Time Serial Change of Proximal Contact Tightness after Crown Restoration (인공치관 수복 후 인접면 접촉강도의 경시적 변화)

  • The purpose of the study was to observe the time serial changes in proximal contact tightness after single crown restoration. Initial proximal contact tightness before crown preparation and after prosthesis restoration were measured repeatedly in 12 subjects. In consequence proximal contact tightness of temporary setting on prosthesis was bigger than contact tightness before initial preparation, contact tightness of the prostheses of 1-2 weeks after the setting was similar to that before the preparation, tightness of 2-4 weeks after the setting showed little change and maintained constant contact tightness.

  • In implant restorations, it is difficult for the patient to percept any symptoms. In addition, they are absent of shock absorbers, which can lead to mechanical failure if stress distribution is not considered. Since maxillary anterior multiple-implant restorations play a significant role in guiding the functional movement of the mandible by distributing lateral force, it is crucial to form appropriate occlusion. The use of the T-scan system is more advantageous in assessing 'dynamic occlusion', such as the change of occlusion over time, the amount of tooth contact during functional movement, and assessing the occlusion in the less-visible posterior teeth. The case is reported as it has satisfactory results in harmonious anterior guidance of a maxillary anterior multiple-implant restoration using T-scan analysis.

  • A patient with severe periodontitis has causative factors that cause pathological tooth movement, the occlusion is disintegrated, and the vicious cycle of worsening periodontitis is repeated. In particular, when pathological tooth movement occurs in the maxillary anterior region, the patient has an aesthetic sense of atrophy, and the quality of life was reduced. Therefore, when orthodontic treatment was added to patients with severe periodontitis, it promotes the formation of new bone, reduces periodontal cysts, and obtains clinical attachment, which leads to favorable results in prosthetic restoration, thereby enabling ideal occlusion, function and aesthetics. Periodontal treatment, orthodontic treatment, natural tooth restoration, and implant prosthesis were planned for patients with pathological tooth movement in the anterior region due to loss of occlusal support in the posterior region. As a result, an ideal restoration space was secured, a stable restoration of occlusal contact was formed, and the maxillary anterior teeth were aesthetically improved.

  • When wearing complete dentures, patients want to function naturally within a physiologically stable range. To do this, recovery of esthetics, biologically stable arrangement and contour, and occlusal contacts with denture stability are necessary. In this case report, a complete denture patient of adverse conditions was presented. To increase stability of the dentures, functional impression was made by border molding using the neutral zone. The dentures were checked for physiological centric relations and stable occlusion. The clinical results showed satisfactory results on function and esthetics.

  • Objective: This study was performed to evaluate functional changes of occlusion after orthodontic treatment by measuring the occlusal force (OcFr) and occlusal contact area (OcAr), and to compare OcFr and OcAr change according to premolar extractions. Methods: Data were obtained from 74 patients who had finished orthodontic treatment using fixed appliance aged between 18 and 40 years. Subjects were divided into groups who had four premolars extractions or non-extraction (Male extraction-16, Male nonextraction-18, Female extraction-19, Female nonextraction-21). All subjects were asked to bite pressure-sensitive sheets into maximum intercuspation with maximum bite force, and OcFr and OcAr were evaluated by measuring the sheet with a CCD camera. Records were taken right after debonding, 1 week, 1 month, 3 months, 6 months and 1 year after debonding. Results: OcFr and OcAr increased gradually in all groups during the 1 year retention period (p < 0.05). Male groups showed higher OcFr and OcAr than female groups throughout the retention periods (p < 0.05). There were no statistically significant differences of OcFr and OcAr between extraction and non-extraction groups in both males and females (p > 0.05). Conclusions: Occlusion was improved functionally throughout the 1 year retention, and premolar extraction did not induce a decline in the functional aspect of occlusion.

치과용 석고의 사용에 따른 물성 변화

  • 1. 석고를 보관할 때는 건조한 곳에 밀봉하여 보관해야 한다. 따라서 용기 포장을 일단 개봉하면 밀봉용기에 담아 보관하는 것이 바람직하다. 2. 혼수비는 석고의 기계적 성질에 영향을 주기 때문에 정상적인 혼수비를 지켜야 하며, 모형은 경화된 후에도 물과 닿지 않도록 주의한다. 3. rubber인상재의 경우 비눗물을 붓이나 스프레이타입으로 뿌려준 후 흐르는 물에 씻고, 물기를 제거한 후 석고를 주입하면 기포 발생을 줄일 수 있다. 4. 석고를 인상체내에 너무 높이 올려쌓아도 무게로 인해 변형 된다. 5. 석고가 tray밖으로 흘러나가도 변형된다. (공기와 접촉되어 경화된 석고가 아직 경화되지 않은 석고를 잡아당김.) 6. 인상체에서 분리한 모형은 요오드 스프레이를 제조자 설명서에 따라 뿌려 소독한다. 7. 교합기에 모형 부착 시 base끼리 bar나 성냥개비등 다른 고정장치로 고정 시켜야하고, 교합기를 위에서 눌러주어야 한다. 그렇지 않을시 교합관계가 높아질 수 있다. 8. 재료를 구입시 물리적 특성이 많이 기재되어 있는 제품을 선택한다.

COMPUTERIZED EVALUATION OF OCCLUSAL CONTACTS IN CENTRIC CLOSURE IN DISTAL EXTENSION PARTIAL DENTURE CASES (유리단 국소의치 환자에서 T-scan system을 이용한 중심교합시에 교합접촉 양상 연구)

  • This clinical study was to analyze occlusal contacts in maximum intercuspation on distal extension partial denture and to compare tooth contact state between the denture teeth and abutment teeth by time mode and force mode using the T-scan system. The subjects ware twenty-one adult patients with upper natural teeth and lower distal extension partial denture. Conclusion : 1. The patient with simultaneous occlusal contacts both denture tooth and abutment and bilaterally in Kennedy Class I cases was one-sixth, but there was no one with symmetric occlusal and equal force among bilateral denture teeth and abutments. 2. The five-fifteenth of Kennedy Class I case patients resulted in simultaeous occlusal contacts bet-ween denture teeth and abutments but no one has the symmetric occlusal contacts and unifarm force between denture teeth and abutment teeth.

Time Serial Change of Proximal Contact Tightness Between the Implant Prothesis and Natural Tooth (임플란트 보철물과 자연치 사이의 인접접촉강도의 경시적 변화)

  • The aim of this study was to evaluate time serial change of the TPTC(Tightness of Proximal Tooth Contact) between the implant prosthesis and natural tooth at rest state. Thirty-one patients(19 males and 12 females; mean age, 48.2 years) restored with a total 31 single-implant crowns in the left maxillary for 13 single-implant and left mandibular second molars for 18 single-implant crowns, respectively were selected. The TPTC was checked directly after delivery, 1 month after delivery and 6 months after delivery of prothesis at rest state. The TPTC decreased significantly as time goes by both maxilla(p<.01) and mandible(p<.05). The TPTC between the implant prosthesis and natural tooth decreased as time goes by after fixed implant prothesis placement.

Approach to prosthetic treatment for patients with open bite due to mandibular displacement: Case report (하악변위로 인한 개방교합 환자의 보철치료: 증례보고)

  • Open bite is accompanied by decrease in tooth contact and overbite, and causes collapse of occlusal plane, mastication difficulties, speech disorders, changes in appearance, and lower occlusal force than normal. Open bite caused by temporomandibular joint disorder in adults with complete occlusion must be corrected after removal or stabilization of the causative factors. Orthodontic treatment, occlusal adjustment, prosthetic treatment, and surgical treatment can be the option of occlusal correction. This report describes about estimating the cause of occlusion change in two patients who developed an open bite due to mandibular displacement in adults with complete occlusion and different treatment approaches accordingly. In one patient, satisfactory result was obtained in functional and esthetic aspects through occlusal adjustment after stabilization of the temporomandibular joint.

  • 두개하악장애는 환자의 신체적 소인, 관련조직의 구조적 변화, 정신적 스트레스 등의 복합적 요인에 의 발생하는 다인성 질환으로 최근 대두되는 원인중의 하나가 신체, 특히 두경부의 자세이상이다. 비정상적인 두경부자세는 하악의 위치변화를 수반하여 종종 두개하악장애를 야기시키게 되나 이를 설명할 수 있는 과학적 증거는 아직 부족한 실정이다. 저자는 두경부 자세가 교합장치물에 의해 변화되는지를 저작근의 근전도학적 측면에서 규명하고자 21세에서 26세의 치과대학생으로 치아상실 및 교합간섭이 없고 두 개하악장애의 증후 및 경부운동제한을 가지지 않은 정상인 20명을 대상으로 각도계를 사용하여 시상면상에서 인위적으로 유도한 세 가지의 두경부자세변화 (0$^{\circ}$, 30$^{\circ}$ 굴절, 45$^{\circ}$ 신전)에 따른 교합안정장치의 장착(1시 간) 전후 및 안정위와 근접촉위(초기치아접촉위)에서의 두경부 근육(측두근, 교근, 악이복근)의 근전도를 Bioelectric Microprocessor EM2ⓡ를 사용하여 측정한 후 통계학적으로 비교, 분석, 평가하여 다음과 같은 결과를 얻었다. 1. 교합안정장치 장착전 하악 안정위 및 근접촉위에서 두부의 30$^{\circ}$굴절은 악이복근의 근활성도를, 45$^{\circ}$신전은 교근 및 악이복근의 근활성도를 증가시켰다.(p<0.05) 2. 교합안정장치 장착후 하악 안정위에서 두부의 30$^{\circ}$굴절은 악이복근의 근활성도를, 45$^{\circ}$신전은 교근 및 악이복근의 근활성도를 증가시켰으며, 하악 근접촉위에서 두부의 45$^{\circ}$신전은 악이복근의 근활성도를 증가시켰다(p<0.05). 3. 교합안정장치 장착 전후 및 두부의 30$^{\circ}$굴절, 45$^{\circ}$신전에 따른 측두군 및 교근의 하악 근접촉위에서의 근활성도가 안정위에서의 근활성도에 비해 유의한 증가를 나타냈다(p<0.05).
