Shanghanlun Diagnostic System : Exploring Value as Narrative Medicine by Analyzing Cases (사례분석을 통한 『상한론(傷寒論)』 변병진단체계(辨病診斷體系)의 서사의학적 가치의 탐색)

  • Objective : The purpose of this study is exploring value of Shanghanlun six diagnostic system as Narrative Medicine by analyzing cases. Methods : In the article, we examined the theory of narrative medicine and current studies. And then key elements of narrative medicine was defined. On the basis of these, two medical cases were analyzed by reordering in accordance with time sequences and causality for confirming narrative factors. Results : The narrative approach to analyzing cases shows us that different diseases could be the result of same pathological patterns based on Shanghanlun. This tells us following four aspects. [1] Shanghanlun is the causes-oriented records and the text includes narrative factors. [2] Shanghanlun six diagnostic system is a process of constructing plot of medical case history through the clinical interpretation of contextual meaning of patient's life. [3] In the process of diagnosis, Schemata and script are engaged in assuming and confirming six patterns diseases. [4] The subjective factors of an individual can be applied universally through the provision of Shanghanlun. Conclusions : Narrative factors of Shanghanlun six diagnostic system suggest the possibility and value of Shanghan Medicine as Narrative Medicine in terms of Medical Humanities, essence of medical diagnosis and therapeutic action.

Two Cases of Headache Not Otherwise Classified Treated with Saengkangsasim-tang Based on Shanghanlun Provisions (『상한론(傷寒論)』변병진단체계(辨病診斷體系)에 근거하여 생강사심탕(生薑瀉心湯) 투여 후 호전된 달리 분류되지 않는 두통 증례 2례)

  • Objective : This study aimed to report the improvement of two patients with headache not otherwise classified treated using Saengkangsasim-tang based on the disease pattern identification diagnostic system (DPIDS) based on the Shanghanlun provisions. Methods : Two patients with headache not otherwise classified were treated using Saengkangsasim-tang according to Shanghanlun provisions, and the effect was also compared with Soshiho-tang. The results were evaluated using a numerical rating scale (NRS) and the Korea Headache Impact Test-6 (KHIT-6). In addition, new clinical implications for the interpretation of Shanghanlun provision 157 were reviewed. Results : As a result of analyzing the results of the two cases, treatment with Sosiho-tang was ineffective but improved when using Saengkangsasim-tang. After administration of Saengkangsasim-tang, all headaches decreased to the point where they were not uncomfortable in everyday life. Conclusions : The administration of Saengkangsasim-tang to patients with headache diagnosed according to Shanghanlun provision 157, suggesting that it is clinically effective when applied according to the definitions provided by the Shanghanlun provisions.

Review of the converted weights of Shanghanlun prescriptions in 『Herbal Formula Science in Korean Medicine』 (『한의방제학』의 『상한론』 환산 용량 검토)

  • Purpose : In 『Herbal Formula Science in Korean Medicine (HFSKM)』, the weight unit of Shanghanlun '1 Liǎng (一兩)' is mostly converted to 3 g. We tried to confirm whether the usages of the Shanghanlun prescriptions of HFSKM are valid in the view of safety and efficacy. Method : The toxicity of herbs in Shanghanlun were summarized. The prescriptions of HFSKM including herbs recorded by weight in Shanghanlun were compared with the pharmacopoeia usage. Domestic clinical case studies were analyzed to confirm the clinical evidence for safety and efficacy. Results : Glycyrrhizae Radix, Rhei Radix et Rhizoma, Persicae Semen, Ephedrae Herba, Pinelliae Tuber, Aconiti Lateralis Radix Preparata, Asiasari Radix et Rhizoma, Armeniacae Semen and Scutellariae Radix have been reported to have toxicity. As a result of analyzing the 39 Shanghanlun prescriptions of HFSKM, 26 prescriptions were identified when they were within the pharmacopoeia usage, and 13 prescriptions were found in excess. As a result of analyzing the domestic clinical research papers, symptoms considered to be side effects were not mentioned, and improvement of the diseases was confirmed. As a result of measuring the actual weight, it was confirmed that the usage greatly exceeded the pharmacopoeia usage, such as 57.23 g for Pinelliae Tuber and 45.77 g for Armeniacae Semen. Conclusion : The composition of the prescription for Shanghanlun proposed in HFSKM is considered to be a reasonable dose, but continuous review and standard supplementation are necessary.

A Case Report of Obesity treated with Soshiho-tang gagambang based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 소시호탕(小柴胡湯) 가감방(加減方)을 투여한 비만 증례 1례)

  • Objective : The purpose of this paper is to report the effect of Sosiho-tang gagambang on Obesity based on Shanghanlun provisions Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed as Yangmyung-byung, and treated for 90 days with Soshiho-tang gagambang that Scutellariae Radix was substituted with Salviae Miltiorrhizae Radix. The result of treatment was evaluated by Body Mass Index(BMI). Results : After taking Soshiho-tang gagambang for 90 days, the patient's BMI was decreased, and the patient is cured of Obesity. Conclusions : This study suggests that the treatment based on Shanghanlun provision has efficacy on Obesity. In addition, further studies are necessary in order to discover the roles of flavor of Shanghanlun herbal decoctions, and ensure the safety of it.

  • Objective : The purpose of this study is to analyze the naming of 'A diagnostic system based on Shanghanlun six meridian patterns and provisions' and to suggest an alternative naming. Methods : 1. The meaning of 'Six meridian(六經)' was reviewed on existing theories and Shanghanlun provisions. 2. Comparing the name of diangostic system with the term in 'Korean Standard Classification of Diseases-6(KCD-6)' and term in 'WHO international standard terminologies on traditional medicine in the western pacific region' was done. Results : 'Six meridian' is customary used in the Shagnhanlun study but its meaning is not match with original Shanghanlun system and could possibly make misunderstanding. So 'Disease pattern identification' is suitable than 'Six meridian' for this diagnostic system. Conclusions : This study suggests that using 'A disease pattern identification diagnostic system based on Shanghanlun provisions.'is more appropriate instead of using the name of the six meridian diagnostic system.

  • Objective : The purpose of this paper is to report the improvement of patient with pitting edema treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient was diagnosed with Taeyang-byung, number 96 provision, and took Soshiho-tang herb medication 30days. The changing symptom of pitting edema is reported depended on the edema scale and the statement of patient during treatment. Results : The pitting edema of patient disappeared by 96 provision of Soshiho-tang seclected according to Shanghanlun provisions. Conclusions : This case report suggests that two words on the 96 provision of Shanghanlun, '或 心下悸, 小便不利'(palpitations below the heart, inhibited urination) indcate close relationship between palpitation and decrease of genital secretions, which affected the edema in this case.

  • Objective : The purpose of this paper is to report the improvement of patient with hyperhidrosis treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient was diagnosed with Taeyang-byung Kyeolhyung number 156 provision, and took Oryeong-san herb medication 15days. The result of administration was evaluated by Hyperhidrosis Disease Severity Scale (HDSS) and Visual Analog Scale (VAS). Results : After administration of Oryeong-san 15days, based on 156 provision of Oryeong-san seclected according to Shanghanlun provisions, Hyperhidrosis Disease Severity Scale (HDSS) was decreased from 4 to 2 and Visual Analog Scale (VAS) was decreased from 10 to 2. Conclusions : The patient's hyperhidrosis improved. This case report suggests that the word '遂' on the 153-156 provisions of Shanghanlun be a direct emotional cause of hyperhidrosis in this case.

  • Objective: The purpose of this study was to report improvement in patients with autism spectrum disorder (ASD) treated with herbal medication described in Shanghanlun. Methods: The patient was diagnosed with Lesser Yin disease based on the 315th provision of Shanghanlun, and Baektonggajeodamjup-tang was used for treatment. The result was evaluated using the Korean-Childhood Autism Rating Scale (K-CARS). Results: The K-CARS score decreased from 32 to 15 after 7 months of treatment. Conclusions: Lesser Yin disease 315th provision of Shanghanlun may be a provision describing ASD; in addition, Baektonggajeodamjup-tang may be an effective treatment for ASD.

  • Objective: The purpose of this paper is to report the improvement in a patient with benign paroxysmal positional vertigo (BPPV) treated with herbal medication based on the Shanghanlun disease pattern identification diagnostic system. Methods: According to the 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions,'the patient was diagnosed with Guorem-byung number 350 provision, and took Baekho-tang herbal medication for 15 days. The result of administration was evaluated using the Dizziness Handicap Inventory (DHI) and the Visual Analog Scale (VAS). Results: After administration of Baekho-tang for 15 days, based on the 350 provision of Baekho-tang diagnosed according to the Shanghanlun provisions, the DHI score decreased from 74 to 16 and the VAS score decreased from 10 to 1. Conclusions: The patient recovered from symptoms of BPPV. This case report suggests that the words '滑,''厥,''裏'on the 350 provision of Shanghanlun mean physical and psychological causes of BPPV in this case.

  • Objective : The objective of this study was to explore the meaning of "man (滿)" in Shanghanlun through the analysis of four cases. Methods : We analyzed four cases treated with modified Soshiho-tang, which is administered when 滿 aggravates chief complaints. All cases were diagnosed by the disease pattern identification diagnostic system based on the Shanghanlun provisions. Results : All 4 patients' chief complaints were improved after administration of modified Soshiho-tang. Swelling symptoms were reduced in all 4 cases. Through the etymological research and clinical treatment results, the clinical meaning of 滿 was deduced as "swelling." Conclusions : The etymological and clinical analysis suggest that the concept of "滿" may be defined as "swelling" in the Shanghanlun provisions.
