Disease Prevalence, Parent's Educational Needs, and Disease Management according to Severity of Respiratory Infections in Early Childhood (영유아 호흡기 감염 중증도에 따른 이환실태, 부모의 교육요구 및 관리수행정도)

  • Purpose: The purpose of this study was to investigate disease prevalence, parent's educational needs, and disease management according to severity of respiratory infections in early childhood. Methods: Participants for this study were 173 mothers whose child was admitted to I university hospital in Seoul and whose child was an infant or toddler. Data were collected from December, 17, 2014 to February, 15, 2015 using self-report structured questionnaires. Data were analyzed using IBM/SPSS 21.0 program. Results: There were significant differences in the severity of respiratory infection according to neonatal admission due to dyspnea, feeding type, atopic dermatitis in the infant or allergic disease in father and siblings. Parent's educational needs for the severe respiratory infection group were higher than for the non-severe group. Parent's disease management for the severe respiratory infection group was lower than the non-severe group. Conclusion: As important care factors in neonatal admissions include dyspnea, cow milk feeding, eczema, family history of allergies, parent's educational needs and disease management, they should be considered when caring for young children with respiratory infections and their parents.

Recognition and Performance on Management for Nosocomial Infections among Nursing Students (병원감염관리에 대한 간호학생의 인지도와 수행도)

  • Purpose: This study was designed to identify recognition and the performance level for nosocomial infections amongst student nurses and to provide data for preparing on efficient policy and control program for nosocomial infections. Method: 191 senior nursing student participated in this research from 5 nursing college which allowed data collection, in the city of Seoul. The questionnaire was composed of 73 items(likert scale) about eight areas; aseptic technique, disinfection, precaution, hand washing, urinary tract infection, respiratory infection, catheter related infection, and self care about nosocomial infections. Results: The mean score of recognition and performance level for management of nosocomial infection were 4.29 and 3.41 respectively. The mean score of the recognition level was significantly lower than the performance level in the eight areas. The mean score of both recognition and performance were highest in the area of disinfection. However, the mean score of recognition was lowest in the area of aseptic technique and the mean score of performance was lowest in the area of catheter related infections. The correlation of recognition and performance level was statistically significant. Conclusions: These research findings should be useful in promoting an intensive and continuous educational program on nosocomial infection for nursing students and to establish an efficient policy for preventing nosocomial infections.

Identification of Microorganisms from Eggs in Hypermarket in the Northern Gyeonggi Area (경기 북부 일부 지역 대형 마트 유통계란에 오염된 미생물의 분리)

  • Microorganisms or their toxins can be transferred to eggs and cause food poisoning in humans. Therefore, this study was conducted to detect microbial contamination of eggs and to identify microorganisms in any contaminated eggs. Four different brands of eggs were collected from hypermarkets in the northern Gyeonggi area. The total bacterial counts on the shells of the eggs varied greatly between brands. In addition, various bacterial species including Klebsiella pneumoniae, Pseudomonas mendocina, Alcaligenes xylosoxidans, Alcaligenes faecalis, and Enterobacter cloacae were identified on eggshells. Furthermore, mean of total bacterial counts of four brands was $3.4{\times}10^4 cfu/m{\ell}$ and E. coli was detected on the eggshell of one brand egg. However, Salmonella was not identified on all brands of collected eggs. We also demonstrated that the E. coli isolated from the eggshell was not pathogenic based on the absence of pathogen-specific gene expression patterns. Taken together, the result of this study indicate that strict quality control and improved distribution controls are required to decrease microbial contamination and improve human health.

Molecular Genetic Characteristics of Methicillin-Resistant Staphylococcus aureus Isolated from Patients and Environment of General Hospital Intensive Care Unit in a Chungnam Province, Korea (충남지역 종합병원 중환자실 환경과 환자로부터 분리한 메티실린 내성 황색포도알균(MRSA)의 분자유전학적 특성)

  • In the present study, mec complex typing and SCCmec typing were performed to analyze the molecular genetic characteristics of 20 methicillin-resistant Staphylococcus aureus (MRSA) strains isolated from clinical specimens and 4 strains isolated from the ICU environments of secondary medical institutions in a Chungnam province, Korea, from June to July of 2017. Among a total of 20 MRSA strains isolated from clinical specimens, 8 cases (40%) were SCCmec type II, one case (5%) was SCCmec type IVa, and 11 cases (55%) were not-typeable in SCCmec type analysis. Among 4 MRSA isolates from the ICU environment, one strain did not have the mecA gene and 3 strains were typed as SCCmec types II, III, and IVa, respectively. Data from the present study showed that the origin of MRSA isolated from the clinical specimens was different from those from the ICU environment in most cases but the origin was concordant in one case. In this case, MRSA might be transmitted by healthcare workers to the ICU environment. Further study with a large number of cases and other hospital infection-related microorganisms will be needed. This continuous follow-up study might provide useful information on infection control in medical institutions.

Research Areas and Trends in Articles on Pediatric and Adolescent Tuberculosis in Korea (국내 소아청소년 결핵 관련 논문의 연구 영역과 동향)

  • Purpose: Despite the decline in tuberculosis (TB) incidence and mortality rates in the Republic of Korea through a national TB control program, TB remains one of the most critical infectious diseases in Korean children. We investigated the trends and research areas of published articles on TB in Korean children and adolescents. Methods: In 6 Korean and overseas databases, we searched titles and abstracts including "tuberculo*" or "TB," "child*" or "adolescen*" or "neonat*" or "infant*" or "pediatric*," and "korea*." The publication type, publication year, research areas, journal title, and research subjects were analyzed. Results: Out of the 257 searched documents, 120 papers were included in the analysis. Of these, 82 were original articles (68.3%), 33 case reports (27.5%), 4 review articles (3.3%), and 1 guideline (0.8%). In the original articles, the most common subject of studies was the clinical characteristics of patients with TB (36.6%), followed by diagnostics (29.3%), contact investigations (9.8%), epidemiology (6.1%), treatment (4.9%), vaccine (3.6%), latent TB infection (3.6%), complications (3.6%), and surveys on perception of TB (2.4%). From 1962, 4 articles were published in the 1960s, 10 articles in the 1970s, 11 articles in the 1980s, 22 articles in the 1990s, 26 articles in the 2000s, and 47 articles since 2010. Conclusions: The amount of research on TB in Korean children has increased over the past 5 decades; however, it has mainly focused on the clinical characteristics and diagnostics. Research in different areas, such as treatment and vaccine, is needed in the future.

Comparison of Molecular Characterization and Antimicrobial Resistance in Carbapenem-Resistant Klebsiella pneumoniae ST307 and Non-ST307 (Carbapenem 내성 Klebsiella pneumoniae ST307과 Non-ST307의 분자 특성 및 항균제 내성 비교)

  • Carbapenem-resistant Klebsiella pneumoniae (CRKP) is emerging as a worldwide public health threat. Recently, Klebsiella pneumoniae carbapenemase-2 (KPC-2)-producing sequence type (ST) 307 was identified main clone of CRKP, and dissemination of ST307 was reported in South Korea. This study examined the molecular characteristic and antimicrobial resistance pattern of 50 CRKP isolated from a tertiary hospital in Daejeon, from March 2020 to December 2021. Epidemiological relationship was analyzed by Multilocus sequence typing (MLST) and antimicrobial susceptibility test was determined using disk-diffusion method. PCR and DNA sequence analysis were performed to identify carbapenemase genes. CRKP infections were significantly more frequent in males and the patients aged ≥ 60 years. Among the 50 CRKP isolates, 46 isolates (92.0%) were multidrug-resistant (MDR), and 44 isolates (88.0%) were carbapenemase-producing K. pneumoniae (CPKP). The major carbapenemase type was KPC-2 (36 isolates, 72.0%) and New Delhi metalloenzyme-1 (NDM-1) and NDM-5 were identified in 7 isolates (14.0%) and 1 isolate (2.0%), respectively. In particular, 88.9% (32/36) of KPC-2-producing K. pneumoniae belonged to ST307, whereas 87.5% (7/8) of NDM-1,-5-producing K. pneumoniae belonged to non-ST307. These results suggest that proper infection control and effective surveillance network need to prevent not olny the spread of ST307, but also the development of non-ST307.

Strategies for Public Health Service Development in the Times of Local Autonomy (지방자치시대의 공공보건사업 발전 전략)

  • Health is a fundamental human right and a sine qua non for happiness of people and for national development. Government has a responsibility for the provision of health services for their people. Recent changes of disease pattern, i.e. decrease of Infectious diseases and increase of chronic and degenerative diseases Including cancer and cardiovascular diseases, together with universal coverage of health insurance and improved living standard have prompted medical care utilization and skyrocketed the national health expenses. The goal of national health policy is improving the quality of life through the betterment of health level. To achieve this goal it is necessary to establish a healthcare system for lifetime, to improve the efficiency of healthcare delivery system, and to strengthen the public health services for disease prevention and health promotion. The current public health service programs are Inefficient due to an inconsistent policy for health service program, lack of health information system, irrational health program planning and evaluation, and Inadequate training of health workers. Local government has a legal responsibility for health service program planning and promoting the competence of health workers. Thus, municipal and provincial health departments should expand their roles and strengthen their function. The strategies for developing public health service programs at local level are ${\circled}1$ stipulating the goals of health policy, ${\circled}2$ promoting the ability for health program planning and evaluation, ${\circled}3$ establishing health information and surveillance system, ${\circled}4$ training of health workers, ${\circled}5$ establishing an institution for health information management and training of health workers, and ${\circled}61$ collaboration with local universities.

Effect of COVID-19 Stress on Online Shopping Addiction: Mediating Effects of Depression on Moderated by Gender (코로나19 스트레스가 온라인 쇼핑중독에 미치는 영향: 성별에 의해 조절된 우울의 매개효과)

  • This study aimed to identify the correlation between COVID-19 stress, depression, and online shopping addiction in adults, and to confirm the mediating effects of depression on moderated by gender. An online survey was conducted by requesting a survey organization that has panels across the country. The collected 336 data were analyzed using Process Macro for SPSS. As a result of the study, online shopping addiction increased as COVID-19 stress increased. And the mediating effect of depression was found to be significant in the effect of COVID-19 stress on online shopping addiction. Furthermore, in the effect of COVID-19 stress on online shopping addiction, the mediating effect of depression moderated by gender was found to be significant. As the COVID-19 infectious disease prolongs, it is necessary to consider gender in interventions for behavioral addiction prevention, such as online shopping, and mental health management.

Analysis on the Multiple Frequency Disease Trend of Yeongbuk in Gangwon-do (강원도 영북권역 다빈도 질환 추세분석)

  • The purpose of this study was to analyze the frequency of use of hospitalization/outpatient care of patients with addresses in Yeongbuk-gu, Gangwon-do to identify the medical demand and status of medical use in Yeongbuk-do. The National Health Insurance Corporation (www.data.go.kr) conducted an analysis of health insurance and medical benefits (inpatient/outpatient) claims data from 2003 to 2017. The order of the highest frequency of chronic disease was ranked from 1st to 60th according to 'Healing Case and %'. In addition, the use of patients at Sokcho Medical Center, the only hospital in the Yeongbuk region, was analyzed at the same time. According to the analysis, the use of outpatient treatment and In-patients in the Yeongbuk area of Gangwon-do is increasing due to chronic non-infectious diseases from the past acute diseases. In particular, it is necessary to expand the functions of schizophrenia and obstetrics and gynecology and to carry out specific health projects necessary for the health care of the local community. Through this, we should improve the quality of life in Yeongbuk, Gangwon Province.

Study on the Methodology of the Microbial Risk Assessment in Food (식품중 미생물 위해성평가 방법론 연구)

  • Recently, it is continuously rising to concern about the health risk being induced by microorganisms in food such as Escherichia coli O157:H7 and Listeria monocytogenes. Various organizations and regulatory agencies including U.S.FPA, U.S.DA and FAO/WHO are preparing the methodology building to apply microbial quantitative risk assessment to risk-based food safety program. Microbial risks are primarily the result of single exposure and its health impacts are immediate and serious. Therefore, the methodology of risk assessment differs from that of chemical risk assessment. Microbial quantitative risk assessment consists of tow steps; hazard identification, exposure assessment, dose-response assessment and risk characterization. Hazard identification is accomplished by observing and defining the types of adverse health effects in humans associated with exposure to foodborne agents. Epidemiological evidence which links the various disease with the particular exposure route is an important component of this identification. Exposure assessment includes the quantification of microbial exposure regarding the dynamics of microbial growth in food processing, transport, packaging and specific time-temperature conditions at various points from animal production to consumption. Dose-response assessment is the process characterizing dose-response correlation between microbial exposure and disease incidence. Unlike chemical carcinogens, the dose-response assessment for microbial pathogens has not focused on animal models for extrapolation to humans. Risk characterization links the exposure assessment and dose-response assessment and involve uncertainty analysis. The methodology of microbial dose-response assessment is classified as nonthreshold and thresh-old approach. The nonthreshold model have assumption that one organism is capable of producing an infection if it arrives at an appropriate site and organism have independence. Recently, the Exponential, Beta-poission, Gompertz, and Gamma-weibull models are using as nonthreshold model. The Log-normal and Log-logistic models are using as threshold model. The threshold has the assumption that a toxicant is produce by interaction of organisms. In this study, it was reviewed detailed process including risk value using model parameter and microbial exposure dose. Also this study suggested model application methodology in field of exposure assessment using assumed food microbial data(NaCl, water activity, temperature, pH, etc.) and the commercially used Food MicroModel. We recognized that human volunteer data to the healthy man are preferred rather than epidemiological data fur obtaining exact dose-response data. But, the foreign agencies are studying the characterization of correlation between human and animal. For the comparison of differences to the population sensitivity: it must be executed domestic study such as the establishment of dose-response data to the Korean volunteer by each microbial and microbial exposure assessment in food.
