Zinc-citrate compound (SeLava$^{\circledR}$)을 이용한 임신과 동반된 자궁경부 선암 치료 1예

초록

자궁경부암은 암으로 인해 사망에 이르는 주요한 부인암중 하나이며, HPV의 감염이 자궁경부암 발생의 중요한 원인인자로 알려져 있다. 아연은 세포주기 정지 및 세포자멸사를 통해 여러 암세포의 성장을 억제시키는 광물질로 알려져 있다. 본 저자들의 연구에 의하면 zinc-citrate compound (CIZAR$^{\circledR}$)가 융모상피암 및 난소선암에 세포자멸사 유도 효과가 있음을 확인한 바, 본 CIZAR$^{\circledR}$를 통한 세포독성 효과를 이용하여 임신유지를 원하는 11주 자궁경부선암 산모에 대해 매일 CIZAR$^{\circledR}$가 함유된 SeLava$^{\circledR}$ 질세척액으로 국소세척을 시행하였다. 치료 약150일 후에는 자궁세포진 검사에서 AGUS로 변하였고, 임신 39주에 건강한 남아를 분만하였으며, 분만 후 치료 약240일 후에 시행한 자궁경부 원추절제술 후조직 검사 결과HPV 감염이 없어졌으며, 국소미세침윤(<0.1 mm)이 있는 상피내선암 (adenocarcinoma in situ)이 되었고, 치료 260일 후에 시행한 전자궁적출술 결과 잔여 병소는 없었다. 치료기간중 특이한 부작용은 없었다. 향후 체계적인 임상계획으로 SeLava$^{\circledR}$의 국소요법에 대한 치료효과를 기대하며, 간단한 문헌 고찰과 함께 보고하는 바이다.

Cervical carcinoma is the second leading cause of death from cancer in women worldwide. It is well known that human papillomaviruses (HPVs) is the etiologic agent of cervical neoplasia and cervical cancer. Zinc has been shown to inhibit the growth of malignant cell lines by inducing apoptosis and cell cycle arrest. Recently it was reported that zinc-citrate compound (CIZAR$^{\circledR}$) has a cytotoxic effect on choriocarcinoma cell line and ovarian adenocarcinoma cell line and suppresses its proliferation inducing apoptosis. CIZAR$^{\circledR}$ prevents the proliferation by inactivation of m-aconitase activity and induces apoptosis by increasing Bax expression and reducing Bcl-2 expression and inactivation of telomerase. We report one patient of cervical adenocarcinoma with HPV infection, who desires to continue pregnancy, treated by daily topical application of SeLava$^{\circledR}$ which contains zinc-citrate compound (CIZAR$^{\circledR}$). We followed up the cytologic, pathologic and coloposcopic changes of healing process.
