Spasmodic Torticollis Case Managed by Intraoral Balancing Appliance of FCST (FCST의 구강내 균형장치를 이용한 경추부 근긴장 이상증 증례보고)

  • To investigate the effects of the postural balance of TMJ using intraoral balancing appliance of functional cerebrospinal technique (FCST) combined acupuncture and manual medicine therapeutics in the treatment of congenital muscular torticollis. An idiopathic cervical dystonia case was managed with postural training with intraoral balancing appliance, combined with acupuncture and manual medicine therapeutics. Clinical assessment included radiography, Toronto Western Spasmodic Torticollis Rating Scale, and global clinical impression. Clinical improvement was observed over 5 months of treatment period. Clinical observational data on spasmodic torticollis provide an positive prospective for further investigation on the effect of FCST.

  • Stand-alone cervical cage consists of a PEEK body, Ti plate, and screw, which are configured as a single-piece. Through a single operation, this implantable medical device is capable of completely fixing the cervical vertebral body. For example, instead of a plate, which is normally used, the intervertebral disc is removed and replaced with a cervical cage. It should be noted that in Korea, KFDA guidelines for a stand-alone cervical cage have not yet been suggested. Therefore, the aim of this study is to present the systematic study of the static compression test, static torsion test, dynamic compression test, and expulsion test. Further, the test method is designed to refer to the ASTM standard and relative literature.

  • 많은 연구에서 측면 나사 고정(laminar screw fixation)보다 척추경 나사 고정(pedicle screw fixation)의 생체 역학 강도가 더 나은 것으로 알려져 있다. 그러나 비교적 작은 크기의 척추경(pedicle)과 척수(spinal code), 신경 뿌리 및 척추 동맥에 대한 상해의 위험으로 일반적으로 이 방법은 사용에 제한이 있었다. 최근 3차원 모델링 및 3D 프린팅 기술의 진보는 해부학적인 연구, 특히 척추를 포함한 뼈와 관련된 연구를 용이하게 하고 있으며, 로봇 수술을 위한 다양한 아이디어를 제공하고 있다. 본 연구는 로봇 암(robotic arm)으로 경추 척추경에 나사를 삽입할 때 사용될 수 있는 척추경 나사 삽입을 위한 이상적인 궤적을 계산하는 방법을 제시하였다.

The Effect of Cervical Traction on Pain & Symptom for Patients with Cervical Pain (경추부 견인이 경추부 통증 환자의 증세 및 통증에 미치는 영향)

  • The purpose of this study was to investigate the influence of cervical pain and radiating pain after cervical traction for patients with cervical pain. This evaluation was made 81 persons who cervical pain or radiating pain. The result of this study were as following ; 1. There were statistically significant decrease in cervical pain and radiating pain after cervical traction. 2. A type group(only neck pain ; n=5) and B type group(neck to elbow radiating pain ; n=11), there were pain decreased but there were not significant difference (p>0.05), C type group(neck to shoulder radiating pain group ; n = 14) and D type group(neck to hand radiating pain group ; n = 50), there were pain decreased before test then after test by VAS and significant difference(p<0.05).

  • The ottonello method is used to see a front view image of cervical 1-7 number. The study found a systematic examination method to find a better image through an experiment using metronome. The results are as follows: In an evaluation of MTF, when used 0.5 standard of MTF, oral explanation resulted 1.2 LP/mm, Moderato-100 1.4 LP/mm, Allegro-138 1.6 LP/mm, Presto-184 2.4 LP/mm. Among them presto-184 showed the highest sharpness with 2.4 LP/mm. In an image reading by a medicine doctor of radiology, when oral explanation was 17%, Moderato-100 15%, Allegro-138 18%, Presto-184 50%. Among themprest-184 was evaluated as the best image. The result of was a method to reduce an image repeat, patients radiation exposure, patients waiting time for examination, material and so on.

  • Giant cell tumors are potentially malignant tumors in vertebrae, affecting frequently difficult to diagnose and are often inoperable. So it will be treated using radiation because of their high recurrence rate and the mechanical compression of spinal cord, but many surgeons described tumors of the vertebra, and the affected vertebral body can be treated using radical or near to total excision, with anteroposterior vertebral fusion or instrumentation of the spine. we report a case of giant cell tumor affecting the third cervical vertebra which caused neck pain and destroyed the vertebra body had treated using radical excison with fusion of posterior arch using instrumentation of the spine together with a literature review.
