정상 성인에서 경두개 직류 전류자극과 기능적 전기자극에 의한 대뇌피질의 활성화: 사례연구 (Cortical Activation by Transcranial Direct Current Stimulation and Functional Electrical Stimulation in Normal Subjects: 2 Case Studies)

  • Purpose: Recently, many studies have demonstrated that application of external stimulation can modulate cortical excitability of the human brain. We attempted to observe cortical excitability using functional magnetic resonance imaging (fMRI) during the application of transcranial direct current stimulation (tDCS) or functional electrical stimulation (FES). Methods: We recruited two healthy subjects without a history of neurological or psychiatric problems. fMRI scanning was done during? each constant anodal tDCS and FES session, and each session was repeated three times. The tDCS session consisted of three successive phases (resting phase: 60sec dummy cycle: 10sec tDCS phase: 60sec). The FES session involved stimulation of wrist extensor muscles over two successive phase (resting phase: 15sec FES phase: 15sec). Results: The average map of the tDCS and FES analyses showed that the primary sensory-motor cortex area was activated in all subjects. Conclusion: Our findings show that cortical activation can be induced by constant anodal tDCS and FES. They suggest that the above stimuli have the potential for facilitating brain plasticity and modulating neural excitability if applied as specific therapeutic interventions for brain injured patients.

  • Purpose : This study assessed the effects of transcranial direct current stimulation (tDCS) on balance, fall efficacy, and fall-related fitness in stroke patients, using a cohort of 30 stroke patients divided into two groups. Methods : The experimental group (was given transcranial direct current stimulation in a virtual reality program) and the control group was given false tDCS in virtual reality. there were 15 patients in each group, receiving appropriate treatment over 30 sessions (30 minutes per session per week over a six-week period). In order to assess the change in balance before and after the intervention, the Berg Balance scale was utilized. Fall efficacy was evaluated using the Korean Falls Efficacy Scale for the Elderly (FES-K), The following exercises were performed by patients to assess fall-related fitness : sitting down in a chair and standing up : walking a 244 cm round= trip, and standing on one foot. Results : After the intervention, the experimental group exhibited significantly increased fall efficacy and fall-related fitness, while the control group exhibited no change. These findings suggest that tDCS has positive effects on balance, fall efficacy, and fall-related fitness in stroke patients. Conclusion : Using tDCS as an intervention would bring positive effects on balance, fall efficacy, and fall-related fitness in stroke patients undergoing rehabilitation.

  • Purpose : Transcranial direct current stimulation is noninvasive method of cerebral cortical stimulation applied as an intervention to facilitate recovery in stroke patients. The purpose of this study was to examine the effects of transcranial direct current stimulation on depression and anxiety in stroke patients with the aim to finding a reliable method to reduce depression and anxiety in stroke patients. Methods : This study was conducted at A hospital located in D city from December 2022 to February 2023. Patients diagnosed with stroke were selected according to predefined study criteria and divided into two groups. 9 patients in experimental group underwent transcranial direct current stimulation and traditional occupational therapy, while 9 patients in the control group underwent sham transcranial direct current stimulation and traditional occupational therapy for 20 min per session 3 times per week for a total of 8 weeks. Beck depression inventory (BDI) and Beck anxiety inventory (BAI) were used to evaluate depression and anxiety in the patients before and after intervention. Wilcoxon signed rank test was used to compare differences before to after intervention. Mann-Whitney U test was used to compare the two groups. Differences with p<.05 were considered statistically significant. Results : First, both the experimental and control groups presented significant statistical difference before and after intervention on depression (p<.01). Comparison of two groups after intervention showed significant statistical difference in depression (p<.05). Second, the groups presented significant statistical difference before and after intervention on anxiety (p<.01). After intervention, the two groups showed significant statistical difference in anxiety (p<.05). Conclusion : Results of the present study demonstrated the effects of transcranial direct current stimulation on depression and anxiety in stroke patients, suggesting it may be a useful method of rehabilitation in this patient population; as such, further studies are warranted.

경두개직류전류자극이 시열반응과제에 대한 운동 수행 능력에 미치는 영향 (The Effects of Transcranial Direct Current Stimulation in Motor Performance of Serial Reaction Time Task)

  • Purpose: The aim of this study is to investigate whether motor cortex excitability by transcranial direct current stimulation (tDCS) over primary motor cortex (M1) affects motor performance of serial reaction task. Methods: Cathodal, anodal and sham tDCS (1 mA) are applied over right M1 of 24 subjects for 30 minutes including 11minutes for task period time. We applied two electrodes at the same position to both an experimental group and a sham-controlled group, and we made 2 groups recognize to be applicated of stimulation. Flexion, extension of wrist and thumb flexion are carried out following colors of arrows on the monitor. Serial reaction time task was applied to confirm the difference of the reaction time between 2 groups. Results: Reaction time is decreased in both tDCS-group and Sham-controlled tDCS group, and the degree of reduction is much greater in the post-test than pre-test. Reduction of reaction time between groupsis statistically significant. Conclusion: We consider that anodal tDCS increased the cortical excitability of the underlying motor cortex and it can be helpful to modulate motor performance. It seems that tDCS is an effective modality to modulate brain function, and it will be great help to mediate strategy for the brain injury patients.

  • Purpose : Study on the effects of attention(working memory) for normal adults by applying transcranial Direct Current Stimulation(tDCS). Methods : There are two groups of 10 random people aged between 20 to 30, one is an experimental group(stimulation) and the other is a comparison group(simulate stimulation). For the 1st da y, pre-stimulation test is executed, and for the 2nd day to the 4th day, tDCS is applied stimulated both on the right prefrontal lobe and the left occipital lobe through the electrode. The stimulation lasts for 15 minutes with voltage power of 1mA for each day. The post-stimulation test is executed on the day of 5th. In this paper, a FAIR attention test is used for measuring the attention. Results : As the result of the experiment, there are significant gaps both on the value of P, the ability of selective attention, and the value of C, the consistent ability after the stimulation on right prefrontal and left occipital lobe. And there is no significant gap on the value Q, the quality(Control). However, all the P, Q, and C values are increased according to the average value between the pre-stimulation test and the post-stimulation. Conclusion : Finally, ability of selective attention and self-control, and consistent attention is progressed by applying the tDCS to the normal people.

경두개직류전류자극 및 시지각 훈련 프로그램 적용이 치매환자의 시지각 기능 및 일상생활활동에 미치는 영향 (Does the Application Transcranial Direct Current Stimulation and Visual Perception Program Enhance the Visual Perception Function and ADL Performance of Dementia Patients)

  • Purpose : This study aimed to evaluate the changes in visual perception and daily life activities after simultaneously applying the visual perception training program and transcranial DC stimulation. Particularly, this study tried to understand what changes in abilities among daily life activities influenced cognition. Methods : This study selected 50 dementia patients to achieve the objective. This study applied transcranial DC stimulation and a visual perception training program to a treatment group (n=25) and a visual perception training program to a control group (n=25). The intervention was applied five times per week and 30 minutes per session (total of 30 intervention sessions). This study measured visual perception functions and daily life activities using MVPT and FIM before and after applying the intervention. Results : The visual perception, mobility of daily life activities, cognition, and total score of the treatment group (n=25), which received the visual perception training program and transcranial DC at the same time, were significantly (p<.05) different between before and after the intervention. Moreover, when the visual perception and daily life activities of the treatment group and those of the control group after implementing the intervention were compared, they were significantly (p<.05) different between the two groups. Conclusion : The results of this study showed that the simultaneous application of the visual perception training program and transcranial DC stimulation was an effective means to enhance the visual perception and daily life activities of dementia patients. The results implied that transcranial DC stimulation, as well as the application of visual perception training program, as well as a visual perception training program, was effective for improving the visual perception functions and daily life activities dementia patients and simultaneous application of the two interventions would increase the effect even further.

경두개직류전류자극이 무릎관절 전치환술 환자의 통증 및 균형 능력에 미치는 영향 (The Effect of Transcranial Direct Current Stimulation on Pain and Balance of Total Knee Arthroplasty Patients)

  • Background: The purpose of this study was to examine the effect of transcranial direct current stimulation (tDCS) on the pain and balance of patients who receive total knee arthroplasty (TKA). Methods: This study subjects were 24 patients in Hospital T, located in Daegu, South Korea who received TKA after being diagnosed with degenerative arthritis. The subjects were randomly divided into and experimental group and a control group, with each group including 12 patients. Both group received superfical thermal therapy, interferential current therapy (ICT), and continue passive motion (CPM), which are conventional knee therapy on the knee joint. The experimental group received the tDCS treatment three times a week for three weeks, from October 1st to October 20th. The visual analogue scale and Wii Balance Board system were used to measure the pain and balancing ability, respectively, of both groups. In the statistical result analysis, to compare about pre and post test difference in each groups was accomplished. Statistical analysis of independent t-test and paired t-test were conducted using SPSS version 23.0. Results: After three weeks of intervation, there were significantly difference in balance ability in pre and post test in the tDCS group. VAS decreased significantly in both groups(p<.05), There was a significantly difference in pain, balance ability in the tDCS group compared to the sham group. Conclusion: These results indicate that applying tDCS together with conventional knee joint therapy for TKA patients is effective in promoting the patients' recovery.

경두개직류전류자극을 결합한 가상현실프로그램이 경도인지장애환자의 우울, 손기능, 인지와 일상생활활동에 미치는 영향 (The Effect of Virtual Reality Program Combining Transcranial Direct Current Stimulation on Depression, Hand Function, Cognition, and Daily Life Activities of Patients with Mild Cognitive Disorders)

  • Purpose : This study evaluated the effects of transcranial direct current stimulation and a virtual reality program on the depression, hand functions, cognitive function, and activities of daily living of patients with mild cognitive impairment by dividing 20 patients with mild cognitive impairment and depression. The 20 patients were divided into a treatment group (transcranial direct current stimulation + a virtual reality program) and a control group (placebo transcranial direct current stimulation + a placebo virtual reality program). Methods : This study allocated ten subjects to the treatment group and ten subjects to the control group. The treatment was given five times per week for six weeks (30 sessions), and each session was 30 minutes. This study screened depression by using SGDS-K, a short geriatric depression scale, to examine depression before and after treatment intervention. This study also used the box and block test, NCSE, and FIM to evaluate hand functions, cognitive function, and activities of daily living, respectively. Results : The results showed that depression significantly decreased, hand functions significantly increased, cognitive function significantly improved, and activities of daily living significantly increased after intervention in the treatment and control groups. The magnitude of changes in depression, hand functions, cognitive function, and activities of daily living was significantly different between the two groups after intervention (p>.05). Conclusion : The results showed that the application of transcranial direct current stimulation and a virtual reality program could improve cognitive function, hand functions, and activities of daily living by decreasing depression. Therefore, it can be concluded that the simultaneous application of transcranial direct current stimulation and a virtual reality program is an intervention method, which can be applied for decreasing depression, enhancing hand functions, improving cognitive function, and increasing activities of daily living in patients with mild cognitive impairment.

비침습적 뇌자극기술과 법적 규제 - TMS와 tDCS기술을 이용한 기기를 중심으로 - (Non-invasive Brain Stimulation and its Legal Regulation - Devices using Techniques of TMS and tDCS -)

  • TMS와 tDCS는 자기와 전류를 통하여 뇌에 자극을 가함으로서 환자나 개별 이용자의 질병을 치료하고, 이외에도 건강을 관리하거나 증진시킬 수 있는 비침습적 기기를 말한다. 이들 기기의 효과와 안전성은 몇몇 질병에서 입증되고 있으나, 아직도 이에 대한 연구는 진행 중이다. 점차 증가하고 있는 이들 기기의 활용도에 비해 TMS와 tDCS를 직접 규율하는 입법례를 찾기는 어렵다. 미국, 독일, 일본의 TMS와 tDCS에 대한 법적 규율을 살펴보면, TMS는 중등도의 위해도를 가진 의료기기로 승인되어 있는 반면, tDCS는 아직 의료기기로 승인된 상태는 아니다. 하지만, 최근 FDA 가이드집이나 유럽 MDR 규정의 변화, 미국의 리콜사례, 독일과 일본의 관련 법 규정, 전문가 그룹의 제언 등을 검토하면, tDCS도 조만간 의료기기로 승인되어 규율될 것으로 보인다. 물론, tDCS를 의료기기가 아닌 일반제품으로 보더라도 다른 법률과 제도를 통하여 제품의 안전성과 효과를 규제할 수는 있다. 그러나 이 기기가 인간의 뇌에 미칠 수 있는 여러 영향을 고려할 때, 이를 독자적으로 규율할 필요성이 크다. 우리도 TMS와 tDCS를 규율하는 명시적 법률은 없으나, 이 두 기기는 식약처 고시에 따라 3등급 의료기기로 판정된다. 그리고 TMS는 가이드 라인에 따라 미국 FDA 지침에 의해 안전성과 성능을 평가하도록 하고 있다. 하지만, tDCS는 아직 이에 대한 구체적 지침은 존재하지 않는다. tDCS 기기가 일부 병원에서, 그리고 개별 구매자를 통하여 가정에서 사용되고 있는 현실을 고려하면, 이러한 규제의 공백은 신속히 보완되어야 한다. 장기적으로는 비침습적 뇌자극기기를 독자적으로 규율할 수 있는 법적 시스템의 정비가 필요하다.
