Staphylococcal Scalded Skin Syndrome in a Healthy Adult: Easy to Misdiagnose (건강한 성인에서의 오진하기 쉬운 포도구균성 열상 피부증후군의 치험례)

  • A 60-year-old male presented with a three-month history of redness and swelling on his left little finger. His medical history was not informative. Wound culture revealed methicillin-resistant Staphylococcus aureus. After vancomycin administration, the skin lesions became worse and whole body bullae and desquamation occurred. This was initially suspected to be a drug eruption; thus, we switched antibiotics from vancomycin to teicoplanin. However, biopsy revealed Staphylococcal scalded skin syndrome (SSSS). After several days, generalized skin symptoms improved. The patient recovered and is in good physical health without recurrence six months later. We describe a localized form of SSSS, which is very rare in healthy adults. Consequently, there is a high risk of misdiagnosis. Thus, we report a rare case of SSSS in a healthy adult and the importance of early histological examination for accurate diagnosis.

Influence of Trauma Experiences and Social Adjustment on Health-related Quality of Life in North Korean Defectors (북한이탈주민의 외상경험과 사회적응이 건강 관련 삶의 질에 미치는 영향)

  • Purpose: This study was conducted to identify the influence of trauma experiences and social adjustment on health-related quality of life in North Korean defectors. Methods: The subjects were 117 North Korean defectors living in B and Y city. The data were collected from September 1 to September 20, 2014, and analyzed by the SPSS/WIN 18.0 program. Results: Significant differences were found in the health-related quality of life with respect to the subjects' education, marriage status, types of dwelling, family types, duration of residence in South Korea, jobs in South Korea and in North Korea, families left in North Korea or other surrounding countries, and subjective health status. Health-related quality of life is negatively related to trauma experiences during escape from North Korea and during their life in South Korea. Health-related quality of life positively is related to social adjustment. The meaningful variables which influenced the subjects'health-related quality include social adjustment, job in South Korea, job in North Korea, current family structure, and trauma experiences in South Korea. Total explanatory power of these factors for health-related quality in North Korean Defectors is 43.0% and social adjustment is the most influential factor. Conclusion: Therefore, in order to enhance health-related quality of life in North Korean defectors, it is necessary for them to increase social adjustment and decrease trauma experiences. In addition, methods are needed to provide job opportunities, better education and family services for North Korean defectors.

Weight Loss Experience of Highly Obese Young Adult Who Underwent Bariatric Surgery (비만대사수술을 받은 고도비만 청년들의 체중감량 경험)

  • Purpose: This study aimed to explore the weight loss experience of highly obese young people who underwent bariatric surgery. Methods: Data were collected, from October 2021 to April 2022, through individual in-depth interviews with eight highly obese young adults who underwent bariatric surgery and analyzed using Colaizzi's phenomenological method. Results: The following four theme clusters were extracted. First, 'The unbearable sorrow of obese people' explains the process of choosing bariatric surgery after fluctuations and frustration despite countless repeated weight loss attempts. Second, 'The impact aftermath of bariatric surgery' revealed participants struggling in the face of unexpected changes due to bariatric surgery. Third, 'Efforts to rebuild a healthy myself' demonstrated the struggle to regain health by using the surrounding support system to overcome the difficulties of this surgical procedure. Lastly, 'Facing life by getting rid of the yoke of obesity' displayed the experiences of the participants who felt ambivalent about their changed appearance, but coping positively with the new life. Conclusion: This study provides an in-depth understanding of the weight loss experiences of highly obese adolescents who underwent bariatric surgery and presents a new perspective on clinical practice. The results of this study are expected to be useful in developing and applying systematic and customized nursing interventions before and after bariatric surgery.

Spontaneous Degenerative Rupture of Extensor Hallucis Longus Treated with a Split Tibialis Anterior Tendon Autograft: A Case Report (전경골건 부분 이식을 이용한 장족무지신건 퇴행성 파열의 치료 사례)

  • Chronic extensor hallucis longus (EHL) tendon rupture is relatively rare, but in such cases, surgical repair is necessary to prevent hallux dysfunction. To the best of our knowledge, reconstruction of chronic EHL rupture using a split tibialis anterior tendon autograft has not been previously reported. Here we present a case of spontaneous EHL tendon rupture with a 5 cm gap in a healthy 57-year-old woman. At the 1-year follow-up evaluation, hallux function was restored, and the patient was well satisfied with results.

Research Areas and Trends in Articles on Pediatric and Adolescent Tuberculosis in Korea (국내 소아청소년 결핵 관련 논문의 연구 영역과 동향)

  • Purpose: Despite the decline in tuberculosis (TB) incidence and mortality rates in the Republic of Korea through a national TB control program, TB remains one of the most critical infectious diseases in Korean children. We investigated the trends and research areas of published articles on TB in Korean children and adolescents. Methods: In 6 Korean and overseas databases, we searched titles and abstracts including "tuberculo*" or "TB," "child*" or "adolescen*" or "neonat*" or "infant*" or "pediatric*," and "korea*." The publication type, publication year, research areas, journal title, and research subjects were analyzed. Results: Out of the 257 searched documents, 120 papers were included in the analysis. Of these, 82 were original articles (68.3%), 33 case reports (27.5%), 4 review articles (3.3%), and 1 guideline (0.8%). In the original articles, the most common subject of studies was the clinical characteristics of patients with TB (36.6%), followed by diagnostics (29.3%), contact investigations (9.8%), epidemiology (6.1%), treatment (4.9%), vaccine (3.6%), latent TB infection (3.6%), complications (3.6%), and surveys on perception of TB (2.4%). From 1962, 4 articles were published in the 1960s, 10 articles in the 1970s, 11 articles in the 1980s, 22 articles in the 1990s, 26 articles in the 2000s, and 47 articles since 2010. Conclusions: The amount of research on TB in Korean children has increased over the past 5 decades; however, it has mainly focused on the clinical characteristics and diagnostics. Research in different areas, such as treatment and vaccine, is needed in the future.

Effects of Perception of Death, Terminal Care Stress, and Resilience on the Terminal Care Performance of Clinical Nurses (임상간호사의 죽음인식, 임종간호스트레스와 회복탄력성이 임종간호수행에 미치는 영향)

  • Purpose: This study examined the effects of death perception, terminal care stress, and resilience on the terminal care performance of clinical nurses. Methods: As a correlational study, this study conducted a survey total of 189 nurses working at K university hospital and a P university hospital in B metropolitan city for six months or more. The data was collected from August 20th, 2020 to September 15th, 2020. Data were analyzed using the SPSS, Windows version 25.0, according to the purpose of the analysis by descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficient, and hierarchical multiple linear regression. Results: The factors having effects on the terminal care performance of subjects were resilience, perception of death, and the experience of terminal care education. The total explanatory power of those variables on the terminal care performance was 21.0%. Conclusion: For the improvement of terminal care performance, it would be necessary to develop a systematic and professional terminal care education program that could increase the positive perception of death and resilience, and also include knowledge and skills necessary for terminal care. Also, the efficiency of terminal care performance should be increased through periodic terminal care-related nursing education.

Needs assessment for maternal health care in Ermera, Timor-Leste (동티모르 에르메라 지역의 모성보건사업 요구 분석)

  • Objectives: The purpose of this study is to obtain the basic data for the development of maternal health care by analyzing the status and needs of service target persons in Timor-Leste. Methods: The subjects were selected through the non-probability sampling method applying the FGI. Researchers interviewed 3 maternal health service managers, 6 midwives at Gleno and Railaco Health Centers and 2 women between 15 and 45 years of age. Results: In the results of on-site visit of the delivery facility and the FGI, we found poor sanitation in delivery room, lack of medical equipment related to antenatal consultation and delivery. In the case of the health center manager, the public health center provides various maternity health services, but the lack of the staff has difficulty in providing the service and managing the subjects. Midwives asked for regular maintenance education. Women in child bearing age living in mountainous areas had poor access to delivery facilities and lack of awareness of delivery services. Conclusions: It is necessary to increase the maternity management rate through regular maternity and maternal health check service and application to maternal management database, to improve the sanitation of the maternity clinic in the public health center, to strengthen the midwife competency program.

Factors Affecting the Quality of Life of Family with Terminal Cancer Patients Hospitalized in Hospice Institutions (호스피스 전문기관에 입원한 말기암환자 가족의 삶의 질 영향요인)

  • Purpose: The purpose of this study was to identify factors affecting quality of life among family members of patients with terminal cancers hospitalized in hospice institutions. Method: This study used a descriptive design. The participants were 142 families of patients with terminal cancers from 7 hospice institutions from 6 cities in Korea. Data collection was conducted from January 21 through July 31 of 2017 using self-reported questionnaires. Data were analyzed by descriptive statistics, t-test, analysis of variance, the Pearson correlation coefficient, and multiple regression analysis using the SPSS/WIN 18.0 program. Results: Quality of life was positively correlated with spiritual well-being (r=.49) and negatively correlated with burnout (r=-.59). Spiritual well-being was negatively correlated with burnout (r=-.49). Factors influencing the quality of life were burnout, perceived health status and spiritual well-being, which explained 44% of the variance. Conclusion: The findings of this study indicate that it is necessary to develop a nursing intervention program that can improve the quality of life of family members of terminal cancer patients considering burnout.

The Effects of Nursing Work Environment, Job Stress and Knowledge Sharing on Nursing Performance of Nurses in a Tertiary Hospital (일개 상급종합병원 간호사의 간호근무환경, 직무스트레스, 지식공유가 간호업무성과에 미치는 영향)

  • Purpose: The purpose of this study was to identify factors influencing nursing performance of clinical nurses. Methods: This was a descriptive study involving 172 nurses who worked for 6 months or longer at one general hospital in B City. The data analysis was conducted with t-test, ANOVA, Scheffé test, Pearson's correlation coefficients, and stepwise multiple regression. Results: Nursing performance of participants positively correlated with job stress (r=.16, p=.033). The factors affecting nursing performance were total clinical experience (β=.37 p<.001), salary (β=.19, p=.028), and job stress (β=.13, p=.045). These variables had a 27.2% variance to explain nursing performance. Conclusion: Based on these results, administrative and financial support are required for the development of a clinical career program, including the differentiation of salary systems of clinical nurses. There is also a need to develop and implement job stress management and coping programs.

Effect of Emotional Intelligence, Job Stress, and Communication Ability on Nursing Performance of Nurses Caring for Cancer Patients (암환자를 돌보는 간호사의 감성지능, 직무스트레스, 의사소통능력이 간호업무성과에 미치는 영향)

  • Purpose: This study examined the effect of emotional intelligence, job stress, and communication ability on nursing performance of nurses caring for cancer patients. Methods: This is a descriptive study involving 185 nurses with an experience of longer than 6 months at K university hospital in B metropolitan city. The data was collected from March 2nd 2021 to March 31st 2021, and analyzed using the descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficients, and stepwise multiple regression. Results: The factors affect the nursing performance of participants were emotional intelligence, total clinical career, communication ability, job stress and satisfaction of current department. The total explanatory power of those variables on the nursing performance was 43.8%. Conclusion: In order to improve nursing performance, it is necessary to apply a program for improving emotional intelligence and communication ability, and for controlling and coping with job stress, considering the career of a nurse taking care for cancer patients. In addition, efficient manpower management and material support at the hospital organization level are required.
