Nodal Status of the Head and Neck Cancer Patients (두경부 암 환자의 경부 림프절 전이 분석)

  • Purpose : It is well known that the risk of lymph nodes metastases to head and neck cancers are influenced by the location and size of the Primary tumor. as well as the degree and types of histological differentiation. However, data on the statistical analyses of lymph node metastases from the head and neck cancers among Korean Population are not available at present. In order to obtain current status of such data, we have analyzed cancer patients at the department of radiation oncology, korea universityhospital for radiation treatment. Materials and Methods : We have evaluated nine-hundred and ninetyseven (997) head and neck cancer Patients who visited to the Department of radiation oncology, between November 1981 to December 1995. After careful physical examinations and CAT scan, Patients were divided into two groups, those with positive lymph node metastases and with negative lymph node metastases. The nodal status were classified according to the TNM system of American Joint Committee on Cancer (AJCC) Besults : Four-hundred and sixteen Patients out of the 997 patients were lymph node positive $(42\%)$ and 581 patients were lymph node negative $(58\%)$ when they were first presented at the department of radiation oncelogy. According to the AJCC classification, the distribution of positive lymph node is as follow: Nl:106 $(25.5\%),\;N2a:100\;(24\%),\;N2b:68\;(16.4\%),\;N2c:69\;(16.6\%),\;3:73\;(15\%).$ respectively. The frequency of lymph node metastases according to the primary sites is as follow : larynx 283 $(28.5\%)$, paranasal sinuses: 182 $(18\%),\;oropharynx:144\;(14.5\%)\;nasopharynx:122(12\%),\;oral\;cavity\;92\;(9\%),\;hypopharynx:71\;(7\%),\;falivary\;gland:58\;(6\%)$ unknown primary:31 $(3\%),\;skin:\;14(2\%)$,. The most frequent Primary site for the positive Iymph node metastases was nasopharynx $(71\%)$ followed by hypopharynx $(69\%),\;oropharynx\;(64\%),\;oral\;cavity\;(39\%)$ The most common histologic type was squamous cell carcinoma (652/997: $65.4\%$). followed by malignant lymphoma $(109/997:11\%)$. Conclusion : Statistical results of lymph node metastases from head and neck cancer at our department were very similar to those obtained from other countries. It is concluded that the location of Primary cancer influences sites of metastases on head and neck, and stage of the primary cancer also influences the development of metastatic lesions. Since the present study is limited on the data collected from one institute. further statistical analyses on Korean cancer Patients are warrented.

  • This is a report of three cancer patients treated with the different methods of neck dissection considering the state of the each patient. 1. A 76-year old femals patient who showed $T2_N0_M0$ squamous cell carcinoma received the regional neck dissection with resection of primary lesion. The postoperative result was uneventful without recurrence for 3.6 years. 2. A 52-year old male patient who was diagosed $_T3_N1_M0$ squamous cell carcinoma was treated with the bilateral neck dissection and radical resection of primary lesion. Metastasis was noticed on the right scapular area 8 months postoperatively. We treated him with radiation and chemotherapy, but he died 13 months postoperatively. 3. A 55-year old male patient who showed $_T2_N1_M0$ squamous cell cvarcinoma was treated with the classical neck dissection and the radical resection of primary lesion. The postoperative result was good with no recurrence for 4 years.

  • Porphyromonas gingivalis is a gram-negative bacteria of rod shape, and grown in an anerobic condition. It colonizes in subgingival crevice and is known as a major pathogen causing chronic periodontitis. It possesses an invasive property and replicative potential within various cell types, presumably playing an important role in modulating biological behaviors of oral cancer. However, the pathophysiology of P. gingivalis in the malignant transformation of oral cancer has not been fully understood. In this study, we aimed to investigate molecular changes of oral squamous cell carcinoma cells induced by repetitive P. gingivalis infection that clinically resembles chronic periodontitis.

  • 비인두암에서 시행되는 방사선 치료 이후에 구강건조증과 피부 변화는 흔하게 볼 수 있는 합병증 중 하나이다. 비인두암의 좋은 예후를 고려할 때, 그러한 치료와 관련된 독성들은 상대적으로 오랫동안 문제를 야기하며 삶의 질 저하를 불러온다. 특히 류마티스 관절염, 루푸스와 같은 결합조직 질환을 가진 환자들에게서 방사선 치료를 시행하였을 때, 빈번하게 심각한 독성이 관찰된다. 본 증례는 성인형 스틸씨 병에서 병발된 비인두암의 치료로 항암방사선 동시치료를 실시한 결과, 비교적 경미한 구강 건조, 점막염, 불면증 등의 합병증이 관찰되었으나, 완전 관해가 획득되었다. 이에 저자들은 과거에 보고된 적 없는 성인형 스틸씨 병에 병발한 비인두암의 치료 경험을 다른 결합조직질환들에서 방사선 치료의 문헌들과 함께 보고하는 바이다.

Anti-metastatic Effect of Garlic Hexane Extract on Lung Metastasis Induced by Melanoma B16F10 Cells in Mice (Melanoma B16F10 cell에 의해서 유도된 mouse모델에서 마늘 헥산 추출물의 암전이에 억제 효과)

  • Metastatic cancer is one of the main causes of cancer-related death since they rarely respond to available treatments. There is epidemiologic evidence that high garlic consumption decreases the incidence of cancer. Recent studies of our laboratory have revealed that a garlic-extracts is effective in suppressing metastasis. For experimental metastasis, C57BL/6 mice were injected intravenously with melanoma B16F10 cells in the tail vein, and were orally administered various concentrations (0, 50, 100 or 200 mg/kg body weight) of garlic hexane extract (GHE) for 21 days. The incidence and the area of the melanoma cell colony occupied by the poorly differentiated carcinoma were significantly lower in dose-dependent in 50, 100 and 200 mg/kg BW GHE - treated mice compared with control mice. In conclusion, the results of the present study show that GHE administration prevents lung metastasis in C57BL/6 mice.

  • A 65-year-old-male had a mass in the oral cavity, and this was ultimately determined to be squamous cell carcinoma and he underwent wide excision for it. Postoperatively, he underwent the staging work up; this showed that he had other cancers in the esophagus, stomach and lung. We performed a curative resection of all the lesions simultaneously. We report here on a case of synchronous multiple cancers of the esophagus, stomach, lung and oral cavity.

  • Malignant melanoma of the oral cavity is rare, accounting for 1 to $8\%$ of all malignant melanomas. The overall prognosis remains poor despite the available treatments such as radical surgery, adjuvant radiotherapy, chemotherapy and immunotherapy due to failure in early detection and tendency in early metastasis. The etiology of mucosal malignant melanoma remains unkown. However, there are few cases of malignant melanoma of the oral cavity reported in the literature, which might be related to preexisting melanosis and radiation treatment. A case with malignant melanoma developed on the same site after 6 years following irradiation for squamous cell carcinoma of the oral cavity is reported in this article.

  • Defects due to mandibulectomy often cause hard and soft tissue loss and result in esthetic problems and functional disorders such as mastication, swallowing, and pronunciation. After the mandibular reconstruction, several complications including loss of alveolar bone can cause limitations in maintenance or supporting of removable prosthesis. For these patients, implant-supported fixed restorations have been an appropriate prosthetic restorative method. In this case report, we report the patient who underwent mandibulectomy and mandibular reconstruction owing to oral cancer, and then restored the current dentition functionally and aesthetically by applying zirconia frameworks and monolithic zirconia crowns by computer-aided design and computer-aided manufacturing.

  • Simultaneous second primary tumors are not uncommon in patients with oral cavity squamous cell carcinoma. Many studies have previously shown that oral cavity squamous cell carcinoma patients with simultaneous second primary tumor generally have a poor prognosis. Additionally, the choice of the optimal therapeutic modality for oral cavity squamous cell carcinoma patients who present with simultaneous second primary tumor remains problematic. We reported a case of simultaneous squamous cell carcinomas in mouth floor and esophagus, that multidisciplinary team performed resection and reconstruction simultaneously.

  • Germ-line mutations at DNA repair loci confer susceptibility to colon cancer in hereditary non-polypopsis colorectal cancer. Somatic loss of DNA mismatch repair gene has been reported in a large variety of other tumor types. Replication errors(RERs) judged by microsatellite instability(MSI) and its associated mutations have been recognized as an important mechanism in various tumor types. To investigate associations between MSI and oral squamous cell carcinoma, the frequency of MSI using 12 microsatellite markers were analyzed for the series of oral tumors. Of 17 tumors, 8 cases(47%) did not show instability at any of the 12 loci; 5(29%) showed instability at $2{\sim}3$ loci; and 4(24%) showed instability above 4 loci. The 4 cases showing widespread MSI did not differ from those without evidence of instability in terms of age at diagnosis, degree of differentiation, metastasis to lymph node, tumor location or the presence of mutations in the p53 tumor suppressor gene. DCC and D17S 796 were the most frequently detected in MSI analysis. There were no correlation between smoking and MSI frequency, instead, smoking was suggested to increase the mutation rate of p53 and development of oral carcinomas.
