Catamenial Hemoptysis - Report of one case - (월경성 각혈 - 1예 보고 -)

  • Pulmonary endomertiosis is a rare disorder with the typical symptom of hemoptysis during menstruation (catamenial hemoptysis). We report a case of a 19-year-old woman, gravida 0, with 3-month history of catamenial hemoptysis which was confirmed with chest computed tomography. She was treated by means of thoracoscopic wedge resection for the right lesion and fuperior segmental resection through the left thoracotomy, successively. Preoperative fluoroscopy-guided hooking for thoracosopic target lwsion was helpful in circumstances with one lung anesthesia. Four months of follow-up after an uneventful discharge revealed out no recurrence of catamenial hemoptysis in symptoms and images.

  • A 60-year-old female entered the emergency unit with a chief complaint of hemoptysis. Based on the computer tomographic image analysis of the chest, the patient was diagnosed as having an intralobar pulmonary sequestration that accompanied a hemothorax, and the hemothorax was do to the rupture of a pseudocyst. Pulmonary lobectomy of the left lower lobe and primary closure of an aberrant artery were both performed as an emergency operation. After one week following the operation, the patient was discharged without any postoperative complications.

  • 최근 일부지역에서 발병된 바 있는 전염성 후두기관염은 호흡곤란을 주특징으로 하는 닭의 급성 호흡기병이다. 피가 섞인 가래가 기관안에 가득차 있기 때문에 골골거리며 호흡이 대단히 답답하고 곤란하여 목을 길게 위로 빼고 입을 벌리고 괴성음을 내며 호흡을 하기도 한다. 기관의 점막이 붓고 염증이 생기며 점막이 탈락되면서 출혈하기도 하고 각혈을 하면서 응고된 혈액이 기관을 메꾸어 호흡이 곤란하게 되어 질식하여 죽는다. 이러한 증상은 동물이기에 그대로 보아 넘기기 쉬우나 숨통이 피와 가래로 메꾸어져 호흡을 하지 못하고 질식사하는 장면은 차마 보기 어려운 비참한 죽음의 질병이다. 이병은 과거에 발생이 없어 다행으로 여겨 왔으나 이제 부터는 이 전염병에 대한 예방대책을 강구하지 않으면 안되겠다.

  • Pulmonary endometriosis is an uncommon disease, and usually detected by catamenial hemoptysis. Treatment of pulmonary endometriosis may be medical(hormone therapy) or surgical. Since hormone therapy may cause sterility, most of patients who wish to conceive usually choose surgical resection. Although video-assisted thoracic surgery(VATS) has advantage of small scar, reducing postoperative pain and shortening hospital stay, it is not easy to locate the precise lesion and resect whole endometrial tissue not to be remained. 17 years old female with catamenial hemoptysis was treated sucessfully with a partial resection of the lung using VATS, and has been asymptomatic for 7months since the operation.

  • Massive and untreated hemoptysis is associated with a mortality of greater than 50 percents. Since the bleeding was from a bronchial arterial source in the vast majority of patients, embolization of the bronchial arteries has become an accepted treatment in the management of massive hemoptysis because it achieves immediate control of the patients. We have controlled massive hemoptysis in a case with selective bronchial arteral embolization with Gelfoam.

  • A pulmonary actinomycosis is a chronic suppurative disease which leads to abscess formation and fibrosis it was caused by aspiration of contaminated material from to mouth or oropharynx. Recently pulmonary actinomycosis is very rare by widespread use of antibiotics. Clinically, patients with pulmonary actinomycosis initially experience nonproductive coughing and low grade fever, but pulmonary complaints can be minimal. We report a case of pulmonary actinomycosis associated with hemoptysis which was confirmed by lobectomy with a review of literature.
