한식 식이패턴 평가지표(Kdiet-index)와 대사성질환과의 상관관계 - 2017 국민건강영양조사(KNHANES) - (Correlation between Korean Food Pattern Index (Kdiet-index) and Metabolic Disease - 2017 KNHANES -)

  • This study was performed to develop a Kdiet-index based on the definitions and characteristics of traditional Korean food. The Korean Food Pattern Index (Kdiet-index) for diseases was applied using dietary data from the 2017 National Health and Nutrition Examination Survey (KNHANES) and the association between Kdiet-index and metabolic disease was confirmed. The study subjects were 1,971 Korean adults aged >40, and the components were classified according to 14 criteria used to establish the Kdiet-index. Main ingredients up to the top 3 were designated using dish names, ingredients, and contents using KNHANES dietary data. Kdiet-index was classified using scores of ≤3 points, 4-7 points, and ≥8 points and total Kdiet-index were calculated by summing dietary scores. Correlations between Kdiet-index and obesity, hypertension, hyperlipidemia, and diabetes were analyzed. The odds ratio (OR) and 95% confidence interval (CI) for obesity were 0.531, 0.385~0.732 and for elevated cholesterol (≥240 mg/dL) indices were 0.471, 0.282~0.788, respectively, which showed a significant decreased in the risk for each disease for Kdiet-index of ≥8 points. This study confirms that metabolically related clinical results improved significantly as Kdiet-index increased and that higher Korean food pattern indices are associated with lower risks of metabolic disease

체중조절 식이에서 탄수화물의 비중 : 그 질과 양 (A Minireview on Carbohydrate in Weight Management Diet : The Quantity and the Quality)

  • During last few decades dietary guidelines for the weight management mainly have focused on a low-fat, high carbohydrate diet. Carbohydrate was supposed to be low-dense, highly satiating as well as it affects little on the lipogenesis. Although low-fat diet has significant weight-reducing effect, the loss was modest and usually regained after cessation of the diet. Furthermore, low-fat, modest-carbohydrate diet did not impact on the ever increasing rates of overweight and obesity. Alternative approaches include low-carbohydrate diet, high-carbohydrate diet and low-glycemic index diet. Although none of above mentioned diet have sufficient evidence for standard weight management diet, short-term efficacy and safety are being approved continuously. Low-carbohydrate diet contains less than 45% of carbohydrate in daily energy consumption, it is claimed to have more satiating effect and to improve metabolism. However, low compliance due to the limitation of food choice should be considered on prescribing the diet. High-carbohydrate which contains 90% of carbohydrate in total daily energy consumption, is effective in providing satiety and lowering total calorie intake and cholesterol. On the other hand, nutritional unbalance should be took into account. Low-glycemic index diet is based on the theory that contemporary diet contains significantly less fiber and unrefined carbohydrate, therefore insulin secretion is disturbed. Because low glycemic index food slowly increase blood glucose and insulin level, it induces much satiating effect and may decrease calorie ultimate intake. However, poor standardization of glycemic index is one of the main obstacle for the diet to be applied in the clinic. Meanwhile, high fructose food and beverage should be discouraged because it has little satiating effect and may cause insulin resistance. High fiber food is another recommendation for healthy, lean diet.

  • This study classified respondents by degree of obesity using Body Mass Index(BMI) and investigated the differences in self-esteem, food lifestyle, social-cultural attitudes towards appearance, and diet-related attitude and behavior such as trial number of diet, exercise time and period, use of diet information, interest in weight control, diet motivation, and dietetic therapy. A survey was conducted with 258 adults between the ages of 20 and 29 who lived in Daegu and Uijeongbu from November $17^{th}$ to December $14^{th}$ 2014. Data collected from the respondents were analyzed using descriptive statistics, factor analyses, analyses of variance and chi-square tests. This study divided the respondents into three groups (the underweight, the normal weight, the overweight) according to Body Mass Index. The results showed that there were significant differences in food lifestyle (health-conscious, popularity-seeking, taste-seeking), social-cultural attitudes towards appearance, exercise time and period, use of diet information, interest in weight control, diet motivation, and dietetic therapy among groups classified by degree of obesity. However, there were no differences in self-esteem, convenience-seeking, and trial number of diet among them.

흡연 및 음주행태와 저염식생활 실천과의 관련성: 2008년 지역사회건강조사 자료를 이용하여 (The Association between Smoking, Alcohol Intake, and Low-Salt Diet: Results from the 2008 Community Health Survey)

  • The purpose of this study was to identify the association between health-related behaviors including smoking, alcohol intake, and the practice of a low-salt diet using data from a Community Health Survey conducted in 2008. The study subjects were 129,151 persons (19 years and older) without cardiovascular diseases or diabetes mellitus diagnosis history. An index for evaluating low salinity was created by summing three low salinity-related questions (range: 0~3), and a low-salt diet was defined if the index of low salinity was 3. We examined the levels of smoking and alcohol intake according to the index of low salinity, and conducted multiple logistic regression analysis to examine the odds ratios of low-salt diet practice in relation to smoking and drinking behavior, adjusting for general characteristics. The smoking and drinking status significantly improved as the level of low salinity index increase. Adjusting for general characteristics, those with smoking, alcohol intake, or a combined habit of the two behaviors had significantly lower odds ratios for practice of a low-salt diet. In conclusion, smoking and drinking behavior were negatively associated with the practice of a low-salt diet. Based on these findings, it may be necessary to have comprehensive nutritional education programs that consider the multiple effects of smoking, drinking, and a low-salt diet.

The effect of low glycemic index diet on body weight status and blood pressure in overweight adolescent girls: a randomized clinical trial

  • Although several studies have assessed the influence of the glycemic index on body weight and blood pressure among adults, limited evidence exists for the pediatric age population. In the current study, we compared the effects of low glycemic index (LGI) diet to the healthy nutritional recommendation (HNR)-based diet on obesity and blood pressure among adolescent girls in pubertal ages. This 10-week parallel randomized clinical trial comprised of 50 overweight or obese and sexually mature girls less than 18 years of age years, who were randomly assigned to LGI or HNR-based diet. Macronutrient distribution was equivalently prescribed in both groups. Blood pressure, weight and waist circumference were measured at baseline and after intervention. Of the 50 participants, 41 subjects (include 82%) completed the study. The GI of the diet in the LGI group was $42.67{\pm}0.067$. A within-group analysis illustrated that in comparison to the baseline values, the body weight and body mass index (not waist circumference and blood pressure) decreased significantly after the intervention in both groups (P = 0.0001). The percent changes of the body weight status, waist circumference and blood pressure were compared between the two groups and the findings did not show any difference between the LGI diet consumers and those in the HNR group. In comparison to the HNR, LGI diet could not change the weight and blood pressure following a 10-week intervention. Further longitudinal studies with a long-term follow up should be conducted in this regard.

녹각(鹿角)과 홍화자(紅花子)가 난소적출에 의한 실험적 흰쥐 골다공증 모델에 미치는 영향 (Effects of Cervi Pontotrichum cornu and Carthami semen on the Experimental Osteoporosis Induced by Ovariectomy in Rats)

  • This study was carried out to investigate the effects of Cervi Pontotrichum cornu and Carthami semen on the serum osteocalcin level, bone mineral density, bone trabecular area and cortical thickness index in ovariectomized rats. Femal Sprague-Dawley rats were divided into 5 groups. Non-ovariectomized groups were divided into commertial diet fed(normal group) and calcium free basal diet fed(sham operation group). Ovariectomized groups were divided into 3groups, calcium free basal diet fed group(control group or ovariectomized group), 10% Cervi Pontotrichum cornu supplemented diet fed group and 10% Carthami semen supplemented diet fed group. After 4 weeks diet fed, serum osteocalcin level, bone mineral density, bone trabecular areas and cortical thickness index were analyzed. The bone mineral density and bone trabeculae area were significantly increased by Cervi Pontotrichum cornu and Carthami semen supplemented diet fed groups. The osteocalcin level was significantly decreased by Cervi Pontotrichum cornu and Carthami semen supplemented diet fed groups. The trabecular area(%) in epiphysis of tibia was significantly increased by Cervi Pontotrichum cornu and Carthami semen supplemented diet fed groups. The cortical thickness index in distal diaphysis of tibia was significantly increased by Cervi Pontotrichum cornu and Carthami semen supplemented diet fed groups. the alkaline phosphatase level was significantly decreased by Cervi Pontotrichum cornu and Carthami semen supplemented diet fed groups. These results suggest that Cervi Pontotrichum cornu and Carthami semen might have inhibitory effects on osteoporosis by increasing bone mineral density and accerelating bone formation in ovariectomized rats.

신체질량지수, 자아존중감, 외모에 대한 사회문화적 태도가 다이어트 인식에 미치는 영향 (Effects of Body Mass Index, Self-esteem, and Sociocultural Attitude toward Appearance on Diet Awareness)

  • This study examined differences in body mass index (BMI), self-esteem, sociocultural attitude toward appearance, and diet awareness between genders as well as the effect of BMI, self-esteem, and sociocultural attitude toward appearance on diet awareness. This study surveyed adults between the ages of 20 and 29 who lived in Daegu and Uijeongbu from November $17^{th}$ to December $14^{th}$ 2014. Data collected from 258 respondents were analyzed using descriptive statistics, t-test, regression analysis, and ${\chi}^2$ test. The study results showed that the respondents could be divided into three groups (underweight group, standard weight group, and overweight group) by BMI and a large percentage of respondents were in the standard weight group. The percentage of standard weight females was higher than standard weight males. There were significant differences in BMI, sociocultural attitude toward appearance (such as self-awareness), and diet awareness between genders; however, there was no difference in self-esteem. Men also had a higher BMI than women; however, women were more self-aware of sociocultural standards for appearance, desired a slim body and wanted educational information on diet. BMI and sociocultural attitude toward appearance had positive effects on diet awareness. Self-awareness was a strong predictor of diet awareness and self-esteem had no significant effect on diet awareness.

다이어트 프로그램이 성인여성의 체중감량과 건강관련지수에 미치는 효과 (Effect of a Diet Program for Adult Women on Body Weight Loss and Health Related Indices)

  • This study was carried out to analyze the effect of a diet program for adult women on weight loss, BMI, eating habits, sleeping habits, health related indices. The subjects was 415 participants of the 10 weeks (20 time participation program). The data was collected by basic somatometry and HRV (Heart Rate Variability) / APG (Accelerated Plethysmograph) Analyzer. The average age, height, weight and BMI were 28.6 years, 162.1 cm, 62.8 kg and 23.9 $kg/m^2$, respectively. Their body types by BMI were under weight (1.2%), normal (45.8%), overweight (24.8%), mild obesity (22.7%) and obesity (5.5%). There were significant reductions of average weight (4.6 kg) and average BMI (1.75 $kg/m^2$) on the 10th week. There were positive changes in vascular age (50.4%), stress index (44.6%), fatigue index (43.9%), health index (54.5%) of the subjects during the program. There was a meaningful difference of the average variation for the vascular age, stress index, fatigue index, and health index between two groups; one improving the eating habit and the other did not (p<0.05), and also there was a meaningful difference of the average variation for the fatigue index in both groups improving the sleeping habit (p<0.05), but was not a meaningful difference of the average variation for the vascular age, stress index, and health index between two groups. There was a meaningful difference of the average variation for the vascular age, stress index, fatigue index, and health index between two groups improving both eating and sleeping habit (p<0.05). In conclusion, the weight loss program was effective on the weight loss and BMI reduction and health related indices.

나복자, 의이인, 백복령, 다엽, 차전자피로 구성된 혼합분말제(HP)와 진피, 산사, 옥미수, 양총피로 구성된 혼합열수추출물(HE)이 고지방식이로 유도된 비만마우스에 미치는 항비만 효과 (Anti-Obesity Effect of Herbal Mixture Powder and Water Extracts in Mice Fed a High Fat Diet)

  • Objective : This study was conducted to investigate anti-obesity effects of mixed-Powder(Raphani Semen, Coicis Semen, Poria, Plantaginis Testa & Theae folium)(HP) & mixed water extract(Citri Pericarpium, Crataegi Fructus, Maydis Stigma, Allium Skin) (HE) in mice fed a high-fat diet. Method : 50 Female ICR mice were divided into six groups; Normal group (Ve), high-fat diet group (Po), Po+HP(0.6g/kg) group(HP), Po+HE(1g/kg) group(HE), Po+(HP+HE) group(HPE), Po+(HP+HE)*2 group(DHPE). For 8 weeks, these groups were fed their respective diets. Body weight, liver weight and weights of adipose tissues, GOT/GPT index changes & Blood Glucose index changes were measured respectively. Lipid profiles in serum were analyzed by kit of blood. Results : Groups HP, HE, HPE, DHPE had significantly reduced body weight, liver weight, adipose tissue weights and GOT/GPT index, blood glucose index compared to the Po group. Also, serum triglyceride and total cholesterol, low density lipoprotein, were significantly reduced when compared to the Po group. Group G1 had significantly increased high density lipoprotein levels. Conclusion : Mixed powder (HP) & mixed water extract (HE) show anti-obesity effects in mice fed a high-fat diet.

Lower fat and better quality diet therapy for children with pharmacoresistant epilepsy

  • The ketogenic diet (KD) is an established, effective, nonpharmacologic treatment for children with pharmacoresistant epilepsy. Although the KD is the most well-established dietary therapy for epilepsy, it is too restrictive and is associated with serious complications; therefore, alternative lower-fat diets, including a modified Atkins diet and low-glycemic index diet, have been developed. Recent ongoing clinical evidence suggests that other dietary therapies have an efficacy almost comparable to that of the KD. In addition, a diet rich in polyunsaturated fatty acids appears to increase the efficacy of diet therapy and reduce the complications of a high-fat diet. Here, we review the systematic information about lower-fat diets and better-quality dietary therapies and the current clinical status of each of these dietary approaches.
