A Comparison of the Results of BPTB and Hamstring ACL Autograft - Function, Stability & Tunnel Expansion - (골-슬개건-골 및 슬괵건을 이용한 전방 십자 인대 재건술의 비교 - 술후 슬관절의 기능, 안정성 및 대퇴, 경골터널 확장정도에 대하여 -)

  • Purpose : The purpose of this study was to compare the results of ACL reconstruction using bone-patellar tendon-bone to hamstring. Materials and Methods : Thirty-two ACL reconstructions with autogenous BPTB and eighteen reconstructions with hamstring were compared in terms of functional outcome, stability and tunnel expansion. Results : The functional score of BPTB group was higher than hamstring group in OAK(Orthopadishe Arbeitsguppe Knie) and IKDC(International Knee Documentation Committee) system. In BPTB group, OAK scores were 71.6$({\pm}10.0)$ preoperatively and 88.5$({\pm}7.9)$ finally. In hamstring group, OAK scores were 73.9$({\pm}11.5)$ and 82.5$({\pm}12.9)$ respectively. There was no difference in stability checked by either $KT-1000^{TM}$ or stress view. Anterior tibial translation measured by $KT-1000^{TM}$ were 2.4$({\pm}1.8)$mm in BPTB and 2.3$({\pm}2.4)$mm in hamstring group. Anterior tibial translation in stress view were 2.8$({\pm}3.4)$mm in BPTB and 2.8$({\pm}2.5)$mm in hamstring group. There was no difference in tibial tunnel expansion but femoral tunnel was more enlarged in hamstring group than BPTB (P=0.03). Conclusion : As there was no difference in stability between two groups, it seems prudent to select either graft defend on such factors as anterior knee pain, skeletal maturity and cosmetic concern. Tunnel expansion seems not to affect stability, but further study is needed to confirm that decrease of which might improve stability.

MR Evaluation of Tendinous Portions in the Subscapularis Muscle (견갑하근의 건 부분에 대한 자기공명영상을 이용한 분석)

  • Purpose: The purpose of this study was to document the structural features of the tendinous portions within the non-pathologic subscapularis muscle by performing high resolution MR imaging of the shoulder. Materials and Methods: Between April 2007 and May 2010, we retrospectively obtained the MR scans of 88 consecutive young patients (88 shoulders) who were in their twenties. MRI and MR arthrography were performed using a 3.0-T system for the evaluation of glenohumeral instability and nonspecific shoulder pain. None of the patient in this study had any evidence of injury to the tendon or muscle belly of the subscapularis. On MR images, we recorded the transverse length of a stout tendinous band and the total tendinous portion of the subscapularis. In addition, we recorded the number of intramuscular tendinous slips of the susbscapularis. Results: The mean transverse length of the tendinous band was 15.0 mm (range: 8 to 20 mm). The mean transverse length of the total tendinous portion was 48.9 mm (range: 40 to 60 mm). The number of intramuscular tendinous slips on the base of the glenoid fossa was 3 in 20 (22.72%), 4 in 45 (51.14%) and 5 in 23 shoulders (26.14%). On the lateral portion, the intramuscular tendinous slips became gradually rounder and thicker and they gave converge in the superior direction. Conclusion: In this study, the structural features of the tendinous portions of the subscapularis on the MR scans were identified. This will in return give good justification for the lines to be pulled during biomechanical stimulation and also for the surgical approach to restore the biomechanical function.

  • 본 연구는 지속적인 교통량의 증가로 인해 발생되고 있는 고속도로의 혼잡을 해결하기 위해 한국도로공사에서 도입, 시범사업을 실시하고 있는 자동요금징수시스템의 효과를 분석하고자 한다. 이를 위해 하이패스라고 하는 자동요금징수시스템의 시범운영이 실시되고 있는 서울외곽순환고속도로의 판교, 청계, 성남 톨게이트를 대상으로 정량적 측면과 정성적 측면에 대해 시범사업 시행 전.후의 상황에 대해 비교.분석하였다. 시행 전.후를 비교해 본 결과, 정량적 측면에서 각 톨게이트 별 시간대별로 다른 결과를 보이긴 하지만 이용율이 비교적 높은 오전 첨두시에는 3개 톨게이트 모두 효과가 있는 것으로 나타나, 일정수준 이상의 이용율이 있는 경우는 효과가 있는 것으로 분석되었다. 정성적 측면의 이용자 만족도 조사에서는 하이패스 이용자의 84%가 만족하는 것으로 나타났으며 비이용자 선호도 조사에서는 응답자의 91%가 하이패스 이용의사가 있는 것으로 조사되어 효과가 있는 것으로 분석되었다. 결론적으로 7,000대의 단말기 과소 보급으로 아직까지는 평균 3.9%의 저조한 이용율을 보여 시범사업의 전반적인 사업효과가 제대로 나타나지 알고 있지만, 비교적 하이패스의 이용율이 높은 오전첨두에는 효과가 있는 것으로 나타난 점을 보아 2000년 12월부터 시행되고 있는 단말기의 추가 보급이 완료되는 시점에는 그 효과가 분명하게 나타날 것으로 예상된다. 따라서. 이용자의 만족도와 비 이용자의 선호도가 매우 높은 상황에서 현재의 시범운영상에서 발생된 문제점을 개선하여 하이패스를 확대.보급한다면 고속도로의 혼잡을 해결할 수 있는 좋은 방안이 될 것으로 판단된다.

Arthroscopic Stabilization Using Remplissage Technique In Recurrent Shoulder Instability with Large Hill-Sachs Lesion - Minimum Six Months Follow-Up Results - (광범위한 Hill-Sachs 병변에서 Remplissage 술기를 이용한 관절경적 재건술 - 최소 6개월 이상 추시 결과 -)

  • Purpose: To evaluate shoulder stability, clinical, and functional results more than 6 months after utilizing the 'Remplissage' technique, consisting of an arthroscopic posterior capsulodesis and infraspinatus tenodesis, to fill Hill-Sachs lesions. Materials and Methods: Seven patients were followed-up more than 6 months after the 'Remplissage' procedures performed in our hospital from August 2008 to August 2009. The mean age of the patients was 28.6 years and the mean follow-up time was 10 months. Evaluations included ROM, ASES score, KSSI score, ROWE score, and postoperative MRI. Results: In a functional evaluation of the patients with an average postoperative time of 10 months, the ASES score improved from 51.4 preoperatively to 76.8 postoperatively, the KSSI score improved from 46.5 preoperatively to 76 postoperatively, and the ROWE score improved from 43.5 preoperatively to 76.3 postoperatively. After an average postoperative time of 10 months, the range of motion was nearly normal (>170 degrees in further flexion, and >45 degrees in external rotation). Conclusion: In recurrent shoulder instabilities with large Hill-Sachs lesions, the 'Remplissage' technique resulted in good outcomes in terms of shoulder stability, clinical, and functional results after postoperative times of more than 6 months.

  • 우리나라의 계절 특성상 여름철 집중호우가 쏟아지는 현상이 빈번하게 발생하는데 이러한 돌발홍수가 예고 없이 일어나 상습적으로 침수 피해를 입는 지역이 증가하고 있다. 본 연구에서 2009년 ~ 2019년 동안 서울시 침수 피해 사건 중심의 인터넷 기사를 기반으로 실제 침수 사례를 조사해본 결과, 침수가 가장 많이 발생한 순으로 반포동(26건), 대치동(25건), 잠실동(21건)으로 집계되었다. 침수피해가 가장 많은 반포동을 연구지역으로 선정하고 그 중 잠수교의 수위를 예측하는 연구를 진행하였다. 기존 연구에서는 수치모형에 비해 신속한 결과를 도출할 수 있는 자료 기반 모형 중 LSTM 기법을 많이 사용하였다. 그러나 이는 선행 시간이 길어질수록 첨두수위에서 과소추정된 것으로 분석된 취약점이 존재하였다(정성호 외, 2018). 본 연구에서는 이러한 단점을 보완하기 위해 GANs(Generative Adversarial Networks)를 이용하였다. GANs는 생성자와 감별자가 나뉘어 생성자가 실제 자료인 첨두수위에서의 잠수교의 수위를 학습하고 실제와 근접한 가상데이터를 결과로 생성하여 감별자는 그 생성된 미래의 잠수교의 수위가 실제인지 가상인지 판별하도록 학습시키는 신경망 구조이다. 사용한 수문자료는 한강홍수통제소, 기상청, 국립해양조사원에서 제공하는 최근 15년간의 (2005년~2019년) 수위, 방류량, 강수량, 조위 자료를 수집하였고 t-test와 상관성분석을 통해 사용한 인자 간의 유의미성 판단과 상관성을 분석했다. 또한, 민감도 분석 결과 시퀀스길이(5), 반복횟수(1000), 은닉층(10), 학습률(0.005)로 최적값을 선정하였다. 또한 학습구간(2005년~2014년)과 검증구간(2015~2019년)으로 나누어 상대적으로 높은 수위가 관측되는 홍수기의 3, 6, 9시간 후의 수위를 예측하고 오차 지표를 이용해 평가하였다. LSTM 기법으로 예측된 수위와 GANs로 예측된 수위를 비교한 결과 GANs으로 예측된 첨두수위에서의 정확도가 5% 정도로 향상되었다. 향후에는 다양한 영향인자와 다른 기법과의 결합을 고려한다면 보다 정확하게 수위를 예측하여 하천 주변 사회기반시설의 침수 피해를 감소시킬 것으로 판단된다.

  • In this study, a sample preparation method and a simultaneous determination method by ultra-high performance liquid chromatography coupled with tandem mass spectrometry for 9 isomers of chlorogenic acid and arbutin in fruits were developed. The samples were extracted using 90% methanol (pH 3.0), with the solutions being shaken and then sonicated for 10 min each. After centrifugation at 4,000 rpm for 10 min, the extraction was concentrated under a vacuum at $40^{\circ}C$ using a vacuum evaporator. The residue was dissolved in 5 mL of 5% methanol and filtered through a $0.45{\mu}m$ membrane before UHPLC-MS/MS analysis. The separations were performed on a C18 column with gradient elution of water (containing 0.1% formic acid) and methanol (containing 0.1% formic acid). The specificity, linearity, limit of detection, limit of quantification, accuracy, and precision of the proposed methods were also evaluated.

Estimation of Hydrodynamic Derivatives and Dynamic Stability for Submarine Using Captive Model Test (구속모형시험을 이용한 잠수함의 동유체력 계수 추정 및 동안정성 평가)

  • In these days, the world has been increasing the development of various underwater vehicles such as ROVs (Remotely operated underwater vehicles) and AUVs (Autonomous underwater vehicles). And the importance of submarine's maneuverability is especially being emphasized. Therefore, accurate values of the derivatives in equations of motion are required to control motion of the submarines. The aims of the present study are to experimentally derive Hydrodynamic derivatives derived by the vertical planar motion mechanism (VPMM) model test, and to estimate vertical dynamic stability was estimated by using the linear hydrodynamic derivatives, the hydrodynamic derivatives of the submarine, which have a high propriety, were provided by using the fourier analysis of measured forces and moments. Furthermore it is confirmed that the experimental derivatives shows well agreement with the theoretical estimations, and the dynamic stability of the submarine was estimated as a good state, which implies that the value is greater than zero.

Double Bundle Posterior Cruciate Ligament Reconstruction by the Tibial Inlay Method - Technical note - (경골 내재술식을 이용한 이중 다발 후방 십자 인대 재건술 - 수술 기법 -)

  • Purpose: The purpose of this study is to introduce the double bundle posterior cruciate ligament(PCL) reconstruction using Achilles allograft by the tibial inlay method and evaluate the clinical results of 11 cases who had PCL reconstruction using this method and were followed for more than 2 years after surgery. Materials and Methods: Our series consists of 11 cases of PCL reconstruction due to chronic posterior instability of knee and acute PCL rupture. The clinical results were assessed using the IKDC(International Knee Documentation Committee) scoring system, posterior stress radiographs and the maximum posterior displacement using a KT-1000TM arthrometer. Results: The average preoperative posterior displacement in 90 degree flexion stress radiograph was measured 13.4 mm and in 10 degree flexion the average posterior displacement using the KT-1000TM arthrometer was measured 11.4 mm. Postoperatively the 13.4 mm reduced to 4.4 mm and the 11.4 mm reduced to 3.9 mm. According to IKDC scoring system, 9 cases(81.8%) were satisfied. One case showed limitation of flexion with mild stiffness in the knee and another one case was not improved the posterior instability sufficiently and no complication of allograft was noticed. Conclusion: The double bundle PCL reconstruction using Achilles allograft by the tibial inlay method is a useful method for acute PCL rupture and chronic posterior instability of the knee including failed PCL reconstruction, because it will also make the posterior stability in the extension and 90 degree flexion position, and avoid the grafted tendon abrasion by acute turn of tibial tunnel.

Analysis of revision anterior cruciate ligament reconstruction (전방 십자인대 재재건술의 분석)

  • Purpose: As the number of the anterior cruciate ligament (ACL) reconstruction has increased, the incidence of revision of ACL reconstruction due to reconstruction failure has been also increased. Therefore, authors analyzed the reason of the failure of ACL reconstruction and the clinical result of the ACL revision. Materials and methods: From February 1998 to July 2010, we selected 36 cases which was followed at least 12months after the ACL reconstruction failure. Duration from reconstruction to revision, the average duration was 60 months (5~334) and on first reconstruction, we used allograft on 23 cases (63.9%) and autograft on 13 cases (36.1%). For the main symptom of ACL reconstruction failure, instability was the most common symptom, and 35 cases (97.5%) were undergone only one reconstruction and 1case (2.5%) was undergone two reconstruction. Clinical results were evaluated by Lysholm knee joint score, pivot shift test, and Telos device. Results: Average follow-up duration of the patients was 21 months (12~40), and the reason for the ACL reconstruction failure, trauma was most common by 19 cases (52.8%), malposition of the femoral tunnel was 13 cases (36.1%), malposition of the tibia tunnel was 1case (2.8%), and failure of osteointegration was 3 cases (8.4%). On performing the ACL revision, we used allograft on 34 cases (94%) and autograft on 2 cases (6%), and 21 cases accompanied injury of the meniscus (medial meniscus 14 cases, lateral meniscus 7 cases). Lysholm knee joint score was improved from 66.5 points, preoperatively to 92 points on last follow-up (p<0.01). In most cases, patients were satisfied (92%) with the operation results. Tegner activity score was also improved from 2.0 points preoperatively to 6.2 points on the last follow-up. On Lachman and pivot sift test, 33 cases and 30 cases were improved to grade I respectively, and on stability test using Telos device, the bilateral difference was improved from mean 15.5 mm preoperatively to 4.5 mm on the last follow-up. Conclusion: After 1 year follow-up, Revision of ACL had a little anterior instability but it showed satisfactory result on clinical result and patient's subjective satisfaction.

Content and leaching characteristics of non-regulated hazardous substances in waste from the paint industry (국내 도료공정 발생 폐기물 중 미규제 중금속류의 배출특성)

  • This study was performed to investigate the contents and leaching characteristics of hazardous wastes from the paint industry. In order to establish a hazardous waste list, samples from industrial discharge have been analyzed for 8 non-regulated inorganic hazardous substances (i.e., Sb, Ni, F, V, Ba, Zn, Be, Se). In more detail, hazardous waste samples from a total of 64 workplaces, e.g. manufacture, formulation, supply and use (MFSU) of coatings, adhesives, sealants and printing inks processing, have been chosen and analyzed. Contents and leaching tests for inorganic metal species in samples show that the non-regulated hazardous substances satisfy all the criteria, while quantitative analyses reveal that some samples of the discharged wastes exceeded the criteria proposed by NIER (National Instituted of Environmental Research). In conclusion, we expect the outcome of this study to align the classification system of hazardous waste management in South Korea with international legislations, and consequently contribute to reduce environmental pollution as well as health risks by toxic wastes.
