Physical and Psychospiritual Care Need by End-of-life Stages among Non-cancer Patient at Home: Based on the Importance and Difficulty of Care Need (재가 비암 환자의 생의 말기 단계별 신체적, 영적 간호요구도 - 간호요구의 중요도와 난이도를 중심으로 -)

  • Purpose: This study was conducted to assess needs at each end-of-life care stage and to analyze importance and difficulty of care needs for home care nursing among non-cancer patients. Methods: We used a retrospective design. Total eligible patients were 117 at the ages of 40 and over, who continuously received home care nursing throughout beginning, stable, and near death stages, and finally died at home from January 1, 2006 to December 31, 2006. Descriptive statistics, Cochran's Q test, Friedman's test were used for data analysis. Results: In the area of physical care, the care need for 'assistance for activities in daily life' was significantly highest in the beginning stage. The care need for 'aggravation or adverse changes in physical symptoms' was significantly increased in the near death stage. In the area of psychospiritual care, 'family's psychological burden' was revealed as having the highest rate of care needs in the every stage. Conclusion: Future intervention should consider assessing care needs in end-of-life care for non-cancer patients who are provided with home care nursing.

Development of a Home Care Need Assessment Tool - Focused on Home Care Nursing Diagnoses based on Self Care - (우리 나라 가정.방문간호 사업을 위한 가정간호요구 사정도구 개발 - 자가간호개념에 근거한 가정간호진단을 중심으로 -)

  • Objectives; The purpose of this study was to develop a framework for home care and a Home Care Need Assessment Tool. Method 1. Identifying common domains in the provision of home care. 2. Charts of 253 home care clients were reviewed to obtain a classification of the nursing diagnoses. 3. A focus group methodology was used to develop the domains. 4. The tool was applied to 439 home care clients.(Kappa value=0.460-1.000, sensitivity, 0.444-1.000: specificity, 0.743-1.000). 5. Some refinements and extractions of the defining characteristics and related factors were made based on the results of the focus group. Results Home Care Need Assessment Tool consists of three parts; -Part I : factors related to basic conditions -Part II : a screening component that enables home care nurses to assess 30 multiple domains of 53 nursing diagnoses. -Part III : summative nursing diagnoses and nursing need intensity for the clients. Conclusion This tool provides a comprehensive assessment that helps the recognition of many strengths as well as problems of the clients. It will be usefully utilized in scheduling home care nursing plans and evaluating client outcomes.

Self-care Education Experience, Self-care Performance, and Home Care Service Needs of Patients With Rare and Incurable Diseases Who Use a Home Ventilator (가정형 인공호흡기를 적용하고 있는 희귀, 난치성 질환자의 자가관리 교육 경험과 수행정도, 가정간호 요구도)

  • Purpose: This study aimed to investigate the self-care education experience, self-care performance, and home care service needs of patients with rare and incurable diseases who use a home ventilator. Methods: From2 ventilator rental companies and 5 hospitals, records of 162 subjects were collected to obtain data regarding self-care education experience (23 items) and home care service needs (25 items). Results: Of the subjects, 65.4% were male and 55.6% had a myotrophic lateral sclerosis. The items with the highest and lowest levels of self-care education experience were "using the ventilator"(94.0%) and "community-available resource information"(27.0%), respectively. Meanwhile, self-care was performed well in terms of "personal hygiene"(59.6%) but not performed well in "community-available resource information"(23.7%). "Health status assessment" had the highest need(88.2%) of home care services. Generally, the need of home care services was higher for the patients using invasive ventilators than that for those using non-invasive ventilators. Conclusion: Home care nurses need to not only provide physical care for the patients but also strengthen their roles of enhancing patient access to and utilization of various community resources to provide systematic and individualized training and care.

A Study on post-hospital Home health Care Needs in a General Hospital (일 종합병원 퇴원환자의 가정간호요구조사)

  • The purpose of this study is to provide a basic data of home health care nursing centered in the hospital by analyzing home health care needs. Data were collected from June 15. 2000 to June 23. 2000 through questionnaires taken by 208 patients to be discharge in a general hospital. The Home Health Care Need instruments used for collecting data was developed by the researcher. The data obtained were analyzed using frequency. percentage. mean. standard deviation. Chi-square test. t-test. one-way ANOVA. Pearson's correlation coefficients. Cronbach's alpha coefficients and Factor Analysis. The results of the study were as follows: 1. The perception of home health care nursing were 65.4% of subjects had never heard about home health care service and 2.4% of subjects knew about the methods & contents in detail. About the demands of home health care, 58.2% of the subjects were willing to use home health care and 41.8% weren't. 2. Regarding the areas of home health care needs. basic nursing care area was the highest. Education/ counselling, exercise/ hygiene. therapeutic nursing care was orderly. Injection and medication management of basic nursing care area were the highest. 3. In relation to characteristic-related diseases and home health care needs. as for the discharge type, the existence of sores, paralysis showed significant differences with home health care needs in the area of exercise/ hygiene(p<,05). Home health care needs in the area of therapeutic nursing care and exercise/ hygiene showed significant differences with the existence of pain. Home health care needs in the area of therapeutic nursing and basic nursing areas were significantly different in the use of catheter/ assistant instrument(p<.05). Home health care needs in the area of therapeutic nursing. exercise/ hygiene and basic nursing care showed significant difference with diagnosis(p<,05). In conclusion, awareness about home health care nursing were very low, home health care needs in the area of basic nursing was the highest. Home health care needs showed significant difference with discharge type, sores, paralysis, existence of pain, use of catheter/ assistant instrument and diagnosis.

Influencing Factors on the Need of Community Care Services in the Family Caregivers of Hospital?based Home Care Patients (의료기관 가정간호 환자의 주 간호자가 인식한 재가복지서비스 요구 및 영향요인)

  • Purpose: The purpose of this study were to examine the need of community care services and the influencing factors of the need in the family care givers of hospital-based home care patients. Methods: Data were collected from 256 family caregivers, who were recruited from 10 hospitals in a metropolitan city. A structured questionnaire on the characteristics of caregivers, resources, and patients was administered. Also, questions on the need of community care services were added. Logistic regression analysis was used to identify the influencing factors of the need for community care services. Results: The participant needed more transportation service, lease of health care devices, visiting bath, caring, visiting hair dressing than that of housekeeping, short-term care, and day care service. Various variables from the three factors were found to be influenced on the need of community care services. Conclusion: The accessibility of the higher need of community care services should be increased for hospital-based home care users. Also, the factors of Family care giver, Resource, and Patient might be considered to provide community care services of hospital-based home care users.

  • Purpose: The purpose of this study was to identify nursing demands according to general characteristics and diseases of inpatients to whom comprehensive payment systems apply. Methods: This study was designed as a retrospective research study using the electronic medical records of Hospital Information Systems (HIS). Participants were 836 subjects who received seven Diagnostic-Related Group diagnoses among inpatients of one tertiary hospital from January 1, 2015 to June 30, 2015. Data were analyzed using SPSS 23.0. Results: Nursing demand among inpatients'seven Diagnostic-Related Group diagnoses was relatively higher for appendectomy, cataract surgery, and hysterectomy, while there was a significant difference depending on the age, duration of admission, admission path, hospital entry method and Diagnostic-Related Group. Conclusion: The results can be utilized as basic data on accurate nursing demands that reflect various features of patients.

Development of Educational Components of Managerial Cost Accounting for Nurses

  • Purpose: In accordance with changes in the healthcare environment, it is necessary to understand managerial cost accounting for nurses. This study aimed to develop educational components of managerial cost accounting for nurses. Methods: This study is comprised of a comprehensive literature review, nominal group technique using focus group interview, audit by an expert group, and priority analysis. Results: The comprehensive literature review identified 15 educational components of managerial cost accounting. In the nominal group technique, the participants finally agreed on eight educational components of managerial cost accounting for nurses. The average content validity index of these components was 0.99. "Activity-based costing" scored the highest for relative importance (15.7%). Conclusions: These results can be used to develop an education program to strengthen nurses' managerial cost accounting competencies. Furthermore, it will be helpful to use a basis for the development of cost management curricula for nursing students and on-the-job training courses of nurse managers and executives.

  • This study was conducted with whole home care nurses nationwide to provide secondary analyzed data to understand on their usage of medical equipments and their need of them for a month. This study found that treatments given by home care nurses were nelaton catheterization, bladder washing/urethral washing, newborn care, exchange and care for nasogastric tube and suction in that order of frequency. Second, instruments and equipments used for home care were reported to be stethoscope, patient monitor, blood pressure measuring equipment, air flotation mattresses, beds for patients, mattresses, suctioning device sets, enteral feeding equipment and dressing set in that oder of frequency. Moreover, need assessment of medical instruments and equipments showed renal dialysis was most needed and patient monitor, blood pressure measuring equipment, enteral feeding equipment, solution and other supplies for renal dialysis and beds for patient were necessary in that order. In conclusion, the results of this study investigating special treatments and medical instruments and equipments used for home care patients and analyzing patients' need, were expected to be useful for expansion of application of long-term care insurance for the elderly and health insurance as well as for quality control of home care and development of medial instruments and equipments used at home.

Analysis of Functional Independence Status and Home Care Needs among Stroke Patients (뇌졸중 환자의 기능적 독립 상태(FIM) 및 가정간호 요구도 분석)

  • Purpose: The purpose of this study was to analyze functional independence and need for home nursing care in stroke patients. Method: This was a descriptive study. The subjects comprised 117 stroke patients who were supposed to discharge in less than a week. The instruments used for this study were the functional independence measure(FIM) and the need assessment for home nursing care of stroke patients. The data were analyzed using frequency, mean, t-test, ANOVA, and Pearson correlation coefficient. Results: Pearson correlation analysis revealed that there were negative correlations between subcategories of the FIM and the need for home nursing care in stroke patients. Especially, in case of the subjects who recorded lower scores at self-care they showed higher needs for home nursing care in the domain of physical problem and rehabilitation. Conclusion: For the operation of the home nursing care, the protocol for home nursing care is needed to the stroke patients living at home. The FIM instrument is recommended as a useful scale in order to assess the disability for the stroke patients and the need for home nursing care because this one has correlation with the scale of need for home nursing care.

A Comparison of Characteristics between Home Health Care Needers and Non-needers in Rural Areas (농어촌지역의 가정간호 요구자와 비요구자의 특성 비교)

  • Purpose: The aims of this study were to describe general characteristics and needs of home health care, and to find the differences between home health care needers and non-needers. Method: In this study, 642 subjects participated who lived in Muan, Jollanam-do. Data were collected in August 2001 using a self-reported questionnaire. The questionnaire was a revised and simplified form of the Organization of Community Health System Program at the Seoul National University. Collected data were analyzed through Kruskal-Wallis test. t-test. and Chi-squire for cross-sectional analysis. Result: The average age of the subjects was 52.6 years and 33.3% of them aged over 65 years. Twenty six percent of them had chronic degenerative diseases. The percentages of hypertension patients and D.M. patients were 6.4% and 2.5%, respectively. The number of family members was 2.95 on the average, 2.19 in cases of families with the elderly and 3.33 in cases of families without the elderly. The rate of disability of the elderly was 10.5%. Marital status (p=.000), the number of family members (p=.000), education (p=.000), job (p=.000) and health insurance (p=.027) were significantly different between home care needers and non-needers. Home care needers had less living expenses (p=.001), more frequent hospital admissions (p=.004), higher chronic disease rate (p=.000) and more frequent visits to public health center (p=.027) than non-needers. Home care needers who wanted free service were twice as many as non-needers. Conclusion: Home care need was very high in rural areas and the needers had worse characteristics (low educational level. low income, no job and no family). Therefore, it is necessary to develop cheaper and more accessible services for home care needers in rural areas.
