The Effect of Stress and Sense of Coherence on Health Behavior Compliance in Outpatients with Stroke

뇌졸중 환자의 스트레스와 통합성이 건강행위 이행에 미치는 영향

Abstract

The purpose of this study was to identify the effect of stress and sense of coherence on health behavior compliance in patients with stroke. The subjects were 124 outpatients with stroke at a general hospital in Ulsan from May to July, 2019. The data were analyzed using descriptive statistics, t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient and hierarchical multiple regression analysis by using SPSS 25.0. In the results, the health behavior compliance showed a significantly positive correlation with sense of coherence(r=.71, p<.001) and a negative correlation with stress(r=-.53, p<.001). The predictors of model 1 were gender(t=4.07, p<.001), education level(t=4.18, p<.001), and monthly income(t=-2.34, p=.021) and the statistical power was 39.6%. The predictors of model 2 were gender(t=4.65, p<.001), education level(t=43.24, p<.001), and sense of coherence(t=4.33, p<.001) and the statistical power was 60.4%. Thus, a nursing intervention program which is focused on a sense of coherence could promote health behavior compliance in patients with stroke.

본 연구는 뇌졸중 환자의 스트레스와 통합성이 건강행위 이행에 미치는 영향을 알아보기 위한 서술적 조사연구이다. 자료수집은 2019년 5월에서 7월까지 U광역시에 소재한 종합병원에서 뇌졸중으로 퇴원 후 외래를 방문하는 환자 124명을 대상으로 실시하였다. 자료분석은 SPSS 25.0 프로그램을 활용하여 뇌졸중 환자의 통합성, 스트레스 및 건강행위 이행의 정도는 평균과 표준편차, 일반적 특성에 따른 건강행위 이행의 차이는 t-test와 one-way ANOVA, 사후검증은 Scheffé test, 상관관계는 Pearson correlation coefficient, 건강행위 이행에 미치는 영향요인은 위계적 다중회귀분석을 실시하였다. 연구결과에서 건강행위 이행은 통합성과 유의한 양의 상관(r=.71, p<.001), 스트레스와 유의한 음의 상관(r=-.53, p<.001)을 나타냈다. 영향요인으로 모델 1에서는 설명력 39.6%로 성별(t=4.07, p<.001), 교육수준(t=4.18, p<.001), 월수입(t=-2.34, p=.021)이며, 모델 2에서는 설명력 60.4%로 성별(t=4.65, p<.001), 교육수준(t=3.24, p=.002), 통합성(t=4.33, p<.001)이었다. 이상에서 나타난 연구결과에 근거하여 통합성 프로그램의 개발 및 적용을 통해 뇌졸중 환자의 건강행위 이행을 증진시켜야 할 것이다.
