MPTP 파킨슨 모델의 트레드밀 운동이 알파시누크린 변성과 흑질선조체내 신경성장인자 활성화에 미치는 영향 (Effects of Treadmill Exercise on Alpha-synuclein Mutation and Activated Neurotrophins in Nigrostriatal Region of MPTP-induced Parkinson Models)

  • 목 적 : 규칙적인 운동이 신경보호 효과와 도파민성 신경원의 재구축, 운동기능 향상에 영향을 미친다는 실험실적 연구결과에도 불구하고, 아직까지 파킨슨병 질환자의 트레드밀 운동이 뇌신경 변화에 영향을 미치는지에 대해서는 논란이 되고 있는 상황이다. 더군다나, 증상의 진전이 흑질선조체의 뇌신경 변화에 의한 것인지, 운동에 의한 전반적인 효과인지, 의욕에 영향을 받은 것이지 또한 확실치 않은 상황이다. 이에 본 연구자는 트레드밀 운동이 파킨슨 유발 실험쥐의 뇌신경 변화를 유발하는 것을 밝히고자 본 실험을 수행하였다. 방 법 : 본 실험에서는 파킨슨 모델을 만들기 위해 수컷 C57BL/6 쥐에 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP) 30 mg/kg과 프로베네시드 20 mg/kg을 매 12시간마다 10회 투여(총 5일)하여 파킨슨병을 유발하였다. 이후 운동군을 경사도 $0^{\circ}$, 18 m/min의 속도로, 하루 40분의 트레드밀 운동을 수행하였다. 운동수행의 마지막에는 모든(염류 비교군, 비운동 비교군) 동물의 뇌를 적출하여 신경원성, 신경화학적 변화가 어떤지 비교군, 비운동군과 비교분석하였다. 본 실험에서 Synphilin 단백질은 알파시누크린의 발현 징후로 사용되었다. 흑질과 선조체의 뇌세포를 western blotting에 의해 염색하여 분석하였다. 결 과 : 염류 비교군의 경우 synphilin 단백질의 발현이 발견되지 않았다. 파킨슨 유발을 위한 MPTP(1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine) 투여는 알파시누크린의 응집을 의미하는 synphilin 단백질의 발현이 급증하였다. 하지만, 트레드밀 운동군에서는 synphilin 단백질의 발현이 비운동군에 비해 유의하게 낮았다. 이는 트레드밀 운동이 알파시누크린의 응집도를 낮추는데 영향을 미친다는 것으로 사료된다. 결 론 : 본 연구에는 트레드밀 운동이 파킨슨 모델의 뇌에서 알파시누크린 응집체의 제거를 촉진하고, 병의 진행, 세포사멸을 억제하는 것으로 밝혀졌다.

Wolff-Parkinson-White 증후군 환자의 심근 관류 이상 (Abnormal Perfusion on Myocardial Perfusion SPECT in Patients with Wolff-Parkinson-White Syndrome)

  • 목적 : 좌각차단이나 우심실조율박동과 같은 부정맥을 가진 환자들에서 심근관류는 의미있게 변화한다는 것이 알려져 있다. 비정상적인 심근관류는 심실조기흥분에 의해서도 야기될 수 있다고 알려져 있지만, 그 위치 범위 강도와 부전도로와의 관계는 아직 정립되어 있지 않았다. 이에 WPW 증후군 환자에서 부전도로의 위치와 SPECT상에서 관류양상의 관계에 대해 알아보고자 하였다. 대상 및 방법 : WPW 증후군 환자 11명에 대해 Adenosine 99m-Tc MIBI 또는 Tl-201 심근관류 SPECT를 시행하였다, 관류결손은 Fitzpatrick's algorithm을 기초로 한 심전도 또는 전기생리학적검사 및 전극도자 절제술을 이용한 부전도로의 위치와 비교하였다. 결과: 11명의 환자들의 평균 나이는 $39.9{\pm}8.6$세 였고, 비특이적인 흉통을 호소하거나, 증상이 없었다. 11명 모두 관삳동맥의 위험도에 관한 계산도표를 이용하여 관상동맥질환의 확률을 예측했으나 0.1 이하로 위험도가 낮았고 이중 4명은 관상동맥조영술을 시행한 결과 3명은 정상이었고 1명은 관동맥 협착 (50%) 부위의 심근혈류가 정상이었다. 4명의 환자에서 전기생리학적검사 및 전극도자 절제술을 시행하였다. 9명의 가역적 그리고 1명의 비가역적 관류결손이 관찰되었고. 범위는 소에서 대까지 강도는 경도에서 중등도까지 나왔다. 부전도로의 위치가 우외측인 1명은 관류결손이 없었으나, 그 외의 환자들은 다양한 양상을 보였다. 부전도로의 위치가 좌외측인 환자중 1명에 대해 전극도자 절제술을 시행하였고 6주후에 SPECT를 한 결과 시술 전에 있던 관류결손의 범위가 현저하게 감소하였다. 결론: WPW환자에서 심근관류결손은 다양한 범위, 강도 및 위치를 가진다. 거의 대부분의 환자에서 비정상적인 관류결손이 나타났으나, SPECT 소견으로 부전도로의 위치를 특이적으로 예측하기는 어려웠다. 그러므로 WPW증후군 환자에서 심근관류 SPECT의 결과를 주의 깊게 해석해야 한다.

간세포암에서 방사선 치료의 역할 (The Role of Radiotherapy in Treatment of Hepatocellular Carcinoma)

  • 간세포암에 대한 방사선 치료의 효과를 평가하고자 1984년 1월부터 2000년 1월까지 영남대학교 의과대학 부속병원에서 간세포암으로 진단된 환자들 중에서 방사선 치료 단독 혹은 간동맥 색전술과 병합요법을 한 18명의 환자들의 의무기록과 방사선 사진을 검토하여 다음과 같은 결과를 얻었다. 남자가 15명, 여자가 3명이었으며 평균 나이는 51세였다. 복수가 4명(22.2%)에서 있었으며 간경변이 11명(61.1%)에서 있었다. 간기능은 Child-Pugh class A, B, C가 각각 6명, 3명, 2명이었다. 14명(77.8%)에서 HBs 항원 양성이었으며 anti-HCV 양성은 없었다. 사망원인은 간성혼수를 포함한 간부전이 2명(11.1%), 위장관출혈이 1명(5.6%), 암사망이 1명(5.6%), 현재 생존이 4명(22.2%)였으며 10명(55.6%)은 불명이었다. 종양의 위치는 간우염이 10례 좌염이 3례 양쪽 모두가 5례였으며, 종양의 형태는 결절형이 4례, 괴상형이 7례, 미만형이 7례였다. 8례(44.4%)에서 동정맥우회(shunt)가 있었으며 간문맥혈전증이 4례(22. 2%)에서 있었다. 병기는 I기, II기, III기, IV기 각각 0례, 5례, 4례, 9례였다. 종양의 크기(volume percentage)는 4025%(5-96)였다. 치료에 대한 반응은 partial response가 2례(11.1%), minimal response가 4례(22.2%), no change가 11례(61.1%), progressive disease가 1례(5.6%)였다. 평균 생존률은 97개월(2-25)로 반응율(response rate)은 33.3%였다. 3개월, 6개월, 12개월, 24개월 생존률은 각각 81.3%, 43.8%, 18.8%, 6.3%였다. 결론적으로, 간세포암에 있어서 방사선 치료는 생존률을 향상시키지는 못하더라도 종양의 크기를 줄이는데 효과가 있었다. 간동맥 색전술과의 병합치료가 치료효과를 강화하는 것으로 생각되나 방사선 단독 치료의 효과를 알기 위해서는 더 많은 연구가 필요할 것으로 생각된다.

골재생 과정에서 혈소판유래성장인자-BB와 덱사메타존의 병용 효과 (The Effects of Dex and PDGF-BB on Bony Healing of Calvarial Defect in Rats)

  • Bone remodeling results from the combined process of bone resorption and new bone formation which is regulated in part by some of Dexamethasone related proliferation & mineralization of cultured bone cell and polypeptide growth factors such as platelet derived growth factor(PDGF), which has been known to be an important local regulator of bone cell activity and participate in normal bone remodeling. To evaluate the effects of Dex and PDGF on bony healing of calvarial defect in rats, 10 ng/ml PDGF were applied on P group and 10 ng/ml PDGF and $10^7$ M Dex were applied PD group. 4 rats in each group were sacrificed at 7, 14. 21 days after operation respectively, and the tissue blocks were prepared for light microscope with H-E for evaluation of overall healing, with TRAP(tartrate resistant acid phosphatase) for evaluation of osteoclastic activity and with immunohistochemical staining for macrophages. The results were as follows : 1. In all group, healing aspects were progressed from 7 days to 21 days in soft and bony tissue, but complete repair were not observed in bony defect 2. PDGF and control group were showed similar bony healing aspect , but bony healing in combination of PDGF-BB and Dex were observed slower aspect compared to PDGF and control group from early healing times. 3. There were no significant difference on activities of osteoclast and macrophages in bony healing between control and experimental group In conclusion, PDGF were not influenced on bony healing of defect and combination of PDGF-BB and Dex were showed slower healing through early healing times. it was considered that Dex compared to PDGF did influenced on early hone formation factors in healing period

식이(食餌)에 의한 흰쥐 간지질(肝脂質) 및 효소활성(酵素活性) 변화(變化)에 관한 연구(硏究) (Changes in the Lipid Composition and Some Enzyme Activities in the Rat Liver as Affected by Diets)

  • Albio rats right after weaning, weighing $50{\sim}55g$, were divided into the control, high-carbohydrate-, high-lipid-, and high-protein-fed groups. and were fed for 12 weeks with the respective diets to observe the increase in body weights as well as changes in the chemical constituentes and enzyme activities in the liver tissue, with the following results. (1) There was little difference in the rate of increase in the body weights among the groups, showing normal growth, except the high-protein-fed group which showed decrease in rate of body weight increase from the 7th week after feeding. (2) The liver weight was either increased after 12 weeks of feeding with the high-carbohydrate and high-Lipid diets, or showed no difference with the high-protein diet, as compared to the control weight. (3) The liver cytosol protein content was increased when fed with the high-protein diet, but decreased when fed with the high-carbohydrate and high lipid diets, as compared to the control content. (4) The triglyceride and cholesterol contents in the liver were decreased in the high-protein-fed group, but increased markedly in the high-carbohydrate- and high-lipid-fed groups as compared to the control values. (5) The hepatic glucokinase, G6PD, LDH, and fatty acid synthetase activities were increased in the high-carbohydrate and high-lipid-fed groups, and GOT and CPT activities were increased in the high-protein-fed group. From the above results. it was known that the high-carbohydrate and high-lipid diets stimulated the hepatic lipid metabolism, giving rise to lipogenesis, but the high- protein diet could prevent the lipogeuesis leading to the body weight increase.

Haematologic Parameters in Metastatic Colorectal Cancer Patients Treated with Capecitabine Combination Therapy

  • Background: The standard treatment in the metastatic colorectal cancer consists of 5-FU based infusional regimens. However, with oral fluoropyrimidines, equal tumor responses may be obtained. Capecitabine causes macrocytosis of the cells by inhibition of DNA synthesis. In this context, a relationship was found between mean corpuscular volume (MCV) and response to therapy in breast cancer patients treated with Capecitabine, but whether this relationship also pertains in colorectal cancer has not been established. Materials and Methods: A total of 102 metastatic colorectal cancer patients treated with a oxaliplatin (XELOX)${\pm}$Bevacizumab combination were retrospectively evaluated. Patients were randomized into three groups. Hematological parameters (MCV, MPV, PCT, PLT, NLR) were recorded retrospectively, before treatment and after 3 cycles of chemotherapy. Results: After three cycles of therapy, 20 (19.6%) patients had progressive disease (PD), 41 (40.1%) had stable disease (SD), and 41 (40.1%) demonstrated a partial response (PR). In 62 (60.7%) treatment was with capesitabin plus XELOX therapy, and in 40 (39.2%) it was XELOX-Bevacizumab combination therapy. There was no difference among three groups before the treatment in terms of MCV, MPV, PCT, PLT, and NLR. MCV showed significant increase in chemotherapy response groups (PR and SD). In addition, a significant decrease was observed for platelet count in chemotherapy response groups. While NLR decrease was seen in only a PR group, PCT decrease was observed in all three groups. PCT and PLT values were higher in patients receiving Bevacizumab. Conclusions: PLT, PCT, MPV, and NLR values were decreased due to Capecitabine-based chemotherapy, however MCV was increased. PCT and PLT values were higher in patients who received Bevacizumab than those who did not. MCV, PLT, and NLR can be considered as important factors in predicting response to colorectal carcinoma treatment.

알코올 투여에 따른 혈압의 변화 양상 : 용존산소량에 따른 혈압 변화 (Aspect of Blood Pressure after Oral Administration of Alcohol : Effect of Dissolved Oxygen on Blood Pressure)

  • Purpose: The purpose of this study was to elucidate the effect of dissolved oxygen in alcohol to blood pressure of healthy persons. Methods: Subjects (n=30) were randomized in a double blind crossover study to receive 120 mL, 240 mL, 360 mL of alcohol (Korean spirit, 19.59 v/v%, dissolved oxygen is 8 ppm and 20 ppm). Blood alcohol concentration (BAC) and blood pressure were measured applying Lion SD-400 Alcolmeter$^{(R)}$ Breathalyser and Tensoval duo control. Pharmacokinetic parameters ($C_{max}$, $T_{max}$, $AUC_{last}$, $K_{el}$, $V_d$, Clearance) were calculated using Winnonlin$^{(R)}$ program. The difference of parameters and values were analysed by student t-test using Microsoft$^{(R)}$ Excel program. Results: The $AUC_{last}$ values of 8 ppm group and 20 ppm group in 240 ml administration were $6.15{\pm}2.60cg{\cdot}min/ml$, $5.33{\pm}1.84cg{\cdot}min/ml$ (p<0.05) and those in 360 mL were $11.93{\pm}5.70cg{\cdot}min/ml$, $10.33{\pm}4.60cg{\cdot}min/ml$ (p < 0.01), respectively. Thus, the $AUC_{last}$ was significantly decreased. On the other hands, there was a significant change in systolic blood pressure (SBP) after alcohol administration. All measured value after 360 mL of alcohol administration was significantly decreased (p < 0.01). Conclusions: The dissolved oxygen in alcoholic beverage has no effect on blood pressures but the alcohol administration has an effect on blood pressure. Thus, SBP can be used as a biomarker of alcohol administration and utilized in PK/PD modeling of alcohol.

과학교육평가(科孝敎育評價)와 목표(目標)에 관(開)한 중등고학교사(中等科學敎師)의 인식(認識) 조사(調査) 연구(硏究) (A Survey Study on the Science Education Evaluation and Objectives -attached perception and actual state of middle school science teachers in pusan-)

  • The purpose of our survey study was to investigate the status on perception and actual state toward science educational evaluation and objectives of middle school science teachers in Pusan. The perception and actual state were surveyed by the questionaire which was developed by Jong-Ok Woo et. al. We developed two kinds of questionaire. One is designed to use Likert type scale and is composed of two main category(SE; Science educational evaluation, SO; Science educational objectives) which contains 21 question items, and the other has 16 question items From 313 science teacger's responses of 75 middle schools in pusan, the tendency. of total(SE+SO) and each sub category, and the contrast with their background and actual state were analized by SPSS program of KAIST. Some results of our survey study can be summarized as follows; 1. Total tendency of science educational evaluation(SE) and objectives(SO) of middle school science teachers in pusan shows a positive and right tendency of conception in general(m-85.8/100). 2. Tendency of SE(m-87.8/100) are slightly higher than SO(m-81.7/100). At the level of 5% significance, there are correlated but shows low correlation (r=0.12(4). 3. In each correlation toward total tendency(SE+SO), SE is highly correlated(r-0.8486) but SO is moderately correlated(r=0.6297) at the level of 0.1% significance. 4. Tendencies of 5sub category (PE, PC, PD, PO, PB) shows considerably right tendencies (m=$73.4/100{\sim}92.7/100$), there are moderately correlated foward total tendency(SE+SO) at the level of 0.1% significance (r=$0.49{\sim}0.60$). 5. At the level of 5%significance, total means are no differences which their background(Sex, Final alma mate, Major, Difference of training), but there is differences to teaching career at the level of 5% significance.

근골격계 통증환자에서 반복 자기자극치료의 통증완화효과: 예비연구 (Short-Term Pain Relief by Repetitive Peripheral Magnetic Stimulation in Patients with Musculoskeletal Pain: A Pilot Study)

  • Objective: Recent studies have shown that repetitive peripheral magnetic stimulation (rPMS) reduces pain in various conditions. This pilot study aimed to investigate the effects of rPMS depending on the pain characteristics. Method: Adult patients aged 19~85 years evaluated at our institution between September 1, 2017 and February 28, 2018 for subacute to chronic musculoskeletal pain equivalent to a numeric rating scale of 3 or higher for at least one month were enrolled. Pain scores as determined using a numeric rating scale at baseline and at the end of treatment were set as the primary outcome. Additionally, we classified the pain into nociceptive, intermediate, or neuropathic pain using the PainDETECT questionnaire and compared the responsiveness to rPMS according to the type of pain. Results: The average pain scores significantly decreased after the 2-week rPMS treatment in all enrolled subjects (p<0.001). There was no statistically significant difference in pain reduction between groups divided by PainDETECT questionnaire. Conclusion: This study suggests that rPMS could safely relieve various types of pain.

Effect of Restricted Feed Intake on Early Reproductive Development in Large White Gilts

  • Forty-five Large White gilts were used to study the effect of energy intake from 28 to 176 d of age on body composition and reproductive development. From 28 to 60 d, the gilts were fed ad libitum a 16.6 MJ DE/kg, 24% crude protein and 1.3% total lysine diet. From 61 d of age three dietary treatments were used; 1) ad libitum access to feed (15.6 MJ DE/kg, 21% crude protein and 1.07% total lysine) (H), 2) feed offered at 75% (M) of the previous days intake of H, and 3) feed offered at 60% (L) of the previous days intake of H. ADG from 61 to 176 d of age was (p<0.05) affected by treatment. Although live weight at 176 d of age did not differ (p>0.1) the H gilts had higher (p<0.08) carcass weights than the M or L gilts. Back fat depths were similar (p>0.1) for all treatments at 115 d of age, however by 176 d of age M and H gilts were fatter (p<0.1) than L gilts. The mean lipid deposition (LD) from 115 to 176 d of age for L gilts (78.9 g/d) was less (p<0.05) than for M gilts (143.6 g/d) and H gilts (135.6 g/d). There were no differences between treatments for protein deposition (PD) over the same period. More (p<0.05) H gilts (n=8) attained puberty (first observed estrus) than either M gilts or L gilts (n=4 for both). Follicle numbers were similar (p>0.1) across treatments. For gilts that attained puberty, H gilts had fewer (p<0.05) follicles (13.5) than M gilts (19.7) and L gilts (21.3). For gilts with follicular development, H gilts had the heaviest (458.7 g) reproductive tract weight (RTW). However, for those that attained puberty, L gilts had the heaviest RTW. RTW were lowest for those with no follicular development. Energy restriction had a negative impact on puberty attainment, i.e. it took longer to reach puberty. However, for gilts that attained puberty, the number of follicles was greater for those on r feed intakes. It would appear that rate of fat deposition, but not necessarily the total amount of fat, plays an important role in puberty attainment.
