BONE GRAFT PROCEDURE WITH ENDOSSEOUS IMPLANTS : A REVIEW OF THE LITERATURE (골유착성 임프란트와 관련된 골 이식술에 대한 문헌 고찰)

  • Recent development of dental implantology has taken an interest in the bone graft procedure. This is a review of literature, published from 1994 to November 1999. This study is provided by MEDLINE search. In this study, 718 patients received 829 graft with placing 2,677 endosseous implants. In mandible, nonvascularized or vascularized block bone grafts provided better results(success rate 95.2%) than particulate grafts(83.6%). But in maxilla, particulate grafts provided better results(93.7%, 86.2%) and more cases especially in sinus elevation. There were many cases using autogenous bone graft and revealed good results, but allogenic or alloplastic bone graft materials also were used by many surgeons.

  • The purpose of this study was to investigate the characteristics of mesiodens among children in Korea. The study population consisted of 20 children whose ages ranged from 4 years to 26 years. The characteristics of mesiodens were obtained from radiographs and clinical examination results, and were confirmed by surgical intervention. Results showed that males were affected four times as frequently as female. Thirty-five percent of the patients had more than one supernumerary tooth. About 67% of the mesiodens were conical in shape and about 34% of the mesiodens were in the upward position. The age and sex distribution, number of mesiodens per patient, shape, direction, size, and effect on permanent upper incisors are also presented in this study.

NORMAL TEMPOROMANDIBULAR JOINT SPACE (정상인의 악관절 간극)

  • Disc displacement, deformation, perforation or osteoarthritis may induce the joint space narrowing. This study was designed to get the normal joint space width which could be used as a standard comparing with damaged joints. Twenty asymptomatic volunteers' MR images were evaluated and coronal images of thirty joints having normal disc position in sagittal and coronal views, were selected. The joint spaces were measured at center, medial and lateral side on the coronal views. In normal joints, the mean joint space was $3.57{\pm}0.63mm$ at center, $2.77{\pm}0.51mm$ at medial and $2.41{\pm}0.41mm$ at lateral side of mandibular condyle. The mean of joint space of male was wider than that of female at center, but not significantly different at medial and lateral side of the condyle.

  • Human alveolar bone cells were isolated from alveolar bone fragments obtained from normal individual undergoing third molar extractions. Alveolar bone fragments were cultured as explant. Cells began to migrate in the first $5{\sim}7$ day and were confluent in $5{\sim}7$ week. Matrix mineralization was observed by 4 week. Our studies utilize established protocols for the characterization of these cells as osteoblasts by means of alkaline phosphatase activity determination, identification of osteocalcin antigens, establishing the presence of cells expressing type I collagen and determining the ability of cells to produce calcification. Transmission electron microscopic observations confirmed the presence of a collagen matrix undergoing a mineralization process. This new model, using human alveolar bone cells, may provide a tool to investigate alveolar bone development and physiology and to set up new therapeutic approaches.

  • Orbital injuries are common with facial trauma. Direct injuries to the globe are not rare but it can result in complications such as chemosis, subconjunctival hemorrhage and hyphema. Periorbital trauma or injuries to the extraocular muscles and blow-out fracture may result in lid edema, ecchymosis & ptosis and diplopia or limitation of ocular motion respectively. Indirect injuries to the optic nerve come up without any injuries but its complication is irreversible and severe such as loss of vision. The aim of this study is to review the literature on blindness or ptosis following facial trauma and present the cases of blindness after facial trauma and ptosis after mandibular fracture without specific clinical findings.

MYIASIS IN A SUBMANDIBULAR METASTATIC SQUAMOUS CELL CARCINOMA ; A CASE REPORT (악하부에 전이된 편평상피암종에 발생한 승저증의 치험례)

  • We report a case of myiasis on the left submandibular area after radiation therapy for squamous cell carcinoma. A 77-year-old man visited our hospital, 7 weeks after complicating a course of radiation therapy, had a 7cm diameter necrotic ulcer on the left submandible. The margin of the lesion were undermined by numerous interlinking passages containing 20 maggots. The maggots were removed by simple extraction and were identified as Calliphoridae Lucilia. The probable origin of the myiasis in this case was attributed to poor environmental condition and general weakness of the patient. Oppotunistic cutaneous myiasis can be prevented by protecting open wounds from flies, wound should not be left without a covering capable of excluding flies, especially in the summer month.

  • The purpose of this retrospective study was to evaluate the treatment outcome of proximal tibial graft. Thirty-nine patients were grafted with proximal tibia for the repair of maxillofacial defect. Complications can be considered incidental and neglectable clinical significance. It is concluded that the proximal tibial metaphysis offers a promising alternative sources of cancellous bone for the repair of maxillofacial defect.

  • The objective evaluation of velopharyngeal closure function is the key to diagnosis and therapy control of velopharyngeal incompetency. The aim of this study is to evaluate the aerodynamic and acoustic characteristics of the velopharyngeal closure function of patients who have developed velopharyngeal incompetency after management with speech aids. The test words were composed of sustained vowels /a/, /i/, /e/, /u/, /ja/, /je/, /wi/ and polysyllabic words /p'ap'i/, /siso/, /mami/ for measuring nasalance, The data was collected before the placement of the speech aids and one to three months after. The results were as follows: The nasalance score of the velopharyngeal incompetency speakers was higher than that of the normal control group, except for nasal sounds, and was decreased after placement of the speech aids, especially in high vowels /i/ (P<.01) and /wi/ (P<.05).

  • Pleomorphic adenoma is the most common intraoral salivary gland tumor and characteristically presents as slow painless growth. The cheek are rarely affected site of all pleomorphic adenoma arising from minor salivary glands. In this report, we present the case of a patient with pleomorphic adenoma that affects the cheek. The purpose of this paper is to report this case and review the features of the pleomorphic adenoma occuring in this site.

SQUAMOUS CELL CARCINOMA OF THE MAXILLA ORIGINATED IN ODONTOGENIC CYST - A CASE REPORT - (상악골에 발생한 치성낭종에서 유래된 편평상피세포암)

  • Primary intraosseous carcinoma(PIOC) is defined as a squamous cell carcinoma arising within the jaw, having no initial connection with the oral mucosa. The squamous cell carcinoma within the bone can be presumably developed from residues of the odontogenic epithelium, therefore, it is seen in the jaw only. Metastatic carcinoma from another primary site should be excluded in the diagnosis of Primary Intraosseous Carcinoma. This is a case of 62-year-old man, who initially diagnosed as odontogenic cyst on maxilla, but its pathologic examination was diagnosed as squamous cell carcinoma with odontogenic cyst. We treated this patient with partial maxillectomy, modified radical neck dissection(mRND), and postoperative radiation therapy.
