Geometric Assessment of Scapular Thickness by Computed Tomography (견갑골 두께의 전산화 단층 촬영을 이용한 기하학적 모델)

  • Purpose: Understanding exactly detailed anatomy and morphology of scapula is very important for further surgical procedures. This study was intended to provide accurate anatomic and morphologic information of scapula by exactly measuring scapular thickness using computed tomography and reconstructing its geometric model. Materials and Methods: Eight average lengths and two angles of 102 scapular structures obtained from 51 cadavers were generally measured by computed tomography. Also, to measure the scapular thickness, sagittal planes of each scapula were divided almost equally and the thicknesses of each sagittal plane was measured by computed tomography. After measuring every thickness, average results were calculated and the gender difference was compared by Student t-test. Results: Average results of the thickness of glenoid fossa, lateral border, medial border, and the middle 1/3 portion of the scapular body were 22.4 mm, 13.83 mm, 4.44 mm and 2.06 mm, respectively. Also, male scapulars were found to be significantly thicker than female. Based on these measured thicknesses, we reconstructed the 3-dimensional geometric model of scapula. Conclusion: From these results, glenoid fossa and lateral border were the thickest part of scapula, while the middle 1/3 portion of the scapular was the thinnest.

  • Purpose: Scapular dyskinesis is an alteration in the normal position or motion of the scapula during coupled scapulohumeral movements. Vast majority of shoulder pathologies are known to be related with scapular motion abnormalities. Because there being enthusiasm about scapular pathology in recent literatures, understanding scapular dyskinesis seems to be an important subject. The authors describe the importance of scapular abnormality in terms of shoulder pathology. Materials and Methods: Usually the inhibition or disorganization of activation patterns in scapular stabilizing muscles lead to scapular dyskinesis. This motion abnormality has more important values in Elite Athletes because it might be the sign of future shoulder pathology; for example, SLAP and internal impingement. Treatment of scapular dyskinesis is directed at managing underlying causes and restoring normal scapular muscle activation patterns by kinetic chain-based rehabilitation protocols. Treatment is also important to prevent secondary shoulder injuries. Results and Conclusion: Understanding scapular pathology may be the main key to approach to the shoulder pathology. Also treating scapular pathology might be important in preventing secondary shoulder injuries.

  • True lateral scapula image was very important to diagnosis the scapula fracture and dislocation induced by traumatic injury. The aim of this study was to know the patient rotation angle to be showing the scapula true lateral in korean. d Materials Thirty patients(22men, 8 women, mean ages 53.4)with scapular pain, had supine Anteroposterior projection taken with $30^{\circ}$, $40^{\circ}$, $45^{\circ}$ trunk rotation angle changing the manual angulation material. Radiographs were evaluated independently by 5 experienced observers(1 orthopedics surgery specialist, 1 diagnostic radiology specialist, 3 radiological technologist) They assessed overlap of vertebral border and axillary border of scapula as follows; Totally overlapping of vertebral border and axillary border is 4, partially overlapping is 3, not overlapping is 2 and oblique location with two borders is 1. All observers scored using by PACS monitor.$30^{\circ}$ trunk rotation was scored $1.53{\pm}0.39$, $40^{\circ}$ trunk rotation was scored $3.83{\pm}0.15$ and $45^{\circ}$ trunk rotation was scored $2.17{\pm}0.43$. There was no difference(p<0.05) between group of more than 100cm of the girth of the chest and group of less than 100cm. Similarly, men and women group showed no difference(p<0.05) with trunk rotation statistically. There was no result of trunk rotation angle to radiograph the true scapula lateral image up to date. This studies were summarized as follows; Adaptation of $40^{\circ}$ trunk rotation was the best to show the true scapula lateral image in korean. Our results were very useful to get the true scapula lateral images in clinic.

  • Prior to the advent of microvascular surgery, conventional prosthetic rehabilitation offered limited success to re-establish the physiological function in oromaxillofacial reconstruction. Microvascular surgery provided a new frontier and there are multitude of flaps. Each flap has the benefits and limitation for the application to various defects. Advantage of the scapular flaps over other reconstructive methods include the ability to design multiple cutaneous panels on a separate vascular pedicle allowing improvement in three-dimensional relationship and osseointegrated implants can be palced to restore occlusal and masticatory function. Here I present the detailed description of the important surgical anatomy as well as graft dissection and clinical application of free vascularized scapular flap.

  • We reported a patient with a large pedunculated cavernous hemangioma located beneath inferomedial border of the scapular bone and the trapezius muscle checked by ultrasonography before and after the operation. In ultrasonographic finding, the peduncle was difficult to be visualized preoperatively due to acoustic shadowing.

  • Intraosseous ganglion is a benign cystic lesion. It is composed of fibrous tissue with mucoid changes located in the subchondral bone adjacent to a joint. Intraosseous ganglion has been reported in various skeletal sites and most commonly in the lower end of the tibia and femur. However reports of intraosseous ganglion of the glenoid are rare, with only 14 cases in the literature. We repot 2 cases of intraosseous ganglion of the glenoid with literature reviews.

  • Purpose: We wanted to evaluate the results of surgical treatment for fracture of the scapular body. Materials and methods: The subjects of this study were seven patients with fracture of the scapular body and all these patients were managed surgically. We considered the operative indications as being 100% translation of the lateral border, or $25^{\circ}$ of angular deformity of the glenoid, or 1cm medialization or the variant of the double disruption of the superior scapular suspensory complex. There were five males and two females, and their ages ranged between 40 and 58 years (mean age: 49) with average follow up of 11 months (6~24 months). Two patients sustained multiple fractures of the ribs and spines. The surgical results were evaluated according to the subjective satisfaction, the UCLA score and the Korean shoulder score. Results: The mean UCLA score was 29 (17~33) and the mean Korean shoulder score was 86 (63~94). The self assessment for subjective satisfaction was 7.7 (4~9). There were two complications; one case of screw pull-out without the loss of the fracture fixation and the other case with screw penetration of the glenoid cavity. Conclusion: Surgical treatment may be effective for the management of severely displaced fracture of the scapular body in order to prevent the impairment of the shoulder function caused by the altered glenohumeral and scapulothoracic kinematics.

Development of Appendicular Skeleton in Rivulus ocellatus marmoratus (teleostomic: Aplocheilidae) (점박이송사리 Rivulus ocellatus marmoratus (진구아강 : 점박이송사리과)의 부속지골격 발생에 관하여)

  • 점박이송사리에 관한 해부학적 자료축적의 일환으로 부속지골격의 형댈 발생 및 골화 진전양상을 조사하였다. 각 부속지의 구성골은 전피성골이 연골성골보다 먼저 골화되었다. 부속지의 기조는 가슴지느러미, 구리지느러미, 등 및 됫지느러미, 그리고 배지느러미의 순으로 골화되었다. 골화 진전에 따라 초기 자어시 유함되어 있던 수직지느러미의 기골은 방사기골과 중담기골로, 가슴지느러미의 오초-견갑골은 오훼골과 견갑골로 분리, 분화되었다. 그러나 골화진전에 따른 구성 골간의 유합은 나타나지 않았다. 5가지 부속지의 골화 완성은 가슴지느러미, 꼬리지느러미, 등지느러미, 됫지느러미 그리고 배지느러미의 순으로 이루어졌다.
