Objective : Ankylosing spondylitic is a chronic inflammatory disease which most frequently the sacro-iliac joints of young men and known as rare disease. There was no clinical report on functional assessment of ankylosing spondylitis. In order to evaluate the effect of clinical treatment, it was necessary to apply valuable and useful of assessment. Methods : We evaluate 1 cases of diagnosed patient with askylosing spondylitis by AIMS2, M-HAQ, K-HAQ, BASFI, and BASDAI after a series of herbal acupuncture treatment.(Solution of herbal acupuncture is made of vaporizing extraction method from the prescription Sineumheo 1.) Results : Herbal acupuncture had relieved pain and morning stiffness successfully and recovered ankylosing spondylitis functionally. Conclusion : 1. Herbal acupuncture had relieved pain and morning stiffness of ankylosing spondylitis. 2. AIMS2 was not useful to evaluate ankylosing spondylitis because of long evaluation time and low specificity. 3. Although there is no translation version of them, BASFI, and BASDAI are clinically useful of assessment, because of short evaluation time and high specificity. 4. K-HAQ with M-HAQ is useful to evaluate the treatment of ankylosing spondylitis because of snort evaluation time and high specificity.
Background & Object: Ankylosing spondylitis (AS) is a chronic inflammatory disease that causes ankylosis and deformation of axial joints. Since current medicine cannot cure the disease yet, alleviating pain and preventing deformation with medications are the main therapy for patients with AS. The key medications for these purposes include nonsteroidal anti-inflammatory drugs (NSAIDs), and tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) inhibitors. This study aims to analyze prescribing patterns of AS patients in South Korea. Method: National Patients Sample data compiled by the Health Insurance Review and Assessment Service from 2013 was analyzed. Patients with AS were identified with Korean Standard Classification of Diseases code-6, which was M45. The rates of prescription, discontinuation, and switching ingredients were calculated for each medication during 2013. Results: Total number of patients was 655, and most of them were male (n = 514, 78.5%). Of all age groups, the proportion of 30-40 year old patients was the greatest (35.1%). The most utilized drug class was NSAIDs (82.4%). Less than half of patients were prescribed $TNF-{\alpha}$ inhibitors (n = 212, 32.4%). Meloxicam, aceclofenac, and celecoxib were the most frequently prescribed NSAIDs. In case of $TNF-{\alpha}$ inhibitors, adalimumab, etanercept and infliximab were the top three most prescribed drugs. Although not recommended by the current practice guideline, significant proportions of patients were identified using disease modifying anti-rheumatic drugs (DMARDs). Conclusion: Considering the current practice guideline and previous studies about the efficacy, the use of DMARDs should be reduced and medical insurance term in South Korea should be re-examined.
Ankylosing spondylitis(AS) is one of the most common forms of chronic arthritis causing pain and progressive disability. The purposes of this study were 1) to develop an offline educational program and online educational program for the patients with ankylosing spondylitis; and 2) to test the effectiveness of each program in terms of changes in ankylosing spondylitis self-efficacy, exercise continuity disease activity, physical functioning, and depression. The convenient samples for three groups were selected acceding to the inclusion criteria. Online education group consists of 7 patients, and 14 patients were included for the offline group, while 12 patients were assigned to the control group. Outcomes variables included self efficacy, exercise continuity, disease activity, physical function, and depression and the reliability of each measure was ranged from .88 to .95 in the present study. The offline educational program demonstrated positive effects on self-efficacy, exercise continuity, disease activity, and depression, whereas the online educational program on exercise continuity, disease activity, and depression. The self-efficacy was showed positive effect only in the offline educational program, whereas physical functioning did not improve in both intervention groups. These partial improvements may be due to the small sample size and the methods of online education program. In conclusion, the effectiveness of the offline and online educational programs was partially supported in the present study. For the improvement of these programs, the further study would be necessary to apply this kind of intervention program with larger sample and to test the psychometric of the newly development tools.
To evaluate the diagnostic usefulness of quantitative sacroiliac joint scintigraphy and its clinical significance in the ankylosing spondylitis, we measured SIS ratio (Sacroiliac Joint/Sacrum Uptake ratio) by region of interest method in 52 patients with ankylosing spondylitis. As compared with radiological grade, SIS ratio was significantly elevated in the early stage of ankylosing spondylitis and returned to normal range in the late stage. Scintigraphic finding of focal uptakes of radiotracer at multiple spinal apophyseal joints was characteristic and proximal large joints were more involved than distal joints. The incidence and extent of the arthropathy revealed by scnitigraphy exceeded that which was clinically evident, and scintigraphy was more sensitive to detect symptomatic lesion than radiography. Therefore, it is suggested that bone scan is useful for the early diagnosis and the assessment of clinical activity of involved joints of ankylosing spondylitis.
Objectives This study was performed to research the trends of Chinese acupuncture treatment for ankylosing spondylitis (AS). Methods We searched clinical studies about Chinese acupuncture treatment for AS through China National Knowledge Infrastructure. We analyzed the characteristics of selected studies according to research design, sample size, publication year, treatment methods, evaluation criteria and adverse events. Results 16 studies published from 2015 to 2020 were selected. They consisted of 11 randomized controlled trials and 5 case reports. In these studies, several types of acupuncture treatments were performed and especially there were many treatments that applied thermal stimulation of needles. The most commonly used evaluation criteria was effective rate. Conclusions This study showed that acupuncture treatment could be effective for AS. It suggested that various studies should be conducted to provide reliable evidence about acupuncture treatment for AS in Korea.
Objective: The aim of the study was to determine the effects of traditional Korean medicine treatments on a patient diagnosed with ankylosing spondylitis. Case Summary: A 29-year-old male patient suffering from lower back pain and chest pain was diagnosed with ankylosing spondylitis. Korean medicine treatments, including acupuncture, pharmacopuncture, and herbal medicine, were applied. The treatment effect was evaluated with a numeral rating scale (NRS) for pain and by changes in C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). After 44 days of treatment, the patient's symptoms of ankylosing spondylitis were improved. The NRS score for lower back pain decreased from 3 to 2, and the NRS score for chest pain decreased from 10 to 1. In addition, the ESR level decreased from 13 to 10 and the CRP level decreased from 1.39 to 0.84. Conclusion: The integrative Korean medicine treatments appeared to be effective in relieving pain associated with ankylosing spondylitis.
Objectives : Ankylosing Spondylitis, inflamatic disease, appeared symostosis, pain and dyscinesia in spine and sacroilic joint. It repeated an improvement and a depravation. We observed one patient with Ankylosing Spondylitis. Methods : This patient was juged incurable in points of western medical's View. But he was improved by treating oriental medical therapy. The therapies which used in this patient was acupunture, bee venom, korean chuna therapy and kinesiatrics etc. We measured the R.O.M, special test and degree of pain every day. Results : The R.O.M of extension in lumbar ${vertebrae(0_{\circ}\;{\rightarrow}5_{\circ}\;)\;and\;patrick\;sign(++/-\;{\rightarrow}\;-/-)}$ was improved a little. Degree of pain was improved better than admission. Conclusions : This case has demonstrated that the Ankylosing Spondylitis treated by oriental medical therapies proved good results.

Purpose: The purpose of this study was performed to understanding of family support, pain and self-esteem, activities of daily living(ADL) in patient with ankylosing spondylitis(AS) and to identify the relationship among variables. Method: This study was conducted on 55 adults diagnosed with AS in C hospital in Daejeon during January 15th to June 20th, 2004. Instruments in this study were pain scale (VAS), family support scale, self-esteem scale, ADL scale. The data were analyzed mean, standard deviation, one-way ANOVA, Pearson's Correlation Coefficient using SPSS WIN(ver 10.0) program. Results: 1) 83.6% of the subjects was men, 30.9% of the subjects' age was below 29. And the 90.9% of the subjects was no experience of education about AS, 41.8% of the subjects was doing exercise. 2) The pain score was 48.36, family support score was 3.98, self-esteem score was 3.52 and ADL score was 81.41. 3) Family support was positively correlated with self-esteem and ADL and negatively correlated with pain. self-esteem was negatively correlated with pain and ADL. Conclusion: The results in this study can help nurse who care patients with AS understanding the relationships among family support, self-esteem, ADL, pain. And this findings showed that the patients with AS have few experience for disease-related education to manage and to understand AS. Therefore disease-related educational nursing program based on family support, pain, ADL of AS is needed to understand and manage AS. Because family support, pain, ADL of AS were significant correlated.
Objective : Ankylosing Spodylitis(AS) is a chronic inflammatory disease that is notorious for its difficulty to cure. This study aims to interpret the mechanism of AS in the Traditional Korean Medical(TKM) perspective, and to understand the role of the spine. Method : Clinical manifestations of AS, epidemiological data and clinical cases were interpreted by traditional methodology. Result : The young age group of patients suffering from this condition suggests that the cause of this condition is not solely linked to deficiency of the Kidney[腎], which is a general cause for bone disease. Its symptoms are linked to obstruction in the upper body, which results in disturbance of Jeong(精) collection in the lower body. Based on other accompanying symptoms, together with its character as an autoimmune disease, a strong link to the spirit of the heart[心神] can be suggested. Conclusion : AS is closely linked not only to essence of kidney[腎陰], but to the spirit of the heart[心神] as well. From the TKM analysis of the spine, we can understand it as the basic pathway for the fundamental Gi(氣) circulation.
