원발성 난관 악성종양의 영상소견
Imaging Findings of Primary Tubal Malignancy

목적: 원발성 난관 악성종양의 영상소견을 알아보고자 하였다. 대상과 방법: 수술과 병리조직 소견으로 원발성 난관 악성종양으로 확진된 40명의 환자 중 확인 가능한 영상이 있는 27명, 28예를 대상으로 후향적으로 분석하였다. 환자의 나이는 34~78세로 평균 58.2세였다. 23명에서 전산화단층촬영을 시행하였으며 3명에서 자기공명영상, 4명에서 초음파 검사를 시행하였다. 종양의 크기, 낭성부분의 비율, 모양, 동반된 소견 등을 분석하였다. 결과: 종양의 크기는 평균 5.3 cm(범위, 2~13.1 cm)였다. 낭성부분의 비율에 따라 형태를 나누었을 때(1형: 낭성비율이 75% 이상, 2형: 낭성비율이 50~75%, 3형: 낭성비율이 25~50%, 4형: 낭성비율이 25% 이하) 1형에 10예(35.7%), 2형이 2예(7.1%), 3형에 3예(10.7%), 4형이 13예(46.4%)였다. 모양을 원형, 소엽형, 관상형(round, lobular, tubular)으로 나누면 각각 7예(25%), 12예(42.9%), 9예(32.1%)였다. 8예(28.6%)에서 종양내 중격을 동반하였고 13예(46.4%)에서 벽비후 소견을 보였으며 11예(39.3%)에서 수난관을 동반하였다. 결론: 원발성 난관 악성종양은 여러 형태로 관찰되며 낭성부분의 비율도 매우 다양하였다. 종양의 위치와 형태로 인해 원발성 난소종양과 감별하는 것이 어려우나, 수난관을 동반하거나 그 모양이 소엽 또는 관상형일 경우 감별진단으로 고려하여야 한다.
Purpose: To describe the radiologic findings of primary tubal malignancy. Materials and Methods: This study described the imaging findings of 27 patients with 28 cases of primary tubal malignancy. Of the 27 patients examined, 23 underwent CT, 3 underwent MRI and 4 underwent ultrasonography. Image findings were analyzed according to size, proportion of cyst, shape, and associated findings of tumor. Results: The mean tumor diameter was 5.3 cm (range 2.0-13.1 cm).
