강직성 척추염은 천장 관절의 강직뿐만 아니라 척추체 주변의 관절막과 인대의 골화로 인해 척추 강직을 일으킨다. 이는 척추 움직임 제한을 일으키고 많은 경우에서 통증을 동반한 경추, 흉추, 요추 분절의 척추 변형을 유발한다. 방치할 경우 최종적으로는 전신 관절의 강직과 골화를 유발하게 되는 심각한 질환 중 하나이다. 일반적으로 30세 이전에 증상이 발현되며 질병의 점진적인 진행으로 신체적 기능, 환자의 직업적 능력과 삶의 질에 악영향을 끼치게 된다. 이는 우울증 같은 정신과적 질환으로 발전할 가능성을 높인다. 저자들은 이러한 심각성을 인지하고 전신 골화로 악화되기 이전에 다양한 약물 치료로 수술적 치료를 방지하고자 하는 최근의 경향과 연구를 소개하고자 하였으며, 또한 진행을 예방하지 못하여 수술적 치료에 이르게 된 환자들에서 시행되는 다양한 수술 방법과 그에 따른 합병증을 제시하고자 한다.
본 연구는 무릎 골관절염 환자를 대상으로 수중운동과 지상운동의 효과를 비교하기 위해 수행된 체계적 문헌고찰 및 메타분석 연구이다. 문헌은 Ovid-Medline, Cochrane Library CENTRAL, CINAHL과 국내 DB인 RISS와 KISS를 통해 ((osteoarthriti* OR OA) AND (aqua* OR water OR hydrotherap*)), (골관절염 AND 수중운동) 등을 주요어로 조합하여 검색하였으며, 선택배제과정을 거쳐 최종 7편(n=449)의 문헌이 분석에 포함되었다. 문헌에 대한 질 평가는 SIGN의 무작위임상시험평가도구를 사용하였으며, 질 평가 결과는 6편에서 ++, 1편에서 +로 평가되어 전반적으로 비뚤림 위험은 없는 것으로 판단하였다. 메타분석결과 운동 중재 후 수중운동군과 지상운동군의 통증변화에 대한 차이는 Standardized mean difference (SMD) -0.26(95% CI -0.49, -0.03, p=0.03, $I^2=14%$)으로 수중운동군이 지상운동군에 비해 통증이 통계적으로 유의하게 감소하는 것으로 나타났으나, 그 외 두 군간 굴곡 관절가동범위, 신전 관절가동범위, 신체기능, 삶의 질의 SMD는 각각 -0.12(95% CI -0.51, 0.27, p=0.53, $I^2=0%$), -0.04(95% CI -0.55, 0.48, p=0.89, $I^2=43%$), -0.12(-0.44, 0.19, p=0.44, $I^2=0%$), -0.15(-0.54, 0.24, p=0.46, $I^2=0%$)로 두 군간 통계적으로 유의한 차이가 없는 것으로 나타났다. 현재 시점에서 무릎 골관절염환자를 대상으로 수중운동과 지상운동의 효과를 분석한 무작위 대조군 실험 연구 수가 비교적 적어 효과크기에 대한 확증적 결과를 얻는 데에는 다소 제한이 있었으므로 향후 이와 관련된 무작위 임상시험연구와 장기적인 효과검증에 대한 노력이 지속적으로 이루어져야 할 것으로 사료된다.
The purpose of this study was to identify functional impairment and psychological status and to analyze their relationships in patients with chronic arthritis. The sample was consisted of 75 arthritic patients who visited H hospital and S welfare center. Functional impairment was measured by ADL, pain, and the number of painful joint. Psychological status was measured by depression, self-efficacy, and quality of life. Data was analyzed by frequency, mean ${\pm} SD$, Pearson's correlation coefficient, and Stepwise multiple regression. The results of this study were as follows : 1. In functional Impairment, ADL was below average which means somewhat difficult, pain was above average, and the number of painful joint was 9.20. 2. In psychological status, scores of quality of life was 97.89, depression was 41.28, self-efficacy was 895.35. 3. Pain was negatively correlated to self-efficacy, ADL and quality of life, and positively correlated to depression. 4. Self efficacy and depression explained 45% of the variance in quality of life. Therefore, it is suggested that nursing intervention improving psychological status would be useful for patients with chronic arthritis. Especially, it is very important to implement nursing intervention focused on increasing self-efficacy and decreasing depression.
측두하악관절에서 발생하는 골관절염은 저작시 통증, 개구제한, 관절잡음, 교합변화를 유발하여 환자의 삶의 질을 저하시키는데 관여하는 주요한 질환이다. 골관절염의 진행과정과 그에 따른 제어방법을 연구하기 위하여 다양한 연구방법이 시도되고 있다. 이 중 골조직면에서의 조직변화, 활막액의 성분변화, 활막의 조직변화, 주변조직의 변화, 통증에 대한 반응을 확인할 수 있는 동물모델이 주로 많이 이용된다. 본 연구에서는 실험동물에서 다양한 방법으로 측두하악관절 골관절염을 유도하여 하악관절표면에서 일어난 조직변화를 비교 관찰하였으며 각 동물모델의 연구 활용방안에 대하여 탐색하였다. 측두하악관절의 관절강에 monosodium iodoacetate (MIA) 와 interleukin-$1{\alpha}$(IL-$1{\alpha}$)를 직접 주입하여 약물로 관절염을 유도하여 관절조직면을 관찰하였다. 또한 여성호르몬인 에스트로겐에 의한 골변화를 확인하기 위하여 난소절제술로 인한 골다공증 유발모델에서의 하악관절을 관찰하였으며, 노화된 백서의 관절면을 확인함으로써 노화로 인한 골변화를 관찰하였다. Safranin-O 염색 법과 Masson's trichrome 염색법을 이용하여 각각의 관절면에서 세포배열의 변화, 콜라겐의 변화, 골조직의 변화 및 파괴양상에 대하여 확인하였고 Safranin-O의 양성반응을 확인하여 proteoglycans (PGs)의 조직내 분포를 추적하였다.
심한 치주염을 가진 환자는 잔존 치조골의 흡수가 증가함에 따라, 병적인 치아 이동이 증가하고 구치부 교합지지 상실의 가능성이 높다. 특히, 상악 전치부에서 병적인 치아 이동이 발생하는 경우, 환자는 심미적인 위축감과 삶의 질 저하를 경험할 수 있다. 중증 치주염 환자에게 교정치료는 새로운 뼈 형성을 촉진하고 치주낭을 감소시키며, 임상적 부착을 얻어 보철 수복에 유리한 결과를 가져옴으로써 이상적인 교합과 기능 회복을 가능하게 한다. 구치부의 교합지지 상실로 인해 전치부 병적 치아 이동이 있는 환자에게 치주치료, 교정치료, 임플란트 치료를 계획함으로써 이상적인 수복공간 확보와 안정적인 교합접촉을 형성하였고, 구치부 교합회복을 통한 기능적인 전악수복을 통해 심미적인 결과를 얻었다.
Purpose: The purpose of this predictive study was to identify factors affecting health related quality of life (HRQoL) in patients with rheumatoid arthritis (RA). Methods: The participants in this study were 131 patients with RA who were recruited from the outpatient clinic of a university hospital in Seoul. Disease activity in rheumatoid arthritis was evaluated by calculating the Disease Activity Score 28. Disability in activities of daily living (ADL) was assessed with the Korean Health Assessment Questionnaire, and depression with The Center for Epidemiologic Studies Depression Scale. HRQoL was evaluated using The Short Form 36 Health Survey. Data were analyzed using descriptive statistics, correlation, and hierarchical multiple regression. Results: Pain, disability in ADL, disease activity, and depression correlated negatively with physical and mental dimensions of HRQoL. But hierachical multiple regression analysis revealed that disability in ADL and depression were the only variables negatively influencing physical and mental QoL after adjustment for influences of sociodemographic variables. Conclusion: Results of this study suggest that disability in ADL and depression, rather than disease activity and pain have profound effects on HRQoL in patients with RA. Further studies are needed to assess the predictive ability of disease activity and pain on HRQoL in this population.
Purpose: The purpose of this study was to identify predictors of depression and quality of life among older adults with osteoarthritis. The predictors included in the model were the client's characteristics(age, pain, disease duration, ADLs), personal resources(hardiness, self-care agency and family support), and depression. Method: 150 subjects who were older than 65 years and had diagnosis of osteoarthritis participated in the study. To answer the research questions, descriptive analysis, Pearson correlation, and hierarchical multiple regression were utilized using SPSS WIN program. Result: Older adults who were younger and had lower levels of pain and dependency on ADLs, and higher levels of self care agency and hardiness reported lower levels of depression($R^2=0.517$). Older adults who had lower levels of depression, pain, and dependency on ADLs, higher levels of family support and hardiness, and who are younger reported higher levels of quality of life($R^2=0.084$). Conclusion: Based on the findings of this study, development of nursing intervention program including pain reduction, enhancing ADL abilities and personal resources (hardiness, family support) can be suggested. Further study is needed to increase the ability of generalization of the study findings to the broader population.
Purpose: This study examined the effects of exercise programs on the pain and quality of life in patients with knee osteoarthritis. Methods: Electronic bibliographic databases of KiSTi, KoreaMed, KISS and KERIS were searched to identify studies of randomized controlled trials and non-randomized controlled trials. As a result, 1213 publications identified and 16 studies met inclusion and exclusion criteria. All studies examined for quality assessment of studies using Cochrane's risk of bias. Results: The 16 studies were eligible for inclusion criteria, then the meta-analysis was conducted to examine effectiveness of exercise programs on pain and quality of life in patients with knee osteoarthritis. The meta- analysis based on the random effect model showed that the exercise program was beneficial in decreasing pain (effect size .69; 95% confidence interval 0.45~0.91). Also, the exercise programs have been shown to be effective in significantly improving the quality of life. However, it was not statistically significant due to severe deviations of studies (effect size 1.06; 95% confidence interval -0.34~2.46). Conclusion: Exercise programs had positive effects on pain relief and improvement of the quality of life in patients with knee osteoarthritis, although the quality of life was not statistically significant due to severe deviations of studies.
Purpose: The purpose of this study was to determine the effects of water exercise program on physical fitness, pain and quality of life(QoL) in patients with osteoarthritis. Methods: Thirty-four old women were divided into the exercise(EG, n=18) and control groups(CG, n=16) after 6-week self-help education program. The EG carried out 6-week supervised water exercise program(60min/day, 2days/wk). Physical fitness, pain, and QoL were assessed by Senior Fitness Test Manual(Rikli & Jones, 2005), Pain rating scale(0-10) and World Health Organization QoL BREF(Min et al., 2000), respectively. Results: Both EG and CG increased upper and lower-body strength(all, p<.05), which were measured by arm curl and chair stand, respectively. For flexibility test, the EG increased upper and lower body(all, p<.05). Balance of the EG increased(p<.05), but not in the CG. Pain significantly decreased in the EG post training(p=.000). However, both EG and CG did not significantly improve for QoL. Conclusion: Six weeks of water exercise program did induce significant improvement in physical fitness and pain control in patients with osteoarthritis.
Purpose: The purpose of this study was to examine the effects of a self-management program on self-efficacy and quality of life for Korean women with rheumatoid arthritis. Methods: A quasi-experimental study using a nonequivalent control group pretest posttest design was used. The data were collected from July 1 to Sep. 15, 2007. Thirty one arthritis patients (15 in the experimental group and 16 in the control group) were participated in this study. A self-management program developed by Korean Society of Muscle and Joint Health in 1994 was used. The program was consisted of 6 sessions, 2 hours each and held every Friday for 6 weeks. General Self-Efficacy Scale and Quality of Life were used to test the effect of the program. Results: Hypothesis 1; "Self-efficacy in the experimental group after program would be higher than the control group" was supported. Hypothesis 2; "Quality of Life in the experimental group after program would be higher than the control group " was not supported. Conclusion: The self-management program was effective for Korean women with rheumatoid arthritis in terms of increasing self-efficacy. Therefore, to increase self-management skills and self-efficacy for female rheumatoid arthritis patients, the program can be useful.
