초록
목적: 중둔근 건에 발생한 석회화의 임상 양상과 진단 및 치료에 대하여 조사하였다. 대상 및 방법: 2004년 2월부터 2008년 12월까지 석회화 건염으로 진단되었던 9명의 환자를 대상으로 하였으며 평균 연령은 57세(41~80)였고 남자 1예, 여자 8예였다. 전 예에서 단순 방사선 검사 및 초음파를 시행하였다. 단순 방사성 검사에서는 석회화 모양, 초음파 검사에서 중둔근 건의 두께 변화 및 석회화 모양, 약물 투여와 증상과의 관계에 대하여 조사하였다. 결과: 석회화 모양이 제 1형(솜털 경계)의 경우 내원 전 증상의 지속 및 소실 시간이 제 2형(균일한 경계면)의 경우보다 짧았다(p=0.03). 초음파 검사에서 보여준 중둔근 건의 두께, 석회화 모양(구형, 무정형)과 증상의 지속 시간, 소실 시간과의 관계에서는 통계적인 유의한 차이는 없었다. 비스테로이드성 소염제를 투여한 경우에 증상의 소실 시간과 관계에서는 의미 있는 상관 관계를 확인 할 수 없었다. 결론: 중둔근 건에 발생한 석회화 건염에서 솜털경계를 가진 경우 증상의 지속 및 소실 시간이 짧았고 비스테로이드성 소염제의 투여는 상기 질환의 진행에 영향을 주지 않았다.
Purpose: This study evaluated the clinical symptoms, diagnosis and management of calcific tendinitis around the insertion of the gluteus medius tendon. Materials and methods: Between February 2004 and December 2008, 9 patients (8 women and 1 man; mean age, 57 years; age range, 41~80) with a diagnosis of calcific tendinitis were enrolled in this study. All patients underwent radiography and sonography. The relationships between the clinical aspects, medication, the shape of calcification under radiography, thickening of the tendon, and the shape of calcification under sonography were analyzed. Results: Type 1 (fluffy margin) under radiography had a shorter symptom duration and faster recovery than type 2 (well-defined and homogenous margin) (p=0.03). Tendon thickening and the shape of calcification (ovoid or amorphous) from the sonographic findings was not significantly related to the symptom duration and recovery time. Non-steroidal anti-inflammatory medication had no effect on the symptom duration or recovery time. Conclusion: Patients with calcific tendinitis around the gluteus medius tendon with a type 1 margin (fluffy margin) showed a shorter symptom duration and faster recovery than those with a type 2 margin. Non-steroidal anti-inflammatory medication had no effect on the symptom duration or recovery time.
