Advanced Treatment Planning Method for Gamma Knife Radiosurgery of Cerebral Arteriovenous Malformations (뇌동정맥기형의 감마나이프 방사선 수술 -치료 계획 방법의 개선을 중심으로-)

  • Since March 1992, total 200 patients who visited our hospital as functional or organic lesions of central nervous system were treated by gamma knife stereotactic radiosurgery for 27 months. Thirty-nine patients of total cases was diagnosed as cerebral arteriovenous malformation. The rate of magnification on X-ray film was reduced by cutting fixation adaptor from 1.0 to below 1.45 times. In order to treat the deep- and lateral-seated cerebral arteriovenous malformation, we slightly modified the angiographic indicator, the commercial Leksell system, by cutting each inner sides about 5mm, We performed the more distinction of the scales by adapting 0.5mm or 1mm copper filter to angiographic indicator. The center point of indicator(X=100mm, Y=100mm, Z=100mm) is corrected by adjusting scales of X-, Y-, Z-axis to each inner 100 and outer 100 point within 1-2mm by repeated exposure of X-ray on films in trial-and-errors. We have developed the 'GKANGIO' programed as the Fortran-77 in Microvax - 3100, which can save treatment planning time and perform accurate pretreatment planning using the theoretical target metrix center. The theoretical description of the simplified method is presented for the reduction of experimental and numerical errors in treatment planning of radiosurgery.

THE EFFECT OF SPRUE DESIGN ON THE MARGINAL REPRODUCIBILITY OF CAST TITANIUM CROWNS (주입선 형태가 티타늄 주조관의 변연재현성에 미치는 영향)

  • There has been a great interest in the use of titanium for fixed and removable prostheses in recent because of its excellent biocompatibility. However the high melting temperature and chemical reactivity of titanium necessitates casting systems different from those used in conventional casting. The current titanium casting systems are based on an electric-arc design for melting the metal in an argon atmosphere and its exclusive investment. Despite the new developments in Ti casting systems, inadequate mold filling and internal porosity are frequently observed casting defects. Therefore, the study on the fabrication technique including sprue design to solve these casting defects is still necessary. In this study to evaluate the effect of sprue design on the castability of simulated cast titanium crowns, 10 cylindrical cast crowns for each group with four different sprue design(Single group. Double group, Runner bar group. Reservoir group) were fabricated. An impression of the entire casting margin was made and cut at $90^{\circ}$ intervals, and the sections were photographed in a microscope at $100{\times}$ magnification to record marginal discrepancy. The internal porosities of the cast crowns were disclosed by radiographs. Within the limits of this study. the following conclusions were drawn. 1. The overall mean marginal discrepancies for each group were as follows: Double group, $43.65{\mu}m$; Reservoir group, $50.27{\mu}m$; Single group, $54.17{\mu}m$; Runner bar group, $58.90{\mu}m$ (p<0.05). 2. The mean of marginal discrepancies for wax patterns was $10.65{\mu}m$. 3 The numbers of internal porosity showed the most in Runner bar group followed by Single group, Reservoir group, and Double group.

A STUDY ABOUT THE VARIATION OF STYLOID PROCESSES IN PANORAMIC RADIOGRAPHS (파노라마 방사선사진상에서 경상돌기의 변이에 관한 연구)

  • The purpose of this study was to investigate the variation in the length and shape of styloid processes and the relationships between the elongated styloid processes and the styloid process syndrome, using panoramic radiogrpahs and questionaires. The subjects were 342 patients consisted of 170 males and 172 females aged from 11 to 78 years, not showing facial asymmetry, who visited infirmary of dental college of Yonsei University. Also, the accuracy was determined for measuring the length of styloid processes from panoramic radiographs, using dried skulls. The results were as follows: 1. The length of styloid processes was magnified approximately 1.1 times, but there was no statistically significant differencies in the magnification rate of length between right and left side. 2. The mean radiographic length of styloid processes was 29.72±7.92㎜ in males, 27.93±6.69㎜ in females, and 28.82±7.37㎜ in total. And elongated styloid process (>30㎜) was seen in 31.3% of total subjects. 3. The growth in the length of styloid processes was completed in the third decade. 4. The most common shape of styloid processes was straight followed by segmented and bent form. The incidence of segmentation was reduced with increasing age. 5. The 74% of subjects with elongated styloid process (>30㎜) showed symptoms of the styloid process syndrome. The most frequent symptom was headache followed by discomfort in the neck when turning the head from left to right, tinnitus or earache, vague facial pain, discomfort or pain when swallowing, feeling that an object is caught in throat.

Cryptosporidium suis Infection in Post-Weaned and Adult Pigs in Shaanxi Province, Northwestern China

  • Cryptosporidium spp., ubiquitous enteric parasitic protozoa of vertebrates, recently emerged as an important cause of economic loss and zoonosis. The present study aimed to determine the distribution and species of Cryptosporidium in post-weaned and adult pigs in Shaanxi province, northwestern China. A total of 1,337 fresh fecal samples of post-weaned and adult pigs were collected by sterile disposable gloves from 8 areas of Shaanxi province. The samples were examined by Sheather's sugar flotation technique and microscopy at${\times}400$ magnification for Cryptosporidium infection, and the species in positive samples was further identified by PCR amplification of the small subunit (SSU) rRNA gene. A total of 44 fecal samples were successfully amplified by the nested PCR of the partial SSU rRNA, with overall prevalence of 3.3%. The average prevalence of Cryptosporidium infection in each pig farms ranged from 0 to 14.4%. Species identification by sequencing of SSU rRNA gene revealed that 42 (3.1%) samples were Cryptosporidium suis and 2 (0.15%) were Cryptosporidium scrofarum. C. suis had the highest prevalence (7.5%) in growers and the lowest in breeding pigs (0.97%). C. suis was the predominant species in pre-weaned and adult pigs, while C. scrofarum infected pigs older than 3 months only. A season-related difference of C. suis was observed in this study, with the highest prevalence in autumn (5.5%) and the lowest (1.7%) in winter. The present study provided basic information for control of Cryptosporidium infection in pigs and assessment of zoonotic transmission of pigs in Shaanxi province, China.

Profitability and Publicity of the Regional Public Hospitals in Korea - With focus on administration assessment of regional public hospitals - (공공의료기관의 공공성과 수익성 상호관계에 대한 연구 - 지방공사의료원의 경영평가를 중심으로-)

  • The administration of a regional public hospitals are expending from profit preference to publicity preference. The weight rate for a profitability and publicity of performance assessment has changed from 84:16 which was resulted by the assessment executed firstly in 1989 to 39:61 as resulted in 2004, the final assessment execution in 2005. Regional public hospitals are exerting and promoting a magnification in public sector to raise up the public-score. With comparison between publicity scores and profitability scores in original scores basis excluding weight rate, the publicity scores ranked higher than profitability scores although the latter was higher by 2002. However, for the administration performance of the regional public hospitals, the deficits increased 11 times from \92.6billion deficits with \460.3billion cost increased by 457% although income as \367.7billion increased by 394% comparing the last 2004 year to the first 1989 year for profit & loss statement of a regional public hospitals. There was analysis for the relation in yearly basis partitioning publicity and profitability for the assessment scores of the to regional public hospitals confirm the accumulated deficits of the hospitals like this attribute to the extension of public sector. The result showed that there was distinct plus relationship from 1999 although a minus relationship in general until 1997 except 1992 and there is a more plus relationship as approaching 2004. That is, it is hard to tell that the accumulated deficits increase of regional government medical center attributed to extension of public sector. On the contrary, the analysis showed the extension of public sector has a mutual relationship with uplift of profitability Meanwhile, it showed that operation cost rate and labor cost are the factors which influence a revenue & expenditure rate among the profitability index according to the results of relation analysis for the representative index of profitability and that of publicity.

COMPARISON OF THE INFLUENCE OF CANAL CURVATURE ON THE SEALING ABILITY OF LATERAL CONDENSATION AND SYSTEM BTM (측방가압법과 System BTM를 이용한 근관충전시 근관만곡도에 따른 폐쇄효과의 비교)

  • The purpose of this study was to compare sealing ability between lateral condensation and continuous wave of condensation in depend of root canal curvature. In this study, we divided fifty-six human molar teeth into two group in depend of Schneider method, and then subdivided them into four experimental group (each group is composed of twelve teeth) by canal curvature and obturation method, and eight teeth were served as positive and negative controls. Specimens were prepared by Quantac 2000 series file and obturated by lateral condensation or System B. Specimens were immersed in india ink for 7 days, decalcified by 10% nitric acid, dehydrated by 75, 95 and 100% alcohol in order, cleared by methyl salicylate and then measured of dye penetration with stereomicroscope(${\times}6.5$ magnification) and Image Pro plus. The data were analyzed stastically by one-way ANOVA and Scheffe test. The data were as follows: 1. The mean leakage was $0.725{\pm}1.167$ for group A, $0.813{\pm}0.921$ for group B, $0.809{\pm}0.997$ for group C, $1.111{\pm}1.147$ for group D, but no significant difference among them(p>0.05). 2. Lateral condensation had better sealing ability than continuous wave of condensation, but no significant difference among them(p>0.05). 3. There was no significant difference between root canal curvature degree and microleakage(p>0.05).

THE EFFECT OF THE DENTINE PRETREATMENT ON THE MARGINAL LEAKAGE OF A GLASS IONOMER CEMENT (상아질 표면처리가 글라스 아이오노머 시멘트의 변연누출에 미치는 영향에 관한 연구)

  • The purpose of this study was to evaluate the effect of the dentin pretreatment on the marginal leakage of a glassionomer cement. 1n this study, 60 molars with sound and healthy crown portion were used. The dentin surface of these teeth were exposed and polished with 600 grit silicon carbide paper. Square - shaped cavities were prepared on the flattened dentin surfaces and these were divided into 4 groups according to the dentin pretreatment procedures. Group I : Dentin pretreatment with distilled water as a control group. Group II : Dentin pretreatment with 5% sodium hypochlorite solution. Group III : Dentin pretreatment with Ketac conditioner. Group IV : Dentin pretreatment with 40% polyacrylic acid. The degrees of dye penetration in the cavity walls were assessed using a stereoscope at ${\times}40$ magnification according to the maximum dye penetration. The results were analyzed by using Mann - Whitney U test. The results were as follows : 1. All groups showed varying depth of dye penetration. 2. Distilled water group showed the most severe marginal leakage when compared with the other groups(P<0.05). 3. 40% polyacrylic acid group showed the least amount of marginal leakage compared with the other groups (P<0.05). 4. There were significant differences between Goup I(distilled water) and Group IV (40% polyacrylic acid)(P<0.05), but there were no significant differences among Group I(distilled water), Group II(sodium hypochlorite), Group III(Ketac conditioner) (P>0.05).

Training of Microanastomosis with Chicken Wing Brachial Artery (닭 날개 혈관을 이용한 혈관문합술의 교육)

  • Purpose: Microsuturing is a difficult job for beginners of microsurgery. It is because they are not familiar with microscopic environment and, it needs much time for them to get used to microanastomosis. Before the real microsurgery, sometimes a surgeon wants rehearsal. But, microsurgical exercise has been performed with surgical glove, silastic drain or rat femoral artery. Rat femoral artery is a very good training material. But, it needs animal laboratory, anesthesia and its keeping facilities. And the surgeon should appoint the time to exercise with the laboratory. Methods: We used chicken wing brachial artery for education material of microsuturing. The artery is 5 cm long and the diameter is about 1 mm. Monofilament 10-0 was used for suture material. Results: Six persons of Seoul National University medical school students and one resident attended in this program. Each of them performed arterial anastomosis ten times. They were satisfied with chicken wing brachial artery for anastomosis training under the magnification environment. Conclusion: We think that chicken wing brachial artery is a very cheap and an effective training material for the beginners of microsurgery.

Rapidly and Accurately Processing of Low Melting Block for Shielding of Radiotherapy (방사선(放射線) 치료(治療)의 신속정확(迅速正確)을 위한 저온용융(低溫熔融) 차폐물(遮蔽物)의 제작(製作)과 응용(應用))

  • For accurate and easily shielding irregular shaped organ, its minimized penumbra region and a low melting point alloy 'Lead Y' and synchronizing instrument have been developed. The 'Lead Y' is the quaternary eutectic alloy and it is composed of Lead 30.0% Tin 11.5% Bismuth 48 5% Cadmium 10.0% The density of its at $22^{\circ}C$ is $9.8g/cm^3$ and the melting temperature has $40^{\circ}C\;to\;68^{\circ}C$. The thickness of 'Lead Y' for perfect shielding of Co-60 gamma ray and LINAC 10MeV x-ray is 6cm and 7cm respectively. The 'Lead Y' shielding block is casted directly on the styrofoam from which is cut with hot wire of synchronizer device. The special features and advantages of the Lead Y shielding block could be summarized as follows; 1. The shielding block for radiotherapy is rapidly processed only with boiling water and styrofoam. 2. It is not injure one's health and not danger of a fire, because of not generating of any metals vapor and evil smelling. 3. It is very effective to minimize secondary penumbra for the protection of healthy tissue from unnecessary ionizing radiation regardless of the magnification source to skin distance. 4. The HVL of the Lead Y is 1.2cm for Co-60 gamma ray and it's shielding effect is almost same as the pure lead block. 5. The hardness of Lead Y is 1.5 times higher than lead block. 6. It's reavailability is higher than lead block and then one block of Lead Y is reavailable about 30 to 40 times. 7. It is usefull for shielding of x-ray, gamma ray, beta-ray, electron and neutron radiation. 8. The materials for Lead Y are easy to acquire with reasonable price and tractable.

Affinity of Endothelial Cells to a Polyurethane Vascular Graft: A Preliminary Animal Study (폴리우레탄 인공혈관에 대한 혈관내피세포의 친화성: 예비동물실험)

  • Purpose: Autologous vessels remain the gold standard for vascular grafts in microanastomoses. However, they are sometimes unavailable and have a limited long - term patency. Synthetic vessels have high success rates in large - diameter reconstructions but failed when used as small - diameter grafts due to graft occlusion. It has been proved that endothelial cell seeding improves prosthesis performance and long - term patency. Among polyurethane, PET and ePTFE, polyurethane has the best affinity to endothelial cells and mechanical properties closest to human vessels. We examined the ability of endothelial cells to attach to a polyurethane graft manufactured by the electrospinning method. Methods: Endothelial cells, which were cultured from porcine internal jugular veins, were attached to polyurethane grafts with an internal diameter of 3 mm. The same cells were attached to allogeneic decellularized porcine internal carotid artery grafts as controls. Both of the 10 mm - long grafts were exposed to endothelial cells in a well for 1 hour. Each well contained $2{\times}10^5$ endothelial cells. The graft materials were rotated through 90 degrees every 15 minutes in order to minimize the effect of gravity. The extent of cell attachment was examined with the MTT assay. Results: The MTT assay showed good incorporation of endothelial cells into both grafts. For the evaluation of affinity, the number of attached cells was counted at 10 fields of microscopic examination with ${\times}40$ magnification. Endothelial cells adhered more to polyurethane grafts (mean, $127.4{\pm}6.2cells$) compared to porcine artery grafts (mean $45.8{\pm}5.1cells$)(p<0.05,Mann - Whitney test). Conclusion: In this study, we attached porcine endothelial cells to polyurethane grafts, manufactured by electrospinning. The grafts exhibited a better affinity to endothelial cells than allogeneic decellularized porcine internal carotid artery grafts. It is suggested that the time required for endothelial cells to attach to decellulized artery grafts may be longer than that which is required for attachment to polyurethane grafts.
