통증을 동반한 족무지 종자골의 골연골종: 증례 보고 (Painful Osteochondroma of the Hallucal Sesamoid: A Case Report)

  • Osteochondroma is one of the most common bone tumors. It can occur anywhere, although it is most frequent mainly around the metaphysis of long bones. Prediction sites are distal femur, proximal humerus, proximal tibia, and so on. However, osteochondroma in sesamoid is very rare. Herein, we report a case of a 56-year-old woman with symptomatic extra-articular osteochondroma in hallucal sesamoid with a brief literature review.

  • The osteochondroma, also known as osteocartilaginous exostosis, is one of the most common benign tumors of the axial skeleton. These tumours rarely affect the facial skeleton and a true osteochondroma of the mandibular condyle is a very uncommon entity. The tumors are usually slow growing, and symptoms develop over a long time. The most common presentation of the condylar osteochondroma consists of a changing occlusion, the development of facial asymmetry, and a posterior open bite on the affected side. In this case, we treated an osteochondroma of Lt. mandibular condyle through excision of the lesion via the temporal approach to the temporomandibular joint.

  • Osteochondroma is one of the most common benign tumors of the axial skeleton, but is rarely found in the facial bones and extremely rare in mandibular condyle. The clinical signs and symptoms of osteochondroma of mandibular condyle may resemble those seen in patients with temporomandibular joint (TMJ) dysfunction. Condylectomy have been the first choice for treatment of osteochondroma, but it may be with some complicaitons, loss of condylar vertical height, etc. A 57 years old female patient who had an osteochondroma on left mandibular condyle visited to our clinic. We did surgically remove the mass with favorable result, so we present the case with review of literatures.

  • An osteochondroma is an osseous protuberance with cartilaginous growth potential, usually arising in skeletal bone and relatively uncommon in the craniofacial bone. Osteochondroma of the craniofacial region usually occurs at the condyle or the tip of the coronoid process, and rarely arises in the mandibular body, symphysis, ramus, and similar areas. Excision of the lesion including the periosteum is curative, and recurrence or malignant change (usually to a chondrosarcoma) after treatment is rare. We present an atypical case of osteochondroma in the left mandibular inferior border with review of literature.

족저부에 발생한 골외 골연골종 - 1례 보고 - (Extraosseous Osteochondroma of the Foot - A case report -)

  • Although osteochondromas are common bone lesions, extraosseous osteochondromas are rare. The most common sites of involvement are the hand, knee, and foot. The authors experienced a case of soft tissue osteochondroma in the plantar aspect of the left foot. The patient complained of a palpable mass for 5 years. The mass grew slowly and had been asymptomatic until 5 months prior to admission when the patient developed slight pain on ambulation and numbness distal to the mid foot. Radiologic studies demonstrated well-circumscribed, lobulated, ossified mass in the plantar aspect of the mid foot. Histological examination confirmed a osteochondroma of soft tissue origin.

거골 내측 결절에 발생한 골연골종과 통풍 발작에 의한 발목 내측 충돌 증후군: 증례 보고 (Medial Ankle Impingement Syndrome due to Talar Osteochondroma and Gout Attack: A Case Report)

  • Osteochondromas are benign bone tumors typically found in the metaphyseal region of long bones. These tumors are often asymptomatic and detected incidentally. However, their occurrence in atypical sites such as the talus can pose significant diagnostic and treatment challenges. This report describes a rare case of osteochondroma of the medial tubercle of the talus, which is an unprecedented location based on a review of relevant literature. A 28-year-old male presented with worsening medial ankle pain and limping. Imaging revealed a lesion consistent with osteochondroma contributing to medial ankle impingement syndrome. Uniquely, this case also featured a coinciding gout attack in the ankle joint. Surgical removal of the lesion resulted in significant symptom relief and functional improvement. This case underscores the need to consider rare diagnoses, such as talar osteochondroma, when presented with persistent medial ankle pain and highlights the potential presence of concurrent conditions, such as gout.

족부 및 족관절에 발생한 종양 (Tumors of the Foot and Ankle)

  • 목적 : 족부 및 족관절에 발생하는 종양은 다른 부위에 비해 그 발생 빈도가 낮으며, 특히 악성 종양은 매우 드물고 양성 종양으로 간과되는 경우도 있어 족부 및 족관절에 발생한 종양의 임상 양상에 대해 알아보고자 하였다. 대상 및 방법 : 1989년부터 1998년까지 10년 동안 족부 종양으로 진단받은 72명을 대상으로 조사를 시행하였다. 결과 : 남자가 40예, 여자가 32예였다. 양성 종양이 53예(74%)로, 연부 조직 종양이 30예, 골 종양이 23예였다. 악성 종양은 19예(26%)였으며, 연부조직 종양이 17예, 원발성 골종양이 1예, 전이성 골종양이 1예였다. 양성 종양은 연부 조직에서는 섬유종이, 골종양에서는 골연골종이 가장 많았다. 악성 종양은 악성 흑색종이 7예로 가장 많았다. 호발 부위는 양성종양의 경우 족지부, 악성 종양의 경우 발 뒤꿈치 부위였다. 악성 종양 19예 중 국소 재발은 3예, 원격 전이는 8예에서 발생하였다. 결론 : 악성 종양이 26%였으며 원격 전이가 42%로 높았다. 따라서, 족부 종양의 경우 악성의 가능성을 염두에 두고, 치료 시에 병리학적인 확진이 필수적이라 하겠다.

  • Osteochondroma is one of the most common benign tumors of the axial skeleton, but is rarely found in the facial bones. Osteochondroma shows an irregular radiopaque lesion and chondromatic area surrounded by osteoma. It may appear different findings as calcification levels. When it develops in the long bone, it has a marked tendency in the ages from 10 to 20 years and ceases with the end of pubertal growth. However, when it develops in the condyle, it is prevalent in the third decades (average 39.2 years) and continues to develop. Lesions developed in the long bone have a predilection for men (M:F = 2:1), but for women in the mandible. Osteochondroma is differentiated from chondroma, osteochondromatosis and osteoma. Mandibular condyle osteochondroma presents asymptomatic facial swelling, rarely posterior openbite, pain during mouth opening and internal derangement of the temporomandibular joint disc due to condylar lengthening and condylar hyperplasia. The first choice of treatment of the massive osteochondroma is the surgical removal. We report osteochondroma of the mandibular condyle showing good result to treat the lesion.

  • The osteochondroma is a cartilage-capped exostosis resulting from an error in the regulation of normal chondrocyte proliferation and maturation that leads to a normal bone growth. Although exostoses are benign lesions, they are often associated with characteristic progressive skeletal deformities and may cause clinical symptoms. Surgery can prevent progression and provide correction for certain deformities. We experienced a rare case of solitary osteochondroma in a 21-year-old male which caused the valgus deformity of the ankle.

관절원판의 유착을 동반한 하악과두의 골연골종의 치료: 증례보고 (Treatment of Osteochondroma of the Mandibular Condyle with Inferior Adhesion of Meniscus: A Case Report)

  • Osteochondroma is a benign neoplasm, osseous projection surrounded with cartilage, 35.8% of benign osseous tumor, 8.5% of whole osseous tumor and usually arises from the skeletal bone. Osteochondroma is a cartilaginous derivation and relatively uncommon in the craniofacial bone. Osteochondroma of the mandible has slow growth rates which mainly affect women around forty years of age and it can appear through the coronoid process and mandibular condyle, especially in the medial half. Clinical finding associated with osteochondroma of condyle are primarily a palpable, painless temporomandibular area mass with facial asymmetry, malocclusion and midline deviations. Sometimes pain and dysfunction like trismus often accompany the anatomic derangement. Other features include malocclusion with open-bite on the affected side and cross-bite on the contralateral side. In this study, a 45-years old female patient exhibits pain on the left temporo-mandibular joint area and malocclusion due to loss of the molar region with osteochondroma on the top left of her mandibular condyle head. The patient is able to recover gradually from the symptom through treatments on manipulation, stabilization splint, arthroscopic lavage and surgical excision, thus, this is reported as a clinical case.
