Sinus membrane elevation and implant placement

  • Sinus Schneiderian membrane elevation surgery is widely performed for dental implant placement in the maxillary posterior region. With regard to sinus elevation surgery, various complications can occur and lead to implant failure. For successful implants in the maxillary posterior region, the clinician must be well acquainted with sinus anatomy and pathology, a variety of bone graft materials, the principles of sinus elevation surgery, and prevention and management of complications.

  • Guided bone regeneration (GBR) is a surgical procedure that utilizes bone grafts with barrier membranes to reconstruct small defects around dental implants. This procedure is commonly deployed on dehiscence or fenestration defects ≥2 mm, and mixing with autogenous bone is recommended on larger defects. Tension-free primary closure is a critical factor to prevent wound dehiscence, which is critical cause of GBR failure. A barrier membrane should be rigidly fixed without mobility. If the barrier is exposed, closed monitoring should be utilized to prevent secondary infection.

  • Computerized tomography has become more widely available in numerous centers throughout the world and is now recognized in many specialities as an important diagnostic procedure. The use of computerized tomography in oral and maxillofacial surgery is discussed. It has been found to be of special value in the diagnosis and assesment of expanding and infiltrating lesions of the jaws, fracture and infections, especially in the midface, in both adults and children.

  • The present case report describes an external auditory canal injury following extraction of the lower molar. The external auditory canal was torn in the same fashion that occurs in an anterior tympanic plate fracture. This case demonstrates one of the rare complications associated with dental extractions.

  • 구강은 다양한 기능을 가지고 있다. 즉, 소화, 호흡 및 발음의 기능을 주로 하며, 심미적으로 매우 중요하다. 그러므로 토진 및 구개열과 같은 기형은 위에 열거한 작용을 훼손시켜, 환자 본인은 물론, 주위 사람들에게 커다란 불행이라 할 수 있다. 그리하여 토진 및 구개파열을 수정하려는 노력은 고대로부터 출발된다. Boo-Chai는 A.D390년경에 중국에서 토진수술이 있었다고 1966년에 보고한 바 있다. 이후에는 수많은 사람들이 토진 및 구개파열에 대한 수술방법을 제시하여 왔으며 현재에도 여러 가지 다양한 수술방법이 이용되고 있다. 그러므로 토진 및 구개파열을 전반적으로 다루는 것은 매우 방대한 것이기 때문에, 먼저 발생빈도, 원인 및 분류에 대하여 간단히 이야기하고 수술시기와 장 많이 쓰이고 있는 수술방법을 다루기로 한다.

Extraction socket preservation

  • Extraction socket preservation (ESP) is widely performed after tooth extraction for future implant placement. For successful outcome of implants after extractions, clinicians should be acquainted with the principles and indications of ESP. It is recommended that ESP be actively implemented in cases of esthetic areas, severe bone defects, and delayed implant placement. Dental implant placement is recommended at least 4 months after ESP.

  • Bruxism is defined as a parafunctional activity during sleep or while awake that includes locking and grinding of teeth and clenching. It generates excessive occlusal force that may lead to implant failure. Therefore, diagnosis of bruxism and providing specific protocols such as occlusal splint and/or injection of botulinum toxin before implant installation are important to prevent increases the risk of implant failure in bruxism patients.
