There is a need to develop an app for a caregiver health management that can provide continuous management in response to changes over time, because elderly people have low digital utilization capabilities, difficulty maintaining regular and continuous self-management. Based on this need, this study designed an app with a user-friendly UI and simple structure for the elderly. The app developed in this study supports regular management of health data such as blood pressure, blood sugar, and heart rate, as well as specific information on physical, disease, cognitive, communication, and environment in the care field. The app developed in this study supports care services by automatically entering data through integration with health management devices, automatically analyzing and visually representing recorded data to understand trends and volatility, and adding scalability to connect with various health management and medical support platforms. The effectiveness and satisfaction of the developed app were confirmed to be significant in the field verification results.

But It has a problem that each individual people do not confirm medical information correctly. because Medical information based on HIS is use in hospitals. The purpose of this study was to identify the effect of Home Health Nursing based Respiratory Management Program (HHNbRMP) on unmet healthcare need and healthcare resource utilizations of patients applying the home mechanical ventilator in the home with amyotrophic lateral sclerosis. The subjects of this study were 40 patients placed in an experimental group(n=19) and a control group(n=21), respectively. This HHNbRMP based on Cox's interaction model was consisted of cognitive assent (education, specialized medical care, case management), internal motivation (airway clearance, thoracic and air accumulated exercise) and psychological response (meditation & active listening). The intervention was applied to experimental group during 12 weeks. As variables was measured at baseline, twelve, twenty-four weeks and healthcare unmet need, resource utilizations (admission, out patient department, emergency room) was measured at 24 weeks. The data were analyzed by t-test, ANOVA and Repeated Measures ANCOVA. This intervention was not effective the unmet healthcare need. But the admission in to the hospital among the healthcare resource utilizations variables showed a significant difference at twenty-four weeks(t=4.17, p=.049). This results suggest that applying this program tailored to patients condition, utility of medical resource would be decreased, specially admission.

This study survey research of infection control, wastewater management, and instrument disinfection according to characteristic at Daegu, Gyeongbuk Province In 2012 and 2017, the same 114 dental clinics to identify the infection management behavior and prevent infection of dental medical institutions. Scored mean 3.37 points on 8 items of infection control, 95.5% in "records of the patient's medical history", 1.8% in "presence of a wastewater facility." Scored 94.7% in "disinfection of metal trays", 17.5% in "storage from a spitting receptacle in the waste bin and commissioned management.", Scored higher in of infection control, wastewater management and instrument disinfection according to general characteristics and dental characteristics in 2017 than in 2012. Points were higher dental hygienist, University graduation, Type of duty was counseling and management, hospital or higher, Number of dentist(dental hygienist, chair, patient) was high. Therefore raise a need for infection control into consideration the dental characteristics and education and promotion regardless of the hospital size.

The importance of palliative care for terminal cancer patients has been emphasized globally. Korea has formulated and implemented its policy for cancer control as it drew up a 10-year plan for cancer patient care. We examined Korea's National Cancer Act and the second 10-year plan for cancer patient care, which are legal grounds for palliative care projects for terminal cancer patients, to check the current status of Korea's efforts to establish a hospice and palliative care system. Institutionalization of hospice and palliative care has been developed within a framework of the national cancer project. Efforts such as expansion of hospice units, experts training and quality improvement should continue after the reimbursement of hospice and palliative care begins in 2013.

병원 전 단계에서는 일반인 단계, 일차 반응자 단계, 기초응급의료 제공 단계 그리고 전문응급의료 제공단계로 구분되는데 각 단계의 적절한 병원 전 응급의료 서비스의 제공은 환자의 예후에 긍정적인 영향을 미치는 것으로 알려져 있다. 그러나 국내의 일반인 단계 및 일차 반응자 단계의 응급의료 제공은 매우 낮은수준이며, 응급의료체계는 전문 응급의료가 뒤따르지 않는 기초 응급의료만을 제공할 뿐인데도 그 적절성에 대하여 많은 문제점이 지적되어 오고 있다. 우리나라의 병원 전 응급의료체계는 비교적 짧은 기간 동안에 소방서를 축으로 운영되고 있다. 병원 전 단계에서 응급처치의 적절성을 높이기 위해서는 119구급대에 의한 응급처치에 대하여 체계적으로 조사를 수행함으로써 기초 응급의료에 대한 지속적인 질 관리를 해 나가는 것이 매우 중요하다. 이러한 질 관리의 평가를 위해서는 적절한 평가도구가 필요한데 객관성이 높으면서도 비용-효과적인 도구와 지표는 아직 확립되지 못하고 있는 것으로 보인다. 그동안 119 구급대에 의한 응급처치의 적절성에 대하여 몇 차례 보고가 있었지만, 단일 기관 또는 단일 질병을 대상으로 수행된 연구였거나 일개 응급의료기관으로 이송된 환자들만을 대상으로 했던 비교적 소규모의 보고들이었으며, 한 도시를 전체를 대상으로 한 광범위한 조사가 이루어진 적은 아직 없었다. 이에 이 연구에서는 대구 경북 지역의 119구급대원을 대상으로 병원 전 응급처치의 실태와 및 교육현황을 조사하여 응급처치의 질적 향상을 위한 프로그램 개발에 필요한 기초 자료를 제공하고자 한다.

With the paradigm shifts towards consumer-centered health service, it is expected that more health care consumers will keep their health information and manage their own health in the future. Thus, this study was conducted to compare "Understanding", "Utilization" and "Management" of Personal Health Record(PHR) between medical users(healthcare students) and health care providers(medical students). We collected data from 208 health and medical students via using self-reported questionnaires form April to June, 2011. The collected data were analyzed using frequency, t-test, Chi-square on SPSS 19.0 version. There was no significant difference in "Understanding" of PHR between two groups. Looking at the order of the importance of PHR contents, two groups equally emphasized medical records, surgical history, and test results. There was significant difference in both time and effectiveness of PHR(p=0.02). Intergrated type of PHR was preferred by both groups. Recently, PHR reflects needs and demands of users more than ever. However there are many limitations to promote the utilization. In the future, it is necessary to implement targeted strategies for the elderly groups and specific types of disease.

Today, the medical system is changing into a comprehensive health care system in which collaborative relationships between medical professionals and non-medical personnels in neighboring occupational areas. The current medical act brands such "collaboration" as unlicensed medical practice, and punishes non-medical personnel who acted in the risk management of doctors as well as doctors collaborated with non-medical personnel as unlicensed medical practice. In order to narrow the gap between the legal system that regulates unlicensed medical practices and the medical reality, it is necessary to overcome the structural limitations of dualistic, nationalistic, and identity-oriented regulation of unlicensed medical practices. The legal interests of unlicensed medical practice have a dual nature as a personal legal interest of "human life and body" as well as a national legal interest of "maintenance and protection of the nation's medical license system", and it should be noted that the criteria for judging the legal interests protected by the regulations of criminal punishment should be found in "personal legal interest theory." In addition, when determining which behavior is a medical practice and evaluating its risk, the dimension of behavior and measures should be considered in a fair manner without being biased against the subject (identity) of the action. In other words, judging unlicensed medical practice should depend on whether the risk of side effects that may result from the act is reasonably managed. Considering the prospect of therapeutic dialogue between medical professionals and patients, it would be desirable for medical law policies to move in a way that does not fundamentally block the possibility of collaboration among pluralistic medical personalities.

This study was aimed to investigate the influencing level of nursing staff's recognition for the health care accreditation on patient safety managing activities by identifying the relationship between them and other factors affecting on patient safety managing activities. Subjects of this study were 182 nursing staffs working in six geriatric hospitals accredited for health care in Seoul metropolitan area. Data was collected during April, 2016 by structured questionnaires. And SPSS/WIN 15.0 program with t-test, ANOVA, Pearson's correlation, and stepwise multiple regression analysis were employed for analyzing them. 96.2% of the subjects have recognized the accreditation and 31.8% of them have acknowledge it in detail. The average level of recognition for the health care accreditation and patient safety managing activity were 3.60 and 4.39 point (max.5.0) respectively. Correlation between them was positive(r=.339, p<.001). Significant factors influencing patient safety managing activity were internal service quality promotion (t=5.292, p<.001) and academic background (t=2.836, p=.005). Education program or information on health care accreditation system, and action plans for promoting internal service quality for the nursing staffs including job standardization of the jobs are recommended for the better patient safety managing activities.
