Nonunion of Tibial Eminence Fracture in Child - 2 cases Report - (소아 경골 극 골절의 불유합 - 2예 보고 -)

  • The general consensus is that tibial eminence fractures have an excellent prognosis. But symptomatic nonunions requiring reduction and grafting have been reported. This report presents two cases of nonunion after tibial eminence fracture in eight years old boys which were treated with arthroscopy-guided reattachment and internal fixation and obtained good outcome.

Arthroscopic Treatment of Tibial Spine Fracture using Suture Hook and pull-out PDS (Suture Hook과 pull-out PDS를 이용한 경골극 골절의 관절경적 치료)

  • Displaced tibial spine fractures need the anatomical reduction of the displaced bone fragment to achieve normal range of motion and anterior stability of the knee joint. The purpose of this paper is to describe details of arthroscopic technique using suture hook and pull-out PDS and to evaluate the clinical results. We report 7 cases who underwent arthroscopic reduction and internal fixation using suture hook and pull-out PDS. All cases had fresh fractures generated within 3 weeks. The follow up period was at average 16.6 months. The fracture union was achieved at average 7.4 weeks. Knee exercise was started 2 weeks after the operation. One of the patients, who had combined injury of posterior cruciate ligament and lateral meniscus, showed limitation of knee movement. But he was underwent the arthroscopic fibrolysis at one year later, he returned to normal range of motion. Arthroscopic treatment of displaced tibial spine fracture using suture hook and pull-out PDS showed good results including rigid fixation and early mobilization. Therefore it is thought to be one of the effective operative techniques in treatment of the tibial spine fractures.

Anterior Instability of the Knee after Arthroscopic Treatment for Tibial Spine Fractures (경골극 골절의 관절경적 치료 후 슬관절의 전방 불안정성)

  • Purpose: We evaluated the result of the degree of reduction and anterior instability after arthroscopic treatment of tibial spine fractures. Materials and Methods: Thirty-two cases of tibial spine fractures treated with arthroscopic procedure could be evaluated and the mean follow-up period was $18\;(12{\sim}48)$ months. Evaluations included Lachman test, rotatory instability examination and side to side difference (SSD) using KT-2000 arthrometer. The degree of radiologic reduction was classified as accurate reduction, anterior elevation and reduction failure, according to which the degree of anterior instability was measured. Results: There were twenty three cases (71%) of accurate duction, eight cases (25%) of anterior elevation and one case (4%) of education failure. Eight (29%) of twenty eight cases with pull-out method showed anterior elevation. The SSDs of the accurate reduction and the anterior elevation group were 2.0 mm and 2.8 mm, respectively showing no significant difference. However, the percent age of grade 0 Lachman test and SSD less than 3 mm of the accurate reduction group were significantly higher. Conclusion: Accurate reduction is important in reducing anterior instability especially in unstable fractures. Therefore, arthroscopic pull-out method should be performed with great caution to avoid anterior elevation of the fracture fragment.

  • It is well known that the two main variants of the anterior cruciate ligament (ACL) ruptures in an adolescent include tibial eminence fractures and midsubstance tears. Authors report a case of 17-year-old girl with simultaneous bilateral anterior cruciate ligament ruptures at the ligamento-osseus junction of tibial attachment which was treated with arthroscopic primary repair.

Arthroscopic Treatment of Displaced Tibial Spine Fracture in Children (소아에서의 전위된 경골극 골절의 관절경적 치료)

  • Purpose: To analyze the clinical result of the arthroscopic reduction and fixation for the displaced tibial spine fractures in children and report the utility of the arthroscopic treatment with the review of the literature. Methods: Between December 2000 and July 2004, five patients (average age 9.1 years) received an arthroscopic reduction and fixation of displaced tibial spine fracture. A male-to-female ratio was 3:2 and mean follow-up was 38.2$(13{\sim}56)$ months. All five patients were classified type III by Movers and McKeever classification. The average period from injury to operation was 4.8 days $(3{\sim}8days)$, the avulsed fragment was reduced by operative arthroscopy and fixated by pull-out suture in 3 patients and by cannulated screw in 2 patients. Postoperatively long leg cast was applied for 2 weeks, and then gradual range-of-motion exercise was permitted. Full weight-bearing ambulation was permitted after 6 weeks. The clinical evaluation was performed by range of motion, Lachman and pivot shift test, KT-1000 arthrometer, Lysholm knee score and the modified Feagin score. Results: All five patients had no symptom and recovered full range of motion of the affected knees. Lachman test was positive finding of 1+ laxity in one patient, the others were negative, and all patients were negative findings for pivot shift test. The result of KT-1000 arthrometric assessment is mean maximum side-to-side differences 1.9 mm. Average Lysholm knee score was 99.4. All patients had excellent results in modified Feagin score. Conclusion: Arthroscopic reduction and fixation of displaced tibial spine fracture in children showed excellent result without complication. Both pull-out suture fixation and cannulated fixation provide an effective treatment option for fixation of the displaced anterior tibial spine fracture.

  • Purpose : To evaluate the clinical and radiological characteristics of osteochondral ridges of talus and ankle. Materials and Methods : We have analyzed their clinical symptoms and signs, radiologic and CT findings and post-operative results in 17 ankle joints of 14 patients (bilateral in 3), followed them for average 13 months after surgical excision. Results : No definite trauma, but mostly in male after middle age. Their chief complaints are pain on ankle, especially in dorsiflexion or squatting position, and symptom durations are very long, more than average 15 months. Definite diagnosis was made by lateral radiograms of ankle joint. Osteochondral ridges are common in talar neck (10 cases), tibia (4 cases) and both side (3 cases). After excision of osteophytes, all patients gained normal ankle without pain and any limited motion. Conclusions : Anterior impingement syndromes are common in middle aged male, but no definite correlation with sports. Plantar and dorsiflexed lateral radiographs are helpful in definite diagnosis for impingement, and surgical excision is best for treatment.

Ultrastructure of the Fertilized Egg Envelope from Pale Chub, Cyprinidae, Teleost (경골어류 잉어과 피라미의 수정란 난막 미세구조)

  • The ultrastructure of the fertilized egg envelope from pale chub , Zacco platypus belong to Cyprinidae was studied using scanning and transmission electron microscopes. The fertilized egg was an adhesive type, have a single micropyle resembling the pathway of sperm in the area of the animal pole. The micropyle was surrounded by 5 peaks of hill. An outer surface of the fertilized egg envelope was arranged by pore canals irregularly. In section of fertilized egg, the egg envelope consistes of three layers, an outer adhesive layer, a middle electron dense layer and an inner lamellae layer consisting of 9 layers. These ultrastructural characters of fertilized egg envelope from pale chub can be utilized in taxonomy of teleost.

  • The ultrastructure of the matured egg envelope in Pond smelt, Hypomesus nipponensis belonging to Osmeridae, Osmeriformes were investigated by routine light and electron microscopes. The matured egg have two egg envelopes and have a single micropyle, which is thought to the pathway of sperm in the area of the animal pole. An outer egg envelope was surrounded by a follicular layer and outer surface of inner egg envelope have structure with high electron density. Also, the inner egg envelope consisted of 6 horizontal lamellae with higher electron density alternating with 5 interlamellae of lower electron density. Many grooves distributed on the outer surface of outer egg envelope, and the outer surface of inner egg envelope was covered by amorphous structures. In conclusion, the egg of teleost is surrounded by one egg envelope according to the studies on morphology of egg envelope up to the present. The fact that have two egg envelopes is a species specificity of Pond smelt and these ultrastructural characters of egg envelope can be utilized in taxonomy of teleost.

Osteological Development of Korean Striped Bitterling, Acheilognathus yamatsutae (Cyprinidae) (줄납자루, Acheilognathus yamatsutae (잉어과)의 골격 발달)

  • Osteolgical development of Korean striped bitterling, Acheilognathus yamatsutae (Cyprinidae), were investigated using cartilage and bone staining. The parasphenoid, dentary, pterygoid, ceratohyal, branchial arch and parts of the caudal skeleton were formed first as cartilaginous elements at $6.82{\pm}0.08mm$ total length (TL). Formation of frontal, parietal, and maxillary were notable at $7.76{\pm}0.09mm$ TL, and the proximal radial, scapula,and coracoid in the pectoral girdle were developed at this stage. At the same time, the neural and hemal spine were present in the caudal vertebra. The opercle and branchiostegals were observed at $9.68{\pm}0,14mm$, and the posttemporal was formed at $12.9{\pm}40.64mm$ TL, respectively. Ossification in the parasphenoid, pharyngeal bone, dentary, premaxillary, maxillary, and opercle began at about $9.68{\pm}0.14mm$ TL. The cleithrum, supracleithrum, urostyle, and caudal fin were calcified at this stage. The vertebral column was formed and ossified at $11.52{\pm}0.13mm$ TL, and the frontal, preopercle, subopercle, and hyomandibular were ossified at $15.30{\pm}0.68mm$ TL. This fish was late in developing the skeletal formation and ossification as compared with other fishes.
