The aim of this study was to compare the clinical performance of 4 types of orthodontic wires, indicated for initial tooth alignment: stainless steel, multistranded steel, superelastic and thermoactivated nickel-titanium. A prospective randomized clinical trial was conducted on a sample of 45 patients, at the Dental School of the State University of Rio do Janeiro, Brazil. Fixed appliances were fitted and study casts were obtained from each patient. Randomly, the wires were allocated as follows: 26 dental arches for superelastic NiTi wires, 22 for stainless steel, 22 for multistranded and 20 for thermoactivated archwires. After 8 weeks, the archwires were removed and impressions for study casts were taken again. Using a 3D digitization technique of defined anatomical points on the study cast crowns, a Dental Irregularity Index (DII) was created for each study cast. The difference between DII before and after the archwire insertion expressed the aligning effect of the wires. ANOVA tests were employed to evaluate the anatomical point approximation (positive DII) and separation (negative DII), for each area of the dental arches: upper and lower whole arch and anterior arch. Results showed no significant difference between the different archwires.
Alveolar crest is the section of interproximal alveolar bone which includes the free edge of the alveolar process. An increase of the normal forces within limits of tolerance leads to deposition of new bone. If forces are beyond the limits of tolerance, resorption of bone will result whether the force produces pressure or tension. This study was designed to evaluate changes of alveolar bone levels in mesial and distal surface of the left, right first molar, by using pre-treatment, post-treatment panorama films. Two hundreds sixteen subjects were divided into adolescent group of 104 subjects and adult group of 112 subjects, to which orthodontic treatment with a bicuspid extraction (adolescent group-50 subjects, adult group-50 subjects) or without a nonextraction (adolescent group-54 !subjects, adult group-62 subjects) was applied by fixed appliances. Pre- and post-treatment Panorama films were traced, and alveolar crest height was measured. Amounts of changes in alveolar crest height by treatment were calculated md compared in terms of side of tooth, extraction, age. The results were as follows ; 1. When pre-treatment alveolar crest bone levels were compared, levels of adult group were significantly lower than those of adolescent group. 2. Post-treatment alveolar crest bone levels were significantly lower than pre-treatment levels. 3. When changes of alveolar crest height were compared, between adolescent and adult group were not significantly. 4. When changes of alveolar crest height were compared, significantly larger changes were noticed in ex윤action than nonextraction cases. 5. When changes of alveolar crest height were compared, significantly larger changes were noticed in maxilla than mandible. 6. When mesio-distally compared, significantly larger changes were observed in the distal than mesial sides of adult group.

Down syndrome (DS) is a genetic disease known as trisomy 21. Congenital cardiac anomalies are present in about 40% of DS patients. Dental anomalies are also common among DS patients. In DS patients, canine impaction is 10 times more frequent and transposition of maxillary canine and first premolar is 50 times more common than in general population. A female DS patient with congenital heart disease was diagnosed as having a transposed impacted maxillary canine. Sectional fixed appliance with Nance holding arch was used for the orthodontic treatment. After space was regained for the eruption of the canine, orthodontic button was attached using flap operation with closed technique. Traction and alignment of the tooth followed. To prevent endocarditis, prophylactic antibiotics were prescribed for the recommended dental procedures. Total treatment time was 25 months and no complication was found.

Impacted molars are not usual with the prevalence rate of less than 1%, however problems such as infraocclusion, extrusion of the opposite tooth and lack of space are caused. If molars are impacted, radiographic check-up and observations are needed as they play an important role in skeletal development and mastication. Causes of impaction are underdevelopment of roots, supernumerary teeth, cysts, odontomas, ankylosis and etc. In our first case, we present impacted molar which was surgically exposed and orthodontically traded. As the developmental state of the root was less than half at first visit, we observed until the root was 2/3 in its length and traction was carried out. The second case is orthodontic traded molar with compleate enucleation of the existing cysts. When trading impacted molars, direct bonding of attachments to the tooth and light continuous forces are recommended so as to ankylosis, external absorption and periodontal attachment loss avoid. Fixed appliances tract the teeth on arch and obtain eruption spaces without patient's cooperation. We report surgically exposed and orthodontically traded molars which resulted in good occlusion and patients satisfaction.
Fixed orthodontic appliances for the treatment of malocclusion has iatrogenic side effect such as demineralization of enamel, gingivitix and gingival hyperplasia. The purpose of this study is to longitudinally investigate the salivary microorganisms and immunoglobulin A after delivery of fixed orthodontic appliances for 10 months. Eight orthodontic patients were included in this study and the author has investigated the numbers of general bacteria, Streptococcus mutans Staphylococcus aureus and concentration of immunoglobulin A from unstimulated whole saliva. The author examined these parameters at prebracketing, 1 month after, 4 months after, 7 months after and 10 months after delivery of fixed orthodontic appliances. The obtained results were as follows : There were significant increases in the number of salivary general bacteria, Streptococcus mutans and Staphylococcus aureus after delivery of fixed orthodontic appliances The numbers of general bacteria were significantly increased at 1 month after (p<005), 4 months after (p<0.05), 7 months after (p<0.01), compared with prebracketing. However it showed no difference at 10 month after compared with 7 months after bracketing. The Numbers of Staphylococcus aureus were significantly increased at 1 month after (p<0.05), 4 months after(p<0.01), 7 month(p<0.01), compared with prebracketing. However it showed decreasing pattern at 10 months after compared with 7 months after bracketing. There was no significant difference in the concentration of immunoglobulin A after delivery of fixed orthodontic appliances.
Objective: The purpose of this study was to evaluate clinical and microbiological changes in periodontal tissue around the banded molars after debanding. Methods: This study included 17 young adult patients treated with fixed orthodontic appliances including bands on the last molars more than 1 years. Probing depth and bleeding frequency were measured and plaque samples were collected from the last banded molars in all quadrants of each patient. All the data were collected immediately after debanding and 4 weeks after debanding. Results: Using polymerase chain reaction based on 16S rDNA, the presence of Porphyromonas gingivalis, Tannerella forsythia and Treponema denticola was detected. After debanding, probing depth, bleeding frequency, and prevalance of periodontopathogens were reduced. Probing depth and bleeding frequency were most decreased in the buccal site of the mandibular left molar and were least decreased in the lingual site of the maxillary right molar. Conclusion: The results of this study indicated that proper management of oral hygiene after debanding can recuperate unfavorable periodontal condition caused by orthodontic treatment.
Retrostpective study of two groups of patients was conducted to evaluate the physiologic drift of the mandibular teeth following the extraction of four first premolars. The concept of physiologic drift, commonly referred to as 'driftodontics', following first premolar extractions has been gaining acceptance in the orthodontic community, the exact nature and amount of drift has not been adequately documented. There were also no guide lines as to when drift should be allowed to now. The purpose of this study was to quantify physiologic drift of the untreated mandibular dentition following extraction of the four first premolars during the early permanent and late permanent dentition stages. The early permanent dentition extraction sample(Group 1) included 26 Patients and the mean age at pretreatment was approximately 13.5 years. The observation period following extraction was approximately 6.96 months. The late permanent dentition extraction sample(Group 2) included 31 patients. The mean age at pretreatment was 21.3 years, followed by a observation period of 7.26 months. During the observation period, except for the extractions, no other mandibular therapy was rendered. Pre-and post-treatment lateral cephalograms and dental casts were analyzed. The obtained results were as follows 1. Group 2 showed marked changes in movements of the mandibular incisors and canines but minimal changes in molars. 2. The amount of changes in movements of the mandibular incisors and canines were significantly greater in Group 1 than in Group 2. The results showed no differences in rates of molar movements between groups. 3. Physiologic drift of the dentition produced desirable changes such as decreased Incisor Irregularity.
Objective: This study was performed to investigate the influences of extraction and nonextraction treatment on smile esthetics by measuring dental arch width changes. Methods: Pretreatment and posttreatment study models of 30 first premolar extraction cases and 30 nonextraction cases were randomly selected to determine whether extraction treatment results in narrow dental arches, and a consequent unaesthetic smile. Arch widths were measured from the cusp tips of the canines and the first molars. Posterior arch widths were also measured at a constant arch depth derived by averaging randomly chosen nonextraction models. Results: The intercanine widths increased significantly in the extraction sample, whereas the intermolar widths decreased significantly. The arch width at a standardized arch depth was significantly wider in the extraction subjects. Conclusion: These results elucidate that constriction in arch width is not a materialized consequence of extraction treatment. It leads to postulate that an esthetically compromising effect from narrow dental arches on smile is hardly anticipated with extraction treatment.

Purpose: The purpose of this study was to investigate the differences of displacement pattern depending on type of sliding jig and application method during maxillary molar distalization with temporary anchorage devices (TADs). Materials and Methods: Maxilla with normal tooth size and arch shape was selected to create a 3-dimensional finite element model, which included the bracket, orthodontic main archwire, removable sliding jig (R-jig). The orthodontic mini-implant anchorage was set 8 mm superiorly from main archwire, buccally between the second premolar and first molar. The base experimental design was Condition 1, which was composed $0.019{\times}0.025$ inch stainless steel (SS) of wire size of R-jig, 200 gm force, un-tied state. And the other designs varied to wire size of R-jig, magnitude of force. The results are as follows. Results: As the wire size of R-jig was increased, the deformation of R-jig was decreased. However, the displacement of second molar wasn't different each other. As the force to second molar was increased, the more displacement of second molar was observed, and the more distal tipping movement, vetical displacement was observed. Conclusion: R-jig can get distal teeth movement in orthodontic treatment without side effects.

Extracting mandibular incisors for orthodontic treatment may adversely affect the occlusion. However, when properly used, extraction of mandibular inciors is a selection for the correction of the malocclusion. Generally, treatment for crowding needs to select between nonextraction and four premolar extraction. Approaches for crowded mandibular incisors include distal movement of posterior teeth, lateral movement of canines, labial movement of incisors, interproximal enamel reduction, removal of premolars, removal of one or two incisors, and various combinations of the above. Extraction of incisors is used in case of crowding, anterior tooth size discrepancy, absent of maxillary lateral incisors, and ectopic eruption. But severe overjet. overbite, and space are the contraindication of it. A patient had severe crowding on upper anterior teeth, impacted upper left lateral incisor, palatal ectopic eruption of upper right incisor and severe crowding on lower anterior teeth. Lower lateral incisors are extracted for space availability and facial esthetics. We report the case of orthodontic treatment of upper and lower anterior crowding through extraction of lateral incisor.
