A case of unexpected adjacent tooth extrusion after implant fixed prosthetic treatment, who had undergone mandibular resection and reconstruction due to ameloblastoma (법랑모세포종으로 하악골 절제 및 재건술 시행한 환자에서 임플란트 고정성 보철물 수복 후 원인 미상의 인접 치아 정출이 발생한 증례 및 고찰)

  • Ameloblastoma is a benign odontogenic epithelial tumor with high recurrence rate and requires extensive resection of the surrounding tissue and reconstruction of defect site. Because of the anatomical limitation of the reconstruction site, prosthetic treatment with implants is the first recommendation. This is a case of prosthetic restoration of the reconstruction site with implant fixed prosthesis in patient who underwent mandibular resection and iliac bone reconstruction due to ameloblastoma. However 14 months after completion of implant prosthesis, adjacent natural tooth erupted unexpectedly, resulting in 1mm infra-occlusion occurred including posterior implant prosthesis and anterior natural teeth. In adults, implant infra-occlusion may occur due to residual growth after placement of the maxillary anterior implant. But this case, hypo-occlusion of molar implant and open bite of anterior natural teeth is occurred due to extrusion of adjacent tooth, is rare. Thus we report the treatment process including orthodontic treatment with intrusion of the posterior tooth, and investigate the causes of sudden, unexpected tooth extrusion.

  • A conventional approach for the treatment of long-span edentulous areas is the use of removable dentures. However, placing implants in these areas results in superior functional outcomes by increasing the stability, support, and resistance of the prostheses and improving the masticatory efficiency. Treatment modalities utilizing implants can be further classified into either removable or fixed-type prostheses. Several factors such as the amount of alveolar bone resorption, inter-arch relationship, patient preferences, and socioeconomic status should be considered when determining the appropriate treatment approach. Monolithic zirconia has been considered a suitable material for implant-supported fixed dental prosthesis, because of the drastic improvement in its mechanical properties. It exhibits fewer incidences of fracture and chipping of the prostheses, and has greater bulk of material than metal-ceramic crowns and zirconia-veneered ceramics. Moreover, highly translucent monolithic zirconia is also available in the market, and its application is gradually increasing for anterior tooth rehabilitation. The present report describes a patient who underwent full-mouth rehabilitation with fixed dental prostheses (eight upper and three lower implant placements). All teeth, except bilateral mandibular canines and left mandibular first and second premolars, were extracted after the diagnosis of generalized chronic moderate-to-advanced periodontitis of the remaining teeth. The patient reported satisfactory esthetic and functional outcomes during the one-year follow-up visit.

Full mouth rehabilitation of edentulous patient with fixed implant prosthesis (고정성 임플란트 보철물을 이용한 완전 무치악 환자의 구강회복 증례)

  • There are various treatment options such as conventional complete denture, hybrid prosthesis and implant-supported fixed prosthesis for fully edentulous patients. In case of implant-supported fixed prosthesis, compared to removable prosthesis, it is difficult to place the implant in the correct position considering the anatomical contours of the final prosthesis. In this case, a full mouth rehabilitation with implant-supported fixed prosthesis was performed for a patient who required extraction of all remaining teeth due to dental caries and chronic periodontitis. In the implant placement stage, the implant was placed in the desired position using a surgical guide fabricated considering the anatomical contours of the final prosthesis, and the function and esthetics were evaluated through correction and re-fabrication of the fixed provisional restoration. A final restoration of porcelain fused to gold prosthesis was delivered to the patient based on the provisional restoration. To cope with complications such as loosening of screws and fracture of porcelain, a screw-retained type prosthesis was fabricated for the posterior part and a screw-cement-retained type prosthesis for the anterior part. As a result, the patient showed an improved prognosis in terms of functional and esthetics after the final prosthesis was delivered.

  • Women have been entrusted with responsibility for pregnancy, childbirth, and nurturing by means of socially imposed 'maternity' along with their physical characteristics. Abortion too involves the bodies of women, and women are the ones most affected by it. However, women do not yet have the right of self-determination over their bodies. In the "pro-choice versus pro-life" abortion debate in South Korea, women's self-determination is often treated with less significance than the alleged "respect for life." Moreover, as Korea's declining fertility rate has become a serious social problem, women's perspectives on the issue of abortion have been sidelined. Yet even in this context, there is a double standard between the treatment of disabled and able-bodied women. The purpose of this study is to examine the issue of self-determination, especially for women with disabilities, from the perspective of a capability approach. The following three recommendations are proposed: (a) that the various contextual variables of disabled women are included in the concept of self-determination; (b) that a solid relationship between individuals and communities is established in order to ensure the realization of the right of self-determination for disabled women; and (c) that the discourse of "reproduction rights" (i.e. comprehensive rights of women with disabilities) be expanded.

Use of digital scan data for evaluation of edentulous ridge relationship: A case report for removable prosthesis with unilateral cross bite (디지털 스캔 데이터를 활용한 무치악 치조제 관계의 평가와 인공치 선택: 편측 교차교합의 가철성 의치 수복 증례)

  • After the teeth were extracted, maxillary and mandibular alveolar ridges show the opposite resorption pattern and as a result, the mandibular arch is enlarged than maxillary arch relatively. In this situation, we should evaluate both alveolar ridge relationship and arrange the artificial teeth properly for stability of removable prosthesis. This case is a 77 years old male patient who wishes to make removable prosthesis and has atrophic alveolar ridge. By use of model scanner and CAD software, the angle between interalveolar crest line and occlusal plane was easily measured. Depending on the measurement, the artificial teeth are arranged in unilateral cross bite and after completion, patient was satisfied with the denture which showed proper stability, retention, support.

  • There are several unfavorable conditions regarding alveolar bone condition that may compromise the denture patient's satisfaction. Chewing efficiency may not be satisfactory when alveolar bone is deficient, and the denture stability could hardly be achieved when alveolar bone shape is irregular. Implant overdenture can be useful to provide satisfactory denture experience compared to conventional denture. The attachment for implant overdenture can be classified into bar attachment and solitary attachment. When the positions of the implants are in the mandibular anterior region, bar attachment may be favorable to obtain a rigid support of the entire denture. When implants are distributed both on anterior and posterior region, a solitary attachment could be considered for ease of removal and maintenance. This report presents implant overdenture cases with the patients that had unilateral mandibular alveolar bone atrophy conditions. Different abutments were chosen based on the individual patient's mandibular alveolar bone condition and the treatments were successful in terms of patient satisfaction.

Records of the Prosecutor's Office at Gyeongseong District Court(京城地方法院檢事局) and Set up the Department of Ideology(思想部) (경성지방법원 검사국 기록과 '사상부(思想部)'의 설치)

  • Three frequently used documents by researchers on the Japanese colonial period-Classified Police Records of the Japanese Government-General of korea on the Anti-Japanese Movement 舊 朝鮮總督府 警務局 抗日獨立運 動關係 秘密記錄, Trial Records of Gyeongseong District Court 京城地方法院 裁判記錄 and Filed Documents of Gyeongseong District Court 京城地方法院 編綴文書(while the latter two were collected by the National Institute of Korean History, the former was collected by Asiatic Research Institute of Korea University)-are among the same records group that the Prosecutor's Office of Gyeongseong District Court produced through registration and compilation. As contents related to ideology suppression and thought control were numerous, it could be assumed that most of the materials were produced by the 'Department of Ideology' of the Prosecutor's Office. By examining the administrative records of the 1920's within this group, the process of how the 'Department of Ideology' was formed is clearly shown. As ideological movements for Korea's independence and revolution grew, execution of regulations was further expanded by the Prosecutor's Office. Since the mid-1920's, information on ideology had been separated from general information, was routinely collected heavily, and was considered more valuable than the general information. By the end of the 1920's, the term Ideology Prosecutor 思想係 檢事(meaning a prosecutor who specializes in ideology verification) and the named 'Department of Ideology'(思想部) emerged on the records.

Prosthetic treatment for Down's syndrome patient with dental cross bite problem using maxillary double crown denture (교차교합을 가진 다운증후군 환자의 상악 이중관 의치를 이용한 수복 증례)

  • Patients with Down's syndrome have several dental complications such as small teeth caused by underdevelopment of dentin and enamel, periodontitis, agenesis of teeth, prolonged retention of primary teeth and malocclusion due to narrow palate. Removable denture with maxillary double crowns would be a good treatment option to solve the problems of the patient with Down's syndrome. Double crowns compensate the insufficient support and retention of denture and easily solve the cross bite problem. Double crowns also allow easy repair of denture in case of abutment teeth extraction. In this case, 26-year-old female patient with Down's syndrome and dental phobia had small number of teeth with enamel hypoplasia, prolonged retention of primary teeth and dental cross bite. Prosthetic treatment was done using removable denture with double crowns in the maxilla. In the mandible, teeth preparation was done on enamel margin without anesthesia. Anterior laminate and posterior complete zirconia crown restorations were performed. As a result, the cross bite was effectively corrected by denture with double crowns. Pronunciation and appearance were also improved without extraction of teeth and dental anesthesia.

Effect of image matching experience on the accuracy and working time for 3D image registration between radiographic and optical scan images (술자의 영상정합의 경험이 컴퓨터 단층촬영과 광학스캔 영상 간의 정합 정확성과 작업시간에 미치는 영향)

  • Purpose. The purpose of the present study was to investigate the effects of image matching experience of operators on the accuracy and working time of image registration between radiographic and optical scan images. Materials and methods. Computed tomography and optical scan of a dentate dental arch were obtained. Image matching between the computed tomography and the optical scan (IDC S1, Amann Girrbach, Koblah, Austria) was performed using the point-based automatic registration method in planning software programs (Implant Studio, 3Shape, Copenhagen, Denmark) using two different experience conditions on image registration: experienced group and inexperienced group (n = 15 per group, N = 30). The accuracy of image registration in each group was evaluated by measuring linear discrepancies between matched images, and working time was recorded. Independent t test was used to statistically analyze the result data (α = .05). Results. In the linear deviation, no statistically significant difference was found between the experienced and inexperienced groups. Meanwhile, the working time for image registration was significantly shorter in the experienced group than in the inexperienced group (P = .007). Conclusion. Difference in the image matching experience may not influence the accuracy of image registration of optical scan to computed tomography when the point-based automatic registration was used, but affect the working time for the image registration.

Comparison of study affecting the use of dental sealant in consequence of its inclusion in the National Health Insurance coverage: Using data from the 5th-7th National Health and Nutrition Examination Survey(KNHANES) (치면열구전색 급여화에 따른 치면열구전색 수혜 비교 연구: 국민건강영양조사 제5기~7기 자료를 활용하여)

  • Dental sealant is a procedure to prevent dental caries on the occlusal surface of healthy teeth. Raw data from the 6th-7th Korean National Health and Nutritional Examination Survey (KNHANES) were analyzed. This study was conducted to provide basic data which is necessary for the national health care project and for expanding the standards of coverage for the Dental sealant procedure. In this study, subjects were a total of 4,366 children from the age of 6 to 18 and surveyed for the Amount of Received Dental sealant and rate of Received Dental sealant. In 2012, the coverage rate for pit and fissure sealing procedures differed significantly in terms of age, residential area and average monthly income. This coverage varied as per average monthly income in 2014 and as per age and average monthly income in 2017 (P < 0.05). In 2014, average monthly income had a significant influence on the coverage rate, while age and average monthly income had a significant influence on it in 2017 (P < 0.05). These findings suggest that in order to increase the sealing procedure coverage, continuous promotion of dental sealant projects, reduction of copayments, inclusion of varied tooth types and age groups, expansion of coverage to isolated areas and inclusion of socially disadvantaged groups are necessary.
