A PHOTOELASTIC STUDY ON THE INITIAL STRESS DISTRIBUTION OF THE MOLAR ANCHORING SPRING(MAS) DURING RETRACTION OF THE MAXILLARY CANINE (상악견치 후방견인시 저항원 조절을 위한 MAS(Molar Anchoring Spring)의 초기 응력분포에 관한 광탄성학적 연구)

  • The efficiency of maxillary canine retraction by means of sliding mechanics along an 0.016 continuous labial arch and an 0.009 inch in diameter with a lumen of 0.030 inch NiTi closed coil spring was compared with that using the same NiTi closed coil spring and Molar Anchoring Spring(MAS) which was designed by author. MAS was made of .017" X .025" TMA wire and was given 60 degree tip-back bend on the wire close to the molar tube. This study was designed to investigate molar and canine root control during retraction into an extraction site with continuous arch wire system. Two techniques were tested with a continuous arch model embedded in a photoelastic resin. A photoelastic model was employed to visualize the effects of forces applied to canine and molar by two retraction mechanics. With the aid of polarized light, stresses were viewed as colored fringes. The photoelastic overview of the upper right quadrant showed that stress concentrations were observed in its photoelastic model. The obtained results were as follows. 1. Higher concentration of compression can be seen clearly at the distal curvature of the canine and mesial curvature of the molar and premolar when NiTi closed coil spring was applied only, which means severe anchorage loss of the molar and uncontrolled tipping of the canine. 2. The least level compression was presented at the mesial root area of the molar and premolar, and mesial root area of the canine when NiTi closed coil spring and MAS were used simultaneously. Especially mesial alveolar crest region of the canine was shown moderate level of compression that means MAS can be used as a appliance for anchorage control and prevention of canine extrusion and uncontrolled tipping during canine retraction.

A comparison of on masse retraction of six anterior teeth with separate canine retraction (6전치 일괄(on masse) 견인과 견치 견인 후 4전치 견인 시 공간폐쇄 양상에 관한 연구)

  • The purpose of this study was to compare on masse retraction of six anterior teeth with separate canine retraction in the amount of the anchorage loss and the retraction of the anterior teeth. The subjects consisted of 30 adult female patients with Angle Class 1 malocclusions who were treated by .022' straight wire appliance with 4 first permolar extraction. They were composed of two groups. Group 1 consisted of 15 subjects, whose six anterior teeth were retracted by on masse retraction. Group 2 consisted of 15 subjects, whose canines were retracted separately. Pre-treatment and post-treatment lateral cephalometric radiographs were analyzed. All data were processed statistically with independent samples t-test, and the conclusions were as follows. 1. There was no significant difference in the amount of the anchorage loss between two groups(p>0.05). 2. There was no significant difference in the amount of the retraction of the anterior teeth between two groups(p>0.05). 3. There was a significant difference in the amount of the inclinational change of the upper incisors between two groups. It was greater in Group 2. 4. There was a significant difference in the vertical positional change of the upper incisal edges between two groups. The upper incisal edges in Group 2 were more extruded than Group 1 by about 1mm. 5. There was no significant difference in the vertical positional change of the root apex of the upper incisors between two groups(p>0.05). And there was no significant difference in the vertical positional change of the upper molar(p>0.05).

Retrospective Analysis of Incisor Root Resorption Associated with Impacted Maxillary Canines (매복된 상악 견치에 의한 전치의 치근 흡수에 대한 후향적 연구)

  • Root resorption of the permanent maxillary incisors can occur due to ectopic eruption of the permanent canines. Severe root resorption threatens the long-term survival of the affected incisors. The aim of the present study was to identify risk factors for root resorption of the maxillary incisors associated with impacted maxillary canines. In the present study, we retrospectively analyzed the Cone-Beam Computed Tomography (CBCT) scans of 65 children and adolescents with ectopically erupting maxillary canines (total of 88 impacted canines). Root resorption of central incisors was significantly associated with the mesiodistal position and root development of the adjacent canine. Root resorption of lateral incisors was significantly associated with sex, age, and the buccolingual and vertical position of the adjacent canine. However, enlargement of the dental follicle was not significantly associated with root resorption of adjacent incisors. Although incisor resorption is difficult to diagnose and predict, our findings suggest that changes in the dental follicles of the erupting maxillary canines do not cause resorption of the adjacent permanent incisors. CBCT should be utilized to ensure early diagnosis of impacted canines and precise evaluation of incisor root resorption.

An occlusal contact analysis of lateral mandibular movement using T-Scan system (T-Scan system을 이용한 하악 측방운동 시 교합접촉 분석)

  • Purpose: Most of the former studies about the occlusal contact patterns during the mandibular movement focused on foreigner. The purpose of this study is analyzing the occlusal contacts of young Koreans by using T-Scan system. Materials and methods: The sample size was 87 and the occlusal contacts of each right and left lateral movements were measured from the maximum intercuspation to the 3mm excursive position for three times respectively. All of the occlusal contacts were double checked through the thin metal foil. The results were categorized as two; 1) considering occlusal contact patterns on working side only, 2) considering occlusal contact patterns on working and nonworking sides. Results: The results showed that the nonworking side occlusal contacts play major roles in the lateral mandibular movement. In both cases of considering with and without the nonworking side occlusal contacts, the group function was the most prevalent. In the working side, the contacts were the most frequent in canine and the frequency of contacts was decreased as the distance was increased from canine to molar. In the nonworking side, the contacts were the most frequent in second molar. And the gender factor was statistically significant (${\alpha}$=.05), as females have more nonworking side occlusal contacts in this study. Conclusion: Among the three factors of anterior guidance, the group function was the most dominant factor. The analysis of 87 samples showed that canine contact was the most frequent in the working side and second molar contact was the most common in the nonworking side.

  • Upper and lower cuspids are very important teeth in point of function as well as esthetic. However, failure of eruption of the canine sometimes happens and is frequently encountered clinical problems. Upper cuspids are more frequently impacted as compared with lower cuspis and palatally impaction is more frequent event than labially impaction. Impaction of the mandibular canine is unusual event, especially crossing the midline (transmigration) is very rare. Because of, in most cases, there is no signs and symptoms, impacted teeth can be detected by periodic radiograph examination. The etiology of impaction is unclear and multifactorial. The treatment of impacted canine is influenced by presence of obstacles, location and direction of impaction, space for alignment and development of root. There are several treatment options; (1) Surgical removal and/or observation, (2) Window opening and orthodontic traction, (3) Autotransplantation. The first patient (impaction of upper right cuspid) is treated with window opening and orthodontic traction. The second patient (bilaterally impaction of lower cuspids) is treated with autotransplantation, because of transmigration.

Factors Influencing the Duration of Forced Eruption in Impacted Maxillary Canines (상악 매복 견치의 견인 기간에 영향을 미치는 요인에 대한 분석)

  • The aim of this study was to identify factors that affect the treatment duration and to predict the duration of forced eruption in impacted maxillary canines using panoramic radiographs and cone-beam computed tomography. This retrospective study was performed by reviewing medical records and radiographs of 73 patients (93 impacted maxillary canines) from the age of 8 to 18 years who were treated with surgical and orthodontic interventions on impacted maxillary canines from January 2012 to December 2020 in Ajou University Dental Hospital. Stepwise multiple regression analysis showed that the distance between the canine cusp tip and the occlusal plane, mesio-distal location, bucco-palatal location, patient's age, and use of rapid palatal expansion are significant factors with regard to the duration of forced eruption. There was a statistically significant correlation of the treatment duration with the angulation between the axis of the canine and the occlusal plane and unilateral or bilateral impaction. It can be concluded that the duration of forced eruption in impacted maxillary canines could be shorter when the impacted canine is closer to the occlusal plane and located in the lateral incisor or canine area, buccal or middle, the patient is younger and uses rapid palatal expansion.

  • Crowding은 치열의 어느 곳에서나 발생하나 측철치와 견치 사이에서 가장 많이 발생하고 특히 하악 전치 부위에서 다발하여 이 부위의 정상 배열은 그렇게 많이 관찰되지 않는다. 서양 백인과 동양인의 경우에서는 연령의 증가에 따라 발생 빈도가 증가한다. 그러나, 흑인에 있어서는 치아의 크기가 큰데도 불구하고 연락의 증가에 다라 발생 빈도가 증가하지 않는다. 남녀의 발생빈도에 대한 연구 보고에 의하면 여자에서 많이 발생한다고 한다.

  • Composite Resin(콤포짓트 레진) 수복은 3급,4급(견치의 원심면 제외) 및 5등급 전치부와동에 주로 사용되었으나 재료의 개선으로 인해 1급,2급 및 견치 3급와동의 원심면에도 제한적으로 사용이 가능하게 되었다. 물론 실제적인 와동의 설계는 수복요건에 따라 다양한 형태변화를 부여할 수도 있다.
