History of definitions of the Zheng (證) - a study based on 20th century Chinese literatures - (현대 증(證) 정의의 역사 -20세기 중국의 문헌을 중심으로-)

  • Objectives Although the word Zheng (證) is widely used basic term in nowaday's East Asian traditional medicine, it's definition can't be found in ancient texts before 20th century, and the history in which that term got general meaning in public, have not been clearly introduced. So the present author show the way Zheng confirm it's modern implication. Methods To search the books that contain any contents on Zheng's definition, the author mainly used the electronic texts of Super Star Reader (超星閱讀器). To search modern study article on Zheng's definition, the author used China National Knowledge Infrastructure (www.cnki.net). Results The present study shows that : although Treatments according to disease Pattern Identification (辨證施治) have been prominent treatment modality in East Asian traditional medicine, the general definition of disease Pattern (證) was given in late 20th century. Especially from 1955 to 1965, some major scholars like Ren Ying Qiu (任應秋), Zhu Yan (朱顔), Qin Bo Wei (秦伯未) and Jiang Jian Fu (蔣見復) did important roles in it's concept creation. Conclusions Modern concept of Zheng (證) was defined in late 20th century.

Clinical Observation on 1 Case of Both Leg Paralysis Patient Diagnosed Wei Symptom (위증(療證)으로 진단한 하지마비 환자의 치험 1례)

  • Wei symptom(?證) is symptom that reveals muscle relaxation without contraction and muscle relaxation occurs in the lower or upper limb, in severe case, leads to death. This is the clinical report about the Wei symptom(?證)-patient doubt as Transverse Myelitis and Conversion Disorder. The patient was treated by acupuncture, moxibustion, herb medication(十全大補湯), electriccal stimulation theraphy, Bee Venom acupuncture, and had significant improvement in Wei symptom(?證). these results suggest the surface temperature differ remarkably from before being treated. The temperature is measured by using Digital Infrared Thermal Imaging(DITI). The results suggest that oriental medicare is an effective treatment for Wei symptom. We expected that therapeutic value of treatment of both leg Paralysis in the oriental medicine will be higher if more clinical studies and researches are accomplished.

Inferential Structure and Reality Problem in Diagnosis of Oriental Medicine (한의 진단의 추론형식과 실재성)

  • Inferential structure and reality problem is a serious issue to O.M.(oriental medicine). The study will analyze this issue through a philosophical and historical comparative study of W.M.M(Western modern medicine) and O.M. First, I presuppose some basic ideas. The first is the division of the 'the philosophy of medicine' and 'the medicine itself'. Second, there is a 'visibility' that discriminate between 'the abstractive concept' and 'the concrete object' in diagnostic terminology. The third is the separation of disease, the entity and disease, the phenomenon. Finally, the distinction between the cause of disease and the nature of disease. Through these basic concepts, this study will analyze O.M's diagnostic methodology, 'Pattern identification of the S.A.S(sign and symptom)'. The results are follows: 1. O.M's views disease as a phenomenon. So, the S.A.S, which is visible, is the disease itself. Tough the analysis and inference of the S.A.S, 證(zheng) the essence is derived. 2. 證(zheng) can be considered as 'the abstractive concept' reflecting the essence of a disease. 3. 證(zheng) is not arrived through causal sequence reasoning but rather by analogical reasoning. 4. 證(zheng) is 'the non-random correlative combination of S.A.S', pattern. These patterns secure the abstractive deduction in reality. that is, The causality, the positivism, the view of disease as entity, and anatomical knowledge are the traits peculiar to W.M.M. But, these properties can not be applied universally to every medical systems. Also, these properties do not indicate the superiority or inferiority of any medical system. 5. 證(zheng) summarizes the patients condition simultaneously with the S.A.S. However, 證(zheng) doesn't necessarily indicate the knowledge about the actual internal organ. That is, Early in O.M.'s history, the diagnostic terminologies including 證(zheng) were analogical reflections of a naive knowledge of internal organs and external environmental factors. Later, the naive knowledge in 證(zheng) changed int new nature, an abstractive concept. The confusion of the concept of disease, the indiscriminate acceptance of Western anatomical knowledge, and the O.M.'s theoretical evolution et are the challenge facing modern O.M. To find solutions, this study looks at the sequence of the birth of W.M.M. and then compares it's system with the O.M. system. It is recommended that O.M. diagnostics should pay close attention to the ambiguity of the diagnostic methodology in order to further development. At present time, the concept and the system peculiar to O.M. can not be explained by common language. but O.M.'s practitioner can not persist in this manner an: longer. Along with the internal development of O.M., the adjustment of O.M.'s diagnostic terminology needs to be adopted.

  • Among oriental medicine,s literatures, through Hwang-Je-Nae-Kyung(黃帝內徑) to Chung(淸), I extracted contents related to atrophy syndrome(?證). And studied it,s pathophysiology, therapy and treatment. Then, I concluded that result same below 1. The pathophysiologies of atrophy syndrome are lung heat(肺熱) & decreasing of it,s circulation, making liver and stomach weaken & difficulty it's circulation, injurious to liver and kidney & atrophy of bonemarrow and muscle. 2. The most important point of atrophy syndrome therapy is Yang-Myung(陽明). The Priority of therapy is stomach & liver,s balance. And then we must protect acquired human function & clean humidity & temperature of Yang-Myung(陽明). under the principle of decreasing south organ,s function & protecting north organ,s function, we should Ja-Yeum-Chung-Yeul(滋陰淸熱). so remove temperature of lung & protect liver and kidney & make strong stomach. 3. Among the therapy of atrophy syndrome in literatures Yi-Jin-Tang(二陳湯), Sa-Gun-Ja-Tang(四君子湯), Sa-Mul-Tang-Je(四物湯劑), Ho-Jam-Hwan(虎蠶丸), Dong-Won-Geun-Bo-Hwan(東垣健步丸) and Chung-Jo-Tang(淸燥湯) were many. These make strong spleen & dry humidity organ using Sa-Gun-Ja-Tang(四君子湯)and Yi-Jin-Tang(二陳湯) by Dog-Cheu-Yang-Myung(獨取陽明) method. Sa-Mul-Tang(四物湯), Phellodendri cortex(黃柏), Radix sophorae flavescentis(苦蔘), Carapax Testudinis(龜板) bitter taste make strong Yeum(陰) & decrease Yang(陽) so important human muscle powerful. Ho-Jam-Hwan(虎蠶丸), Dong-Won-Geun-Bo-Hwan(東垣健步丸), Chung-Jo-Tang(淸燥湯) make Chung-Yeul-Jo-Seup(淸熱燥濕), protect liver and kidney & strong muscle and bone. Besides Gum-Gang-Hwan(金剛丸), Yi-Myo-Hwan(二妙丸), Nok-Gak-Geu-Hwan(鹿角膠丸)&Ga-Mi-Sa-Geun-Hwan(加味四斤丸) were used in treatment of atrophy syndrome.

A case report of Guillain-Barre syndrome (Guiillain-Barre 증후군 환자의 치험 1례)

  • Objective : This is clinical report about the Wei syndrome(?證)-patient diagnosed as Guillian-Barre syndrome. Guillain-Barre syndrome(GBS), what is called acute inflammatory polyneuritis, is a disorder in which the body's immune system attacks parts of peripheral nervous system. GBS is subclassified into acute inflammatory demyelinating polyneuropathy(AIDP), acute motor or motor-sensory axonal neuropathy(AMAN, AMSAN), and the other variants. The cause and mechanism of this syndrome are unknown yet. The typical Guillain-Barre syndrome could be diagnosed by the patient's syndroms and physical exams as the rapid onset of weakness, paralysis and loss of reflexes. The analysis of CSF and electrical test of nerve and muscle function can be performed to confirm the diagnosis. Most of the cases usually occur shortly after a viral infection. Method & Result : This is the clinical report about the one patient daignosed as Guillain-Barre syndrome. The patient, 9-year-old girl had the hemiparesis after upper respiratory infection. We characterized her as Wei syndrom(?證). The patient was treated by acupunture, indirect moxibustion, herb medication(通竅湯 加味方, 四物湯合檳蘇散 加味方) and had significant improvement in the Wei syndrome(?證). Conclusion : We report that we had good effects of oriental medical treatment on Guillain-Barre syndrome.

  • 동의보감(東醫寶鑑) "내경편(內景篇)"에 "사람의 형은 긴 것이 짧은 것만 못하며, 큰 것이 작은 것만 못하고, 살찐 것이 마른 것만 못하다. 사람의 색은 흰 것이 검은 것만 못하고, 옅은 것이 짙은 것만 못하며, 엷은 것이 두터운 것만 못하다. 더욱이 살찐 사람은 습이 많고, 마른 사람은 화가 많으며, 흰 사람은 폐기가 허하고, 검은 사람은 신기가 넉넉하다. 사람마다 형색이 다르면 오장육부 역시 다르기 때문에 외증이 같더라도 치료방법을 달리해야 한다."고 하였다. 이것은 "동의보감(東醫寶鑑)"의 체 병 증을 서로 결합하여 진단 치료하는 사상으로 "이체질동병이치", "이체질동증이치"의 체질진료사상을 나타낸 것이다. "동의보감(東醫寶鑑)" "탕액편(湯液篇)"에서 "약이란 병을 치료하는 것이다. 변화하는 것은 병에 있고, 병 치료를 주관하는 것은 약에 달렸으며, 약을 만들어 쓰는 것은 사람에게 달려 있다. 이 셋 중 하나라도 빠지면 안된다."고 하였다. 이것은 체 병 증과 방약이 서로 대응하는 용약사상을 보여주는 것으로 동의약의 진료체계를 건립하였는데 이 점이 바로 동의약학의 장점이자 특색이라 할 수 있다.

  • Objectives : In this study, the changes of concept of Bianzheng(辨證)[syndrome differentiation] in the traditional medical history are investigated for the purpose of understanding conditions of Korean Medicine in modern times. Methods : The concepts of Zheng(證)[syndrome] and Bianzheng[syndrome differentiation] in Sanghanron(傷寒論) and many important medical literatures were selected and analyzed to overview the historical changes of those. Results : To the modern ages, the concept of Zheng had included the two kinds of concepts, that is, symptom/sing and syndrome with slight changes of meaning. As a abstract meaning of syndrome, Zheng(證) has been systematized and complicated with the times, that means changes of syndrome differentiation. The concept of Zheng has been recognized as the symbol that expresses the characteristics of Traditional Medicine since the modern age that concepts of sign and symptom have flowed from Western Medicine into Traditional Medicine. Conclusions : One of the main key of studies about Bianzheng(辨證) in future would have been harmonizing the balance between the two trends of modern Traditional Medicine, ideation and objectification.

  • It is different from the DongEuiBoGam(東醫寶鑑) of Korea traditional medicine that China BeunJungLonChi(中醫辨證論治) was made for the scientific and systematic assortment of China medicine. Symptom(症)is more important than symptom complex(證)in china BeunJungLonChi at DonEuBoGam. And unusual symptoms generally are diseases in DongEuiBoGam. DongEuiBoGam is emphasis on body form, temper, body color, pulse(形氣色脈) and the functional difference of organs by body form and body color. there are many problems in order to unify the theory of Korea traditional medicine by china BeunJungLonChi. I suggest that a new assortment that can combine Sasang constitutional medicine(四象醫學), Constitution-Acupuncture(體質針), and Hyungsang medicine(四象醫學) will be necessary. BeunJungLonChi : diagnosis and treatment based on overall analysis of symptoms and signs, the cause, nature and location of the illness and the patient's physical condition according to the basic theories of traditional Chinese medicine
