노인의 우울 완화를 위한 디지털 헬스케어의 활용: 체계적 문헌고찰 및 메타분석 (The use of digital health care for the relief of depression in the elderly: A systematic review and meta-analysis)

  • 본 연구는 디지털 헬스케어가 노인의 우울 완화에 미치는 영향을 알아보기 위하여 체계적 문헌고찰을 수행하였으며, 메타분석을 통해 그 효과를 분석하였다. 체계적 문헌고찰을 위해 CINHAL, Embase, Medline, DBpia, KERIS, KISS, RISS를 검색한 결과 총 4,071편이 검색되었고, 문헌 선정 및 배제 기준에 따라 최종적으로 6편의 논문이 분석에 포함되었다. 그 중 메타분석이 가능한 5편의 연구를 분석한 결과 우울 감소에 효과가 있는 것으로 나타났다(SMD=-4.73, 95% CI -7.44 to -2.01, Z=3.41, p=.0007). 디지털 헬스케어를 제공한 프로그램의 종류로는 웹 기반 프로그램만이 포함되었으므로, 향후 스마트 기기 등 다양한 부분에서의 디지털 헬스케어의 적용으로 노인의 우울 증상을 줄이는 방안을 모색할 것을 제안한다.

Reconstruction of Pharyngolaryngeal Defects with the Ileocolon Free Flap: A Comprehensive Review and How to Optimize Outcomes

  • Several reconstructive methods have been reported to restore the continuity of the aerodigestive tract following resection of pharyngeal and hypopharyngeal cancers. However, high complication rates have been reported after voice prosthesis insertion. In this setting, the ileocolon free flap (ICFF) offers a tubularized flap for reconstruction of the hypopharynx while providing a natural phonation tube. Herein, we systematically reviewed the current evidence on the use of the ICFF for reconstruction of the aerodigestive tract. A systematic literature search was conducted across PubMed MEDLINE, Web of Science, ScienceDirect, Scopus, and Ovid MEDLINE(R). Data on the technical considerations and surgical and functional outcomes were extracted. Twenty-one studies were included. The mean age and follow-up were 54.65 years and 24.72 months, respectively. An isoperistaltic or antiperistaltic standard ICFF, patch flap, or chimeric seromuscular-ICFF can be used depending on the patients' needs. The seromuscular chimeric flap is useful to augment the closure of the distal anastomotic site. The maximum phonation time, frequency, and sound pressure level (dB) were higher with ileal segments of 7 to 15 cm. The incidence of postoperative leakage ranged from 0 to 13.3%, and the majority was occurring at the coloesophageal junction. The revision rate of the microanastomosis ranged from 0 to 16.6%. The ICFF provides a reliable and versatile alternative for reconstruction of middle-size defects of the aerodigestive tract. Its three-dimensional configuration and functional anatomy encourage early speech and deglutition without a prosthetic valve and minimal donor-site morbidity.

근육에 대한 역학적 스트레스의 영향 (Review of Effect of the Mechanical Stress on Muscle)

  • Purpose : Mechanical stress activates signaling cascades and leading to a specific response of a network of signaling pathways. The purpose of this study is to review the effect of mechanical stress-induced adaptation in skeletal muscle involves a biological mechanisms. Methods : This is literature study with Pubmed, Medline and books. Results : Skeletal muscle tissue demonstrates a malleability and may adjust its metabilic response, vascularization and neuromuscular characteristic makeup in response to alteration in functional demands. The adaptation in skeletal muscle involoves a multitude of signalling mechanisms related with insuline-like growth factor, vascular endothelial growth factor, neurotrophins. Conclusions : The identification of the basic relationships underlying the malleability of skeletal muscle tissue is likely to be of relevance for our understanding with PNF technique.

  • Purpose : This article reviewed the advances in the understanding of the effect of motor rehabilitation and brain plasticity on functional recovery after CNS damage. Methods : This is literature study with Pubmed, Medline and Science journal. Results : The inability of CNS neurons to regenerate is largely associated with nonneuronal aspects of the CNS environment. Especially, this neuronal growth inhibition is mediated by myelin associated glycoprotein, olygodendrocyte-myelin glycoprotein, and NOGO. Enriched environment, motor learning, forced limb use have been utilized in scientific studies to promote functional reorganization and brain plasticity. Especially, enriched environment and motor enrichment may prime the brain to respond more adaptively to injury, in part by expressed neurotrophic factors. Conclusions : These reviews suggest that activity-induced neural plasticity occur in damaged brain areas in order to functional reorganization, where it could contribute to motor recovery, and represent a target for stroke rehabilitation.

Protein Named Entity Identification Based on Probabilistic Features Derived from GENIA Corpus and Medical Text on the Web

  • Protein named entity identification is one of the most essential and fundamental predecessor for extracting information about protein-protein interactions from biomedical literature. In this paper, we explore the use of abstracts of biomedical literature in MEDLINE for protein name identification and present the results of the conducted experiments. We present a robust and effective approach to classify biomedical named entities into protein and non-protein classes, based on a rich set of features: orthographic, keyword, morphological and newly introduced Protein-Score features. Our procedure shows significant performance in the experiments on GENIA corpus using Random Forest, achieving the highest values of precision 92.7%, recall 91.7%, and F-measure 92.2% for protein identification, while reducing the training and testing time significantly.

요통에 대한 운동치료 효과의 최근 연구에 따른 증거 (The current evidence for the effectiveness of exercise in low back pain)

  • 컴퓨터를 이용하여 MEDLINE, AMED, CINAHL과 Cochrane Libra를 검색하여 요통환자를 위한 다양한 형태의 운동치료 효과에 대해서 발표된 연구 보고서 중 1991년부터 2001년 사이에 영문으로 출판된 보고서를 중심으로 본 연구에 합당한 주제를 충족시키는 연구 보고서 40편을 선택하였다. 수집된 자료를 통하여 요통 관리의 운동치료 효과에 대하여 연구하였으며 일반적인 요통환자들만을 대상으로 급성, 아급성, 만성단계로 구분하였다. 실험논문은 수정된 rating system을 이용하여 논문의 질 수준과 연구결과를 측정하였으며 고찰 논문과 관찰 연구는 grading system을 사용하여 주요 연구 결과를 측정하였다. 본 연구의 결과는 운동치료가 통증감소와 기능 향상에 효과적임이 증명되었고, 총괄적으로 타당도를 지지하는 정도는 중등도에 해당되었다. 연구보고서들을 scale system을 통해서 세부적으로 분석했을 때, 상급의 연구 방법론적 질을 충족시켰다. 결과에 따르면 급성, 아급성과 만성요통에 대한 운동치료가 효과적이었다라는 확실한 증거를 나타냈다. 본 연구의 결론은 요통환자에 대한 운동치료는 효과적으로 입증되었으며, 특별히 만성요통환자의 경우에 더욱 더 효과적인 것으로 나타났다. 이러한 결론을 더욱 지지하기 위해서는 더 많은 고질의 연구 논문들이 이루어져야 할 것이다.

MeSH를 이용한 개념 기반 검색 엔진 시스템 (Concept-based Search Engine System Using MeSH)

  • 본 논문에서는 개념 기반 검색엔진 시스템(Concept-based Search Engine System)의 검색 정확도를 향상시키기 위한 방법으로 MeSH를 이용하였다. MeSH는 Medical Subject Headings의 약자로서 MEDLINE 논문의 원활한 검색을 위하여 주제어를 코드화한 것으로 이를 개념 그래프의 시소러스로 사용하여 개념 그래프의 가장 중요한 부분인 개념 추출의 정확성을 보장하도록 하였다. 본 논문은 2003년 MeSH의 Descriptor Data의 Term 항목을 사용하여 개념과 관련이 있는 유의어를 추출했다. 추출된 유의어로 개념 그래프를 구성한 것과 문서 내에서의 단어 빈도수에 의하여 개념 그래프를 구성한 것의 검색 결과를 비교한 결과 MeSH 를 시소러스로 사용하여 개념 그래프를 구성한 것이 훨씬 더 정확한 결과를 내는 것을 확인할 수 있었다.

제2형 당뇨병 환자에게 인크레틴 기반 약물치료요법 (Incretin-based Treatment for Type 2 Diabetes Mellitus)

  • Incretin hormones such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide delay gastric emptying, increasing satiety, and enhance insulin secretion. Two new classes of treatments related to incretin hormones for the management of type 2 diabetes mellitus have emerged: GLP-1 receptor agonists (e.g., exenatide, liraglutide) and the dipeptidyl peptidase-4 (DPP-4) inhibitors (e.g., sitagliptin, saxagliptin, vildagliptin, alogliptin), which prevent the degradation of GLP-1. A MEDLINE search was conducted in order to evaluate the efficacy and safety of incretin-based therapies and publications were reviewed. Data from clinical trials indicated incretin-based treatment showed clinically significant reductions in hemoglobin A1c with low risk of hypoglycemia. Weight reductions were observed with GLP-1 receptor agonists where as DPP-4 inhibitors are weight neutral.

Effects of Dioscoreae Rhizoma (SanYak) on Peripheral Neuropathy and its Safety

  • Objectives: This study aimed to evaluate the evidence available in the literature for the safety and efficacy of Dioscoreae Rhizoma (DR) for the treatment of peripheral neuropathy. Methods: Literature searches were performed in MEDLINE and three Korean medical databases up to April 2013. All studies evaluating the effects on peripheral neuropathy or the safety of DR monopreparations were considered. Results: Three studies - DR extract per os (po) on diabetic neuropathy in mice, DR extract injection on the peripheral sciatic nerve after crush injury in rats and DR extract injection to patients with peripheral facial paralysis proved that DR treatments were effective for the treatment of nerve injuries. Conclusions: In conclusion, we found the DR has a strong positive potential for the treatment of peripheral neuropathy, but studies addressing direct factors related to the nerve still remain insufficient.

A Concept Analysis of Gratitude in Patients Based on Rodgers' Evolutionary Method

  • This article presents a concept analysis of the gratitude experience of diseased patients. The Rodgers' Evolutionary Method was used for conducting the analysis. A search of CINAHL, MEDLINE, PsycARTICLES, and Springer databases was conducted using "gratitude or appreciation or thanks" and "patient or illness" as a key word, 22 final articles were selected. Three critical attributes of gratitude in patients were identified: positive emotions, acceptance of the current status, and a driving force to plant the will of life. In addition, two antecedents of gratitude in patients were identified: interactions with people or the environment, and the perception of a favorable stimuli or help. Two consequences of gratitude in patients were identified: an increased compliance in implemented treatment, and an enhancement of trust relationship. The concept analysis describes diseased patients' gratitude. This paper will become the basis for future clinical research related to diseased patients' gratitude.
