A Study on the Urinary Incontinence and Overactive Bladder Syndrome of Women in a Rural Region (일부 농촌여성에서 요실금 및 과민성방광증후군의 실태에 관한 조사)

  • Objectives: This investigation was carried out to understand the status of the urinary incontinence (UI) and overactive bladder (OABs) syndrome of women in a rural region. Methods: The authors conducted a questionnaire survey among the 322 females who voluntarily participated in a health examination, from 16 to 18 August 2001. Using by definition of UI and OABs, we calculated the proportion of UI, OABs I, and OABs II. The data collected was evaluated using the SPSS 12.0 statistical package, and the differences of symptoms and problems related to daily life between UI, OABs patients and the others were analyzed using a Chi-square test or Fisher's exact test. Results: The overall proportion of UI was 35.4%, and stress UI (32.9%) is more common than urgency UI (17.4%) and mixed UI (14.9%). The proportion of OABs I and OABs II were respectively 36.0%, 14.0%. Symptoms related to UI or OABs were nocturia (35.1%), frequency (23.9%), urgency (21.4%) etc. Of the incontinence cases, 27.2% had experienced UI for a period of one to three years. The proportion of OABs increased significantly by age (p<0.05), UI didn't. The most frequent symptoms in UI and OABs were respectively 'slow stream', 'urgency'. The most frequent problem of daily life in UI and OABs was 'seeking toilet firstly at stranger place'. Conclusions: The proportion of UI and OABs in our study were respectively 35.4%, 14.0%. UI and OABs must be very significant health problems in women, especially rural region. Systemic and profound interventions for UI and OABs need to administer to women in Korea.

  • This study is designed to explore the effects of symptoms severity and stress on depression in patients with overactive bladder syndrome in the middle age. The subjects of the study were 167 middle-aged patients aged 40 to 60 who were diagnosed with overactive bladder syndrome in outpatient clinics and hospitals located in G Metropolitan City. The data analysis was using the SPSS 23.0 program and frequency analysis, Pearson's correction, t-test, and multiple regulation. The study found that the factors that affect depression in patients with the disease in middle age are symptom severity, age, living with a spouse. Therefore, in order to reduce the subject's depression, it is necessary to develop a program that can educate active treatment induction, lifestyle habit correction, and behavior therapy, and perform customized nursing interventions according to age group and spouse presence.

Symptom Bother, Physical and Mental Stress, and Health-related Quality of Life in Women with Overactive Bladder Syndrome (과민성방광증후군 여성의 증상 심각도, 스트레스 및 건강 관련 삶의 질)

  • Purpose: The objective of this study was to identify the relationships among symptom bother, physical and mental stress and health-related quality of life (HRQoL) in women with overactive bladder (OAB) syndrome. Methods: The participants were 106 women who were diagnosed with OAB (urgency, urge urinary incontinence, frequency, and/or nocturia) at P university hospital. Data were collected from Dec 23, 2011 to Aug 31, 2012. Results: The mean score for symptom bother was 43.1 points, for physical stress, 12.8 which was slightly higher than mental stress (11.8), and for HRQoL, 63.9. For symptom type, there were statistically significant differences in the symptom bother (F=8.67, p<.001) and HRQL (F=3.32, p=.023). The Symptom bother of OAB was positively correlated with physical stress (r=.23, p=.014) and mental stress (r=.33, p<.001) and negatively correlated with the subscales of HRQoL; coping (r=-.66, p<.001), concern (r=-.71, p<.001), sleep (r=-.59, p<.001), and social interaction (r=-.58, p<.001). Conclusion: From the results, bother symptom was associated with physical, mental stress and HRQoL. These results suggest that nursing intervention programs for OAB should be developed not only to relieve the symptoms but also to reduce stress and improve the quality of life.

Overactive Bladder and Urinary Incontinence in Adult Women: Prevalence and Effects on Daily Life and Sexual Activity (성인 여성의 과민성방광 증후군과 요실금의 실태조사)

  • The purpose of this study was to investigate the prevalence, relative factor and the effects that overactive bladder or urinary incontinence have on women's lives. Data was gathered through an internet survey. The subjects included 3,372 women ranging from 20 years old to 49 years of age. The questionnaire included general characteristics (age, parity, family history, and history of UTI), overactive bladder symptoms (urgency, frequency, nocturia, urgent incontinence), urinary incontinence, discomfort in ADL and sexual activity. The data was analyzed by the SPSS/PC+ program using frequency, $x^2$ test, and multivariate logistic regression. The results were as follows; 1. The prevalence was 12.7 % in overactive bladder and 21.0% in urinary incontinence. 2. 19.2% of the subject have urge incontinence, 5% of them have coped with the use of a pad. 3. The significant factors to overactive bladder were age($x^2$=6.6, p<0.05), history of urinary tract infection ($x^2$=50.8, p<0.01) and family history ($x^2$=26.1, p<0.01). The significant factors to urinary incontinence were age ($x^2$=6.2, p<0.05), occupation ($x^2$=11.0, p<0.05), history of urinary tract infection($x^2$=20.2, p<0.01), parity ($x^2$=8.6, p<0.01), and family history ($x^2$=4.9, p<0.05). 4. Overactive bladder impacts on individual daily life was 5.0 times, urinary incontinence was 2.9 times higher than in non-symptoms. Also, overactive bladder impacts on their sexual activity disturbance was 4.3 times, urinary incontinence has 3.9 times higher than in non-symptoms. In conclusion, overactive bladder symptoms and urinary incontinence were health problems that disturbed women's lives. Also the most problem among overactive bladder may be urgency in Korean. Intervention based on these results needs to be provided for these women.

  • Objective : The purpose of this case series was to investigate the effectiveness of GagamZihwang-tang for enuresis and frequent urination. Method : We prescribed GagamZihwang-tang and provided acupuncture therapy weekly. Results of before and after therapy were evaluated and scored twice a month. Results and conclusion : The study showed decrease of urination frequency from 17th-20th a day to 6th a day.

A Case of Urologic Manifestation of IARS2-associated Leigh Syndrome (IARS2 유전자 연관 리 증후군(Leigh syndrome) 여아에서 방광기능장애 증례)

  • Leigh syndrome is a rare progressive neurodegenerative mitochondrial disorder with clinical and genetic heterogeneity. Recently, balletic IARS2 variants have been identified in a number of patients presenting broad clinical phenotypes from Leigh and West syndrome to a rare syndrome CAGSSS characterized by cataracts, growth hormone deficiency, sensory neuropathy, sensorineural hearing loss, and skeletal dysplasia syndrome (OMIM#616007). We describe a child with Korean Leigh syndrome with urologic manifestations resulting from a compound heterozygote mutation in IARS2. A 5-year-old girl visited the emergency room with a complaint of abdominal pain accompanied by abdominal distension. Abdominal-pelvic CT showed a markedly distended urinary bladder without definite obstructive lesions. She was diagnosed with neurogenic bladder dysfunction based on a urodynamic study. She had global delayed development due to neurologic regression after 6 months of age and a history of bilateral cataract surgery at the age of 2 years. Her brain magnetic resonance imaging showed symmetrically increased signal intensities in the bilateral putamen and caudate nuclei with diffuse cerebral atrophy. No gene variants were identified through whole-mitochondrial genome analysis. Whole exome sequencing was performed for diagnosis, and compound heterozygous pathogenic variants were identified in IARS2: c.2446C>T (p. Arg816Ter) and c.2450G>A (p. Arg817His). To the best of our knowledge, this is the first case report of bladder dysfunction manifestation in a patient with IARS2-related Leigh syndrome. Thus, it broadens the clinical and genetic spectrum of IARS2-associated diseases.

A Clinical Case Report of Soyangin Patient Diagnosed as Sjögren's Syndrome with Irritable Bladder (과민성 방광을 동반한 쇼그렌 증후군 소양인 환자 치험 1례)

  • Introduction This study aims to report a significant improvement of Sjögren's syndrome patient with irritable bladder, diagnosed as 'Soyangin chest-binding symptomatology' based on Sasang medicine. Methods The patient had treatment with 'Dojeokganggi-tang' and western medications. We evaluated the treatment outcome of Sjögren's syndrome symptoms by using a numerical score of 0 to 10, based on the patient's subjective discomfort of dry eye and dry mouth, and irritable bladder symptoms by recording the number of night urination every month. Results Sjögren's syndrome symptoms were significantly improved, and the number of night urination was decreased. After 41 weeks of treatment, the patient didn't have dry mouth any more and had only a little dry eye. The number of night urination was decreased from 3-4 times to once a night. Discussion The patient diagnosed with Sjögren's syndrome, treated with Dojeokganggi-tang and western medications for 41 weeks, showed a significant improvement in dry eye, dry mouth, and night urination compared to the first visit.

  • Objective: Overactive bladder syndrome refers to the symptom of frequent urination, as the function of the bladder is so sensitive that the bladder muscles contract regardless of the patient's intent and the patient feels the need to urinate quickly. Overactive bladder syndrome is common among the elderly, especially in women, and the prevalence increases with increasing age. Overactive bladder can decrease social activity, increase isolation, and decrease quality of life due to sleep disorders caused by night urination; therefore, active treatment is required. In this study, we report the effects of Korean medical treatment on a Korean hospital patient with overactive bladder symptoms, including urine frequency, urine incontinence, and night urination that began at a similar time to an onset of coughing. Methods: We treated the patient with herbal medicines and acupuncture therapy. The International Prostate Symptom Score (IPSS), Overactive Bladder Symptom Score (OABSS), and a numeric rating scale (NRS) were used to assess symptom changes. Results: The patient with urine frequency, urine incontinence, and nocturia was hospitalized for 8 days, and the overall symptoms of overactive bladder syndrome were improved. Conclusion: The results indicate that Korean medical treatment is effective in patients with overactive bladder syndrome.

Clinical Manifestations and Characteristics in Patients with Horseshoe Kidney (소아 및 성인 마제신 환자들의 임상적 특징과 비교)

  • Purpose: We aimed to investigate the clinical characteristics and associated diseases in children with a horseshoe kidney and compared these data between children and adults. Method: We performed a retrospective analysis of the medical records and radiological findings of 43 patients diagnosed with a horseshoe kidney in the Busan Paik Hospital. The subjects were divided into the children's group (14 cases, age <18 years) and the adult group (29 cases, age ${\geq}18$ years). Results: The study group consisted of 17 males and 26 females with a median age of 34 years. In the children's group (14 cases), 5 subjects were male and 9 were female, with a mean age of $6.7{\pm}6.2$ years. Most of the subjects were asymptomatic and were incidentally diagnosed with horseshoe kidney during their evaluation for another disease. Among the associated diseases in the children's group, Turner syndrome was the most common (5 cases), whereas ureteropelvic junction (UPJ) stricture was observed in 2 cases (14.2%). None of the children exhibited abnormal renal function during the follow-up period. In the adult group (29 cases), 12 subjects were male and 17 were female, with a mean age of 48 years. Eighteen patients were incidentally diagnosed with horseshoe kidney during their evaluation for another disease, and 11 patients had hematuria or abdominal pain due to renal stones. Among the associated diseases in the adult group, Turner syndrome was the most common (5 cases), and UPJ stricture was observed in 5 cases; the other accompanying diseases included hydronephrosis and overactive bladder. Six patients exhibited decreased renal function (serum creatinine level >1.5) during the follow-up period. Conclusion: Horseshoe kidney is usually diagnosed incidentally in both children and adults. In the present study, we noted that Turner syndrome was the most common associated disease in children. In addition, most children were asymptomatic but had a high risk of urologic complications after the transition to adulthood. Therefore, children with horseshoe kidney require continuous follow-up.
