)

  • 본 연구는 방문건강관리사업 대상자 중 취약한 노인여성과 노인증후군 관련요인에 대한 인과관계 모형을 규명하여 취약한 노인 여성의 보건의료서비스 제공 시 효율적인 방안제시를 위한 기초자료로 활용하고자 시행하였다. 연구대상자는 2008년 7월 1일부터 9월 30일까지의 기간 동안 전국 보건소에 등록된 방문건강관리사업 대상자 중 동의된 65세 이상 노인여성 1,350명을 선정하여 훈련된 조사원이 직접면담을 통해 조사하였다. 수집된 자료 중 불충분한 29명을 제외한 1,321명을 최종 분석대상으로 하였다. 수집된 자료는 SPSS 17.0 프로그램을 이용하여 빈도분석 및 카이제곱 검정, t-검정, ANOVA, 사후검정은 Tukey를 이용하였으며, 유의한 변수들을 선정하여 로지스틱 회귀분석을 이용하여 분석 하였다. 연구결과를 요약하면 다음과 같다. 첫째, 허약과 관련 있는 노인증후군위험요인은 우울, 요실금, 낙상력, 일상적 수행활동(ADL)으로 나타났다(p<0.05). 둘째, 허약에 영향을 미치는 요인으로는 연령이 75세 이상의 고령일수록, 우울과 요실금의 증상이 있을 때, 최근 낙상경험이 있을 때 허약의 증가 위험이 높은 것으로 나타났다(p<0.05). 셋째, 노인증후군의 위험요인 증가에 영향을 미치는 요인으로는 평생 동안 담배를 5갑 이상 피웠을 때, 당뇨병이 있을 때로 나타났다(p<0.05). 이상 결과를 볼 때, 취약 노인 대상으로 노인증후군, 허약의 향상을 위해서는 만성질환관리 프로그램 뿐 만 아니라 체계적인 근력강화 운동 및 정신건강관리 프로그램의 충분한 확대보급이 반드시 필요하다.

Analysis on the Effects of the Lower Extremities Muscle Activation during Muscular Strength Training on an Unstable Platform with Magneto-Rheological Dampers (MR 댐퍼를 적용한 불안정판에서 하지 근력 훈련이 근 활성도에 미치는 영향 분석)

  • Adequate postural control depends on the spatial and temporal integration of vestibular, visual, and somatosensory information. Especially, the musculoskeletal function is essential to maintain the postural control. The experimental studies was performed on the muscular activities in the lower extremities during maintaining and moving exercises on an unstable platform with Magneto Rheological(MR) dampers. The unstable platform of the developed system was controlled by electric currents to the MR dampers. A subject executed the maintaining and moving exercises which are presented through the display monitor. The electromyographies of the eight muscles in lower extremities were recorded and analyzed in the time and the frequency domain: the muscles of interest were rectus femoris(RF), biceps femoris(BF), tensor fasciae latae(TFL), vastus lateralis(VL), vastus medialis(VM), gastrocnemius(Ga), tibialis anterior(TA), Soleus(So). The experimental results showed that the muscular activities differed in the four moving exercises and the nine maintaining exercises. For the anterior-posterior pattern, the TA showed highest activities; for the left-right pattern, the TFL; for the 45, $-45^{\circ}$ pattern, the TFL and TA. Also, the rate of the increase in the muscular activities were affected by the condition of the unstable platform with MR dampers for the maintaining and moving exercises. The experimental results suggest that the choice of different maintaining and moving exercises could selectively train different muscles in various intensity. Futhermore, the findings suggested that the training using this system can improve the ability of postural control.

Characteristics of Surface Electromyography During Strength Training of 12 weeks (12주 근력강화훈련 동안 표면근전도 신호의 특성)

  • This study tested whether repeated measurement of median frequency (MDF)-related variables could express the muscle power changes during a 12-week DeLome strengthening program, by using consecutive overlapping FFT (Fast Fourier transformation) and integrated EMG (IEMG) from surface EMG data for isometric and isotonic exercise. To evaluate the effect of training, the following were recorded every 3 weeks for the elbow flexors and knee extensors of 5 healthy male volunteers: MVC, lRM, limb circumference, and surface EMG during isometric MVC or isotonic contraction at 10RM load. From the EMG data, IEMG and variables from a regression analysis between MDF and time were obtained. MVC, lRM, IEMG, and initial MDF increased linearly over the training period. The fatigue index and slope of the regression line increased temporarily until the 6th week and decreased thereafter. From these results, there appeared to be enhanced neural recruitment of fast twitch fibers in the first 6 weeks and continued enhancement in the recruitment and hypertrophy of fast twitch fibers, which led to increased fatigue resistance, over the last 6 weeks. Accordingly, the MDF and IEMG analysis technique could demonstrate the effect of the program detected significant changes in both isometric and isotonic contractions. EMG analysis methods can be used to estimate the electrophysiological and histological changes in skeletal muscles during a strengthening program.

The Effects of the Task-Oriented Trunk Training on Muscle Strength and Muscle Activity of Trunk, Balance and Gait in Stroke Patients (뇌졸중 환자의 과제지향몸통훈련이 몸통 근력과 근활성도, 균형 및 보행에 미치는 효과)

  • Purpose : This study aimed to determine whether task-oriented trunk training can improve muscle strength, muscle activity, balance, and gait in stroke patients. Methods : A total of 27 stroke patients who agreed to participate in the study were randomly divided into the following two groups: (1) experimental group 1, task-oriented training applied to the proximal part (n=14) and (2) experimental group 2, task-oriented training applied to the distal part (n=13). Thereafter, task-oriented trunk training was accordingly applied in each group for 60 minutes per session, 5 times per week for 6 weeks. Muscle power, muscle activity, balance, and gait were assessed using a digital dynamometer, surface electromyograph, Timed Up and Go (TUG) test, and gait analyzer (G-WALK), respectively, before and after training. Results : Trunk muscle strength significantly increased in both groups after training (p<.05). and there was a significant difference between the groups. Muscle activity in the stance phase during gait significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. Muscle activity in the swing phase during gait significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. The TUG test values significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. Gait significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. Conclusion : The results of this study show that task-oriented training can improve trunk muscle strength, muscle activity, balance, and gait in stroke patients.

The Effects of Trunk Muscle Strength Training on Sitting Balance of Children with Spastic Cerebral Palsy (체간 근력 강화 훈련이 경직성 뇌성마비아의 앉은 자세 균형에 미치는 효과)

  • The purpose of study was to evaluate effects of a trunk muscle strength training on sitting balance of children with spastic cerebral palsy. Five individuals with spastic cerebral palsy(three females. two males; mean aged 6.6) participated three times a week over 6 weeks. Five individuals with spastic cerebral palsy(three females. two males; mean aged 7.0) who did not receive strength training served as control. BPM(Balance Performance Moniter) was used to measured sitting balance and EMG(electromyograph, NORAXON, USA) was used to measured activities of abdominal and erector spine muscle. The obtain result are as follows. 1. The result of this study were following that maximum perturbation area and perturbation velocity were significantly reduced strength training group compared with control group(p<.05). 2. The result of this study were following that maximum perturbation area and perturbation velocity were significantly reduced after strength training compared with pre strength training in strength training group(p<.05). 3. The result of this study were following that maximum perturbation area and perturbation velocity were not significantly reduced at interval of six week in control group(p<.05). 4. The result of this study were following that abdominal muscle activity was significantly increased strength training group compared with control group(p<.05). 5. The result of this study were following that erector spine muscle activity was significantly increased strength training group compared with control group(p<.05). 6. The result of this study were following that abdominal muscle activity and erector spine muscle were significantly increased after strength training compared with pre strength training in strength training group(p<.05). 7. The result of this study were following that abdominal muscle activity and erector spine muscle were not significantly increased at interval of six week in control group(p<.05). In conclusion, the result of this study suggest that improved sitting balance and increased activities of trunk muscle by trunk strength training in spastic cerebral palsy. The result was proposed that therapeutic approach of spastic cerebral palsy with impaired sitting balance should use trunk strength training.

The Effects of Virtual Reality Training with Upper Limb Functional Electrical Stimulation to Improve on Muscle Strength, AROM, and Function of Upper Limb Joints in Patient with Chronic Stroke (가상현실훈련과 위팔 기능적 전기자극이 만성 뇌졸중 환자의 위팔 근력, 능동관절운동과 기능에 미치는 효과)

  • Purpose : The purpose of the present study is to examine the effects of VR training with FES on improving the muscle strength, AROM, and function of the upper limb joints in patients with chronic stroke. Methods : The present study makes use of a pre-post control group design. Thirty patients with chronic stroke were randomly assigned to two groups according to treatment method - the VRFES group and the control group. The VRFES group received 15 minutes of VR training and 15 minutes of FES treatment. The control group received 15 minutes of conservative physical therapy and 15 minutes of VR training. All subjects received 30 minutes of treatment, three times a week, for eight weeks, which amounted to 24 sessions of training. The muscle strength, AROM, and function of the upper extremities were measured before the training and eight weeks after. Upper limb muscle strength was tested using the Digital Manual Muscle Tester while AROM was measured using the Digital Dual Inclinometer. The clinical assessment tools for upper extremity function included the use of the Manual Function Test and the Jebsen-Taylor Hand Function Test. Results : Both groups exhibited great improvements in muscle strength and upper extremity function during the intervention period. The VRFES group exhibited a significant difference in muscle strength, AROM, and function of the upper extremities in comparison with the control group(p<.05). Our results reveal that VRFES is more effective for the muscle strength, AROM, and function of the upper extremities in patients with chronic stroke. Conclusion : VRFES treatment will be used as an important intervention for improving the muscle strength, AROM, and function of the upper extremities in patients with chronic stroke and achieving the functional recovery of the upper extremities.

Effects of endurance and strength training of cyclists and triathlon athletes on efficiency, economy and VO2 kinetics (사이클 선수와 철인3종 선수들의 지구력 및 근력 트레이닝이 효율성 및 경제성과 VO2 kinetics에 미치는 효과)

  • The purpose of this study was to evaluate the economy and efficiency of two trained athlete groups of different training status. The subjects were 20 elite athletes composed of the cyclists group (n=10) and triathlon group (n=10). All subjects performed steady state testing at 50, 100, and 150 watts, with ample time to reach resting $VO_2$ and ventilation values between stages. The efficiency of two groups appears to be quite different, with triathlon competitors displaying superior efficiency values for the transitions from 0 to 50 and 100 watts. This same principle likely explains the economy of the groups, as triathlon competitor was again more economic at 50 and 100 watts. As though this matching of oxygen consumption and workload they can reduce the amount of oxygen deficit that must be repaid post exercise.

  • Purpose: The purpose of this study was to investigate the effect of trunk-stabilization training using stabilizing reversal and rhythmic stabilization techniques of PNF on trunk muscle strength and respiratory function in elderly stroke patients. Methods: There were 26 stroke patients included in the study. Patients were divided into two groups, and all patients performed exercise 30 min five times per week for six weeks. The experimental group performed trunk stability exercise using stabilizing reversal and rhythmic stabilization techniques of PNF, and the control group performed flexibility and strength training. Trunk muscle strength, forced vital capacity, maximum inspiratory pressure, and maximum expiration pressure were measured to determine the changes after the intervention. For statistical processing, a paired t-test was performed within the group, and the value after intervention was performed as an independent t-test to find out the difference between the two groups. Results: In the experimental group, all of the trunk muscle strength, forced vital capacity, maximum inspiratory pressure, and maximum expiration pressure showed significant differences according to the intervention. In the control group, there were statistically significant differences in trunk muscle strength and forced vital capacity, but the maximum inspiratory pressure and the maximum expiration pressure did not show any statistical change. Conclusion: From these results, it can be seen that the trunk stability exercises that use the proprioceptive neuromuscular promotion method of stable reversal and rhythm stabilization can be a good intervention for the respiratory function of stroke patients.

Long-term Effect of Robot-assisted Step Training on the Strength of the Lower Extremity and Gait Speed in a Chronic Stroke Patient: A Preliminary Study (장기간의 로봇 보조 스텝훈련이 만성 뇌졸중 환자의 하지 근력과 보행속도에 미치는 영향: 예비 연구)

  • PURPOSE: The present research examined the effects of progressive robot-assisted step training on the strength of the lower extremity and gait speed of an individual with stroke through changes between the baseline and the intervention stage (1, 3, 6, 9, and 12 months). METHODS: A single-subject (A-B) design was performed for a chronic stroke patient aged 70 years old. The robot-assisted step training was conducted three times a week during 12 months (40 minutes/session), and the assessment was conducted a total of seven times between the baseline and the intervention (No. 1, 3, 6, 9, and 12 months) to determine the effect of the intervention. RESULTS: As a result of the intervention, the muscle strength at the lower extremity of the paralysis side increased by the greatest extent 12 months after the intervention compared to the baseline, and the gait speed via the 10-meter walk test was increased as well. CONCLUSION: Long-term robot-assisted step training might be an effective intervention for improving the strength of the paretic lower extremity muscles and gait speed in stroke patients with difficulty walking independently. Further studies with sufficient sample sizes and a randomized control group will be needed to evaluate the long-term effects of robotic stepping rehabilitation.

Effects of Gluteus Medius Strengthening Training Using Sling on Ankle Instability, Muscle Strength, Balance in Adults in Their 20s with Chronic Ankle Instability (슬링을 이용한 중간볼기근 강화훈련이 만성 발목 불안정성을 가진 20대 성인의 발목 불안정성, 균형, 근력에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effect of ankle strengthening exercise combined with sling-assisted gluteus medius strengthening on ankle instability score, and static and dynamic balance ability, muscle strength in adults in their 20s with chronic ankle instability. Methods : Twenty-eight adults in their 20s with chronic ankle instability were recruited. After screening test, they were randomized through R studio program as an experimental group (n=14) to apply an ankle strengthening exercise combined with sling-assisted gluteus medius strengthening and a control group (n=14) to apply ankle strengthening exercise. The intervention lasted two times a week for 6 weeks. To compare the intervention effects, the Cumberland ankle instability tool (CAIT) score, static and dynamic balance ability, and muscle strength of lower extremities were measured. Results : The experimental group showed a significant increase in pre and post-intervention Cumberland ankle instability tool (CAIT) score, static and dynamic balance ability, and muscle strengt (p<.05). The control group showed a significant increase in pre and post-intervention CAIT score, dynamic balance ability, and muscle strength (p<.05). The experimental group showed a significant increase in CAIT score, dynamic balance ability, and muscle strength compared to the control group (p<.05), and showed a high effect size. Conclusions : The results of this study confirmed that ankle strengthening exercise combined with sling-assisted gluteus medius strengthening on people with chronic ankle instability the possibility that it could be effective in improving ankle instability and improving dynamic balance ability, and strength by movement. Although additional research is needed to increase the number of participants due to the small sample size, it is hoped that this study will be an optimistic clinical protocol for people with chronic ankle instability.
