Influences of Emotional Labor, Hardiness on Job Satisfaction of Nurses in Comprehensive Nursing Care Service Units (간호·간병통합서비스 병동 간호사의 감정노동, 강인성이 직무만족도에 미치는 영향)

  • The purpose of this descriptive research study was to identify the effects on job satisfaction of nurses working at comprehensive nursing care service units. The study was conducted in five hospitals, two of 250 beds or more in three cities and three of 800 beds. The study subjects were nurses working at comprehensive nursing care service units. A structured questionnaire of 150 questions was distributed to them and 146 were collected. The data collection period was from August 1, 2019 to November 15, 2019. The collected data were analyzed using the SPSS 22.0 program, using x2-test, t-test, ANOVA, Pearson correlation, and multiple linear regression. Emotional labor and toughness were negatively correlated (r=-.20, p=.018), and emotional labor and job satisfaction were positively correlated (r=.25, p=.004). The predictive variables affecting the job satisfaction of the subjects were the aptitude of the ward service (β=-.35, p=<.001) and hardness(β=.21, p=.009). The explanation of job satisfaction in the variables was 18.2% (F=3.54, p=.009). The study results revealed that the predictive variables affecting job satisfaction were the hardiness and aptitude of the ward service. Therefore, in order to increase the job satisfaction of ward nurses, it is necessary to provide administrative support to control the hardiness of the nurse and to allocate manpower in consideration of the aptitude in the hospital personnel system.

Development and Application of an Online Clinical Practicum Program on Emergency Nursing Care for Nursing Students (간호학생의 응급환자간호 임상실습 온라인 프로그램 개발 및 적용)

  • Purpose: Clinical practicums via non-face-to-face methods were inevitable due to the COVID-19 pandemic. We developed an online program for emergency nursing care and identified the feasibility of the program and the learning achievements of students. Methods: This was a methodological study. The program was developed by three professors who taught theory and clinical practicum for adult nursing care and clinical experts. Students received four hours of video content and two task activities every week in four-week program. Real-time interactive video conferences were included. Qualitative and qualitative data were collected. Results: A total of 96 students participated in the program. The mean score for overall satisfaction with the online program was 4.72(±1.02) out of 6. Subjects that generally had high learning achievement scores were basic life support care, fall prevention, nursing documentation, infection control, and anaphylaxis care. As a result of a content analysis of 77 reflective logs on the advantages of this program, students reported that "experience in applying nursing process," "case-based learning and teaching method," and "No time and space constraints" were the program's best features. Conclusion: Collaboration between hospitals and universities for nursing is more important than ever to develop online content for effective clinical practicum.

Influence of Humanistic Knowledge and Occupational Stress on Professional Quailty of Life of Clinical Nurses (임상간호사의 인문학적 소양과 직무 스트레스가 전문직 삶의 질에 미치는 영향)

  • This study examined the influence of humanistic knowledge and occupational stress on the professional quality of life of clinical nurses. The data from 217 survey questionnaires collected from three general hospitals located in a metropolitan area from Jun 3 to 14, 2019 were used. The data were analyzed using a t-test, ANOVA, Scheffè test, Pearson's Correlation Coefficient, and Stepwise multiple regression with SPSS Statistics 23.0 program. The findings revealed a positive correlation between compassion satisfaction and humanistic knowledge and a negative correlation with Burnout. Burnout had a negative correlation with humanistic knowledge and a positive correlation with occupational stress and secondary traumatic stress. Secondary traumatic stress showed a positive correlation with occupational stress. Burnout, satisfaction for nursing, and working period were significant variables of the compassion satisfaction with an explanatory power of 48.5%. Compassion satisfaction, secondary traumatic stress, satisfaction for nursing, marriage, and occupational stress were significant variables of burnout with an explanatory power of 68.8%. Burnout and income were significant variables of secondary traumatic stress with an explanatory power of 21.9%. Burnout was found to be a significant variable of not only compassion satisfaction, but also compassion satisfaction. Therefore, efforts are needed to make efforts to prevent burnout.

Affecting Factors on Clinical Competence of Nursing Students (간호대학생의 임상수행능력 영향 요인)

  • The purpose of this study was to investigate the level of clinical competence and influencing factors of clinical competence in nursing students. Convenience sample of 197 subjects were recruited from two colleges in D city. The survey was conducted from March 2nd to 21th 2011. The data was collected by questionnaires and was analyzed with SPSS statistics 21.0. The mean score of clinical competence was 3.96. Significant positive correlation among clinical thinking disposition, problem solving ability, major satisfaction, satisfaction with clinical practice, clinical competence were found. It was found that the effect of clinical thinking disposition, completion of logic course, problem solving ability on clinical competence was 27.2% and clinical thinking disposition influence on clinical competence in nursing students. The study findings suggest that to develop the effective learning program with achievement level of clinical competency for nursing students.

구전 커뮤니케이션 유형에 따른 의료서비스 만족도에 관한 연구

  • 오늘날 병원마케팅에서 간호서비스는 병원 내 어느 전문직보다도 가장 많은 시간을 고객인 환자와 합께 나누며 환자의 욕구 충족을 위하여 고도의 분업화된 제반 부서들간의 활동을 조정하고 융화시키는 역할올 통하여 환자들이 지각하는 병원서비스의 만족도에 종요 한 부분을 차지하고 있으므로 병원마케팅의 전략적 초점으로 전환될 필요가 있다. 의료 서비스는 특성상 전문성이 강해서 정보가 소비자에게 공유되지 못하고 있으며 비영리조직이라는 인식하에 의료 서비스에 대한 광고를 제한합으로써 실제적으로 소비자에게 필요한 의료 관련 정보들마저도 제한을 받는 경우가 많이 있다. 그리고 의료서비스의 무형성, 비분리성, 이질성, 소멸성의 특성으로 어떠한 의료 서비스를 이용할 것인지해 대한 결정을 고객이 하기 어렵기 때문에 오늘날 의료 소비자들이 의료제공 기관을 선택 할 때는 밑올만한 다른 사랑의 추천에 크게 의존하고 있는 것으로 알려져 있으며, 국내의 연구에서도 의료소비자들이 병원을 선택하는 주된 정보 획득 원천이 주변의 제 3자인 것으로 알려져 있다. 이러한 정보 전달에 구전 커뮤니케이션이 주요한 제3자적 억할을 한다. 이러 한 관접에서 불 때 의료 소비자둘 사이의 구전 커뮤니케이션은 마케 팅 전략상 매우 종요하 며 마케팅 관리측면에서 의료 소비자들의 구전 커뮤니케이션 형태톨 파악하는 것은 매우 중요한 일이라고 할 수 있다. 이에 본 연구는 구전 커뮤니케이션의 내용 및 경로 그리고 유형을 파악하고 이들 유형간 의료서비스의 만족도를 알아봄으로써 효파적인 병원 마케팅 전략을 수립하는데 필요한 자료를 제공하고자 한다. 본 연구의 결과를 증합해 보면 의료소비자들이 연령 성별 학력, 소득, 직업, 주거지가 비슷한 사람과 구전이 활발하게 일어냐는 것을 파악 할 수 있어 특히 상업적 광고가 제한 된 병원의 마케팅 전략에 oH우 유용한 것이 훨 수 있다. 또한 불만족한 소비자일수록 발신 자적 입장에 서서 주위의 사람들에게 활발한 구전 활동을 수행하고 었음을 알 수 있다. 이 는 의료 소비자들이 만족한 경우보다는 불만족한 경우에 더욱 구전을 수행하려는 의지가 강해짐올 시사한다. 따라서 병원은 소비자들의 불만을 감소시키기 위해 노력해야 한다. 특히 의료인의 친절함이 가장 많이 구전이 되는 것을 알 수 있었는데 의사나 간호사는 치료 나 간호의 직접적인 제공자로서 의료 소비자들의 만족도에 중요한 영향을 미치므로 의료소 비자들의 요구에 부응하는 서비스를 충족시켜 줄 수 있도록 노력해야하며 이러한 바탕 위에 병원마케팅 전략이 수립되어야 할 것이다.

ASSESSMENT OF THE NEEDS OF SELF-CARING RETIREMENT VILLAGE RESIDENTS FOR COMMUNITY SERVICES (RETIREMENT VILLAGE에 거주하는 자가 돌봄이 가능한 노인들의 지역사회 서비스에 대한 욕구 사정)

  • 지역 사회 서비스는 노인들이 질적으로 보다나은 삶을 영위할 수 있도록 돕는 기능을 해야 한다. 적절한 서비스의 공급을 위해서는 지역사회 서비스 욕구 사정의 과정이 우선적으로 요구되어지며, 이에 본 연구는 호주 NSW주 Illawarra지역내의 retirement village에 거주하는 자가 돌봄이 가능한 노인들의 지역사회 서비스에 대한 욕구를 평가하는데 그 목적을 두었다. Illawarra지역내의 여덟개 retirement village에 거주하는 98명의 자가 돌봄이 가능한 노인들을 대상으로 그들의 1) functional status; 2) 지역 사회 서비스에 대한 지식정도; 3) 지역 사회 서비스 사용정도; 4) 지역 사회 서비스 만족도; 5) 잠재적인 지역 사회 서비스 욕구 등에 대한 자가 보고형 설문조사가 이루어졌다. 수집된 자료들은 JMP(for Mackitosh) program으로 통계 처리되었고, computed frequencies가 본 연구의 목표 달성을 위해 주로 사용되었다. 연구 결과를 요약하면 아래와 같다. 1. 평균 연령 76세인 대상자들의 80%이상이 그들의 일상 생활 운영에 다른 사람의 도움을 필요로 하지 않으며 건강상태 또한 양호하다고 자각했다. 2. 90%이상의 대상자들이 지역 사회 서비스를 이용하지 않았다. 3. 70%이상의 대상자가 지역 사회 서비스의 존재에 대해 지각하고 있었다. 4. 60%이상의 대상자가 필요시에는 지역 사회 서비스에 관한 정보를 어렵지 않게 얻을 수 있다고 생각했다. 5. 40%의 대상자가 기존의 지역 사회 서비스에 대한 양과 질에 만족을 나타냈고, 그 외의 대상자들 (60%)은 그 지역 사회 서비스에 대한구체적인 지식이 없다고 보고했다. 6. 25%의 대상자들이 여러 지역 사회 서비스에 관한 관심을 보였다. 결론적으로 대다수의 대상자들은 대부분의 지역 사회 서비스들을 필요로 하지 않았고, 이러한 결과는 현재 자가간호가 가능한 retirement village 거주 노인들에게 당연한 것으로 보여진다. 그러나, 자존감이 강한 노인들에게 서비스 욕구는 자가 돌봄이 건강에 대한 포기정도로 왜곡되어 자각될 가능성이 있는 만큼, 본 연구를 통해 보고된 대상자들의 지역 사회 서비스에 대한 관심도는 고려해 볼만한 가치가 있다고 여겨지며, 이에 노인들의 자존감이 서비스 욕구에 미치는 영향에 대한 연구를 제언하는 바이다.

Professionalism of the Nurse: A Concept Analysis (간호사의 전문성에 대한 개념분석)

  • This study was aimed to clarify the concept of professionalism of the nurse. Walker and Avant's concept analysis process was used. The main attributes of professionalism of the nurse were advanced nursing knowledge, excellent nursing skill, person-centered nursing care, ethicality, responsibility, peer cooperation, autonomy, and outstanding situational judgement, and problem solving. The antecedents were a long enough period of nursing education and experience, research activities, personal qualities, and self-directed training. The consequences were improvement of professional identity and low turnover rates of nurses; improvement of nursing satisfaction, health recovery and well-being for patients; improvement of nursing quality, revenue, and reputation for hospital; gaining social recognition and development of nursing profession; obtaining financial benefits in national health care for nation. The findings of the study provide clarification of the concept of professionalism of the nurse and lay the foundation to establish professionalism among nurses to deliver good nursing care.

Development of Postoperative Self Care Mobile App for Kidney Transplantation Patients (신장이식 환자의 수술 후 자가간호 어플리케이션 개발)

  • The purpose of this research was to develop a self-care application for kidney transplantation patients based on a review of previous literatures and the results of a survey that evaluated the needs of patients. The research proceeded in ADDIE order of analysis, design, development, implementation, and evaluation stages. In the analysis phase, interviews were conducted on over 5 kidney transplantation patients. Moreover, related applications and literatures were reviewed to develop application contents. App-based postoperative self-care program composed of the following: Health teaching, measurement, checklist, views, alarms, and App information. For the evaluation phase, a survey was conducted on 9 experts and 5 patients, using a smartphone application. SPSS/WIN 21.0 program was used for data analysis. Descriptive statistics were used to analyze the validity and suitability of data obtained from experts and users. Content was validated using CVI. Expert assessment of application for the self-care after kidney transplant showed 3.5 out of 4. Patient assessment showed 3.7 out of 4. We determined that a self-care application for patients that underwent kidney transplant is helpful. Moreover, a future study is necessary to test and verify the effects of using this application on self-care and self-care knowledge.

Relationship of Anxiety, Empathy, Ego-resilience and Clinical Competency in Nursing Students (간호대학생의 불안, 공감능력, 자아탄력성과 임상수행능력과의 관계)

  • The purpose of this study was to investigate the levels of anxiety, empathy, ego-resilience, and clinical competency of nursing students, and to examine the relationship of the variables. The participants were 197 nursing students. Data were collected from December 3 to 15, 2017 by taking a survey and analyzed with SPSS WIN 19.0 program(SPSS Inc., Chicago, IL, USA). The results of this study were as follows: The mean score of anxiety was $2.16{\pm}.51$, empathy was $3.51{\pm}.36$, ego-resilience was $2.83{\pm}.44$, and clinical competency was $3.65{\pm}.48$. There was significant differences in clinical competency by satisfaction with major, satisfaction with clinical practice. There were a negative correlation between clinical competency and anxiety(r=-.37, p<.001), a positive correlation between clinical competency and empathy(r=.34, p<.001) and a positive correlation between clinical competency and ego-resilience(r=.49, p<.001). Variables such as ego-resilience, empathy, anxiety, satisfaction with major and satisfaction with clinical practice explained 35.6% of variance in clinical competency. According to these result, there is a need to develop interventions and programs to increase the ego-resilience and empathy and to reduce the anxiety for improving clinical competency in nursing students.

A Comparison of Empowerment, Job Satisfaction, Organizational Commitment between Advanced Practice Nurses and Registered Nurses of Hospitals (전문간호사와 일반간호사의 임파워먼트, 직무만족, 조직몰입 비교)

  • The purpose of the study was to identify and compare levels of recognition of empowerment, job satisfaction, organizational commitment between Advanced Practice Nurses(APN) and Registered Nurses(RN) of hospitals. Data collection was done by the 85 APN and 120 RN of six large hospitals, four medium sized hospitals in Seoul and Incheon. T-test and ANOVA using SPSS 18.0 were performed. There were no significant differences of empowerment between APN($3.56{\pm}0.53$) and RN($3.50{\pm}0.54$)(t =-. 861, p = 0.390). job satisfaction between APN($3.36{\pm}0.53$) and RN($3.25{\pm}0.46$)(t =- 1.575, p = 0.117), organizational commitment between APN($3.11{\pm}0.34$) and RN($3.05{\pm}0.36$)(t =- 1.301, p = 0.195). Empowerment, job satisfaction, organizational commitment of APN and RN showed no significant differences. There are some reasons why it resulted in no significant difference between APN and RN. APN's role in Korea is not clearly established and distinguished from RN, lack of appropriacy of employment and reward, and lack of the institutional foundation for APN's operation and efficiency.
