Pyloric Obstruction with Advanced Gastric Cancer: Stent vs. Bypass (악성 위출구 폐쇄 치료의 선택: 스텐트 삽입술 혹은 수술적 우회술?)

  • In the past, conservative bypass surgery was usually performed for palliation of malignant obstruction of the gastrointestinal tract. However, endoscopic stenting was developed recently, and technical advances and clinical experience have made it possible to establish stent implantation as one of the main treatment options. There are several advantages in stent implantation over bypass surgery, such as high feasibility and technical success rate, non-invasiveness, rapid symptomatic response, short hospitalization, and cost-effect benefits. Complications, such as stent ingrowth, stent injury by bile or acid, and migration, may occur and early re-insertion is frequently needed. Recently, diverse novel stents which are powered to predict stent migration or ingrowth have been developed and are being used in the clinical setting. In general, stent implantation is known to be beneficial in patients who are expected to survive <6 months, and surgical bypass may be more effective in patients who can survive >6 months. In this review, we have compared the technical feasibility, clinical outcomes, complications, and cost-benefit between stent implantation and bypass surgery, and determined the optimal treatment strategy in malignant upper gastrointestinal obstruction.

A Case of Complete Remission after Palliative Chemotherapy and Salvage Radiotherapy for Lymph Node Recurrence in Advanced Gastric Cancer (근치적 절제술 후 림프절 재발이 발생한 진행성 위암에 대한 고식적 치료 1례)

  • A 54-year-old male patient who was diagnosed with advanced gastric cancer underwent a distal gastrectomy, D2 lymph node dissection, and adjuvant chemoradiotherapy. After a year, in a follow-up PET-CT, lymph node metastases were observed in the neck and abdomen, and therefore, the patient underwent chemotherapy. After treatment, the follow-up PET-CT revealed a growth of the posterior neck lymph node. Thus, an excisional biopsy was performed, and the growth was diagnosed as metastatic adenocarcinoma. Therefore, the patient received chemotherapy with FOLFIRI. Another follow-up PET-CT after chemotherapy revealed a growth in the right inguinal lymph node, and the patient underwent salvage radiotherapy for this lesion. The PET-CT taken for the response evaluation showed no evidence of further metastasis of the lymph node. We hereby report a case of advanced gastric cancer with neck and inguinal lymph node recurrence showing complete remission after palliative chemotherapy and salvage radiotherapy.

  • 원격 전이된 비인두암 환자에서 주요한 치료 방법은 고식적 항암화학치료에 국한되어왔다. 그러나 적극적인 항암화학치료로 비인두암 환자 중 많은 환자에서 치료 반응을 보이며, 치료 반응을 보인 환자군에서는 국소 제어가 중요한 문제가 된다. 본 저자들은 원격 전이 된 환자 1예를 보고하고자 한다. 환자는 큰 크기의 다발성 전이가 있었으나 항암화학요법으로 관해 상태를 보였다. 이후 공고화 항암화학방사선요법을 추가하였고 현재 30개월 간 무병상태이다. 원격전이된 비인두암에서 공고화 항암화학방사선 요법의 역할에 대하여 문헌 고찰을 통해 논의하고자 한다.

Why Do Patients Drop Out During Radiation Therapy? - Analyses of Incompletely Treated Patients - (불완전 방사선치료 환자의 분석)

  • Purpose : This study is to see how much proportion of the patients receiving radiation therapy drop out during radiation therapy and to analyze the reason for the incomplete treatment. Materials and Methods : The base population of this study was 1,100 patients with registration numbers 901 through 2,000 at Department of Radiation Oncology, Samsung Medical Center, Seoul, Korea. Authors investigated the incidence of incomplete radiation therapy, which was defined as less than 95$\%$ of initially planned radiation dose, and the reasons for incomplete radiation therapy. Results : One hundred and twenty eight patients (12$\%$) did not complete the planned radiation therapy. The performance status of the incompletely treated patients was generally Poorer than that of the base population, and the aim of radiation therapy was more commonly palliative. The most common reason for not completing the planned treatment was the patients' refusal of further radiation therapy because of the distrust of radiation therapy and/or the poor economic status. Conclusion : Careful case selection for radiation therapy with consideration of the socioeconomic status of the patients in addition to the clinical indication would be necessary for the reduction of incomplete treatment, especially in the palliative setting.

Extraordinary Response of Metastatic Pancreatic Cancer to Chemotherapy (항암 치료에 좋은 반응을 보였던 전이성 췌장암 증례)

  • A 58-year-old woman presented with right flank and back pain for one month. After undergoing an abdominal computed tomography (CT), she was referred to our hospital. The abdominal CT showed a hypodense pancreatic tail mass with multiple retroperitoneal lymph node metastases. Positron emission tomography-computed tomography (PET-CT) scan showed high 18F-FDG uptake in pancreatic tumor and enlarged lymph nodes. Endoscopic ultrasound fine needle aspiration (EUS-FNA) revealed adenocarcinoma, which stained strongly in hENT1 (human equilibrative nucleoside transporter 1) on immunohistochemistry. She received gemcitabine 1,000 mg/m2 + nanoparticle albumin-bound paclitaxel 125 mg/m2 as a palliative chemotherapy. Follow-up abdominal CT and PET-CT after 4 cycles of chemotherapy showed that both pancreatic mass and the metastatic retroperitoneal lymph nodes were nearly disappeared. We report a case of 58-year-old female with metastatic pancreatic cancer who had a dramatic response to palliative chemotherapy (gemcitabine plus nanoparticle albumin-bound paclitaxel).

  • 방사선치료의 발전으로 3차원치료보다 진보된 호흡연동방사선치료가 시행되어지고 있다. 호흡의 규칙성과 환자의 위치 재현성이 중요한 치료적응 인자이며, 호흡연동 방사선치료의 효율을 높일 수 있는 지표이다. 국가암통계상 고령의 암환자가 증가하며, 수술, 화학요법을 병행하는 암 치료법이 널리 이용이 되고 있다. 고식적인 치료를 요하는 고령의 복부 암 환자분들에 호흡연동 방사선치료법을 사용하는데 에는 호흡의 불규칙성과 체위의 재현성의 문제점으로 인한 치료 효율의 저하를 가져온다. 본 연구에서는 호흡에 의한 종양 움직임이 있는 방사선 치료에서 내부표적체적 기반의 치료계획과 호흡연동 기법을 적용한 치료계획과의 선량비교 분석하였다. 2가지 치료법 모두 정상조직 보호선량에 부합한 것으로 나타났으며 치료체적은 처방선량의 95%이상 포함된 선량분포로 적합하였다. ITV 설정을 통한 3D Plan은 고식적 치료을 목적으로 하는 고령의 환자, 체위 및 호흡의 불안정성 환자에게 처방선량의 95% 이상의 4D Plan의 치료법 보다 짧은 시간에 치료함으로써 치료효율을 높일 수 있을 거라 사료된다. 다만 정상조직보호선량(NTCP)에 부합하는지에 대한 평가가 전제되어야 한다.

Stereotactic Radiosurgery for Recurrent Glioblastoma Multiforme using Yeungnam Localization Device - Technical note and Clinical trial - (뇌정위적 방사선 절제술에 필요한 위치선정용기구 제작과 치험 2예)

  • Authors performed a stereotactic radiosurgery with multiple noncoplanar convergent photon beams of linear accelerator (NELAC-1018 18 MeV, NEC) using a specially designed Yeungnam localization device for two patients with recurrent glioblastoma multiforme. One patient had 2 cm sized and the other 4 cm sized mass on the CT images. After single session of treatment with 15 and 20 Gy, headache was improved in a few days after radiosurgery with no remarkable untoward reactions. Our experience with these two patients were encouraging and we found that our localization device, which is easily adjustable and inexpensive, could be a valuable tool for stereotactic radiosurgery particularly in the treatment of recurrent brain tumor.

  • 기도흡인은 후두의 고유기능중 방어기능을 소실함으로써 지속되는 경우 하기도에 치명적인 손상을 초래하게된다. 기도흡인에 대한 치료는 고식적 치료법과 수술적 치료법으로 대별되는데 일반적으로 고식적으로 치료법이 우선적으로 선택되어지고 있으나 흡인의 정도가 심하여 하기도에 폐렴, 무기폐 등의 위급한 합병증을 유발하거나 그 원인이 비가역적이어서 탈진 또는 악액질을 초래하는 경우 부득이 수술적 요법율 선택하게 된다. 수술적 요법의 선택시기에 대하여는 논란이 많으나 현재까지는 고통을 받고있는 환자의 상태에 따라 결정되고 있는 실정에 있다. 저자들은 최근 5년간 경험한 만성 기도흡인 환자 6례에 대하여 수술적 요법 즉, cricopharyngeal myotomy 1례, cricopharyngeal with medialization laryngoplasty 1례, laryngeal suspension with cricopharyngeal myotomy 2례, laryngeal diversion 1례, total laryngectomy 1례를 각각 시행함으로써 얻은 치료경험을 분석 검토하여 만성 기도흡인에 대한 치료상의 문제점과 보다 효율적인 치료법의 선택에 도움을 얻고자 문헌고찰과 함께 보고하는 바이다.

The Role of Radiotherapy in Patients with Brain Metastasis (고형암의 뇌전이시 방사선치료 효과)

  • Purpose : Brain metastases are the most frequent metastatic neurologic complication of systemic cancer. Even if the prognosis of brain metastases is grave, with available treatments, most patients experience effective palliation of neurologic symptoms and meaningful extension of life. We evaluated the clinical features and prognostic factors of the patients who were diagnosed as solid tumors with brain metastasis and received radiotherapy for brain metastases. Materials and Methods: Between January 1987 and January 1998, 71 patients with brain metastases from solid malignancy were included. We reviewed neurologic symptoms and signs of patients and evaluated improvememt of neurologic symptoms and signs. Survival durations after brain metastasis were ana lysed according to several factors such as age, performance status, primary malignancies, the pre-sence of brain metastasis at initial diagnosis of primary tumor, multiplicity of brain metastass, the presence of metastases other than brain, and treatment method. Results: Frequent symptoms associated with brain metastasis were headache (47.9$\%$), motor weakness (40.8$\%$), nausea and vomiting (19.7$\%$) and mental change (19.7%). Palliation of these symptoms was accomplished in 64.9$\%$ of cases. The overall median survival time was 16 weeks and 1- and 2-year survival rates were 15.0$\%$ and 5.1$\%$ respectively. Patients without extracranial metastases (n=27) had longer median survival than patients with extracranial metastases (33 weeks vs 10 weeks, p=0.0018). In patients with single brain metastasis (n=37), the median survival time was longer in patients treated with surgery plus radiotherapy than in patients treated with radiotherapy alone (40 weeks vs 16 weeks, p=0.0438). Conclusion: Patients who has brain metastases only constitute a prognostically favorable group and they may be benefited from radiotherapy and surgery if indicated.

The Palliative Radiation Therapy in Malignant Extra-Hepatic Biliary Obstruction (간외 폐쇄성 황달 환자에서 고식적 방사선치료의 결과)

  • Purpose : To evaluate the effectiveness of external radiation therapy and the prognostic factors, we retrospectively analyzed therapeutic results of malignant extrahepatic biliary obstruction (EHBO). Methods and Materials : We analyzed the results of the external radiation therapy in 59 patients of inoperable malignant EHBO who had been treated with more than 10 Gy of external radiation therapy from April 1984 to December 1990. There were 21 stomach cancer ($35.6\%$), 12 pancreas cancer ($20.3\%$), 15 extrahepatic biliary cancer ($18.0\%$) and 11 another cancer ($18.0\%$). Their pathologies were confirmed in 31 patients ($52.5\%$). They divided into 27 adenocarcinoma and 4 nonadenocarcinoma. Their chief complaints were jaundice in 47 patients ($79.7\%$) and abdominal pain in 15 patients ($49.2\%$). Twelve patients had slightly increased bilirubin level in liver function test without jaundice. We treated twenty four patients ($40.6\%$) with percutaneous transhepatic biliary drainage (PTBD) and 32 patients ($54\%$) with systemic chemotherapy (CT). We performed external radiation therapy (ERT) upto $10.8\~55.8$ Gy (median 37.8 Gy) with palliative aim. Results : Overall median survival duration was $7.80\pm1.15$ months. The response rates of jaundice were $81.8\%$ in PTBO group and $66.7\%$ in non-PTBD group without statistical significance. The improving rate of jaundice was not significantly different in decreased ratio of total bilirubin level. But abdominal pain was more decreased in CT group than non-CT group (p<0.05). The significant prognostic factors were high performance status (Karnofski Performance Status >70), total radiation dose more than 35 Gy and good response of pain after therapy. There were increased in bacterial cholagitis in PTBD group and gastrointestinal complications in CT group. Conclusion : External radiotheapy could improve jaundice and abdominal pain in malignant EHBO patients, Overall survival duration was prolonged in patients with higher performance status and patients who had been treated with more than 35 Gy of total radiation dose. In the future, we expect not only better palliative role but also the prolongation of survival of using the ERT combined with other treatment method. But to achieve certain conclusion, we need luther study consisted with many kinds of treatment methods including new technologies in RT.
