A correlation between bruxism and eccentric occlusal interferences (이갈이와 편심위 교합간섭의 상관관계 분석)

  • Purpose: The purpose of this study was to analyze correlation between bruxism and occlusal contacts on balancing side. Materials and Methods: The purpose of this study is to compare the difference of group function and balancing side occlusal contacts according to bruxism and sex, A total of 100 adults that aged 26-37 years (39 bruxers and 61 non-bruxers) were examined. The lateral excursion and balanced occlusal contacts were analyzed to determine the correlation with bruxism. The occlusal contacts were recorded by T-Scan system and articulating paper. Results: The group function was the highest in 61.5% of bruxers and 47.5% of non-bruxers. In comparison between males and females, group function was 58.9% in males and 37.0% in females. Occlusal contacts on non-working side occurred in 48 out of 100 patients. There were 51.2% of the cases in the bruxers and 46.0% in the non-bruxers. Statistically, there was no correlation between the bruxism and occlusal contacts on non-working side. There was no correlation between sex and balancing occlusal contacts. Conclusion: There was no significant correlation between bruxism and occlusal contacts on non-working side. The group function was the highest in bruxers when lateral excursion was occurred.

  • 저작운동을 중추신경계와 말초조직의 작용에의해 일생동안 학습과 적응의 가정을 통해 이루어진다. 저작의 양상은 유치가 구강내에 맹출되면서 이루어지며, 상하악 치아의 접촉, 점막과 혀 및 치아상호간에 일어나는 접촉과 치근막의 고유수용기와 악관절 수용기등에 의해 조정되어 최종적으로 에너지소비가 적고 동통이나 불편함이 없이 최대의 효율을 나타내는 양상으로 된다. 그러나 원활한 저작과 하악은동을 이룩하기 위해서는 저작근, 악관절, 상하악 치아의 교합등이 서로 조화를 이루어야 한다. 만일 외상에 의해 악관절의 기능장애가 오면 저작근활동이 변화하고 이에따라 교합장애를 야기한다. 또한 치아를 상실하거나 대합치아간의 조기접촉이 있으면 교합장애가 원인이 되어 근육활동이 변화와 악관절 기능장애를 초래한다. 그리고 때로는 지나친 근육의 활동(이갈이, 지속적 긴장성 수축)에 의해 지나친 치아의 마모로인한 교합장애나 악관절장애를 일으킬 수 있다. 따라서 하악의 기능을 충분히 이해하기 위해서는 교합의 형태학적 관계와 더불어 저작계의 기능적인 면도 고려하여 그 상호관계를 검토하여야 하며 본문에서는 교합에 관련된 저작근의 작용과 이에 영향을 주는 요인 및 교합장애와 근육활동장애의 연관성에 관해 고찰코자 한다.

Comparisons of occlusal force according to occlusal relationship, skeletal pattern, age and gender in Koreans (한국인에서의 부정교합 여부와 골격형태, 연령, 성별에 따른 교합력의 비교)

  • Objective: The aim of this study was to evaluate the occlusal force and contact area and to find its associating factors in Koreans. Methods: Occlusal force and contact area in maximum intercuspation were measured using the Dental $Prescale^{(R)}$ system in 651 subjects (15 with normal occlusion, 636 with various malocclusions divided into subgroups according to the skeletal pattern, Angle's molar relationship, age and gender). Results: Occlusal force of the normal occlusion group ($744.5{\pm}262.6N$) was significantly higher than those of the malocclusion group ($439.0{\pm}229.9N$, $p$ < 0.05). Occlusal force was similar regardless of differences in ANB angle or Angle's molar classification, however the increase in vertical dimension significantly reduced occlusal force ($p$ < 0.05). Conclusions: Occlusal force was significantly lower in the malocclusion group compared to the normal occlusion group, and in females compared to males, but it was not affected by age, antero-posterior skeletal pattern or molar classification. Although a hyperdivergent facial pattern indicated lower occlusal force compared to a hypodivergent facial pattern, the differences in skeletal pattern were not the primary cause of its decrease, but a secondary result induced by the differences in occlusal contact area according to the facial pattern.

Occlusal Analysis of the Patients with Temporomandibular Disorders Using T-Scan II System (T-Scan II 시스템을 이용한 측두하악장애 환자의 교합 분석)

  • Correlation between occlusal contact pattern and TMD have been hypothesized and partially investigated but results are controversial and not conclusive. The purposes of this study were to compare right-to-left difference of occlusal contact pattern, through contact points, contact force and occlusal balance, in the patients with unilateral TMD and also to evaluate its change related with TMD treatment. 36 patients with unilateral TMD from Department of Oral Medicine in Dankook University Dental Hospital were selected in this study (M:F=7:29, mean age of $29.2{\pm}14.8$ years). A computerized T-Scan II system (Tekscan, INC., USA) was employed for occlusal analysis and the simultaneity and occlusal balance through the number of tooth contact and magnitude of contact force were determined before and after TMD treatment. The number of contact points and contact force was more on the unaffected side than the affected side before treatment (p=0.056 and p=0.060, respectively) while significant difference between both sides was not found after treatment. The number of contact points and contact force on the affected sides significantly increased after treatment (p=0.038 and p=0.052), but the unaffected sides exhibited no significant difference between before and after treatment. In addition, sides difference in relative contact force decreased from about 27% to about 12% after TMD treatment (p=0.001). According to the results of this study, it is likely that unilateral TMD impairs right-to-left occlusal balance and that conservative TMD treatment alleviates the imbalance, subsequently leading to more symmetrical occlusal condition with increased contact points and force.

  • In cases of extensive prosthetic restoration, correction of occlusal contact is often needed, as it is the essential component for a successful restoration. If occlusal contact is given incorrectly, various symptoms of occlusal trauma can occur of which temporomandibular joint disorder (TMD) is one of them. As one of the common symptoms of TMD, patients may suffer with masticatory muscle disorder and temporomandibular joint pain. This case presents satisfactory results for the improvement of masticatory muscles and temporomandibular joint pain of a TMD patient, caused by incorrect occlusal contact of the restoration, by replacing the prosthesis after occlusion correction.

Rehabilitation of unstable occlusion caused by inter-dental arch discrepancy (치열궁 부조화로 인한 불안정한 교합을 보이는 환자의 수복 증례)

  • Inter-dental arch discrepancy between maxilla and mandible could cause three dimensional occlusal problems, and collapse of occlusal plane, multiple teeth loss and decrease of masticatory efficiency could be observed in patient having unstable occlusal contact. Patient showing posterior bite collapse, unstable occlusal contact and improper anterior guidance should be treated to recover stable centric occlusion, occlusal contact, and anterior guidance in conjunction between prosthodontics and orthodontic treatment. This clinical report describes the favorable results of orthodontic and prosthodontics rehabilitation of patient with above mentioned problems.

A STUDY ON THE CHANCES OF TOOTH CONTACTS FOLLOWING ORTHODONTIC TREATMENT BY USE OF THE T-SCAN SYSTEM (T-SCAN을 이용한 교정치료후의 교합접촉 변화에 관한 연구)

  • This study was undertaken to investigate the changes of tooth contacts following orthodontic treatment , using T-Scan system. Occlusal contacts in maximum intercuspation were examined in 26 patients at the end of the active phase of orthodontic treatment and again 3 months into the retention phase to abbess the initial posttreatment occlusal changes. The obtained results of this study were as follows : 1. The mean total number of contacts increased from 11.27 at the end of orthodontic treatment to 13.15 at the fellow up visit, which was found to be statistically significant (P<0.05). 2. The change of number of contacts on anterior teeth was unchanged. 3. The change of number of contacts on posterior teeth was found to be statistically significnat (P<0.01). 4. The change of fetal number of teeth in contact was significant (P<0.05). 5. The number of ant. teeth in contact was slightly increased. but Which was not found to be statistically significant (P>0.05). 6. The number of post. teeth in contact significantly increased, which was statistically significant (P<0.05). 7. A statistically significant relationship was found between the total number of contacts at the end of treatment and the change in the contacts over time. The fewer the number of tooth contact was at the end of orthodontic treatment, the greater number of tooth contact increased over time(r=-0.38, P<0.05).

Changes of bite force and dynamic functional occlusion analysis after occlusal stabilization splint therapy in sleep bruxism patients: a pilot study (수면이갈이 환자에서 교합안정장치 사용 후 교합력 및 동기능적교합분석: 예비 연구)

  • Purpose: The aim of this study was to compare changes of bite force, occlusal contact area, and dynamic functional occlusion analysis after occlusal stabilization splint therapy during sleep for one month in a patient with bruxism. Materials and Methods: From October 2021 to July 2022, sleep bruxism of 30 patients who visited the Department of Oral Medicine at Yonsei University College of Dentistry Hospital were recruited. The participants were divided into two groups: using an occlusal stabilization splint during sleep (treatment; n = 15) and not using an occlusal stabilization splint (control; n = 15). Before using the occlusal stabilization splint and one month after, bite force, occlusal contact area and dynamic functional occlusion analysis (ratio of left/right bite forces, average bite forces, maximum bite forces, and maximum contact areas during lateral and anterior and posterior mandibular movements) were performed. Results: There was no difference in bite force and occlusal contact area between the treatment group using the occlusal stabilization splint and the control group not using the occlusal stabilization splint during sleep for one month. However, there were significant differences in the average bite force and maximum bite force in the lateral and anterior and posterior mandibular movements and the maximum contact areas in the anterior and posterior mandibular movements. Conclusion: The occlusal stabilization splint is helpful for sleep bruxism patients who lateral and anterior and posterior mandibular movements. In addition, further studies are needed a double-blind study with a large population.

Evaluation of occlusal strength using T-Scan Novus and Dental prescale II in dental prosthodontic treatments: A case report (보철물 수복 형태에 따른 T-Scan Novus와 Dental prescale II를 이용한 교합력 평가 활용 증례)

  • Diagnosis and analysis of occlusal relationships are important factors in prosthetic treatment. A thorough occlusion analysis and evaluation should be performed before treatment to restore a stable interocclusal relationship. Analysis and evaluation are essential during the treatment process and at regular follow-ups. Recently, with the development of dental equipment and digital processing methods, new quantitative analysis methods that can record the patient's occlusal relationship have been introduced. Among them, the T-Scan Novus (Tekscan Inc., S. Boston, MA, USA) displays the strength of the initial contact point and the occlusal contact point of the teeth using a pressure sensor. With this, occlusal contact time of the teeth, anteroposterior and left-right balance of occlusal force can be compared. The Dental prescale II (GC Co., Tokyo, Japan) scans the occlusal contact point using a pressure-sensing film and analyzes the density of the contact point. It can measure the distribution and strength of the occlusal force of the teeth in the most natural occlusion state. Based on this, appropriate prosthetic treatment (four-unit fixed partial denture, removable partial denture, complete denture, and complete oral restoration cases) was performed according to the area and extent of the patient's tooth loss. The patient's occlusion at the first visit, treatment stage, right after treatment, and regular follow-up were compared and evaluated using a quantitative method for appropriate occlusion analysis using T-Scan Novus and Dental prescale II. This report enhances the understanding of occlusion analysis during prosthetic restoration. The results satisfied both the clinician and patients in terms of function and aesthetics.
