A Study on the Recognition of Home Care Services by nurses, doctors and patients. (일 종합병원 의사, 간호사 및 입원환자의 가정간호사업에 대한 인식 조사)

  • Purpose: To provide the data for improving home health care through investigating the recognition of home care services and the possibility of providing it by nurses, doctors and patients. Method: The subjects were 167 nurses and 71 doctors who were working at a general hospital in Seoul and 72 patients who were admitted to that hospital. Data were collected through questionnaire surveys from November 29 to December 17 in 2004. Result: Home care services were recognized by 70.7% of nurses, 64.8% of doctors and 58.4% of patients. The percentages of agreement regarding the necessity of home care services were 70.7% of nurses, 47.9% of doctors and 86.1% of patients. The percentages of agreement to refer the patients to home care when they only want to receive it, were 58.1% of nurses and 57.7% of doctors. There were significantly higher in nurses than doctors between the differences of possible or impossible percentages in agreements about each items in the test-related services, medication-related services and treatment-related services. Conclusion: The above findings indicated that more specific and continuous educations and advertisements are needed to enlarge home care services in general hospitals.

The Effects of Personal Coping Resources and Perceived Organizational Supports on Job Stress among Hospital-Based Home Care Nurse Practitioners (HCNPs) (의료기관 가정전문간호사의 개인대처자원과 조직지원인식이 직무스트레스에 미치는 영향)

  • Purpose: The aims of this study were to describe the levels of personal coping resources, perceived organizational supports, and job stress, and to examine factors that affect job stress in hospital-based home care nurse practitioners. Methods: A cross-sectional survey was conducted. Data were collected from 170 subjects with a structured questionnaire from April to July, 2016 and analyzed using descriptive statistics, t-tests, One-way ANOVA, Pearson's correlation, and multiple regressions. Results: The means for personal coping resources, perceived organizational supports, and job stress were $3.7{\pm}0.43$, $3.4{\pm}0.55$, and $3.4{\pm}0.55$ out of 5, respectively. Personal coping resources and perceived organizational supports were significantly correlated. However, both had no associations with job stress. In the multiple regression analysis, the experiences of home care nursing and hospital location were found to be predictors of job stress. Both personal coping resources and perceived organizational supports were not statistically significant predictors of job stress. Conclusions: It is necessary to carry out organization-based educational programs and support systems aimed at enhancing personal abilities to cope with stress at work. Additionally, further studies are needed to identify other hospital-related characteristics that can lead to job stress in home care nurse practitioners.

Influence of Home Health Care Nurses' Self-esteem and Spiritual Well-being on their Spiritual Care (가정전문간호사의 자아존중감, 영적안녕이 영적간호수행에 미치는 영향)

  • Purpose: This study investigated the degree of, and factors influencing the spiritual care among home healthcare nurses. Methods: The subjects were 168 advanced practice nurses working in 94 hospital-based facilities for home health care located in Seoul, Gyeonggi-do, and Incheon Metropolitan City. Data were collected from January 14, 2019, to March 7, 2019, using structured questionnaires on spiritual care and related variables based on the literature. Data were analyzed by t-test, one-way ANOVA, Pearson correlation, and multiple regression analysis (stepwise) using SPSS Version 23.0 program. Results: The results showed that the degree of spiritual care of home health care nurses was 3.44 points out of a possible 5 points. Factors affecting nurses' spiritual care were spiritual well-being(𝛽=.36, p<.001), recognition of spiritual care as the nature of nursing(𝛽=.17, p=.016) and role of nurses(𝛽=.22 p=.002), regular-based education for spiritual care(𝛽=.23, p<.001), patients' discomfort with spiritual discussion(𝛽=-.18, p=.001), and nurses' experience in home health care area(𝛽=.14, p=.010), which explained 57.2% of the observed variation in spiritual care. Conclusion: This study suggests the need to develop regular-based education programs for facilitating spiritual care and strengthening home health care nurses' spiritual well-being and their positive perceptions toward spiritual care.

Nursing Practices to Address Domestic Violence in a Prenatal and Early Childhood Home Visitation Program in Korea (산전·조기 아동기 가정방문 프로그램 참여 여성이 경험하는 가정폭력에 대한 간호실무 분석)

  • Purpose: Domestic violence is an important issue encountered in nurses' home visitation programs. This study analyzed the types of domestic violence and associated interventions provided by nurses in a prenatal and early childhood home visitation program in Korea. Methods: For 24 families who experienced domestic violence in the Seoul Healthy First Step Project, registration information and home visit nursing records were analyzed through directed qualitative content analysis. Results: Physical violence was found in all 24 cases, followed by emotional violence, coercive control, financial abuse, and sexual violence. Twenty-two interventions derived from existing nurse-family partnership (NFP) program strategies were identified in the nursing practices of the Seoul Healthy First Step Project. Conclusion: In a prenatal and early childhood nurses' home visitation program in Korea, various approaches and interventions were provided to nurses to address domestic violence. However, differences in the level of interventions provided by nurses were found, implying a need to educate and support nurses to address domestic violence in-home visitation programs.

Association of Perceptions and Anxiety of Home Health Nurses about Death, on their Attitudes to Terminal Care (가정전문간호사의 죽음인식과 죽음 불안이 임종간호태도에 미치는 영향)

  • Purpose: The purpose of this study was to identify the extent to which in-hospital-based home health nurses' perceptions and anxiety about death following terminal care, affects their attitudes toward terminal care. Methods: The subjects were 128 advanced practice nurses working in hospital-based facilities for home health care, located in Seoul, Gyeonggi-do, and Incheon Metropolitan City. Data were collected from May 3, 2019, to June 3, 2019, using structured questionnaires, on terminal care and related variables based on the literature. Data were analyzed by performing the Student's t-test, one-way ANOVA, Pearson's correlation, and multivariable stepwise regression using the SPSS Version 25.0 program. Results: The scores of the attitudes toward terminal care of home health care nurses was 3.25 points out of a possible 4 points. Factors affecting nurses' attitudes toward terminal care were their concern about death(β=0.45, p<0.001), religion(β=-0.26, p=0.001) and the anxiety of others about dying(β=-0.23, p=0.003), which explained 32.0% of the observed variance concerning the factors affecting nurses' attitudes toward terminal care. Conclusion: Through this study, concern about death, anxiety about how other people process dying, and religion, were found to be associated factors. The more concerned the nurses were about death and the less their anxiety about how others process dying, the better the home health care nurses' attitude toward implementing terminal care.

Scale Development of Job Stress for Home Care Nurses (가정전문간호사의 직무스트레스 측정 도구 개발)

  • Purpose: The purpose of this study was to develop a job stress scale for hospital-based home care nurses in Korea. The process was construction of the conceptual framework, development of the preliminary items, verification of the content validity, item analysis and test of the reliability. The preliminary items were based on literature review and in-depth interviews with home care nurses. As a result, eight categories and sixty items were selected. These were reviewed by seven specialists for content validity and finally fifty one items were chosen. Data was collected from 180 home care nurses who were engaged in 87 hospitals from August to September 2003. The result of item analysis one was excepted, The final item count was 50. Categories were as follows: overload work(8 items), lack of specialized knowledge and technique(5 items), ethical dilemma(4 items), role conflict(5 items), interpersonal relationships(6 items), visiting home environment(9 items), driving conditions(4 items) and lack of administrative support(9 items), The reliability of the scale by Cronbach's alpha was .948 and the domain's reliability ranged from .649 to .841. The result of this study could be used to measure the job stress of home care nurses. However, for further validity and reliability, repeated studies will be necessary.

The Development of Certificates Criteria and Curriculum in Home Healthcare Nurse Specialist Program (가정전문간호사 교육프로그램 인정기준 및 표준 교육과정 개발)

  • Purpose: To make a contribution to raising the quality of nursing and home healthcare services through reviewing the present state of home healthcare nurse specialist training institutions and education programs and creating concrete measures to establish high-quality education courses. Method: International comparative study of accreditation criteria and curriculum in home healthcare nurse specialist program. Result: The Authorization Standards of home healthcare nurse training institutions consists of 8 items, 23 evaluation criteria and 72 evaluation indexes. Proposal to develop a specialist training program: Curriculum. Modify and complement a present homecare nurse education program. Curriculum I. Designate two forms of certification. The first certification has been granted the authority to serve as a manager and open a home healthcare agency to nurses having masters degrees and clinical experience for five years. The second certification is allowed to perform general home healthcare after having completed a short term training course. Currculum 2. To meet increasing demands, granting a certification to perform home healthcare to registered nurses having clinical experience of more than three years. Conclusion: These results can be utilized in the home healthcare educational program for raising the quality of nurses and home healthcare services.

Moderating and Mediating Effects of Social Support in the Relationship between Work-family Conflict, Job Satisfaction and Turnover Intention among Married Women Nurses (기혼여성 간호사의 직장-가정 갈등, 직무만족 및 이직의도와의 관계에서 사회적 지지의 매개 및 조절효과)

  • Purpose: The purpose of this study was to identify moderating and mediating effects of social support in the relationship between work-family conflict, job satisfaction and turnover intention among married women nurses. Methods: The sample for this study consisted of 225 married female nurses from two hospitals located in Seoul. Data were collected through structured questionnaires and analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and hierarchical multiple regression with the SPSS 21.0 program. Results: (a) Social support of married female nurses had significant moderating and mediating effects between work-family conflict and job satisfaction. (b) Social support of married female nurses had significant moderating and mediating effects between work-family conflict and turnover intention. Conclusion: The results of the study indicate that social support is an important factor in work-family conflict, job satisfaction and turnover intention among married female nurses. Supervisor support is important to married nurses as social support as it is controlled by the organization. Therefore, supportive relationships based on mutual respect between supervisors and nurses are necessary for a creative organizational atmosphere and system.

Job Satisfaction related Factors of Home Visiting Nurses in the Public Health Centers (보건소 방문간호사의 직무만족 관련요인)

  • Purpose: This study was aimed to investigate predictors of job satisfaction of home visiting nurses at the 16 public health centers in Busan. Method: There are two groups of independent factors: non-work related (age. educational level. working duration as nurses in hospitals. certificate). and work related (working duration as nurses in public health centers. working duration as home visiting nurse. position. number of households visited per week, workload, cooperation among staff, support by supervisors, supply of vehicles, supply of materials). The participants were 88 home visiting nurses from 16 public health centers in Busan. Data were collected with self-administrated questionnaires and analysed using an univariate logistic regression and multiple logistic regression analysis. Result: support by supervisors (good vs not-good, OR=3.70. p=0.025), and supply of materials (good vs not-good, OR=3.33, p=0.038) had significants effects on job satisfaction. Conclusion: The results were similar to those of other studies on the predictors of job satisfaction of clinical nurse at hospitals, and were helpful in developing nursing interventions to increase job satisfaction among home visiting nurses in public health centers. A Study on the Type of Role Awareness for Medical Institutions Home Health Care Specialists : A Q-Methodological Approach (의료기관 가정전문간호사의 역할인식)

  • Purpose: This study is to identify the types of role awareness of home health care specialists working at medical institutions, to understand their role awareness correctly by grasping the quality of each type, and to provide useful help in the education of home health care specialists. Method: Q-methodology is used to objectify role awareness of medical institution specialists who may recognize situations differently according to their individual experience and comprehension based on the view of behavior. Q-classification was carried out on 30 home health care specialists working at medical institutions using 30 selected questions. Collected data were examined through factor analysis using QUANL PC program. Results: Three different types of role awareness of home health care specialists working at medical institutions were identified. Type I is 'educational-function-centered', Type II 'patient-centered' and Type III 'professional-service-centered'. Conclusion: Regardless of these types, home health care specialists commonly had a high pride as a specialist and a sense of mission regarding themselves as important persons responsible for patients' health.
