  • The extent of renal biopsy indication is being widened because of the increasing incidence of incidental renal masses; the increasing treatment options for renal cell carcinoma, including ablation therapy and novel targeted treatment; and the increasing incidence of kidney transplantation. However, percutaneous renal biopsy is technically difficult, particularly for beginners, because the skin-to-organ distance is relatively longer than those associated with other organs. In the present review, we will discuss the indications, technical considerations, efficacy, and complications of renal biopsy. Furthermore, we share practical tips of renal biopsy through many examples to help radiologists perform renal biopsy safely and effectively in various situations.

A Case of Oligomeganephronia of the Solitary Kidney, Presented with Isolated Proteinuria (단독 단백뇨로 발현된 단일신의 oligomeganephronia 1례)

  • Oligomeganephronia is rare congenital anomaly characterized by striking reduction of the number of nephrons, which are markedly hypertrophied in renal hypoplasia. Since the first description of oligomeganephronia in bilateral renal hypoplasia in 1962, dozens of cases were reported. Van Acker reported the first case of oligomeganephronia developed in the solitary kidney and 8 cases were searched in the literature. We report a case of oligomeganephronia in the solitary kidney in 13 years old boy, presented with isolated proteinuria as a first case in Korea. (J Korean Soc Pediatr Nephrol 2002 ; 6:92-6)

  • Crossed renal ectopia is a congenital malformation in which both kidneys lie on the same side of the spine, usually side by side longitudinally. More often on the right side. Fusion of the two renal units is eight times more common than nonfusion. Although crossed renal ectopia is uncommon, this unusual entity must be considered in an infant when cystic mass in the abdomen or pelvis paticularly if no kidney can be found on the opposite side. In many cases of crossed fused ectopia with multicystic dysplastic kidney(MCDK), the diagnosis can be strongly suspected from the sonogram, and no other studies may be necessary. However, both intravenous urography and isotope renography is useful to assess the function of the crossed kidney. Crossed renal ectopia and MCDKs are associated with a greater incidence of ureteropelvic junction obstruction and reflux. So, screening voiding cystourethrography should be performed. Very few studies of MCDK in the setting of crossed fused ectopia have been reported. We have experienced a 3-year-old boy with crossed fused renal ectopia with multicystic dysplasia.

  • In this paper, AI methods such as deep learning are applied to provide customized exercise therapy for patients with kidney disease. In order to apply deep learning, a dataset that can determine kidney disease is trained to determine whether it is a kidney disease, and 1RM, which is the user's physical information and muscle strength according to whether it is a disease, can also be calculated through deep learning. The calculated muscle strength of 1RM was converted into resistant exercise for each part through a calculation equation for each part of the body, and was configured to be provided with an aerobic exercise amount tailored to the user's body information. If continuous research is conducted in the manner proposed in this paper, customized exercise therapy can be provided for various diseases.

방사선에 조사된 생쥐의 간 및 신장에서 홍삼 추출물의 방어효과

  • This study was prepared to observe the charge of tissues weight, glutamate oxaloacetate transminase(GOT) and glutamate pyruvate transminase(GPT) activities in the liver and kidney treated with red ginseng extract injection in the gamma ray(5.0 Gy) irradiated mice groups were divied in to control, red ginseng extract injection, irradiation after saline injection and irradiation after red ginseng extract injection. The GOT activity showed earlier recovery at irradiation after red ginseng extract injection group than irradiation after saling injection group in the liver and kidney. The GPT activity showed earlier recovery at irradiation after red ginseng extract injection group than irradiation after saling injection group in the liver and kidney. The above result suggest that red ginseng extract have the protection effect on the change of GOT and GPT activity after radiation injury in the liver and kidney.

A Study on the Sulfametazine residues in Swine Tissues (일부지역 돼지장기 및 근육내 잔류설파메타진에 대한 조사연구)

  • This study was carried out to determine the sulfametazine residues in swine tissues. For this experiment, 22 samples of porks were collected at meat markets in Seoul, 21 samples of swine muscles for export were collected at slaughterhouses in Gyunggi areas, and 18 samples of swine livers, kidneys, and muscles were collected from 6 swine at slaugterhouse in Seoul from the end of August to the early of October and were analyzed by High Performance Liquid Chromatography (HPLC). the results obtained were summarized as follows. 1. The sulfametazine residues were liver > kidney > muscle, and among liver, kidney and muscle were very significantly different(p<0.005). 2. The sulfametazine residue in liver was very significantly higher than that in muscle (p<0.005), the sulfametazine residue in kidney was significantly higher than that in muscle (p<0.005), but, the sulfametazine residue in liver was not significantly higher than that in kidney(p<0.05). 3. the sulfametazine residues in swine muscles for export wer exhibited a tendency to lower in small breeding size farms. 4. the sulfametazine fesidue in one of 22 samples of porks for domestic consumption was exceeded 0.1 ppm.

Effects of Squalene in Mouse Kidney with Contaminated Mercury (흰쥐의 신장에서 수은독성에 대한 스쿠알렌의 효과)

  • The mouse for identifying the histological changes of kidney were also divided into the two groups; treated with only $HgCl_2$ (4 mg/kg), the group treated with $HgCl_2$ and squalene (200 mg/kg). The $HgCl_2$ treated only one time at first day. The squalene treated two times a day (12 hours interval) for every day. Each groups were divided into the five groups; 6, 12, 24, 48 and 72 hours after treated $HgCl_2$ and squalene. Historical changes of the kidneys were investigated by electron microscope. The group with only $HgCl_2$ showed that the nuclear membrane was shrinked, the inner membrane (cristae) of the mitochondria were destructed, and ribosomes on the rough endoplasmic reticulum were lost. The group treated with $HgCl_2$ and Squalene showed that the nuclear membrane was more rounded, the cristae of the mitochondria were almost normal shape, and more ribosomes on the rough endoplasmic reticulum were attached. Therefore , we concluded that squalene has significantly protective effects in kidney to harmful $HgCl_2$.

Differences in the Clinical Characteristics of Children with Urinary Tract Infections Based on the Results of $^{99m}Tc$-Dimercaptosuccinic Acid Renal Scanning (요로감염 소아에서 입원 초기 시행한 DMSA 신 스캔 결과에 따른 임상양상의 차이에 대한 연구: DMSA 신 스캔의 임상적 의미)

  • Purpose: The $^{99m}Tc$-Dimercaptosuccinic acid (DMSA) renal scan is used primarily for the diagnosis of renal scarring and acute pyelonephritis in children with urinary tract infections (UTI). This study aimed to evaluate clinical differences based on the positive or negative results of DMSA scans and kidney ultrasonography (US) in pediatric UTI. Method: We retrospectively reviewed 142 pediatric patients with UTI who were admitted to Myongji Hospital from January 2004 to December 2012. We performed a comparative analysis of clinical parameters such as age, sex, white blood cell (WBC) count, neutrophil count, blood urea nitrogen (BUN) level, creatinine (Cr) level, C-reactive protein (CRP) level, and durations of hospitalization and fever, grouped by the results of the DMSA scans and kidney US. Results: The mean age of the patients was $33.8{\pm}48.3$ months, and 78 (55%) were male. Fifty-two patients had abnormal DMSA findings, and 71 patients had abormal kidney US findings (test positive groups). In the DMSA scan positive group, there were significant differences in age, WBC counts, neutrophil counts, CRP level, BUN level, Cr level, hospitalization duration, number of abnormal findings on kidney US, and incidence of vesicoureteral reflux (VUR) compared with the scan negative group. The kidney US positive group had significant differences in age, neutrophil count, CRP level, BUN level, Cr level, hospitalization duration, number of abnormal findings on the DMSA scans, and more frequent VUR compared with the US negative group. Conclusion: Our data suggest that there were no major differences in clinical parameters based on the results of the DMSA scans compared with kidney US in pediatric UTI. However, as kidney US and DMSA scan were performed to predict VUR, the sensitivity and negative predictive value was increased.

The Effectiveness of Perceived Stress and Social Support on the Quality of Life for Kidney Transplantation Recipients (신장이식환자의 지각된 스트레스와 사회적 지지가 삶의 질에 미치는 영향)

  • Purpose: This study is descriptive study that confirms the affect of the factors of perceived stress and social support of the kidney transplantation recipient affect on the quality of life. Mothods: 167 subjects who have received kidney transplantation in a university hospital in G city. Data obtained are analyzed by SPSS Win 13.0. Results: The perceived stress based on transplantation related characteristics and general traits of kidney transplantation has significant differences only in frequency of admission. The subjects who had been supported by acquittances have more significant social support index. The quality of life has significant differences in number of admissions, gender and occupation. Also, when the perceived stress of kidney transplantation recipients is lower and social support is higher, the quality of life is higher. The perceived stress has 28.1% increment of quality of life. Adding social support, both of them affect 34.8% increment of quality of life. Conclusion: To decrease the factor to cause the stress of kidney transplantation recipients, it is necessary to have social support networks and to develop plans and programs to increase the quality of life of recipients.

Transarterial embolization of renal artery in dogs with experimental hydronephrosis (개의 실험적 수신증에 실시한 신동맥 색전술)

  • This study was performed to validate the procedure of transarterial embolization of the renal artery (TAE-RA) using iohexol-ethanol solution in dogs with unilateral experimental hydronephrosis and to evaluate the embolized kidney using B-mode ultrasonography and selective angiography. Experimental hydronephrosis was induced by ligation of unilateral ureter in 12 dogs. Ultrasonographic findings revealed that size of the kidney was significantly increased at 9 days and 17 days and the length of renal cortex was significantly decreased at 17 days after ligation of the unilateral ureter and it was in accordance with dilation of ipsilateral renal pelvis. No significant change of BUN, creatinine, ALT, calcium, and phosphorus was found immediately after unilateral experimental hydronephrosis. Therefore, it was concluded that unilateral hydronephrosis was established in 12 dogs at 17 days after ligation of ureter. Renal artery embolization was performed using selective catheterization in the hydronephrotic kidney of seven dogs and EKG, $SpO_2$, body temperature, pulse, and respiratory rate were within normal ranges during procedures. Iohexol-ethanol solution was used as embolic material. Average ethanol dose for renal artery embolization was $1.94{\pm}1.24ml/kg$. There were no dogs expired after TAE-RA and no side effects associated with regurgitation of iohexol-ethanol solution. Revascularization of renal artery was not found in angiography in dogs treated by TAE-RA at immediately after TAE-RA and 14 days after TAE-RA. Ultrasonographically, the mean longitudinal length of the embolized kidney decreased significantly at 2 and 3 months after TAE-RA compared to that of contralateral normal kidney. In summary, marked shrinkage of the embolized kidney was observed in dogs with unilateral experimental hydronephrosis treated by TAE-RA with iohexol-ethanol and no adverse effects were observed throughout the observation period. It is concluded that TAE-RA with iohexol-ethanol solution is a viable alternative to nephrectomy in dogs with unilateral hydronephrosis.
