Treatment of Snapping Hip Caused by a Tight lliotibial Tract (장경대 구축에 의한 탄발성 고관절의 치료)

  • Purpose: The purpose of this study was to analyze the treatment results of snapping hip caused by a tight iliotibial tract using Z-plasty . Materials 8nd Methods: Five patients (seven hips) were treated by Z-plasty using Brignall and Stainsby's method from March, 1995 to May,2002 There were three males and two females and their average age at operation was 24.4 years. All patients complained of snapping, pain, and difficulty of squatting with attaching both knees . The average of follow-up was one year and four months. The local anesthesia was done in three patients (3 hips) and epidural anesthesia in two patients (4 hips). Results: Snapping was absent, squatting with attaching both knees was possible, and pain relief was excellent in all patients. One patient (one hip) experienced occasional aching associated with exercise but it is not serious. Conclusion: This Z-plasty of iliotibial band was an excellent surgical technique for the symptomatic snapping hip.

Study for Inferior Alveolar and Lingual Nerve Damages Associated with Dental Local Anesthesia (치과 국소마취와 관련된 하치조신경과 설신경 손상에 대한 연구)

  • Background: Damages of trigeminal nerve, particularly inferior alveolar nerve and lingual nerve, could occur following dental procedures. In some cases, nerve damage may happen as a complication of the local anesthetic injection itself and not of the surgical procedure. Methods: From September 2006 to August 2010, 5 cases of inferior alveolar nerve and lingual nerve damages, which were assumed to happen solely due to local anesthesia, were reviewed. All cases were referred to Division of Oral and Maxillofacial Surgery, Department of Dentistry, Hanyang University Medical Center for legal authentication in the process of criminal procedure. Results: In all five cases, patients complained of altered sensation occurred in the distribution of the inferior alveolar or lingual nerve following block anesthesia. The local anesthetics were 2% lidocaine with 1 : 100,000 epinephrine and the amount of local anesthetics, which were used during injection, were varied. Most of patients experienced the electric stimulation during injection. Recovery was poor and professional supportive care was mostly absent. Conclusions: Dental practitioners should consider that the surgical procedure caused the trigeminal nerve damage, however, dental local anesthesia for inferior alveolar nerve and lingual nerve could be one of the causes for damages. The various mechanisms for nerve damages by local anesthesia are thoroughly discussed.

Comparative Study for the Anesthetic Efficacy between Articaine HCl and Lidocaine HCl during the Surgical Extraction of Bilateral Mandibular Impacted Third Molars (하악 매복 제3대구치 발치 시 Articaine HCl과 Lidocaine HCl의 국소마취 효능에 대한 비교연구)

  • Background: Articaine, a new amide-type local anesthetic, was recently commercially available in Korea. The purpose of this study was to compare the anesthetic efficacy between articaine HCl and lidocaine HCl for the surgical extraction of bilateral mandibular impacted third molars. Patients and Methods: Forty young and healthy patients with bilateral impacted third molars were selected with permission. Randomly, one side of impacted third molar was operated under local anesthesia using 4% articaine and the other side under 2% lidocaine after 1 or 2 week recovery time. Intraoperative pain was evaluated via 0-10 cm visual analog scale (VAS) by both the patient and operator immediately after the operation. After 1 day and 7 days, the complications of local anesthesia were checked. Results: The pain VAS scores in articaine group, evaluated by both the patient and operator, were lower than those in lidocaine group, but they were not statistically significant (P = 0.44 and 0.54, respectively). The incidence of complications of local anesthesia between articaine and lidocaine was similar. Conclusion: In this study, articaine showed at least equal efficacy compared to lidocaine. The further dose-controlled study with more pharmacokinetic parameters will be needed.

Comparison of pain perception using computer-controlled anesthetic device and aspirating syringe (컴퓨터 제어 마취시스템과 통상적 마취방법을 이용한 국소 마취시 동통 비교)

  • Dental phobia is the most prevalent fear in all age groups, across gender, and in all countries. One of the primary identified sources is the fear of dental injections in the dental phobia or the high dental fear and anxiety groups. The purpose of this study was to clinically evaluate the computer controlled anesthetic device and to compare it with traditional methods of dental anesthetic delivery. Fifty(mean age : 25.6 yrs) systemically and periodontally healthy volunteers participated in this study. The subjects were given contralateral buccal and palatal injections. One side was injected with the computer-controlled anesthetic device with a microprocessor and an electric motor to precisely regulate flow rate during administration : The experimental group. The control side was injected with a standard manual syringe, in which flow rate and pressure are operator-dependent and can't be controlled accurately : The control group. The subjects described their perceived pain experiences with two subjective scales. The results of this study were as follows: 1. The computer-controlled anesthetic device was significantly less painful than conventional syringe injection 2. The female subjects reported more pain than the male subjects. But, there were no statistical differences. 3. The anesthetic effect of both methods did not show any difference. In this study, it may be concluded that pain levels decreased significantly when the computer-controlled anesthetic device was used.

Stress Reduction Protocol for Proper Local Anesthesia of Advanced Infected Teeth in Medically Compromised Patients -Review of Literature & Report of Cases- (전신질환자에서 과도한 감염치아부 국소마취시 스트레스 감소법 : 문헌적 고찰 및 증례보고)

  • Common dental procedures (local anesthesia and dental treatment) are potentially stress-inducing in many patients, especially medically compromised patients. The body response to dental stress involves the cardiovascular system (an increase in cardiovascular workload), the respiratory organ and the endocrine system (change in metabolism). To minimize the stress to the medical risk patient, the stress reduction protocol was established. The obtained contents were as follows: (1) Recognize the patient's degree of medical risk (2) Complete medical consultation before dental therapy (3) Schedule the patient's appointment in the morning (4) Monitor and record preoperative, perioperative and postoperative vital signs (5) Use psychosedation during therapy (6) Use adequate pain control during therapy (7) Short length of appointment: do not exceed the patient's limits of tolerance (8) Follow up with postoperative pain/anxiety control (9) Telephone the higher medical risk patient later on the same day that treatment was given. This protocol is predicated on the belief that the prevention of or reduction of stress ought to begin before the start of an appointment, continue throughout treatment, and, if indicated, into the postoperative period. The authors used the stress reduction protocol in the care of local anesthesia infected teeth in medically compromised patients. The final prognosis was comfortable without any complications.

Computer Controlled Local Anesthesia Delivery: Literature Review (컴퓨터를 이용한 속도 조절형 치과 국소마취 기구: 문헌고찰)

  • Background: Today, there are computer controlled local anesthesia devices used clinically. The main principle is to control injection speed by computer aided system, and it relieves pain. However, there are few objective data considering this subject. In this literature review, we researched studies about computer controlled anesthetic delivery. We compared pain control effect of computer controlled to conventional local anesthetic syringe system. Methods: A bibliographic search in PubMed was performed and we reviewed original articles. Results: There were 18 publications that compared pain control effect of computer controlled to conventional system. There were 8 articles reported of children, similar pain control effect was found in 7 of them. One study showed superior pain control effect of computer controlled anesthetic delivery. For adults, 10 studies showed superior pain control effect in computer aided system. Conclusions: Computer controlled anesthetic delivery has similar or superior pain control effect compared to conventional local anesthetic syringe system. For both children and adults, computer controlled anesthetic delivery could be clinically useful, still it may be more effective for adults.

Effect of Soumin Seonghyangjeongkisan Extract on Blood Pressure and Regional Cerebral Blood Flow in Rats (소음인(少陰人) 성향정기산(星香正氣散)이 백서(白鼠)의 혈압(血壓) 및 국소뇌혈류량(局所腦血流量)에 미치는 영향(影響))

  • 1. Purpose : For many years, Soumin Seonghyangjeongkisan(少陰人 星香正氣散, SJ) has been used in Sasang(四象) constitutional medicine as a therapeutic agent for cerebral disease. The effects of SJ on the vascular system is not well-know. The purpose of this study is to make it clear the effects of SJ and composing drugs on blood pressure(BP) and regional cerebral blood flow(rCBF). 2. Method : We measured BP with Pressure Transducer, rCBF with Laser-Doppler flowmeter. Propranolo and methylene blue were used to determine the mechanisms of SJ effects. 3. Results and Conclusion : BP was not affected by SJ in rats, and rCBF was significantly increased by SJ in rats. rCBF was increased by Folium Perillae(蘇葉), Rhizoma Atractylodis(蒼朮), Pericarpium Arecae(大腹皮) and Rhizoma Arisaematic(南星), but decreased by Radix Saussurea(木香) in rats. The increase of rCBF is mediated by adrenergic ${\beta}-receptor$ and guanylyl cyclase which is enzyme producing cyclic GMP.

  • Stomach cancer is the most popular tumor in Korea but the prognosis following extensive surgery and chemotherapy has not improved for many years. Conventional external radiotherapy also has some limitation, namely, a cancerocidal dose can not be delivered to tumors because of low radiation tolerance of adjacent critical sensitive organs. In order to overcome these limitations of curative surgery and external radiotherapy, intraoperative radiotherapy was proposed in many centers and the results were excellent. We treated two cases of locally advanced gastric cancers with IORT by using NEC 18 MeV Linear Accelerator after standard subtotal resection of the stomach. After treatment, the patients are in good conditions so far, but in order to evaluate the effectiveness of IORT, a study will be performed on the survival rates between patients treated by IORT and those treated by surgery alone.

Influence of Acupuncture Stimulation on Electrical Resistance under Adiaphoresis Induced by Regional Anaesthesia (국소마취에 의한 발한 억제 상태에서 침자극이 인체 전기 저항에 미치는 영향)

  • Objectives : When sweating factor is excluded, to know the influence of acupuncture stimulation on electrical resistance. Methods : Regional anaesthesia(Lidocane 2%, 1ml, Subcutaneous injection) is applied to measured regions, 4cm round area including left Neiguan P6. So we can make adiaphoresis. Before and after acupucnture stimulation to Laogong P8, we measured the electrical resistance of anesthetized left Neiguan P6, anesthetized non-acupoint, normal left Ximen P4, and normal right Neiguan P6. For measurment, we use the 1.2V, DC system that has 100$k{\Omega}$ internal resistance Results : Under the adiaphoresis, Acupuncture stimulation makes the significant decrease of electrical resistance. In normal regions, so does it. Conclusions : When the acupuncture stimulation is applied to human boby, the decrease of electrical resistance is due not only to sweat secretion but also to other internal resistance.
