이 연구는 교정치료중인 환자의 구강건강영향지수(OHIP)와 자아존중감에 영향을 미치는 요인을 파악하기 위해 대구시내에 소재한 교정 전문치과 4곳에서 교정치료를 받고있는 환자 200명을 대상으로 설문지 조사방법을 이용하여 대상자의 일반적 특성 및 교정치료 특성, 구강건강영향지수, 자아존중감을 조사 분석하여 교정치료환자의 구강건강영향지수 및 자아존중감에 영향을 미치는 요인을 분석하였다. 그 결과, OHIP에 영향을 미치는 요인으로는 연령이 낮을수록(P<0.01), 구강건강상태가 좋을수록(P<0.001), 가족의 권유로 교정치료 받는사람이(P<0.05), OHIP에 영향을 주는 요인으로 분석되었으며, 자아존중감에 영향을 미치는 요인으로는 남자가(P<0.05), 미혼이(P<0.01), 수입이 많을수록(P<0.05), 자아존중감에 영향을 주는 요인으로 나타났다.

최근 교정치료가 진보함에 따라 치료대상이 성인에까지 폭넓게 적용되고 교정장치에도 심미성이 요구되어지고 있다. 이러한 요구에 부응하여 bracket과 attachment의 소형화, ceramic 또는 plastic을 사용한 투명도가 높은 장치가 개발되었다. 그러나, 순측 치면에서 bracket을 장착하는 치료에 있어서는 심미성에 한계가 있다. 이러한 점에서 lingual bracket은 설측 치면에 접착함으로써 성인교정에 있어서 심미적으로 우수하다고 할 수 있다. 이에 저자들은 lingual bracket을 이용한 교정치료에 있어서 새로운 기공과정과 임상과정을 확립하였기에 소개하고, 현재 많이 사용하고 있는 Ormco사와 Dr. Fujita의 lingual bracket을 이용하여 치료한 증례를 다음의 순서로 4회에 걸쳐서 보고하고자 한다.

1800년대에 유럽에서 가철식 교정장치가 개발되면서 처음으로 교정치료가 시작되었다. 그 후 1900년대에 들어서면서 Edward H.Angle에 의해 미국의 독특한 형태인 고정식 교정장치가 개발되었다. 그 동안 수 많은 선학들에 의해 다양한 재료와 치료방법들이 고안되면서 아름다워지고자 하는 현대인의 욕구와 맞물려 임상교정학 분야는 꾸준히 발전하여 왔다. 국내에서는 1940년대 이후부터 외국의 가철식 교정장치와 고정식 교정장치를 그대로 받아들여 수 많은 부정교합 환자의 치료에 응용하고 있다. 그러나 조금만 더 세밀하게 들여다 보면 가철식 교정장치를 개발한 유럽의 교정의들은 지나치게 가철식 교정장치에만 의존하는 경향이 있고, 고정식 교정장치를 개발한 미국의 교정의들은 지나치게 고정식 교정장치에만 의존하면서 가철식 교정장치의 사용을 아예 금기시하는 경우도 있다. 그러나 이 두가지의 경우 모두 바람직하지 않다. 수혜자의 위치에 있는 우리가 할 수 있는 최선의 방법은 아무련 편견없이 가철식 교정장치와 고정식 교정장치를 적절하게 병행하여 사용하는 것이 바람직하다. 두 장치 모두 확실한 장ㆍ단점을 가지고 있기 때문에 시기와 방법을 적절하게 적용한다면 성공적인 교정치료를 위한 지름길이 될 수 있다.

Objective: The purpose of this study was to evaluate the effects of malocclusion and orthodontic treatment on the self-esteem of adolescents. Methods: The subjects were composed of 3509 female middle school students. Each subject was evaluated with Rosenberg's Self-esteem Scale to measure the level of self-esteem and also evaluated the degree of crowding and soft tissue profile. Results: Results showed that protrusion of lip area had no effects on self-esteem but crowding of upper anterior teeth had significant effects. The fixed orthodontic treatment group and the removable orthodontic treatment group reported no significant difference in self-esteem from the no orthodontic treatment experience group, but the debonding group showed significantly higher Self-esteem index. Conclusion: The results of this study suggest that anterior teeth alignment has influence on the self-esteem of female adolescents.

The purpose of this study was to investigate effect of spectacle correction therapy instead of occlusion therapy for refractive amblyopia treatment. Spectacle correction were prescribed to give the same effect as a occlusion therapy by under correction for normal eye and there was no additional treatment but only spectacle correction for hyperopic amblyopia. The results can be summarized as follows: 1. In hyperopic amblyopia after correction, initial visual acuity($Mean{\pm}SD$) was $0.36{\pm}0.13$ and final visual acuity($Mean{\pm}SD$) was $0.82{\pm}0.23$. 2. Regardless with age, there was significant differences between initial acuity and final acuity, it shows improvement in visual acuity after spectacle correction treatment. 3. Initial correction age did not influence the length of treatment and success rate of treatment, so that ambyopia correction effect not related with age. 4. Compared with initial visual acuity with final visual acuity, initial visual acuity was in proportion to final visual acuity. 5. Incidence was higher in hyperopia and hyperopic astigmatism than myopia and myopic astigmatism in refractive amblyopia group and the therapy was more effective for hyperopia and hyperopic astigmatism than myopia and myopic astigmatism. 6. Treatment was effective even for children who is older than 8 years.

The purpose of this study was to investigate sole effect of therapy of spectacles correction on the refractive amblyopia. Spectacles were prescribed to give the same effect as the occlusion therapy undercorrecting in the case of hyperopia, and effectiveness of the therapy was compared with occlusion therapy without additional prescription. The results can be summarized as follows: 1. The higher anisometropic power was the lower initial visual acuity was. 2. Anisometropic power did not influence final visual acuity. 3. The latter beginning time of therapy was the higher astigmatism was. 4. Therapy of spectacles correction on the hyperopic amblyopia was quite effective.

LINGUAL ARCH APPLIANCE는 1908년 Mershon이 처음 창시한 교정장치로 band를 사용하는 교정장치중 가장 간단하면서도 여러가지 장점을 지니고 있다. 이 장치는 특히 성장발육기의 소아교정에 최적하여 치아의 기능을 제한하지 않으며 악골 및 치아의 자연적인 성장발육을 허용하고 자연적인 치아이동을 도모하게 한다. 또한 이 장치의 특징은 지속적인 교정치료보다는 간헐적인 치료를 한다는 사실이다.예를 들면 Angle씨 2 급부정교합 즉 상악전돌의 치료시는 상악에 Libial arch wire, 하악에 Lingual arch wire를 장착하고 여기에 악간고무를 이용하여 2급의 교합상태를 개선한후 수개월간 장치를 제거하여 치아의 자연적인 적응을 허용하고 다시 장치를 장착하여 치료를 계속하는 것이다.
