Monthly characteristics of rainwater chemistry at a coastal site in southwestern Japan were examined based on an eight year record. In the period November-May when rain was mainly caused by cyclones, the monthly mean concentrations of $nss-{SO_4}^{2-}$, ${NO_3}^-$, ${NH_4}^+$, $nss-Ca^{2+}$, $Na^+$ and $Cl^-$ over the eight years were 25.1-57.8, 9.9-25.0, 11.3-31.4, 5.5-18.7, 24.2-154.9 and $30.0-178.5{\mu}eq\;L^{-1}$, respectively. In June and July when rain was mainly caused by stationary fronts, i.e. Meiyu fronts, the concentrations were 14.4-20.7, 7.2-9.5, 7.7-12.9, 4.1-6.8, 21.7-33.6 and $26.4-40.5{\mu}eq\;L^{-1}$, respectively. In August and September when typhoons contributed substantial rainfall, the respective concentrations of $Na^+$ and $Cl^-$ were as high as 97.7-105.3 and $116.8-122.9{\mu}eq\;L^{-1}$, while the concentrations of other ions were low. These results indicate a large variation of monthly rainwater chemistry, which is basically dependent on the synoptic weather patterns causing rain. From later autumn to early spring, rain contains ions in high concentration and large variation ranges. In the Meiyu season, rain contains less ions which vary in a range much smaller than that in later autumnearly spring. In summer and autumn, the concentrations are low, except $Na^+$ and $Cl^-$ which can be large due to typhoons' contribution.
Sporadic reports of hypernatremic dehydration in breastfed newborn infants have appeared in medical literature for at least 3 decades. We report the first case of hypernatremic dehydration resulting from inadequate breast-feeding in Korea. A 14-day old baby, born to a mentally retarded mother, was transferred to our hospital with a body weight loss of 460 g since birth(17%) and a serum sodium(Na) level of 179 mEq/L, after initial hydration at another hospital. On admission, a cardiac murmur was heard and an enlarged liver was palpated. Cardiac ultrasonogram revealed ventricular septal defect and ostium secundum atrial septal defect. During hydration, a seizure-like attack developed. Serum Na decreased to 135 mEq/L on the 5th day of admission. Brain ultrasonography and brain magnetic resonance image revealed no remarkable abnormalities. Electroencephalography was normal. She suffered from prerenal azotemia, hyperglycemia and disseminated intravascular coagulation at admission but was treated successfully. Heart failure was also controlled with dobutamine, diuretics and digoxin.
Objectives: The Short Form 36 (SF-36) questionnaire is increasingly being used to measure health-related quality of life (HRQoL) in Indonesia. However, evidence that it is valid for use in Indonesian adults is lacking. This study assessed the validity and reliability of the SF-36 in Indonesian middle-aged and older adults. Methods: Adults aged 46-81 years (n=206) in Yogyakarta, Indonesia completed the SF-36, another measure of HRQoL (the EuroQoL visual analogue scale [EQ-VAS]), and measures assessing their demographic characteristics. Fifty-four percent (n=121) completed the SF-36 measure again 1 week later. Confirmatory factor analysis was conducted to confirm the factor structure of the SF-36. Internal consistency reliability was estimated using Cronbach's alpha, and test-retest reliability was assessed using intraclass correlations. Convergent and discriminant validity were assessed by computing correlations among SF-36 subscales, between subscales and the 2 component scores, and between component scores and EQ-VAS scores. Results: Most scaling assumptions were met. The hypothetical factor structure fit the data poorly (root mean square error of approximation [RMSEA]=0.108) and modification was required for a good fit (RMSEA=0.060). Scores on all subscales demonstrated acceptable internal consistency (α>0.70) and test-retest reliability (r>0.70). Divergent validity was supported by weak to moderate interscale correlations (r=0.19 to 0.64). As expected, the 2 summary scores were moderately to strongly correlated with the EQ-VAS (r>0.60). Conclusions: The findings adequately support the use of SF-36 in Indonesian middle-aged and older adults, although the optimal algorithm for computing component scores in Indonesia warrants further investigation.
Objectives The aim of this study is to investigate the effect of Acupotomy on the pain reduction and functional improvement of patients with Acute low back pain. Methods We studied 60 patients with Acute low back pain with Numeric Rating Scale (NRS) 5 or higher in admission and had admitted to Department of Korean Medicine Rehabilitation of Daejeon University from March 1, 2017 to October 31, 2017. 30 patients had received Acupotomy combined Korean Medicine treatment(such as acupuncture, moxabustion, herbal medicine, etc) and the other 30 had received Korean Medicine treatment only. The analysis was conducted as a retrospective study which analyzes the patient's medical records. Statistical analysis was performed using the IBM SPSS Statistics 24 program. We used NRS to evaluate pain reduction, used Range of Motion (ROM) and Roland Morris Disability Questionnaire (RMDQ) to evaluate function improvement, and used EuroQol-5 Dimension (EQ-5D) and EuroQoL-Visual Analogue Scale (EQ-VAS) to assess quality of life. Finally, a five-point Likert scale was used to assess treatment satisfaction. Results The analysis revealed that patients who were treated with Acupotomy showed statistically significant NRS reduction, improvement of ROM and RMDQ, improvement of EQ-5D and EQ-VAS and satisfaction compared to those who just recevied Korean Medicine treatment. Conclusions In conclusion, we found that the Acupotomy showed a positive effect on pain resolving, functional rehabilitation and quality of life in patients with Acute low back pain.

Purpose : To assess the prevalence of hyponatremia in epileptic children receiving carbamazepine or oxcarbazpine, we investigate serum sodium changes according to age, serum carbamazepine level, and daily oxcarbazepine dosage, and the prevalence of symptoms of hyponatremia. Methods : We reviewed the clinical data of the 197 children receiving carbamazepine and/or oxcarbazepine with or without antiepileptic therapy. And these were classified into the carbamazepine treated patients (group I), oxcarbazepine treated patients (group II), and carbamzepine or oxcarbazepine with other antiepileptics treated patients (group III). Potentially predictive values for development of hyponatremia were examined in each group: age, plasma level of carbamazepine and daily dosage of oxcarbazepine. We assessed the symptoms of hyponatremia. Results : The overall prevalence of hyponatremia was 20.8% (group I, II and III : 17.9%, 22.6%, and 21.8%, respectively), and the prevalence in groups II and III compared with controls (P<0.03) was significantly lower. The changes of serum sodium levels relation to age were not significantly different. The changes of serum sodium levels by increasing of serum levels of carbamazepine and dosage of oxcarbazepine were statistically significant (P<0.01). Among the 41 patients who had biochemical hyponatremia, the prevalence of hyponatremic symptoms was 17.1%. Conclusion : Hyponatremia may occur relatively more frequently with oxcarbazepine or combined other antiepileptics than carbamzepine therapy only. Age of children receiving carbamazepine or oxcarbazepine was no predictive value for occurrence of hyponatremia. The patients whose serum level were less than 125 mEq/L showed more severe clinical symptoms than any other study groups.
Purpose: Several studies have shown that magnesium plays an important role in modulating N-methyl-D-aspartate (NMDA)-related seizures by blocking NMDA ion channel receptors. Clinicians usually measure total serum magnesium levels instead of biologically active ionized magnesium levels. We compared the serum ionized magnesium ($iMg^{2+}$) level between epileptic children with and without a history of fever-triggered seizure (FTS). Methods: All epileptic children who visited the outpatient clinic or pediatric emergency department at Korea University Guro Hospital between January 2015 and July 2017 were included. Only epileptic children aged 1-8 years who were newly diagnosed within 2 years were included. Results: There were 12 children with FTS and 16 without FTS. Median serum $iMg^{2+}$ level was 0.93 (0.85-1.14, quartile) mEq/L. Serum $iMg^{2+}$ level was significantly lower in epileptic children with FTS (0.86 mEq/L) compared to those without FTS (1.10 mEq/L) (P=0.005). No difference was noted in clinical variables between the two groups. Lower serum $iMg^{2+}$ level significantly increased the risk of having FTS in epileptic children based on multivariable logistic regression analysis (odds ratio [OR]=0.028). Conclusion: Serum $iMg^{2+}$ level was significantly lower in epileptic children with FTS than in those without FTS. Measurement of biologically active serum $iMg^{2+}$ level could be considered in epileptic children with recurrent FTS. A large-scale prospective study is warranted.

Influence of magnesium and calcium on the serum cholesterol concentration in rabbit blood serum was studied. The subjects were fed with the basal diet and magnesium, calcium and Korea Ginseng. The results obtained are summarized as follows: The subjects body weight fed a basal diet, magnesium, calcium and ginseng powder added did not increased, but the weight of liver was increased. Total cholesterol level of the subject fed ginseng powder 4mg. added diet group showed the lowest concentration. The subjects fed with a basal diet and magnesium/calcium(1:2) added diet showed the lower than anyother group. In case of free cholesterol and cholesterol ester, the subject fed with ginseng powder tap. magnesium/calcium(1:2) added diet showed the lowest concentration. The subjects of the lowest concentration of total cholesterol showed the lowest concentration of low density lipoprotein(LDL) and high density lipoprotein(HDL)/total cholesterol ratio was the higher level than any other group. In case of triglyceride and phospholipid, the subject fed with ginseng powder showed the triglyceride concentration gradually do creased, but phospholipid level was increased positively. In case of electrolytes, sodium and potassium of the subject fed with magnesium, calcium and ginseng powder 4mg added diet showed 120mEq/l 1 and 6.8mEq/l, respectively.
The antioxidant activities of $makgeolli$ and other alcoholic beverages were compared. Based on the same volume (70 ${\mu}L$ eq.) of the alcoholic beverages, the 2,2'-azinobis (3-ethylbenzothiazoline-6-sulfonic acid ammonium salt) (ABTS+) radical-scavenging activities were as follows: whisky > $makgeolli$ crude extract (MCE) > rice wine (RW) > clarified $makgeolli$ (CM) > soju. Based on the same alcohol concentration (6%) of the alcoholic beverages, however, $makgeolli$ showed the highest activity. In addition, based on the same volume (70 ${\mu}L$ eq.), the inhibition effects against the formation of cholesteryl ester hydroperoxide (CE-OOH) were as follows: soju > whisky > RW > MCE > CM. Based on the same alcohol concentration (6%), however, the inhibition effects against the formation of CE-OOH were as follows: RW > MCE > soju > whisky > CM. Therefore, it was suggested that $makgeolli$ may contain radical-scavenging- and metal-ion-chelate-type antioxidants and may increase the antioxidant activity in the blood.
Antioxidative activity and physicochemical characteristics of heat-treated licorice extracted by ethyl-acetate (EtOAc) and ethyl-alcohol (EtOH) were evaluated at various treatment temperatures (110, 120, 130, 140, and $150^{\circ}C$), times (1, 2, 3, 4, and 5 hr), and moisture contents (10, 20, 30, 40, and 50%). Maximum extraction yields of EtOAc treated at $140^{\circ}C$, 2 hr, and 20% moisture content and EtOH extracts treated at $120^{\circ}C$, 2 hr, and 40% moisture content were 9.48 and 32.90%, whereas those of control were 3.74 and 14.60%, respectively. Highest total polyphenol content was obtained from 13.95 mg/g EtOH extract treated at $150^{\circ}C$, 3 hr, and 30% moisture content (control: 6.92 mg/g). Highest antioxidative activity $(IC_{50})$ was obtained from 0.32 g/L EtOAc treated at $140^{\circ}C$, 2 hr, and 20% moisture content (control: 0.57 g/L). Highest ascorbic acid equivalent antioxidant activity value of 2,112.61 mg ascorbic acid (AA) eq was obtained from EtOAc extract treated at $120^{\circ}C$, 2 hr, and 40% moisture content (control: 1,920.27 mg AA eq). Optimum heat treatment conditions were $130-140^{\circ}C$, 3 hr, and 30% moisture content.
