Henoch-Sch$\ddot{o}$nlein purpura can result from exposure to an antigen after infection with several types of organisms. However, Henoch-Sch$\ddot{o}$nlein purpura caused by a primary Epstein-Barr virus infection has been rarely reported. Here, we report the case of a 32-month-old female patient who presented with Henoch-Sch$\ddot{o}$nlein purpura. Based on abnormal liver function test results and positive results for Epstein-Barr virus infection markers, a diagnosis of Epstein-Barr virus hepatitis manifesting as Henoch-Sch$\ddot{o}$nlein purpura was made. Treatment with methyl-prednisolone and hydration improved the symptoms, and a switch to oral steroids was effective in completely alleviating the purpura. No recurrence was noted and no liver function abnormalities were detected during the follow up period.

Henoch-Sch$\ddot{o}$nlein 자반병 신염과 유사한 증상을 가지는 10세 여자에게 발생한 급격한 신기능 저하를 동반하지 않은 현미경적 다발혈관염 1례를 경험하였기에 보고하는 바이다. 이 혈관염의 경우. 진단 시점 또는 치료 시작할 때의 신장기능 저하가 심할수록 병의 예후가 나쁘므로[20], 사구체 여과율이 떨어지기 전에 빠른 진단과 치료가 필요하다. Henoch-Sch$\ddot{o}$nlein 자반병이 의심될 경우 단백뇨와 혈뇨가 지속되면, 다른 혈관염과의 감별을 위해 ANCA, 신장 조직검사가 도움을 줄 것으로 생각된다.

Henoch-Sch$\ddot{o}$nlein purpura is a systemic vasculitis of unknown etiology that is probably related to an autoimmune phenomenon. Henoch-Sch$\ddot{o}$nlein purpura is characterized by a purpuric rash, arthralgia, nephritis, and gastrointestinal manifestations. We reviewed 169 children hospitalized with a diagnosis of Henoch-Sch$\ddot{o}$nlein purpura between 1989 and 1998. One-hundred thirty-nine (82.2 %) had gastrointestinal findings including abdominal pain, nausea, vomiting and gastrointestinal bleeding. Surgical consultations were obtained for ten children, and laparotomy was performed in five. Three children suspected of having appendicitis underwent appendectomy. None had appendicitis. One child thought to have been intestinal strangulation was found to have hemorrhagic edema of the proximal jejunum and of the distal ileum. Another child underwent resection for an hemorrhagic infarct of the distal ileum. A high index of suspicion of this disease entity in the differential diagnosis of abdominal pain in children can avoid unnecessary laparotomy in most cases. However, life-threatening gastrointestinal complications may occur in low percentage of cases. Prompt recognition and adequate radiologic evaluation of the abdominal manifestation of this entity is necessary for early surgical intervention.

Henoch-$Sch{\ddot{o}}nlein$ 자반증에서 복통과 혈변이 가장 흔한 위장관계 침범 증상이며, 주요 위장관계 합병증은 발생빈도가 매우 낮다. 특히 국내 소아에서 단백소실장증이 합병된 보고 예는 아직까지 없다. 저자들은 Henoch-$Sch{\ddot{o}}nlein$ 자반증에서 경과 중에 단백소실장증이 합병된 6세 여아를 경험하였기에 문헌고찰과 함께 보고한다.

We experienced a case of Henoch-Sch$\ddot{o}$nlein purpura associated with Mycoplasma pneumoniae pneumonia in a 28 month old male who suffered from cough, abdominal pain and both leg swelling and pain. Physical examination showed varying sized purpura, characteristic of Henoch-Sch$\ddot{o}$nlein purpura, below both knee. Laboratory test revealed Mycoplasma pneumoniae antibody titer >1:2,560 and cold agglutinins titer 1:64. Chest X-ray showed peribronchial blurring in both lung fields. The patient was treated with midecamycin and prednisolone for 7 days and responded to the treatment well. The authors report a case of Henoch-Sch$\ddot{o}$nlein purpura with Mycoplasma pneumoniae pneumonia with brief review of related literatures.

Henoch-Sch$\ddot{o}$nlein 자반증은 2-38%의 남자 환아에서음낭을침범할수있다. 음낭침범은 Henoch-Sch$\ddot{o}$nlein 자반증의 임상 경과 중 어느 시기에든 나타날 수 있으며, 자반이 나타나기 전 혹은 자반이 사라지고 난 후에도 나타날 수 있어 진단이 늦어지거나 오진될 수 있다. 특히 급성 음낭 통증이 동반된 경우 응급 수술이 필요한 고환염전과 감별하는 것이 중요하다. 첫번째 환자는 Henoch-Schonlein 자반증을 진단 받기 전 급성 음낭 통증이 발생하였으며, 두번째 환자는 Henoch-Schonlein 자반증을 치료받고 약 한 달이 경과한 후 급성 음낭 통증이 발생하여 방문하였다. 두 경우 모두 음낭 도플러 초음파 검사를 시행하였고 부고환으로는 증가된 혈류, 고환으로는 정상 혈류를 보여 부고환염을 진단하고 고환염전을 배제할 수 있었으며, 단기간의 스테로이드 치료를 통해 부고환염은 호전되었다. Henoch-Sch$\ddot{o}$nlein 자반증에서 동반된 급성 부고환염은 보존적 치료만을 통해 빠른 속도로 호전될 수 있는 질환이지만, 고환염전으로 오진되거나 이를 배제하지 못하는 경우 불필요한 수술이 시행될 수 있으므로 증상 발생 초기에 도플러 초음파 검사를 시행하는 것이 중요할 것으로 사료된다.

웨게너 육아종증은 중형내지 소형의 동맥과 정맥을 침범하는 전신적 혈관염의 한 형태로 상, 하부 기도 및 신장의 질환을 특징으로 한다. 신장에 국한된 질환으로 나타날 때에는 Henoch-$Sch\ddot{o}nlein$ 자반증, 현미경적 다발성 혈관염 등의 기타 신장 침범 혈관염과 구분하기 어렵다. 본 증례에서는 초기 신조직검사에서 IgA 양성으로 Henoch-$Sch\ddot{o}nlein$ 자반증으로 진단되고, 15년 후에 폐조직 검사에서 웨게너 육아종증으로 진단된 1예를 경험하였기에 문헌고찰과 함께 보고한다.

목 적 : Henoch-$Sch{\ddot{o}}nlein$ 자반증에서 항호중구 항체에 대한 외국의 여러 논문이 있었으나 이들은 대조적인 연구결과를 보였고, 국내에서는 Henoch-$Sch{\ddot{o}}nlein$ 자반증 환아를 대상으로 한 항호중구 항체의 연구가 없었다. 따라서 한국 소아 Henoch-$Sch{\ddot{o}}nlein$ 자반증에서 항호중구 항체의 임상적인 의의를 알아보고자 본 연구를 시행하였다. 방 법 : 2007년 5월부터 2008년 8월까지 신촌 세브란스병원 소아과에 내원하여 Henoch-$Sch{\ddot{o}}nlein$ 자반증으로 진단받은 30명의 환아들을 대상으로 의무기록을 후향적으로 조사하여 분석하였고, 일반적인 특징으로 나이, 성별, 자반증 등의 증상등을 조사하였다. 검사 소견으로는 백혈구수, 혈소판수, 혈침 속도, Anti-streptolysin O titer, 대변 잠혈 검사, 혈청 면역 글로불린(IgG, IgA, IgM), 혈청 보체(C3, C4), 항핵 항체, 항호중구 항체(c-ANCA, p-ANCA)를 조사하였다. 결 과 : 총 30명의 환아중 남자가 13명, 여자가 17명이었고 평균 연령은 6.0${\pm}$1.9세(범위: 5-12세)였다. 자반은 모든 환아(100%)에서 관찰되었고, 복통은 20 명(67%), 관절통은 17명(57%), 신침범은 11명(37%)에서 관찰되었다. Anti-streptolysin O titer는 검사한 환아 15명 중 1명(7%)에서 증가된 소견을 보었고, 혈청 IgA 수치는 모든 환아에서 증가되지 않았다. 간접면역형광법으로 시행한 항호중구 항체 검사에는 오직 한 명(3%)에서만 p-ANCA가 약양성 소견을 보였으나 이는 추적 검사시 음성으로 전환되었고 나머지 환아에서는 c-ANCA, p-ANCA가 모두 음성 소견을 보였다. 결 론 : 항호중구 항체는 소아 Henoch-$Sch{\ddot{o}}nlein$ 자반증에서 진단적 혹은 면역학적으로 특이한 검사소견은 아닌 것으로 보이며, 이는 항호중구 항체가 소아 Henoch-$Sch{\ddot{o}}nlein$ 자반증의 병인과 연관성이 없음을 암시한다.

There are a considerable number of reports suggesting a common pathogenesis of IgA nephritis(IgANn) Henoch-$Sch{\ddot{o}}nlein$ Purpura(HSP). In previous reports, a patient develops IgAN after kidney transplantation for HSP nephritis, one of Identical twin boys, developed IgAN and the other HSP, and a boy with IgAN later developed HSP. We report two cases, one with IgAN who later developed HSP and the other with HSP who later developed IgAN, suggesting that IgAN and HSP have a common pathogenesis. (J. [ $\beta$ ]-streptococci have been reported as an etiology of Henoch-$Sch{\ddot{o}}nlein$ purpura(HSP), but the role of $\beta$-streptococci in HSP is not yet confirmed. Our report of two HSP patients is as follows : Although the two patients' clinical symptoms resembled those symptoms of HSP, whereas serum $C_3$ decreased in one case, ASO titer increased in both cases and a histological examination of renal biopsy specimens revealed the fetures commonly described in poststreptococcal glomerulonephritis.
