Purpose: To investigate the arthroscopic findings, and to evaluate the clinical outcomes of the treatment of posterior internal impingement of the shoulder in baseball players. Materials and Methods: We followed up 5 cases who were diagnosed as posterior internal impingement for the mean 15 months. All of the cases complained of the pain in the posterior shoulder at late cocking, and were positive in the relocation test added by hyper-horizontal abduction at $120^{\circ}$ abduction of the arm. We sutured posterior labral tear and SLAP lesion arthroscopically, and conducted debridement for rotator cuff. Three cases were performed of anterior capsular plication and the other two were performed of thermal capsular shrinkage. Pain, range of motion, and level of return to sports activity were assessed for the results. Results: As to the arthroscopic findings, all the five cases showed the fraying in posterosuperior labrum, and two of them was accompanied with the flap tear in posterosuperior labrum and the other one was accompanied with type 2 SLAP lesion. All the cases showed the fraying in supraspinatus, and one case showed partial tear. Meanwhile, in all the cases, the rotator cuff was impinged to the labrum at $90^{\circ}{\sim}120^{\circ}$ of abduction and external rotation. As to the postoperative results, all the cases did not complain of pain or instability while pitching, and the competition was recovered to be the mean 88%($80{\sim}100%$) of that before the injury. Conclusion: Definite diagnosis for the posterior internal impingement would be possible through arthroscopic examination. Favorable outcomes could be obtained with capsular plication or shrinkage for anterior microinstability and stretching exercise for posterior capsule tightness inducing the internal impingement.

Purpose: The purpose of this study was to analyze the effectiveness of local steroid injection for infraspinatus under ultrasonographic guidance in patients with posterior shoulder pain during horizontal adduction test. Materials and Methods: Twenty one cases of patients, from May 2013 to May 2014, showed positive horizontal adduction test were retrospectively analyzed. We performed ultrasonography-guided infraspinatus injection using steroid to these patients in the outpatient clinics. We evalulated results for visual analogue score (VAS) for pain, UCLA score for these patients before injection, following 3 months and 6 months after injection. Results: After ultrasonography-guided infraspinatus injection using steroid, there were clinically significant improvements in VAS and UCLA score at 3 month's follow-up period (p<0.05). But there were no significant differences in VAS and UCLA score between 3 months and 6 months (p>0.05). Fourteen of 21 (67%) cases showed much improved and improved satisfaction. Respectively, seven of 21 (33%) cases showed not improved and aggravation satisfaction. Conclusion: Ultrasonography-guided infraspinatus injection using steroid is an effective treatment method for the patients with exacerbated posterior shoulder pain during horizontal adduction test.

Most of the caicific tendinitis have been reported to be found on the rotator cuff, in particularly on supraspinatus. We reported a case of calcific tendinitis on the posterosuperior glenoid labrum. The location of the lesion was diagnosed accurately by the MRI and easily removed the lesion with arthroscopic surgery and got satisfactory results.
We report a case of simultaneous anterior and posterior interosseous nerve syndrome in association with shoulder arthroscopy. Shoulder arthroscopy was performed in a 45-year-old male patient with left shoulder instability. In the right lateral decubitus position, under general anesthesia, traction was applied with elbow extension for 2 hours. One week after surgery, the patient revisited the clinic for weakness of the flexor of the thumb, index finger, and extensor of the fingers. Recovery was not achieved after four months of observation. Therefore, nerve exploration was performed in the anterior and posterior interosseous nerve and hourglass-like fascicular constriction was detected in the posterior interosseous nerve. The area of constriction was removed and epineural neurorrhaphy was performed. Three months after exploration, the extension function of the fingers was recovered. Recovery was achieved gradually, and, five months after nerve exploration, the symptoms were completely recovered. Simultaneous anterior and posterior interosseous nerve syndrome following shoulder arthroscopy is rare. However, it could occur due to the traction and position of the patient. Thus, the operator should be careful of traction and position of the patient.
Purpose: A triple labral lesion represents a combination of injuries of the anterior, posterior and superior glenoid labrum. The injury mechanism and symptoms is not completely understood. Materials and Methods: We encountered a triple labral injury in a 39-year-old male complaining pain and active abduction difficulty after a motor vehicle accident. Currently, he does not complain any instability symptoms. The labrum was repaired using bio-absorbable suture anchors and a suture hook. Results: Eighteen months following surgery, the active range of motion was restored and he had no difficulty in his daily activities.

29 year-old male was suffered from deep shoulder pain and muscle weakness 2 years ago. Recently 2 month ago lancinating shoulder pain and night pain was progressed. physical examination and MRI and EMG finding showed suprascapular nerve compression syndrome due to ganglion cyst in the shoulder joint. Arthroscopic ganglion cyst excision was performed and his symptoms was improved. 2 years later shoulder pain was developed in abduction and external rotation position and mild discomfort was continued. In second operation, SLAP lesion was found and arthroscopic SLAP repair was performed. Our case presented SLAP lesion occurred arthroscopic ganglion excision in the shoulder joint 2 years later.

The authors report one case of isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament. A 13 years old boy was admitted for right knee pain and swelling after the soccer injury on his knee. Hemarthrosis and posterior instability were present. Simple roentgenographic examination showed an isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament. Open approach through the popliteal fossa was used and bony fragment was fixed into its bed using pull-out sutures. Postoperatively, a long leg cast was applied for 4 weeks. The patient was followed for 6 months. He returned to his previous activity levels. There was no residual laxity. Isolated avulsion of the posterior cruciate ligament from the tibia before the epiphyseal fusion is very rare in children. We reported one case of our experience and its result.
Purpose: To evaluate the results and complications of antegrade intramedullary interlocking nailing in humerus shaft fractures. Materials and Methods: We evaluated the clinical outcomes, radiologic results and complications in 47 patients with humerus shaft fracture treated with antegrade intramedullary interlocking nailing, and followed up until bony union. Bony union was confirmed by serial plain radiographs and the clinical outcomes were assessed according to the ASES scoring system. Results: Bony union was confirmed in 41 (87.2%) out of a total 47 patients, and the mean union period was 14.5 weeks. Major complications were as follows: 6 non-union, 3 delayed union, 2 intraoperative posterior cortex fracture in the distal humerus and 2 permanent shoulder pain, including 1 case of adhesive capsulitis. The clinical outcomes were as follows: 29 excellent, 11 good, 4 fair and 3 poor. Satisfactory outcomes were demonstrated in 40 patients (85.1%). Conclusion: Anterograde intramedullary interlocking nailing as treatment for humerus shaft fracture showed satisfactory bony union and clinical outcomes. It is considered an efficacious treatment, especially in patients with associated injury, such as multiple fractures and segmental fracture.
