경북 의성초등학교 10~12세 남아 153명, 여아 156명 총 309명을 대상으로 1994년 12월 3일 부터 12월 20일까지 체격지수(BMI, RI)와 체지방율(%fat)을 근거로한 비만의 실태 파악과 비만 판정에 사용되는 여러가지 방법들간의 상관성을 검토한 결과는 다음과 같다. 1. 조사대상자들의 평균 신장은 남아 143.6cm 여아 145.2cm, 평균 체중은 남아 37.1kg, 여 아 36.6kg으로, 상완위 둘레의 경우 평균이 남아 20.7cm, 여아 20.2cm로 나타났다. 2. 체격지수 중 BMI의 경우 남아 17.7, 여아 17.2, RI는 남아 123.9, 여아 118.2로 남아가 여아 보다 유의적으로 높은 값을 나타내었다(p<0.05). 체지방율의 경우 남아 14.1%, 여아 15.4%로 남아 보다 여아가 유의적으로 높은 값을 나타내었다(p<0.05).3. 체지방율의 기준에 따라 조사대상 아동의 체격분포를 살펴 본 결과, 남아의 경우 여윔군 49명(32.0%), 정상군 91명(59.2%), 비만군 13명(8.5%), 여아의 경우 여윔군 71명(45.5%), 정상군 84명(53.8%), 비만군 1명(0.6%)으로 본 조사대상자의 대부분이 정상군에 속하였고, 비만의 출현율이 가장 높은 시기는 남녀 모두 12세로 나타났다. 4. 체격지수와 체지방율로 비만을 판정했을 때, BMI로는 39명(12.6%), RI로는 31명 (10.0%), 체지방율로는 14명 (4.5%)의 아동이 비만으로 나타났고, BMI와 RI모두에 의해서는 23명(7.4%), BMI와 체지방율 모두에 의해서는 10명(3.2%)의 아동이 비만으로 나타났다. 이와같이 비만판정에 사용한 기준에 따라 비만 출현율에 있어서 상당한 차이를 나타내었다. 5. Triceps과 calf 피하지방두께의 합을 사용하는 Slaughter의 공식으로 산출한 체지방율과 비만 판정에 사용하는 기준간의 상관성에서 BMI는 남녀 각각 r=0.63, r=0.62를, RI의 경우 각각 r=0.56, r=0.49를 나타낸 반면, triceps과 subscapular의 합과의 상관성은 매우 높아 남녀 각각 r=0.91과 r=0.88을 나타내었다. 또한 국내아동을 대상으로 고안된 추정식으로 산출한 체지방율과의 상관관계도 매우 높아 남녀 각각 r=0.93과 r=0.88을 나타내었다. 6. 피하지방두께 중 신체하부조직인 Calf가 다른 피하지방두께에 비해 체격 지수인 BMI(r=0.815), RI(r=0.675) 및 체지방율 (r=0.565)과 높은 상관성을 나타내었다.
This study was conducted to investigate the prevalence of childhood obesity and to figure out the correlation between percentage of body fat and various criteria used for evaluating childhood obesity, and to suggest a new convenient criterion applicable to obesity assessment for large population. subject of 309(male: 153, female: 156) from Uisung elementary school in Kyungbuk, in the age range from 10 to 12 were recruited. The prevalence of obese was evaluated by the methods of Slaughter skinfold equations for children, BMI, and RI. fourteen(4.5%) by Slaughter eq, 39(12.6%) by BMI, 31 (10%) by RI were found to be obese. The results indicate that the evaluation of child obesity is influenced by the methods applied; that is, BMI and RI tend to overpredict the obese while percentage of body fat tend to underestimate. Percentage of body fat by Slaughter equations using triceps and calf skinfold is highly correlated with triceps plus subscapular skinfold thickness(male: r=0.91, female: r=0.88) and with Kim equations for Korean children(male: r=0.93, femlae: r=0.88). The results suggest that, if necessary, practitioners can adequately predict body composition without exposing the upper torso of females.
