Change of Refractive Error after Watching Smart-phone under Low Intensity of Illumination (낮은 조도에서 스마트폰 시청 후 시력 변화)

  • Purpose: To compare the change of visual acuity and NIBUT after watching smart-phone in 1 hour under low intensity of illumination. Methods: 50 subjects (male 22, female 28) aged 20's years old ($20.7{\pm}2.4$ years) who do not have eye disease and have a good eye condition were participated for this study. Objective refraction, corrected distance visual acuity and NIBUT were measured before and after watching smart-phone (Galaxy 2, Samsung, KOREA) under low intensity of illumination (0 lx.) Objective refraction was carried out using auto-chart project (CP-1000, Dongyang, Korea), phoropter (VT-20, Dongyang, Korea) and auto refractor-keratometer (MRK-3100, Huvitz, Korea). Results: Refractive error was changed from $-3.20{\pm}2.00$ D to $-3.38{\pm}2.00$ D (p=0.006) and corrected distance visual acuity was changed from $0.93{\pm}0.08$ to $0.91{\pm}0.10$ (p=0.000) and NIBUT was changed from $10.48{\pm}7.00$ seconds to $10.29{\pm}6.47$ seconds (p=0.761) before and after watching smart-phone under low intensity of illumination. Conclusions: Continuous watching smart-phone under low intensity of illumination lead to temporal change of distance visual acuity and suitable rest may reduce the influence of distance visual acuity and tear safety.

Individual Difference in the Decrease of Visual Acuity and the Change in Contrast Threshold According to the Level of Optically Induced Retinal Defocus (광학적으로 유발된 망막흐림의 정도에 따른 시력감소의 개인차와 인식 대비도의 변화)

  • Purpose: We were aim to investigate individual difference of visual acuity (VA) decrease and the change of contrast threshold (CT) according to the level of optically induced retinal defocus. Methods: A total of 69 eyes were examined using consist of ten-graded decimal vision chart (Landolt's ring). After conducted full correction of subject's refractive error, a monocular VA and CT were measured according to sequential increase by 0.25 D each time. Results: VA gradually decreased according to the increase of retinal defocus level. Individual difference of VA decrease was range from 1.2 to 0.6 in retinal defocus induced by +0.25 D. When retinal defocus was induced as much as +0.50 D and +0.75 D, it was in the range of 1.0 to 0.3 and 0.9 to 0.1 respectively. With +1.00 D, some participants didn't even recognize the 0.1 in the chart. With +1.75 D, whole participant did not recognize the 0.1. Also, CT was gradually decreased with increase of the retinal defocus level. Conclusions: Examiners should consider individual difference in the decrease of VA according to the level of residual refractive error when determining final prescription of a patient.

The Study on the Property of The Heterophoria and Fusional Reserve in Adults in Jeon-buk Area (전북지역 성인의 사위와 융합여력 특성에 관한 연구)

  • The aim of this study was to evaluate the property of heterophoria and fusional reserve in Adults in Jeon-buk Area. We examined the corrected visual acuity, corrected refractive error, heterophoria and fusional reserve of 116 healthy myopes aged from 20 to 44 old who had no strabismus no ocular and phyisical diseases. Using Von Graefe test of horizontal heterophoria Measurement, we measured orthophoria(26.7%), exophoria(52.5%) and esophoria(20.7%) for at near distance. The subjects who had exophoria of 0-6${\Delta}$ in the range of normal state was 38.8%, while the subjects who had exophoria in the range of abnormal state was 61.2%. Reducing fusional reserve was associated with increasing phoria. We found a relationship between asthenopia and fusional reserve of heterophoria and considered that fusional reserve must be examined when we preserve for a patient with heterophoria. Furthermore, Gradient method AC/A ratio was found 4.03 and its relationship to refractive error could not be determined.

A Study on the Accommodative Astigmatism of Near Vision (근거리 주시 시 조절성 난시에 대한 연구)

  • Purpose: The purpose of this study was an accurate grasp of the astigmatic change due to eyes accommodation on near vision and suggested problems occurring accommodative astigmatism using near glasses. Methods: 154 subjects(308eyes) from 20 to 40 years selected for this study who had many opportunity of near vision. First, far vision corrective refraction measured using the phoropter, and then both eyes opened simultaneously for maintaining the function of binocular put away dot chart 40 cm. One eye was fogging and the other eye measured near vision astigmatism axis and degrees astigmatism using cross cylinder, and then compared with far astigmatism. Results: Increased degree of astigmatism persons on near vision more than decreased or did not changed degree of astigmatism persons, which could affect visual acuity more than 0.75 diopters in 30 eyes with an increase of 9.7% of total subjects. Direct astigmatism and oblique astigmatism were increased on near vision. Astigmatic axes were rotated base in direction on both eyes and 66.9% of subjects were more than ${\pm}$5$^{\circ}$ rotation. Due to the rotation axis of astigmatism, residual astigmatism is expected to occur and expect to adversely affect the eyes. Conclusions: Long-term using the glasses without correction of increased astigmatic and rotated axis on near vision should occur new residual astigmatism and increase the asthenopia also. Considered to be taken astigmatic change on near vision during near refraction examination.

A Study on the Relationship Between the Off-Axis Cylinder and Corrected Vision of Astigmatism (난시안의 교정축 이탈과 교정시력과의 관계 연구)

  • This study was classified and compared astigmatism's refractional abnormal degrees with visual acuity state of full correction which turned on axises of only 5 degree, 10 degree, and 15 degree. Subjects of this study were 57 college students (114 eyes) who had neither eye diseases nor binocular abnormality, were from their twenties to fifties, with myopia. It appeared that 30.8% of subjects who had astigmatism wore glasses with wrong axis of astigmatism. After accurate correction of the visual acuity and degrees of astigmatism, when we moved to corrected axises at 5 degree, 10 degree, 15 degree, failure of visual acuity with one line or more were 56.1%, 84.2%, 93.8%, respectively. When we comapre the completely-corrected visual acuity with the visual acuity with dricted axes, the bigger the width of visual acuity's weakness was the bigger the drifted angle. The change of normal visual acuity according to drifting angle of corrected axises of astigmatism, when we compared with full correction, appeared 0.94 in 5 degree, 0.87 in 10 degree, and 0.79 in 15 degree. Drift of 5 degree from fully corrected axis, corresponded to difference of visual acuity about one line, drift of 10 degree to 1.8 line difference of visual acuity, and drift of 15 degree about to 2.6 line difference. Through this study, we were sure that, in the case of drifting away from the right axis of astigmatism, it lead to visual weakness and asthenopia. Therefore we darely advise that optometrists should make mistake of axis least by confirming accucacy of corrected axis after despensing of spectacles of astigmatism.

Development of the Phoropter Simulator for Education using 3D Virtual Reality (3차원 가상현실을 이용한 교육용 포롭터 시뮬레이터의 개발)

  • In this study, we presented a research about the development of the phoropter simulation program for education using the 3-D virtual reality in order to study the ophthalmic optics rather efficiently. Especially, by putting the facility such as the refractive error analysis of the eyeball in the cyber tutor inside, we have made the learner to learn and confirm the present situation of refractive correction through refractive power of principal meridians. By organizing the system which induces the active involvement of the learner and showing the result of the execution to the learner intuitively, the implementation of the more efficient education-environment can be possible. Consequently, it is expected that this program will be greatly helpful for the optician training as well as the ophthalmic optics education. As a next phase of study, we will develope the development technique of this simulator more and extend the cyber tutor contents more and make the web service version of this program to be provided through the internet network in order to inspire the learning desire of the learner more and more.

Availability of Astigmatism Expectation by Jin's and Beam Project Chart (진용한 시력표와 투영식 시력표에서 난시량 예측의 용이성)

  • Purpose: The purpose of this study was to predict the amount of astigmatism through logMAR visual acuity by Jin's chart at best vision spherical power and to compare availability of astigmatism expectation by Jin's and beam project chart. Methods: LogMAR and decimal visual acuity were measured for 150 college students and visual acuity and compared the amount of astigmatism under full correction. Results: Jin's chart was showed marked differences at least more than 0.25 D intervals per line than beam project chart. Correlation with the amount of astigmatism was higher the logMAR visual acuity r = 0.8578 than decimal visual acuity r = -0.7199. Conclusions: LogMAR visual acuity at best vision spherical power was able to predict to amount of astigmatism and Jin's chart was easier than beam project chart to predict difference of each lines.

Scotopic Pupil Size in Myopes (근시안에서 암순응상태의 동공크기)

  • Purpose: This research was performed to measure and analyze scotopic pupil size in myopes and to figure out the factors that influence it. Methods: The pupil size of 191 healthy myopic subjects were measured with the pupillometer (Colvard pupillometer, OASIS medical, USA) in scotopic and analyzed with the age, corneal size, spherical equivalent refractive error, corneal curvature. In addition, it was compared with the measurements of intra-examiner and inter-examiner to verify reproducibility of pupillometer. Results: The mean (${\pm}$SD) scotopic pupil size was $6.64{\pm}0.68$ mm (range, 5.00~8.00 mm), the lower age and the larger corneal size, The bigger the pupil size. The lower spherical equivalent refractive error and steepper corneal curvature tends to be smaller. The reproducibility of intra-examiner and inter-examiner in pupillometer showed the reliability highly (Guttman splithalf point > 0.91). Conclusions: The pupil size associated with age, corneal size, spherical equivalent refractive error and corneal curvature in scotopic condition. It can refer to prevent inconvenience that may occur RGP contact lenses, cataract surgery and refractive surgery.

Correlation of Near Stereoacuity and Phoria, and Refractive Error (근거리 입체시와 사위 및 굴절이상의 상관관계)

  • Purpose: This study was investigated the relationship between the male and female of near stereoacuity and phoria, and was examined the correlations between near stereoacuity and phoria and refractive error. Methods: Adult 83 (male 36, female 47, mean age $21.09{\pm}1.74$(19~29) years old) target was examined after full correction of refractive error. Near stereoacuity was measured by using Titmus-fly Stereotest(Stereo Optical Co., Inc., Chicago, IL, USA), and RANDOT Stereotest (Stereo Optical Co., Inc., Chicago, IL, USA), phoria was measured by using modified torrington method, at distance 40 cm. Results: In near stereoacuity, male was better good than female, and there was no difference between male and female (p>0.05). In near phoria, female was more higher than male, and there was a significant difference between male and female (p<0.05). Near stereoacuity by Titmus-fly test and Randot test result respectively was $262.17{\pm}562.43$ sec (second of arc) $243.08{\pm}68.04$ sec in esophoria, $148.42{\pm}269.54$ sec, $107.40{\pm}263.74$ sec in orthophoria, $113.94{\pm}152.46$ sec, $79.70{\pm}136.83$ sec in exophoria, there was a no difference between three phoria groups (p>0.05), and was a high correlation between phoria and near stereoacity(r=0.68). In addition, near stereoacuity in the refractive error respectively was $80.00{\pm}571.43$ sec, $68.75{\pm}36.82$ sec in myopia, $133.57{\pm}224.15$ sec, $93.14{\pm}214.95$ sec in hyperopia, $511.20{\pm}855.00$ sec, $511.4{\pm}855.60$ sec, there was a significant difference between three phoria groups (p<0.05). when was classfiied near stersoacuity into degree of refractive error, near stersoacuity was best in emmetropia, and was reduced when refractive error was highest in high degree myopia, and hyperopia. Conclusions: Titmus-fly Stereotest and RANDOT Stereotest result, near stereoacuity of adults, when esophoria and high degree refractive error, was reduced, there was a relationship between near stereoacuity with phoria, and refractive error. In this both stereotests if was bad when near stereoacuity can expect a phoria and refractive error.

The Investigation and Development of Astigmatism Correction Treatments by Finite Element Method and Animal Experiments (유한요소법과 동물실험을 통한 난시교정술의 고찰 및 개발)

  • The purpose of this study is to investigate the effects of various factors in keratotomy for astigmatism correction on surgical outcomes by finite element method as well as animal experiments. Three kinds of surgical techniques were mechanically investigated : arcuate, straight, and inverse arcuate keratotomy. Among the three techniques the arcuate keratotomy is the most popular one while the other two techniques are being investigated in this area. The arcuate keratotomy was found to be more controllable and effective in reducing the refractive power than the others. In arcuate keratotomy it was found most effective when the incision was located in the middle position between the apex and the edge of the cornea from the results of experiment as well as finite element study. Regarding to the range of the corneal incision in arcuate keratotomy, the incision angle of 90$^{\circ}$ was found th be most effective in reducing refractive power than other angles even it was incised up to 150$^{\circ}$. Therefore, it was concluded that 90$^{\circ}$ of incision angle results in the largest decrease in refractive power in arcuate keratotomy. However, other important findings were that the effect of the surgery decreased with time so the visco-effect of the cornea and auto-healing process. Therefore, these factors should be considered in future studies.
