Combined Tubal and Interstitial Pregnancies after IVF-ET: a Case Report (체외수정 및 배아 이식 후 난관과 반대편 간질 부위에 동시에 생긴 복합성 자궁외 임신 1예)

  • The incidence of ectopic pregnancy is increased since in vitro fertilization-embryo transfer (IVF-ET) has started. And because of the possibility of combined pregnancy after IVF-ET, bilateral adnexal area and other portion of pelvis should be examined carefully by transvaginal ultrasonography during follow-up period. The rate of combined pregnancy after IVF is estimated to be as high as 0.2~1%. Since the incidence of combined pregnancies is markedly higher with IVF, special attention to this possibility after ET is warranted. According to a recent report, the occurrence of interstitial pregnancy is estimated to be 1:3600 for all pregnancies achieved with IVF. The prevalence of interstitial pregnancies is 2~4% of ectopic pregnancies. Interstitial pregnancies tend to be diagnosed later than most other ectopic pregnancies, and if uterine rupture occurs, the resulting hemorrhage can be severe. Currently, about 2% to 3% of interstitial pregnancies are thought to carry a maternal death, which is twice that of other tubal gestations. We report a case of simultaneous left tubal and right interstitial pregnancies after IVF-ET with a brief review of literature.

  • 뇌성마비(Cerebral Palsy)는 임신 중이나 출산, 또는 신생아기에 발생된 뇌손상에 의해 발생하는 중추성 자세 및 운동 장애를 말한다. 뇌성마비는 여러 가지 합병증 및 동반증상을 가지게 되는데, 그 증상들에는 지적장애, 언어장애, 간질, 시각장애, 자폐, 이갈이 및 섭식연하장애 등이 있다. 뇌성마비 장애인에서 나타나는 치과적 특징과 고려사항, 그리고 이들의 구강건강관리에 대해 알아보고자 한다.

Studies on the Effects of Liver-Fluke Infection on the Reproductive Disorder and Milk and Meat Production in Korean Native Goat (Liver-Fluke의 기생(寄生)이 산양(山羊)의 번식장해(繁殖障害)와 유육생산(乳肉生産)에 미치는 영향(影響)에 관한 연구(硏究))

  • These studies were carried out to investigate the effects of liver-fluke infection on the reproductive disorder and milk and meat production in Korean Native Goat. A survey of infected goat in Chungnam district was conducted with 474 head by interdermal reaction and also a clinical observation was founded. A experiment was carried out to determine the effects of liver fluke extract on the reproductive disorder by subcutaneous injection and milk and meat yield were measured from treat ed goats. The results obtained were summarized as follows. 1. The rate of positive goat was 71.3% among the surveyed goat and the highest rate was appeared at 3~4 years old goats as 81.4%. 2. The treated goats lost condition and failed to thrive, and became progressive weaker. 3. In hemogram, observation, the rate of eosinophil leucocyte was increased significantly by extract treated. 4. It was appeared the pathogenic signs such symptoms poisoning liver function disturbance and reproductive disorder from treated goats with liver fluke extract. 5. In milking goat the milk yield droped significantly and fattening goat did not fatten. 6. The gross income value per capita was lower from infected goat and it was analysed more sensitivly in net income value.

Outcome of pregnant mothers with systemic lupus erythematosus (focusing on congenital heart block) (전신성 홍반성 루푸스 산모의 출산아의 임상적 고찰 (완전 방실 블록을 중심으로))

  • Purpose : Neonatal lupus is characterized by congenital complete heart block(CCHB), cutaneous rash, and laboratory abnormalities in infants born to mothers with systemic lupus erythematosus(SLE). This study aims to examine the incidence of CCHB and clinical outcome in neonates born to mothers with SLE. Methods : The study group consisted of 49 neonates, born from 57 pregnancies of 55 women with SLE, diagnosed at Hanyang University Hospital for the period between January 1997 and January 2005. Clinical and laboratory data were retrospectively identified from medical record. Results : There were 5(8.8 percent) spontaneous abortions and one(1.8 percent) still births among 57 pregnancies of 55 mothers. Of 49 live births, 15(26.3 percent) were premature and eight(12.3 percent) were small for their gestational age. There was one(1.8 percent) CCHB suspected during pregnancy on fetal echocardiograpy in a fetus of mother with systemic lupus erythematosus and the fetus was not born by artificial abortion because of mother. There was no CCHB among EKG findings of 49 newborns. Laboratory testing showed hematologic abnormalities among 25.6 percent(10/39) of the babies. 5.1 percent(2/39) and 7.7 percent(3/39) of them were diagnosed as neutropenia, and thrombocytopenia was seen respectively. Anti-SSA(Ro) and antiphospholipid antibodies were predictive factors for prematurity(P=0.003, P=0.049). Anticardiolipin antibodies were predictive factors for ventilatory care(P=0.018). Conclusion : The incidence of CCHB among neonates born to mothers with SLE, which was measured in this study, was lower than that in earlier studies. A high incidence of hematologic abnormalities was found in our study. It is suggested that careful examination should be made of skin for the diagnosis of neonatal lupus.

Histopathologic Observation of the Aborted Fetus from Pregnant Dairy Cows Naturally Infected with Neospora caninum (Neospora caninum에 자연 감염된 임신우로부터 유산된 태아의 병리조직학적 관찰)

  • This study examined the histopathologic and electron microscopic findings of aborted fetuses from pregnant dairy cows naturally infected with Neospora caninum (N. caninum) at four farms in Gongju city and Yeonki gun of Choongnam province. Systemic subcutaneous edema was observed in the aborted fetuses. The necropsy revealed considerable serosanguinous fluid in the body cavity of the aborted fetuses. Light microscopy showed the infiltration of many inflammatory cells consisting of macrophages, lymphocytes and mononuclear cells, accompanied by congestion, hemorrhage and necrosis of myocardiac cells and hepatocytes in the liver and heart of the aborted fetuses. In the liver, clusters of tachyzoites were formed in the cytoplasm of hepatocytes and the interstitial tissue. In the brain, many tissue cysts of various sizes were observed in the nerve cells and their adjacent areas. Tissue cysts had a round shape and contained a large amount of bradyzoite. In addition, there was diffuse gliosis accompanied by congestion and hemorrhage and focal necrosis in the brain. Infiltration of microglial cells were observed at the periphery of the focal necrosis and perivascular area in the brain. Electron microscopy showed that the tissue cyst wall had a thickness of approximately 1 ${\mu}m$ with an irregular shape. On the interior side, more than 100 bradyzoites with lengths of 2-5 ${\mu}m$ and widths of 1-2 ${\mu}m$ were observed. The nucleus of in the bradyzoites was located approximately 1-1.5 ${\mu}m$ anterior to the posterior tip of the zoite. In the cytoplasm between the nucleus and the posterior tip, there were many amylopectin granules, electron-dense small-sized and electron-thin large-sized round granules, homogeneously electron-dense rhoptries and micronemes oriented perpendicularly to the zoite pellicle. To summarize, tissue cysts were identified on electron microscopy from the aborted fetus from N. caninum seropositive pregnant cow by the ELISA. This led to the confirmed presence of N. caninum.
