노인 연령층에서 비교적 흔히 발생하는 골절로는 척추압박골절과 고관절 및 손목의 골절이다. 65세이상 여성의 33~50%에서 1곳 이상의 척추압박골절이 발생하고 80세 이상 노인의 약 17%에서 고관절 골절이 발생한다. 특히 여성 노인들은 골절 발생 후 3개월 이내 20%에서 사망하고 생존자의 50%는 영구히 보호시설에서 지내게 된다는 점에서 큰 의미를 지니고 있다고 하겠다.

Purpose: We wanted to assess the radiological and clinical results and the prognostic factors after an operation for capitellar fractures associated with/without other injury around the elbow. Materials and Methods: Among the 25 patients (mean age: 49 years-old) who underwent open reduction and internal fixation for capitellar fractures, there were nineteen type 1 fractures and six type 3 fractures. The mean follow up period was 14.8 months. We assessed the factors affecting the radiological and functional results, such as the fracture pattern, the patient age and the surgical approaches. Results: In 24 of 25 patients, bony union was achieved at postoperative 1 year. There were eighteen excellent, four good, two fair and one poor functional results according to the Broberg and Morrey elbow score. The most common type was type 1 and the most common associated injury was lateral condylar fracture. The patients with type 1 fracture rather than the patients with type 3 fracture and the patients who had an extraarticular associated fracture rather than an intraarticular associated fracture had better clinical outcomes. Conclusion: 22 (88%) of the patients were satisfied at the result. The type of capitellar fracture and an associated intraarticular elbow fracture were shown to be important prognostic factors in this study.

Vitamin D is one of important factors involved in the regulation of bone metabolism. In osteoporosis, the therapeutic effect of vitamin D on the healing process of fracture has still been controversial. These studies were designed to understand the healing process of normal fibular fracture and the therapeutic effects of $1\alpha$, 25 dihydroxycholecalciferol on the osteoporotic fracture in rats. The simple transverse fractures of rat fibulae were produced with a rotating diamond saw. The histological and ultrastructural changes of rats were observed. The histological and ultrastructural studies revealed the healing of the fibular fracture in the 5th week after simple transverse fracture. The osteoporosis impaired more the healing of osteoporotic fibular fracture than normal non-osteoporotic fibular fracture. The healing process of osteoporotic fracture was facilitated by the treatment with $1\alpha$, 25 dihydroxycholecalciferol, however, was delayed more than the healing process of normal fracture. These results suggest that $1\alpha$, 25 dihydroxycholecalciferol was effective for reducing the deleterious effects of osteoporosis in fracture healing.

중안면부의 골절은 심미적으로나 기능적으로 많은 문제를 야기할 뿐아니라 두개부와 연관되어 뇌손상의 가능성을 가지므로 생명과 직접적인 연관을 갖는다고 할수 있다. 산업사회의 고도의 발달로 인하여 자동차의 보급이 증가하고 그에 따라 교통사고의 절대수가 증가하고 안면부 골절환자가 증가추세에 있다고 볼 수 있다. 때문에 구강악안면외과의사로서 중안면부의 골절에 관심을 기울여야 할 것으로 생각된다. 중안면부 골절 가운데 비골을 포함한 안와 근심벽의 쉽게 일어나며 골절에 포함된 해부학적 구조물들이 복잡하기 때문에 골절후에 기능적 이상이 쉽게 발생될 수 있다. 본 과에서 처치한 중안면 골절 환자중 비골을 포함한 안와 근심벽의 골절환자를 경험한 바 이에 보고하는 바이다.

본 논문에서는 ART2를 적용하여 X-Ray 영상에서 손목 골절을 추출하는 방법을 제안한다. 제안된 방법에서는 X-ray 영상에서 손목에서의 요골을 추출하기 위해서 요골 및 척골 부위를 ROI 영역으로 설정한다. 설정된 ROI 영역에서 명암 대비를 강조하기 위해 사다리꼴 형태의 Fuzzy Stretching 기법을 적용한다. 사다리꼴 형태의 Fuzzy Stretching 기법이 적용된 ROI 영역에 ART2 기법을 적용하여 요골 및 척골 영역에서 골절이 존재하지 않은 영역을 제거한다. 골절이 존재하지 않은 영역이 제거된 ROI 영역에 다시 ART2 기법을 적용하여 골절의 후보 영역을 추출한다. 추출된 후보 골절 영역을 라벨링한 후, 뼈의 가장자리에 존재하는 골절 후보 영역을 제거한다. 그리고 남아 있는 골절 후보 영역 중에서 가장 큰 두 개의 영역을 골절 부위 영역으로 판단하여 최종적으로 골절 부위 영역를 추출한다.

Although acute traumatic fractures of the clavicle are relatively common, stress fractures of the proximal clavicle are extremely rare. Stress fractures of the clavicle have often been reported after a radical neck dissection or radiation but rarely occur during excessive repetitive exercise in professional athletes. The authors report a case of a stress fracture of the proximal clavicle during exercise in a young man with no specific preceding factors, which has not been reported in the Korean literature.

겨울철에는 낙상사고가 다른 계절보다 3배 이상 높게 나타난다. 추운 날씨 주머니에 손을 넣고 걷다 빙판길에서 미끄러지는 일이 빈번하다. 문제는 노인들의 골절이다. 손목, 척추, 고관절(엉덩이뼈) 골절을 입은 환자는 독립적 보행이 불가능해지며 일상생활에 큰 어려움을 겪게 된다. 특히 고관절(엉덩이뼈) 골절의 경우 1년 내에 사망할 확률이 무려 20%다. 고령 환자의 경우 고관절 골절을 입고 병상에 누워 지내는 기간이 길어지면 심폐 및 방광기능이 저하될 수 있으며, 욕창이나 혈전증 등이 발생할 수 있다. 골절이 노인에게 치명적인 이유다.

To avoid external facial scar which resulted from ordinary infraorbital and lateral eyebrow incision in surgical treatment of zygomatic tripod fractures, the author used intraoral incision and K-wire fixation method in 37 selected patients. This method contraindicated for the patient with multiple comminuted zygomatic fracture, patient combined with hard palatal fracture, and patient combined with herniated orbital floor fracture. The advantages of this method are simple, no external facial scar, and operate under direct visual field. The postoperative results were satisfactory. Balanced facial symmetry and good cosmetic results were obtained.

Patellar fractures in children represent less than 2% of all patellar fractures including adults, and occur about 1% of all pediatric fractures. Because of property of the growing patella, osteochondral or avulsion fractures are more common in children and the most common type of patella fracture in children is sleeve fracture. Avulsion or sleeve fractures of the patella in children can occur at the superior or inferior pole of the patella. Most reported cases of sleeve fracture are involving the inferior patellar pole, but involving the superior patellar pole is very rare. We experienced a case of sleeve fracture occurred at the superior pole of the patella in a sixteen-year-old boy and report this case with literature review.
